Burn patients' use of autohypnosis: making a painful experience bearable.

Autohypnosis (that is performing hypnosis in the absence of the therapist) was used for adjuvant analgesic and anxiolytic therapy in 18 burn patients. It combines the advantages of hypnosis without being limited by the availability of the hypnotic therapist immediately prior to painful activities. Autohypnosis is less time and labour consuming, and therefore less expensive. Out of 180 patients treated with autogenic relaxation techniques only 48 (27 per cent) entered a state of hypnosis that enabled them to cope with their pain, and 18 (10 per cent) achieved autohypnosis. Most of these patients needed some auxiliary devices such as recorded instructions or telephone calls.

Emotional and psychosocial adjustment of women to breast reconstruction and detection of subgroups at risk for psychological morbidity.

Thirty-four women who had undergone breast reconstruction after mastectomy for breast cancer were evaluated as to their emotional and psychosocial adjustment. Standardized psychological tests and a self-report questionnaire were used, along with the surgeon's evaluation of the aesthetic success of the reconstruction. Two psychodiagnostic tests were found to be of use in identifying maladjusted women who do not gain optimal benefit from the reconstruction: a questionnaire that evaluates the level of depression (Beck) and another that assesses the level of optimism (Cantrill). No correlation was observed between the surgeon's evaluation of the reconstruction's aesthetic success and the women's satisfaction. It was found that there are two groups of emotional-high-risk women: single women and women receiving adjuvant therapy. It is suggested that emotional-high-risk patients be given psychological support concerning the importance of the mastectomy and their reaction to breast reconstruction.

Microsurgical replantation of two totally avulsed scalps.

We describe our experience with two patients in whom total avulsion of the scalp occurred. Successful replantation was carried out by microvascular surgery, with 90 and 95% survival of the scalp. Normal hair regrowth and ability to raise the eyebrows was achieved in both cases. A single superficial temporal artery was anastomosed, providing excellent perfusion of the entire scalp. Microvascular replantation should be attempted with all scalp avulsions, regardless of appearance and mechanism of injury. The patient must be transferred to a medical center with comprehensive microvascular surgery capabilities. The avulsed scalp should be cooled to 4 degrees C immediately, but prolonged ischemia time does not preclude a successful result.

[Resection of invasive orbital tumors and reconstruction with temporalis muscle flap].

3 patients with invasive basal cell carcinoma of the skin which necessitated orbital exenteration are reported. In all 3 treatment was delayed, primary therapy was inadequate and the tumor had reached massive size. Excision of the orbital contents is a radical, mutilating procedure, but in advanced cases of malignant orbital tumors it offers the only chance for survival. The temporalis muscle flap is adaptable, well supplied with blood, and contains a large mass of tissue. Its utilization provides a simple, uncomplicated and effective technique for orbital reconstruction following massive resection. It gives a relatively effective result which is usually acceptable to the patient.

[Sex reassignment surgery in a trans-sexual].

Trans-sexualism is a psychiatric disorder characterized by the conviction that one's gender identity does not conform to one's biological sex. The current Diagnostic and Statistical Manual of Mental Disorders gives 5 diagnostic criteria for trans-sexualism which most health care professionals accept. Although many trans-sexuals derive some gratification from assuming roles appropriate to their desired gender, a substantial number are convinced that only sexual transformation can bring meaningful relief of their feelings of despair. Approximately 2/3 of trans-sexuals who undergo sex reassignment procedures are improved at follow-up. A relatively advanced age and secondary trans-sexualism (transvestites and effeminate homosexuals) are risk factors for poor prognosis in those requesting sex reassignment. In recent decades the demand for sex reassignment has increased, as have the number and variety of psychological, hormonal and surgical treatments proposed. The operative procedures currently employed for sex reassignment give reasonably good esthetic and functional results, and in 1986 the Israel Ministry of Health permitted its government hospitals to perform them. A 23-year-old male trans-sexual is reported who underwent successful surgery for sex change and now, 1 year later, is satisfied with the result.

Neck contracture as a rare complication of cervical soft tissue expansion.

An unusual complication using an inflatable silicon implant for removing postburn scarred tissue of the neck and face is described. The appearance of a fibrotic band along the inflating tube caused neck contracture. The purpose of this article is to present this uncommon complication and to emphasize the technical error when using the tissue expander in the neck region.

Pseudolymphoma of the earlobes due to ear piercing.

Keloids are commonly seen as a complication of earlobe piercing. As biopsies are usually not performed routinely in many instances, some of the lesions might, in fact, represent other entities, such as cutaneous pseudolymphoma. Contrary to keloids, for which surgery alone is not recommended, a simple excision or superficial x-ray is sufficient in such cases.

[Early microsurgical reconstruction of extremities with free microvascular flaps].

The key to successful reconstruction of the extremities after complex trauma is often adequate soft tissue cover of the injured area. In the past, when surgical debridement of wounds was too extensive, or it exposed vital structures such as blood vessels, nerves, tendons or bones, the limb either had to be amputated or reconstructed in multiple surgical procedures. The use of free microvascular flaps in the immediate postinjury period allows single stage reconstruction of wounds including exposed vital tissues. It promotes limb salvage and shortens recovery time after severe trauma. 4 patients with severe complex trauma of the extremities underwent reconstruction with immediate free microvascular flaps, which included latissimus dorsi and rectus abdominis muscle and free scapular flaps. The surgical protocol included debridement of wounds on arrival with primary fixation of fractures and additional debridement procedures in the first week until the wound was stable. Then final fixation of fractures was performed and the wound was covered by a free microvascular flap. With this technique, single stage reconstruction is possible in most cases, with improvement in function and cosmetic outcome, multiple operations are avoided and the recovery period is shortened.

Mapping of the human body skin with the transcutaneous oxygen pressure method.

Transcutaneous oxygen pressure (TcPO2) measurements are studied to determine whether location, side of body, sex, and age affect the values at 23 different sites. Measurements were taken on 19 healthy volunteers on one side or the other of the body. Values were similar at all sites with the exception of the forehead, cheek, medial aspect of the leg, and the scapular region. There was no difference between sides of the body or between younger and older subjects. Women had significantly higher values. The results are discussed, and it is concluded that most skin regions can serve as controls for areas under investigation.

T6 positive cells in the peripheral blood of burn patients: are they Langerhans cells precursors?

Peripheral blood mononuclear cells of 14 patients suffering thermal injury were separated by affinity chromatography on peanut agglutinin (PNA) coupled to Sepharose macrobeads. The resulting PNA positive subset was 14% of the total mononuclear population. About 30% of these cells were found to coexpress T6(CD1), Ia-like and the myeloid differentiation antigens My4(CDw14) and Mo1(CD11). In comparison, the PNA+ subset from normal blood donors (about 5% of total mononuclear cells) contained mature monocytes that were found to be T6 negative. Electron microscopic studies using immunogold labeling showed that the T6 positive cells were slightly smaller than monocytes but larger than the classical lymphocytes and had common morphologic features with the Langerhans cells of the skin. Considering that patients suffering extensive damage of the epidermis require fast renewal of all skin elements, it is possible that the cells we identified in their peripheral blood are the precursors of the Langerhans cells of the skin en route from bone marrow to the epidermis.

Factitious lymphedema of the hand: a diagnostic challenge.

A case of factitious lymphedema of the hand is presented. Attention is drawn to this entity and its differential diagnosis. Delayed detection in our case resulted in prolonged and costly hospitalization with some loss of hand function. Treatment should be left to those skilled in psychotherapy.

Complications of liquid nitrogen cryosurgery for verrucae over bony prominences.

Liquid nitrogen cryosurgery is the most frequently used surgical treatment for verrucae. Its success rate is high and complications are rare. However, liquid nitrogen should be used with great care over bony prominences where skin and subcutaneous tissues are thin. We present 3 patients in whom the treatment of simple warts over bony prominences by liquid nitrogen resulted in full-thickness loss of skin. In 2 of them, extensor tendons of fingers were exposed and had to be covered with flaps. We conclude that expertise of the physician is of paramount importance in preventing similar complications in cryosurgery of verrucae.

Melkersson-Rosenthal syndrome: an oligosymptomatic form.

We have described a 35-year-old woman who had had recurrent episodes of swelling of the lips since the age of 14. The patient had a complete remission of symptoms between the ages of 20 to 30, but the swelling recurred, and she has had permanent swelling of both lips and cheeks for the past four years. Various treatments were ineffective. Biopsy of the lip mucosa showed histologic findings of Melkersson-Rosenthal syndrome. This case probably represents an oligosymptomatic form of the syndrome.

Pseudomelanoma following laser therapy for congenital nevus.

Benign nevi can be removed by a variety of procedures including surgical excision, electrocautery, slicing off the protruding portion by a shave biopsy, cryotherapy, dermabrasion, etc. Except in the case of complete excision, these procedures may often be followed by recurrence. We describe a congenital nevus that was incompletely removed by CO2 laser therapy. When the lesion recurred, it had clinical as well as histologic features in common with malignant melanoma, although it was completely benign.
