Baroreflex control of heart rate during cardiac sympathectomy by epidural anesthesia in lightly anesthetized humans.

To evaluate the effects of acute sympathetic denervation on the baroreflex control of heart rate, baroreflex sensitivities were compared in lightly anesthetized humans who had either cervical (N = 20) or lumbar epidural anesthesia (N = 18), or neither (N = 18). Levels of anesthesia during cervical block using 10 ml of 2% mepivacaine without epinephrine were C3-T7 and T7-S1 during lumbar epidural block. Baroreflex sensitivity was assessed with a pressor test using phenylephrine infusion to increase systolic blood pressure by 60 mm Hg within 2 min. There were no statistically significant differences in the baroreflex sensitivities in the absence of epidural block and during lumbar epidural block, the slopes of the regression line (in msec of RR interval change per mm Hg increase in systolic blood pressure, i.e., msec/mm Hg) relating systolic pressure and the succeeding pulse interval being 3.8 +/- 1.4 (mean +/- SD) and 3.7 +/- 1.7, respectively. The mean slope during cervical block, 1.1 +/- 1.2 msec/mm Hg, was significantly different from the slopes observed in the absence of epidural block and during lumbar epidural block (P less than 0.01). The results suggest that cardiac sympathectomy induced by epidural anesthesia can suppress partially baroreceptor function by interrupting sympathetic efferent fibers innervating the heart during high levels of epidural anesthesia, but that lumbar sympathectomy during epidural anesthesia is unlikely to affect baroreceptor activity.

Quartan malaria following splenectomy 36 years after infection.

Quartan malaria developed following splenectomy 36 years after infection in a 63-year-old hypertensive man. The patient underwent nephrectomy because of left renal calculus, increasing proteinuria and hypertension. Splenectomy was done additionally because metastasis of renal tumor to the spleen was suspected at the operation. Attention is drawn to the long silent infection with Plasmodium malariae and to the importance of the spleen in malaria.

[Compared studies of antimicrobial agents against E. coli, Klebsiella, Citrobacter and Proteus isolated from urinary tract infections].

In vitro activities of antibacterial agents against E. coli, Klebsiella, Citrobacter and Proteus which were isolated from patients urinary tract infections at 8 hospitals in Japan, were investigated by agar dilution method from July to October in 1979. The summarized results are as follows. 1. Among oral antibacterial agents, MPC and PPA have showed potent antibacterial activities against E. coli and Klebsiella. Among parenteral antibiotics, CTM was the most active against E. coli and Klebsiella. However, ABPC-resistant E. coli and Klebsiella have appeared to occupy about 40% and 96% of bacteria isolated from urinary tract infections, respectively. 2. In vitro activities of antibacterial agents against Proteus and Citrobacter showed not so potent. 3. Causative organisms in female patients with simple urinary tract infections were mainly E. coli and Klebsiella. 4. Among oral antibacterial agents, PPA have shown similar antimicrobial activities against E. coli isolated from simple and complicated urinary tract infections. ABPC and MPC have been influenced in some degree by these factors. However, parenteral antibiotics are not influenced by these factors. On the other hand, in vitro activities of antibacterial agents against Klebsiella isolated from simple and complicated urinary tract infections were similar.

Association of gastric cancer and nephrotic syndrome. An immunologic study in three patients.

Three patients presenting typical nephrotic syndrome concurrent with gastric cancer are described. The carcinoma was moderately advanced and was histologically an adenocarcinoma in all cases. Morphologic examination of the renal biopsy revealed the characteristic changes of membranous nephropathy and typical subepithelial electron-dense deposits. The glomerular deposits were specifically stained with antihuman IgG and beta-1-c. The eluate of the glomeruli reacted specifically with the surface of the cancer cells from the same patient when studied by the immunofluorescent method. The eluate also reacted with the cancer cell extract as examined with the radioimmunodiffusion technique and with carcinoembryonic antigen (CEA) prepared by the method of Gold. Furthermore, the nephrotic syndrome completely remitted in 1 patient following surgical removal of the cancer. These bits of evidence indicate a possible role of a stomach cancer related antigen and/or CEA in the development of immune complex nephropathy in these patients.

A strong correlation between glomerular filtration rate and filtration surface in diabetic kidney hyperfunction.

The direct relationship between glomerular filtration surface and glomerulaf filtration rate was studied in a group of eight short term, young diabetics exhibiting a 50 per cent elevated glomerular filtration rate with a 2-fold variation within the group. A close correlation was obtained between glomerular filtration surface and glomerular filtration rate, r = 0.8. This supports the idea that the increase in glomerular capillary surface may be a main factor in diabetic kidney hyperfunction. The possible long term implications of the functional and morphologic abnormalities are discussed.

Dipyridamole therapy in the nephrotic syndrome.

Dipyridamole was used in 30 cases of nephrotic syndrome, mostly of intractable type. The results indicate that the drug therapy proved to be effective in decreasing urinary protein and controlling nephrotic condition in 40% of the cases after an initial period of treatment. Long-term results of the drug on urinary protein and on nephrotic condition were rated as good in 36.7 and 53.3%, respectively, of the cases treated. The exact mechanism of action of dipyridamole in the nephrotic syndrome is still obscure in many respects. However, the fact that the drug shares its anti-platelet action with the non-steroid anti-inflammatory drugs, e.g. aspirin and indomethacin, and the rapidity with which it produces its urinary protein-decreasing effect, strongly suggests that it inhibits the release of vasoactive amines and other chemical mediators from blood platelets. As far as the present study is concerned, adverse side effects of dipyridamole were few or minimal, even when the drug used in large doses over a prolonged period of time. From these results it is considered that dipyridamole provides a new remedy which is worthy of trying in nephrotic syndrome as a means of reducing the requirement of steroids and immunosuppressive drugs.
