A disease with immune deficiency, skin abscesses, pancytopenia, abnormal bone marrow karyotype, and increased sister chromatid exchanges: an autosomal recessive chromosome instability syndrome?

A 19-year-old girl is described with microcephaly, short stature, mental retardation, pigmentation of the skin, and recurrent skin abscesses over the whole body. Her elder brother and sister both showed growth and developmental retardation, microcephaly, and anemia. Both died during childhood. Their parents were first cousins. Laboratory studies of the proband revealed hyperchromic erythrocytes with an increased HbF content, thrombocytopenia, an impaired mitogenic response of the PHA-stimulated lymphocytes, and partial impairment of humoral and cellular immunity. She developed pancytopenia in the terminal stage of the disease. Cytogenetic studies of the bone marrow revealed 46,XX, 15p+, -18, +mar karyotype, increased chromosomal aberrations and sister chromatid exchanges, in cultured lymphocytes and skin fibroblasts. She died at age 20. Thus, the disorder in the patient was deduced as an unclassified chromosomal breakage syndrome with an apparently autosomal recessive inheritance.

Leachability of denture-base acrylic resins in artificial saliva.

We studied the influence of salivary acidity on leachability of denture-base acrylic resins with etiological interest in denture stomatitis because denture surfaces are frequently exposed to acidic conditions in the oral cavities. Auto-, heat-, and microwave-polymerized resins were immersed in artificial saliva with pH ranging from 4.0 to 6.8 at 37 degrees C, and leachables were pursued quantitatively with time. Methyl methacrylate, methacrylic acid, and benzoic acid leached from all resins. Their concentrations in the saliva were markedly high for auto-polymerized resins, while leachability of heat- and microwave-polymerized resins was so low that quantitative analysis of leachables was impossible. Lower pH showed higher concentrations of methyl methacrylate, although no apparent association was confirmed between salivary acidity and its own leachability. The concentrations of methacrylic acid increased remarkably with an increase in pH, which was probably due to hydrolysis of methyl methacrylate. These results suggest that chemotoxic actions of auto-polymerized resins are potentially ascribable to methyl methacrylate under more acidic conditions and to methacrylic acid under less acidic conditions.

High-performance liquid chromatographic analysis of catecholamines in biological samples by liquid/liquid extraction prepurification.

A prepurification procedure for the determination of catecholamines in biological samples was studied to simplify the procedure and increase selectivity and recovery. The procedure is based on liquid/liquid extraction of catecholamine-borate complexes. Under optimal conditions, catecholamines formed complexes with diphenylborate in an alkaline NH4Cl/NH4OH buffer that were extracted with n-heptanol. Catecholamines were reextracted into an HCl-acidified solution, followed by reversed-phase ion-pair high-performance liquid chromatographic separation with fluorometric detection. Application of the method to various samples of rat tissues and human urine showed procedural simplicity, high selectivity, and high recovery (about 90% or more for tissue samples). This method can be used for pharmacological studies on catecholamines.

Total sacrectomy and reconstruction for huge sacral tumors.

The authors carried out successful total sacrectomy in three cases, two with giant cell tumors and one with a chordoma. The anterior and posterior approach is feasible for resecting huge sacral tumors en bloc, but it is important to reconstruct the continuity between the pelvic ring and spinal column using spinal instrumentation and sacral rods or AO plates. As total sacrectomy is a large-scale, time-consuming, and collaborative operation, two or three teams should be used in relays. Both pelvic and spinal surgical techniques are required. Post-operatively the patient can stand within 3 to 6 months and well-planned rehabilitation allows ambulation. In spite of the serious structural and neurologic damage caused, total sacrectomy can be rewarding procedure in terms of improved morbidity and mortality.

Elevated expression of proto-oncogenes during interleukin-5-induced growth and differentiation of murine B lineage cells.

Interleukin 5 (IL-5), a lymphokine produced by helper T cells, is involved in the regulation of growth and differentiation of B cells and other hematopoietic cells. To elucidate IL-5-mediated intracellular mechanisms, we have established IL-5-dependent and -independent murine early B cell lines, J6 and MJ88-1, respectively, and examined the effect of IL-5 on the expression of proto-oncogenes during proliferation. Two- to 3.5-fold increases in the levels of c-myb, c-myc, c-fos, and c-fms mRNA were observed in J6 cells, compared with those in MJ88-1 cells. Further, a role of IL-5 in the proto-oncogene expression during differentiation was examined by using thymidine-treated murine B-cell chronic leukemia BCL1-B20 cells with growth arrest. After 4-day culture, the amount of IgM secreted from BCL1-B20 cells was augmented 4-6 fold in the presence of IL-5. Although expression of c-myb, c-fos, and c-fms mRNA did not change, only c-myc mRNA expression was elevated within 30 min of stimulation with IL-5 and reached a maximal level by 1 hr. Addition of phorbol 12-myristate 13-acetate (PMA) or IL-4 to the culture of BCL1-B20 cells inhibited both the IL-5-mediated augmentation of IgM secretion and the elevated expression of c-myc mRNA. These findings suggest that the IL-5 signal may be associated with the up-regulation of c-myc expression.

High-performance liquid chromatographic analysis of alpha-keto acids produced from amino acid metabolism in oral Bacteroides.

Profiles of metabolic alpha-keto acids were determined by a high-performance liquid chromatographic method and applied to characterization of oral black-pigmented Bacteroides. Each bacterial strain was incubated with amino acids in a chemically defined medium. After production alpha-keto acids were purified by hydrazide gel column treatment and converted to u.v.-absorbing derivatives. They were analysed by reversed-phase ion-pair chromatography. Black-pigmented Bacteroides species were differentiated into two groups according to production of aromatic alpha-keto acids. Bacteroides gingivalis, B. endodontalis and B. loescheii produced both p-hydroxyphenylpyruvic and phenylpyruvic acids. However, no such alpha-keto acids were produced by B. levii, B. intermedius and B. denticola. In addition, production profiles of several aliphatic alpha-keto acids (alpha-ketoglutaric, pyruvic, alpha-ketobutyric, alpha-ketoisovaleric, alpha-ketoisocaproic, and alpha-keto-beta-methylvaleric acids) separated each individual species in such groups. The present study offers useful chemotaxonomic information on amino acid metabolic activity of oral black-pigmented Bacteroides species.

Lipid metabolism in acromegalic patients before and after selective pituitary adenomectomy.

Lipid metabolism was studied in 16 acromegalic patients who all underwent transsphenoidal selective pituitary adenomectomy (SPA). Before the operation, their serum lipid levels correlated with none of the basal levels of serum growth hormone (GH), basal levels of plasma somatomedin-C (SM-C), fasting levels of plasma glucose (FPG), peak levels of plasma glucose (PGp) or basal and peak levels of serum immunoreactive insulin (IRIb and IRIp, resp.) in the oral glucose tolerance test (OGTT), and obesity indices. The serum GH levels as well as plasma SM-C levels in the group with decreased serum high density lipoprotein-cholesterol (HDL-C) differed greatly from those of the normal HDL-C group. However, there was no significant difference in either serum GH or plasma SM-C between groups with and without metabolic abnormality of any other lipid examined. After the operation, the basal levels of serum GH and plasma SM-C decreased significantly. In conjunction with these changes, PGp, serum IRIb, serum triglyceride (TG), non-esterified fatty acid (NEFA) and very low density lipoprotein (VLDL) decreased significantly. In contrast, serum HDL-C increased significantly. However, FPG, serum IRIp, obesity indices, serum total cholesterol (TC) and serum low density lipoprotein (LDL) showed no significant change. There were no significant differences in the levels of any serum lipid either before or after surgery among the diabetic, borderline and normal types defined by the preoperative OGTT patterns. Atherogenic indices (AIs) decreased significantly and returned to normal postoperatively. These results suggest that obesity or secondary diabetes is not a direct cause of hyperlipidemia in acromegaly. The prognosis of acromegaly is affected by arteriosclerotic complications. It is intriguing, therefore, that AIs were normalized by transsphenoidal SPA. Being rather a safe procedure, it can be performed without hesitation, aside from a conservative treatment.

[Echocardiographic assessment of mitral annular calcification in hypertrophic cardiomyopathy with asymmetric septal hypertrophy and healthy elderly cases].

The prevalence and degree of mitral annular calcification (MAC) in elderly cases of hypertrophic cardiomyopathy with asymmetric septal hypertrophy (ASHCM) were studied in comparison with healthy elderly and chronic hypertensive elderly with concentric left ventricular hypertrophy (HT + LVH) by echocardiography. In seven elderly patients with ASHCM (mean age 77.3 +/- 8.1 years) severe MAC was observed in all cases (1 male and 6 females) and systolic anterior motion of the mitral valve (SAM) was present in 3 cases, incomplete in 2 and absent in 2 cases. Of 57 healthy elderly with a mean age of 78.8 +/- 8.8 years, 8 subjects (1 male and 7 females, 14%) showed MAC. The prevalence of MAC was 0% in the sixties, 14% in the seventies, 16% in the eighties and 29% in the nineties. MAC was severe in four subjects above eighty years old. In 3 cases with HT + LVH with a mean age of 72.3 +/- 4.7 years no MAC was observed and SAM was present in one, partially observed in one and absent in one case, respectively. Serum calcium (Ca) and phosphorus (P), reported to be related with the formation of MAC, and fasting blood sugar level (FBS) were evaluated. In healthy elderly with MAC, serum Ca was 8.78 +/- 0.46 mg/dl, P 3.55 +/- 0.37 mg/dl and FBS 97.75 +/- 29.01 mg/dl, P 3.55 +/- 0.37 mg/dl and FBS 97.75 +/- 29.01 mg/dl.(ABSTRACT TRUNCATED AT 250 WORDS)

[Application of flow cytometry to the study of hematologic disorders: analysis of oncogene products].

Flow cytometry (FCM) is a useful method for clinical research of oncogene products since it can analyze proteins quantitatively which are located at cell surfaces or inside of cells. Oncogene products are now under study by FCM not only as tumor markers but also as functioning proteins in carcinogenesis. The examples of oncogene products analyzed by FCM are ras, myc, p53, myb and fos; those of cell-proliferation-related proteins are Ki-67, PCNA and DNA polymerase alpha. In some diseases the relationship between these proteins and disease classification, stage, pathophysiology, or prognosis have been clarified. Using dual color FCM of H-ras p21 and DNA, we analyzed the expression of H-ras p21 in human multiple myeloma and leukemias and found that H-ras p21 levels in multiple myeloma strongly correlated to the prognosis of patients (p = 0.03). When AML cells were stimulated by adding G-CSF, it was found that many cells proliferated but some were dying. The percentage of dying cells was small in one AML case whose myeloblasts showed increased expression of H-ras p21 by G-CSF stimulation. Together with other papers reviewed, it is conceivable that H-ras p21 expression is related to cell proliferation and inhibition of cell autolysis. Thus FCM is useful in the classification of the role of oncogene products in carcinogenesis in clinical cases.

[Intra-arterial infusion of cisplatin and caffeine for a recurrent malignant fibrous histiocytoma].

We treated a 43-year-old man with recurrent malignant fibrous histiocytoma in right distal femur with intra-arterial infusion of cisplatin and caffeine 5 years after wide excision and chemotherapy. At the time of the first recurrence in the lateral aspect of thigh, intra-arterial infusion of CDDP (120 mg/m2) was ineffective. We treated him with radiation (7000 rad), and there was no evidence of tumor by radiological evaluation. The second local recurrence was treated with intra-arterial infusion of CDDP (120 mg/m2/1 hour) and caffeine (1.2 g/m2/24 hours x 3 days), and the tumor disappeared, radiologically and histologically. Caffeine did not increase the nephrotoxicity of CDDP, and no insomnia nor palpitation was seen. Intra-arterial infusion of CDDP and caffeine could be useful to increase the effect of CDDP for regional chemotherapy.

Osteosarcoma arising from a multiple exostoses lesion: case report.

A case of osteosarcoma arising from a multiple exostoses lesion is presented. Poorly differentiated osteosarcoma occurred in a twelve-year-old girl's proximal tibia where an exostosis was confirmed from radiographs. We treated this patient with preoperative chemotherapy, thigh amputation and postoperative chemotherapy, but she died of multiple pulmonary metastases seven months after surgery. The osteosarcoma, complicated by multiple exostoses, had a very poor prognosis because it was resistant to various anticancer agents.

Zinc in breast milk during prolonged lactation: comparison between the UK and the Gambia.

A total of 580 breast milk samples were collected from 56 lactating women living in a rural village community in The Gambia, West Africa, and 92 samples were obtained from 57 lactating women living in Cambridge, England. Total zinc content of each sample was measured by atomic absorption spectrometry, and pooled samples of the Gambian breast milks, representing successive periods of lactation, were fractionated into fat, whey and insoluble casein fractions, to examine zinc distribution. The two sets of milks both exhibited a dramatic decline in total zinc concentrations with increasing duration of lactation, in common with previous studies. However, the UK milks unexpectedly had lower zinc contents at all stages than the Gambian milks. Milk zinc levels were not significantly related to either maternal age or parity in the Gambian women. The proportion of zinc found in the sedimentable fraction remained nearly constant with increasing duration of lactation, but the proportion found in the fat fraction increased and the proportion in the whey fraction declined. These observations are potentially relevant for estimations of milk zinc availability and the fulfillment of zinc requirements by infants, and hence for infant feeding practices.

Determination of L-3,4-dihydroxyphenylalanine in blood by high-performance liquid chromatography after solvent extraction.

The use of high-performance liquid chromatography combined with solvent extraction for sample purification is described for the determination of L-3,4-dihydroxyphenylalanine in blood plasma. It is extracted into n-hexanol via complexation of its catechol moiety with diphenyl borate and ion-pair formation of its carboxylic group with tetrapentylammonium ion in an alkaline buffer. Under optimal extraction conditions, L-3,4-dihydroxyphenylalanine and 3,4-dihydroxybenzylamine used as an internal standard are extracted from blood plasma by a simple procedure and in a short time and then separated by reversed-phase ion-pair chromatography. The analytical recovery (100.8%) and reproducibility (coefficient of variation = 2.3% for n = 6) from plasma samples are good enough for routine analysis. L-3,4-Dihydroxyphenylalanine levels in blood can be monitored by this method after oral intake of the substance.

Improvement of human tumor clonogenic assay using cell culture in soft part sarcoma of the extremity.

The human tumor clonogenic assay (HTCA = standard HTCA) gives poor results in sarcomas compared to carcinomas. In order to overcome this problem, a cultivated HTCA, using single-cell suspensions obtained from short-term culture, was developed. In forty specimens taken from 32 patients with soft part sarcomas of the extremities, both standard and cultivated HTCA were performed. Colony-forming ability was low in standard HTCA (0.0053 +/- 0.0051%) while it was high in cultivated HTCA (0.0231 +/- 0.0186%). The number of clonogenic assays suitable for sensitivity tests was low (23.7%) in standard HTCA, while the number was much higher (70.0%) in cultivated HTCA. The histological grade of malignancy (low, moderate, high) was correlated with the growth of colonies in both methods of HTCA; cultivated HTCA, versus HTCA, increased the number of colonies produced in the high and moderate groups of malignant sarcomas. The results of the sensitivity of 8 anticancer drugs on sarcomas were almost the same between the two methods. A retrospective study on the results of the cultivated HTCA and clinical chemotherapeutic effectiveness proved a significant overall correlation rate (91.7%, p less than 0.025). It was concluded that in tumors of a low colony-forming ability such as soft part sarcomas, cultivated HTCA can provide a useful chemosensitivity test.
