Early stage head and neck non-Hodgkin's lymphoma. The effect of tumor burden on prognosis.

Treatment results were investigated in 113 previously untreated patients with clinical Stage I and II (Ann Arbor) non-Hodgkin's lymphoma of the head and neck. Fifty-six Waldeyer's ring, 34 other extranodal sites, and 23 cervical nodal lesions were included. The overall relapse-free survival at 5 years was 41%. Age and Ann Arbor stage influenced relapse-free survival. The results suggested that the tumor cell burden is a fundamental prognostic factor for patients with Waldeyer's ring disease and for patients with only cervical nodal disease. Abdominal relapse was most frequent, followed by generalized relapse. From 1981, patients were randomized in a clinical trial to receive either chemotherapy (cyclophosphamide, vincristine, and prednisone [CVP], five courses) or whole-abdominal irradiation (25 Gy/20 Fr) as an adjuvant therapy. Patients could not tolerate the whole-abdominal irradiation well. A significant improvement in survival has been obtained by adjuvant chemotherapy.

The role of radiotherapy in the management of maxillary sinus carcinoma.

From 1971 to 1983, a total of 208 patients with maxillary sinus carcinomas were treated at Department of Radiology, Hokkaido University School of Medicine, 38 by Method I (radiation + surgery + intraarterial 5-fluorouracil [5-Fu] infusion), and 170 by Method II (radiation + surgery). Radiation doses ranged from 30 to 50 Gy over 3 to 5 weeks in Method I and 52 to 58 Gy over 4 weeks in Method II. Overall actuarial survival rate at 5 years was 45.6%. No statistical difference in survival rates was found between Method I and Method II. With respect to T stage, absolute 5-year survival rates were 100% (4/4) for T2, 49.5% (52/105) for T3, and 24.4% (11/45) for T4. From our data, no advantage in the use of intra-arterial 5-Fu infusion was demonstrated; the local failures in Method I developed earlier and more frequently than in Method II. The ultimate failures for all patients were 46.8%. Since 1980 when treatment planning by using computerized tomography scans and immobilization device was initiated, improvement in survival rate while reducing an incidence of eye complications has been accomplished.

[Analysis of prognostic factors in the multidisciplinary treatment of maxillary sinus carcinoma].

In 247 patients with squamous cell carcinoma of the maxillary sinus, the logistic multiple regression analyses were employed to identify prognostic factors. Among various factors evaluated, use of CT scans in treatment planning (p less than .004), age (p less than .009), location of the tumor (p less than .011), T-stage (p less than .017), and free set-up in radiotherapy (p less than .052) proved to be significant factors independently predictive of survival. The most significant factor in improving survival rate was the use of CT scans in treatment planning which was initiated in 1980. Against our prediction, intra-arterial 5-Fu infusion combined with surgery and radiation did not yield favorable result when compared with combined surgery and radiation. Radiation was combined with surgery, high dose radiation of 50 to 55 Gy delivered in 16 fractions over 4 weeks seemed to show better local control than other dose-fractionations.

Quantitative dose-response analysis of salivary function following radiotherapy using sequential RI-sialography.

A total of 216 99mTc-pertechnetate-sialographies, by applying stimulations by citric acid, were performed for studying radiation effects on the salivary glands in 145 patients. From a scintigram of the salivary gland, the time-activity-curves were obtained for analysis of salivary function. The shape of the time-activity-curve with respect to stimulus-response pattern was classified into four types according to the degree of radiation-induced dysfunction. We found that the acid stimulation was enhancing accumulation of the isotope into the salivary gland as well as influencing salivary discharge. A ratio (Rc) of pre- and post-stimulation counts permitted a construction of quantitative dose-response curves; the curve for the parotids had a slight rise of Rc's over 0-20 Gy, whereas that for the submandibular glands had a significant rise over 10-30 Gy; in both glands the rise of Rc's was followed by a steep fall. The parotids appeared to be more sensitive to radiation than the submandibular glands at 0-3 months following 20-70 Gy, but after 3 months both glands were similarly impaired. The gland volume irradiated was also of great importance in determining the degree of salivary dysfunction. The maximum uptakes averaged per unit area (salivary-to-background ratio) and determined during the initial 30 minutes were greater in the submandibular than in the parotid glands following greater than 20 Gy. In both glands the maximum uptakes were gradually impaired with time over 3-36 months following 50-70 Gy.

A medical record database in radiology.

A database system on the medical records of radiation therapy, computer tomographic and radioisotopic examinations of our department was created in Computing Center of Hokkaido University which has two remote terminals in the department. Old three filing systems which had been kept in three sections of our department independently since 1972 were integrated by the creation of the database. The main functions of our database management system are as follows; 1. data input through two minicomputers in the department; 2. data loading to the database from the minicomputers; 3. production of key word files and link files for generalised data handling. Seven files are defined in the database with total data of 30 Mega bytes at the end of 1981. Many programs for information retrieval and data processings were prepared and every member of the department can share both data and application programs registered. Outline and operation of the database system and some examples of data processings are reported.

Tissue reaction & tumor response with negative pi mesons.

As a prelude to a randomized trial of negative pi meson (pion) radiotherapy, as compared to conventional radiation treatment, tolerance of several normal tissues was investigated. Fifty-three of 108 patients received at least 2700 peak pion rads at a usual dose rate between 100 and 125 rads daily. The major sites treated were head and neck, pancreas, prostate, rectum and brain. Acute normal tissue reactions, late effects, and tumor response are correlated with the two dose levels. Pancreatic tumors have not fared well. At this point, tumors that can be observed disappear more rapidly, and, for tumors that cannot be observed, symptoms disappear more rapidly and normal tissues exhibit less reaction than with conventional radiotherapy. It is believed that the dose level can be raised an additional 7 percent above the current 4100 rads, measured at the maximum within the treatment volume.

[Thirty years of Radiology Department in Hokkaido University Hospital (author's transl)].

Thirty year's history of the Radiology Department of the Medical School of Hokkaido University, the first department of its kind in Hokkaido, is shortly reviewed. Statistics include the number of staff, equipment and patients diagnosed or/and treated in the department. Sections have been formed in the department since several years ago and are functioning to a certain extent independently in order to keep pace with the development of modern medicine and to meet the needs of our hospital. These sections are mainly radiation therapy, nuclear medicine, neuroradiology, X-ray diagnosis in angiology and for the digestive tract. Diagnosis based upon ultrasonography and CT are also becoming recognized sections in the department recently. Further more data processing and statistics seem to grow to become a section in the near future. In conclusion the number of staff, equipment and patients has been and perhaps will be growing increasingly fast. The increasing number of sections which resulted from the establishment of new specialities makes us to anticipate the new and independent departments in addition to the existing one.
