[Levels of alpha-elastin and of desmosine as indexes of elastase-induced lung injury in rats].

Two markers of elastin degradation, alpha-elastin and desmosine levels, were measured in sera from rats after single tracheal instillations of porcine pancreatic elastase (PPE). Biochemical and pathophysiological changes in the lung were also studied. Male Wistar rats were divided into three groups. The control group received one intratracheal dose of saline (0.3 ml). The PPE 60 group received one intratracheal dose of 60 U of PPE. The PPE 120 group received one intratracheal dose of 120 of PPE. Blood samples were collected 0, 1, 3, 5, 7, 14, 28, and 56 days after the intervention, and levels of alpha-elastin and desmosine were measured by enzyme-linked immunosorbent assay. At the end of the study (day 56), lung volumes and static pressure-volume curves were measured by body pletysmography. Then the rats were killed and the mean linear intercept was measured as defined by Thurlbeck. The serum alpha-elastin level in the PPE 120 group on day 7 was significantly higher (p < 0.05) than those in other groups. Desmosine values on day 1 in the PPE 60 group and on days 1 and 7 in the PPE 120 group were significantly higher (p < 0.05) than those in the control group. Functional and morphometric analyses revealed that the PPE 120 group had more emphysematous changes than the control group (p < 0.01) and more than the PPE 60 group (p < 0.05). These results suggest that levels of alpha-elastin and of desmosine reflect changes that occur in the early stages of lung injury, and that these changes are related to abnormalities found in rat lungs on day 56.

Sudden death of adults in Japan.

Epidemiological features, risk factors and preventive methods of sudden death (SD) derived from studies the authors have performed since 1987 together with colleagues in Niigata University School of Medicine were reviewed. When SD was defined as death occurring within 24 hours of the onset of symptoms, the annual incidence was 145/100,000 for people aged 15 years and older in Niigata Prefecture. The incidence increased sharply along with the advance of age, while the proportion of SD to natural death due to circulatory diseases was higher in younger people. Though diseases of the circulatory system made up approximately 90 percent of all causes of death, SD due to ischemic heart disease was less frequent in Japan than in western countries. SD showed various patterns in seasonal and "within-a-day" occurrences according to sex, age and cause of death. The months of the highest SD occurrence differed by occupation and matched the busiest work periods. A decrease in sleeping hours and mental stress experienced during the preceding week were related to the occurrence of both sudden death and non-fatal acute myocardial infarction. People having structural circulatory diseases were shown to be predisposed to SD when stress occurred, because fatal arrhythmia is easily induced by the above factors in such people. Health examinations were shown to have preventive effects, though limited, against SD. Differences in the resuscitated rates in cases where a witness was present and where one was not indicates that educating people about correct resuscitation methods is important to minimizing SD.

Modified Marcy repair of large indirect inguinal hernia in infants and children.

Based on a review of operative notes of recurrent inguinal hernia cases from the authors' primary series, a surgical technique modified from the Marcy repair is described. With this technique, emphasis is placed on preservation of the intact internal spermatic fascia and reduction in the size of the internal inguinal ring. Through the inguinal approach, the sleeve-like extension of the internal spermatic fascia is incised longitudinally along the cord and up to the internal ring. The cord structures are dissected off the sac, and as much of the fascial tissue as possible is preserved intact. Both edges of the fascial defect are approximated with an unabsorbable suture; great care is taken to not penetrate the wall of the sac. The same suture is then used for high ligation, via a stay suture placed on the transversalis fascia on the other side of the neck, to reduce the size of the internal ring. The technique can be used in premature babies who have a flimsy, easily torn sac, and in some cases of giant hernia with a widely dilated internal inguinal ring, if the direct wall integrity remains adequate.

Redistribution of the CDK inhibitor p27 between different cyclin.CDK complexes in the mouse fibroblast cell cycle and in cells arrested with lovastatin or ultraviolet irradiation.

The cyclin-dependent kinase (CDK) inhibitor p27 binds and inhibits the kinase activity of several CDKs. Here we report an analysis of the behavior and partners of p27 in Swiss 3T3 mouse fibroblasts during normal mitotic cell cycle progression, as well as in cells arrested at different stages in the cycle by growth factor deprivation, lovastatin treatment, or ultraviolet (UV) irradiation. We found that the level of p27 is elevated in cells arrested in G0 by growth factor deprivation or contact inhibition. In G0, p27 was predominantly monomeric, although some portion was associated with residual cyclin A.Cdk2. During G1, all of p27 was associated with cyclin D1.Cdk4 and was then redistributed to cyclin A.Cdk2 as cells entered S phase. The loss of the monomeric p27 pool as cyclins accumulate in G1 is consistent with the in vivo and in vitro data showing that p27 binds better to cyclin.CDK complexes than to monomeric CDKs. In growing cells, the majority of p27 was associated with cyclin D1 and the level of p27 was significantly lower than the level of cyclin D1. In cells arrested in G1 with lovastatin, cyclin D1 was degraded and p27 was redistributed to cyclin A.Cdk2. In contrast to p21 (which is a p27-related CDK inhibitor and is induced by UV irradiation), the level of p27 was reduced after UV irradiation, but because cyclin D1 was degraded more rapidly than p27, there was a transient increase in binding of p27 to cyclin A.Cdk2. These data suggest that cyclin D1.Cdk4 acts as a reservoir for p27, and p27 is redistributed from cyclin D1.Cdk4 to cyclin A.Cdk2 complexes during S phase, or when cells are arrested by growth factor deprivation, lovastatin treatment, or UV irradiation. It is likely that a similar principle of redistribution of p27 is used by the cell in other instances of cell cycle arrest.

The presence of platelet-derived endothelial cell growth factor in human endometrium and its characteristic expression during the menstrual cycle and early gestational period.

Decidualized tissues are characterized by the intensive outgrowth of the microvasculature. Several angiogenic factors are assumed to be involved during the drastic change in the vasculature occurring in the process of decidualization. We examined the possible role of platelet-derived endothelial cell growth factor (PD-ECGF), a known angiogenic factor, during the process of early decidualization in humans. The expression of PD-ECGF in human endometrium was demonstrated by Western blot analysis, a marked increase being found in decidualized endometrium. Immunohistochemical staining showed that PD-ECGF immunoreactivity was present mainly in decidualized endometrial stromal cells. We established a primary cell culture of human endometrial stromal cells which were differentiated into decidualized cells in vitro by the addition of progesterone. In this cell culture system, progesterone augmented the expression of PD-ECGF in a dose-dependent fashion. The addition of progesterone also resulted in an increased release of prolactin, a well-known marker for decidualization. These findings suggest that PD-ECGF may play a physiological role as a possible angiogenic factor in the process of decidualization of human endometrium.

[Pathogenic bacteria isolated from the sputum of the patients with pulmonary emphysema].

Isolated pathogenic bacteria from sputum of the patients with pulmonary emphysema who were admitted in our hospital from 1984 to 1994 were examined to elucidate the relationship between isolated bacteria from sputum and pulmonary functions including vital capacity (VC), forced expiratory volume (FEV1.0), PaO2 and PaCO2. VC of the patients from whom MSSA (methicillin-sensitive Staphylococcus aureus) or Enterococcus faecalis (E. faecalis) were isolated was significantly lower than that of the patients from whom Streptococcus pneumoniae (S. pneumoniae), Branhamella catarrhalis (B. catarrhalis) or Haemophilus influenza (H. influenza) were isolated. FEV1.0 had a similar tendency as VC in terms of isolated organisms from the patients with emphysema. Similarly, PO2 of the patients from whom MSSA or E. cloacae were isolated was significantly lower than that of the patients from whom S. pneumoniae, B. catarrhalis or H. influenzae were isolated, and PCO2 of the patients from whom S. pneumoniae, B. catarrhalis or H. influenza were isolated. There was also impaired respiratory function in the patients from whom MSSA, Escherichia coli (E. coli), Pseudomonas aeruginosa (P. aeruginosa), Xanthomonas maltophilia (X. maltophilia) or Enterobacter cloacae (E. cloacae) were isolated, compared with those in the patients from whom S. pneumonia, B. catarrhalis or H. influenzae were isolated. These results suggest that isolated pathogenic bacteria are shifted from S. pneumoniae, B. catarrhalis or H. influenza to MSSA, E. coli, P. aeruginosa, X. maltophilia or E. cloacae in the course of impairment of respiratory function in pulmonary emphysema. The treatment and prophylaxis for acute exacerbation in pulmonary emphysema should be based on these results.

Acute subarachnoid hemorrhage: MR imaging with fluid-attenuated inversion recovery pulse sequences.

PURPOSE: To evaluate the usefulness of fluid-attenuated inversion recovery (FLAIR) magnetic resonance (MR) imaging sequences in the detection of acute subarachnoid hemorrhage (SAH). MATERIALS AND METHODS: MR imaging with FLAIR sequences was performed with a 0.5-T superconducting unit in 20 patients (aged 30-72 years) with acute SAH due to a ruptured aneurysm and in 27 control subjects (aged 32-72 years). FLAIR images were obtained 2 hours to 2 days after ictus. Findings were evaluated and compared with computed tomographic (CT) findings. RESULTS: In all patients, acute SAH was clearly demonstrated as an area with signal intensity that was high relative to that of the normal cerebrospinal fluid and surrounding brain parenchyma at FLAIR imaging. This sequence was especially useful in demonstration of acute SAH in the posterior fossa, which was difficult to show at CT because of beam-hardening artifacts. In a double-blind comparison, no FLAIR images acquired in control subjects were confused with those acquired in control subjects were confused with those acquired in patients. CONCLUSION: FLAIR sequences reliably provide diagnostic images in patients with acute SAH.

Estrogen attenuates progressive glomerular injury in hypercholesterolemic male Imai rats.

Hypercholesterolemic Imai rats spontaneously develop proteinuria and glomerulosclerosis, especially in males. A sex difference in glomerular injury has been recognized, but the role of sex hormones in glomerular injury remains unclear. Therefore, we investigated whether estrogen administration influences the progressive glomerular injury in male Imai rats. Estrogen produced a significant decrease in body weight. Systolic blood pressure in estrogen-treated rats was significantly lower than that in controls. Estrogen attenuated the progression of glomerular injury by significantly reducing proteinuria and glomerular sclerosis. The glomerulosclerosis index was significantly higher in controls than in estrogen-treated rats. Estrogen suppressed serum testosterone levels, whereas it increased GH levels. Results suggest that estrogen appears to play an inhibitory role on the development of glomerular injury, by itself or in association with sex-related factors regulated by estrogen and testosterone.

[Effect of theophylline on disturbances of ventilation and of gas exchange during exercise in patients with pulmonary emphysema].

Factors limiting exercise were studied in 23 patients with pulmonary emphysema, and the effects of theophylline on exercise tolerance and on hypercapnic ventilatory response were studied in 16 patients with pulmonary emphysema. Exercise in these patients was limited by inability to increase tidal volume and by disturbances of gas exchange, as indicated by low PaO2, high AaDO2, and high PaCO2 during exercise, and by low diffusing capacity. Theophylline increased tidal volume during exercise, but it did not improve oxygenation during exercise. Theophylline improved exercise tolerance and hypercapnic ventilatory response. The percent increases in VEmax and in VO2max after theophylline administration correlated significantly with the percent increase in the ratio of mouth occlusion pressure to PACO2 during rebreathing. These results indicate that exercise tolerance in patients with pulmonary emphysema can be improved by theophylline, probably because of an increase in ventilatory drive.

p27, a novel inhibitor of G1 cyclin-Cdk protein kinase activity, is related to p21.

Using a yeast interaction screen to search for proteins that interact with cyclin D1-Cdk4, we identified a 27 kDa mouse protein related to the p21 cyclin-Cdk inhibitor. p27 interacts strongly with D-type cyclins and Cdk4 in vitro and more weakly with cyclin E and Cdk2. In mouse fibroblasts, p27 is associated predominantly with cyclin D1-Cdk4. Recombinant p27 is a potent inhibitor of cyclin D1-Cdk4 and cyclin A-Cdk2 protein kinase activity and a weaker inhibitor of cyclin B1-Cdc2. Overexpression of p27 in Saos-2 cells causes G1 arrest. p27 protein levels do not change as serum-stimulated quiescent mouse fibroblasts progress through the cell cycle. p27 is identical to p27Kip1, a cyclin-Cdk inhibitor present in TGF beta-treated cells. p27 has the hallmarks of a negative regulator of G1 progression and may mediate TGF beta-induced G1 arrest.

Detection of alpha-fetoprotein mRNA, an indicator of hematogenous spreading hepatocellular carcinoma, in the circulation: a possible predictor of metastatic hepatocellular carcinoma.

We attempted to detect circulating hepatocellular carcinoma by demonstrating hepatocyte-associated mRNA in the nuclear cell component of peripheral blood using nested reverse transcription-polymerase chain reaction because of the extremely small number of tumor cells in the circulation. Albumin mRNA was demonstrated not only in the liver tissue (hepatocytes) and HepG2 cells but also in nuclear cells of the blood from normal healthy volunteers (neutrophils and lymphocytes) by reverse transcription-polymerase chain reaction. In contrast, alpha-fetoprotein mRNA was demonstrated in the liver tissue, as well as in HepG2 cells, but not in peripheral blood of normal healthy volunteers, indicating the possibility of using alpha-fetoprotein mRNA for detection of benign and malignant hepatocytes among the population of neutrophils and lymphocytes. alpha-Fetoprotein mRNA in peripheral blood was detected in 17 of 33 cases of hepatocellular carcinoma (52%), 2 of 13 cases of cirrhosis (15%) and 2 of 17 cases of chronic hepatitis (12%). alpha-Fetoprotein mRNA was not demonstrated in 26 cases of normal healthy volunteers (0%). Among the patients with hepatocellular carcinoma, total volume of tumor tissue, maximum size of tumor and serum alpha-fetoprotein level were markedly increased in the patients with alpha-fetoprotein mRNA in blood. In addition, alpha-fetoprotein mRNA was detected in the blood of all 6 patients showing metastasis at extrahepatic organs (100%), in contrast to 11 of 27 cases without metastasis (41%). From these results, we conclude that the presence of alpha-fetoprotein mRNA in peripheral blood may be an indicator of circulating malignant or benign hepatocytes, which might predict hematogenous spreading metastasis of tumor cells in patients with hepatocellular carcinoma.

Testosterone eliminates the attenuating effect of castration on the progressive glomerular injury in hypercholesterolemic male Imai rats.

Hypercholesterolemic Imai rats, especially males, spontaneously develop proteinuria and glomerulosclerosis. We have shown that castration attenuated the progression of glomerular injury in male Imai rats. The present study was designed to investigate whether the attenuating effect of castration on glomerulosclerosis is eliminated by administration of testosterone. Testosterone propionate (TP) eliminated the attenuating effect of castration on the progression of glomerular injury by significantly and dose dependently increasing proteinuria and enhancing glomerular sclerosis. TP at 1 mg failed to reverse glomerulosclerosis to the control levels, despite a dose of TP sufficient to restore urinary protein excretion, serom testosterone level and glomerular growth. The results indicated that besides testosterone other sex-related hormones may be involved in the development of glomerulosclerosis in male Imai rats.

[Effect of E. coli endotoxin and D-galactosamine on pathophysiology in rat lungs].

The effects of repeated intravenous injection of E. coli endotoxin (ETX) and intraperitoneal injection of D-galactosamine (GAL), which decreases the circulating level of alpha 1-antitrypsin, on the pathophysiology of chronic lung injury was studied in rats. Four groups were prepared as follows for 8 weeks. Group 1 (control): Intravenous injection of saline. Group 2: Intravenous injection of ETX (2 mg/kg) once a week. Group 3: Intraperitoneal injection of GAL (200 mg/kg), 2 times daily on 3 consecutive days each week. Group 4: Injection of both ETX and GAL, at the same dosages as used in groups 2 and 3. Total lung capacity and static lung compliance divided by weight were high in the ETX group and the ETX + GAL group, comparative when compared with those in the control and GAL groups, even though weight gain rates in the ETX + GAL group was less than in other groups. Mean linear intercept of rats in the ETX + GAL group was significantly greater than in other groups. These results suggest that ETX + GAL-treated rats have more emphysematous changes in pulmonary function and structure.

MR of acute subarachnoid hemorrhage: a preliminary report of fluid-attenuated inversion-recovery pulse sequences.

We report preliminary results applying fluid-attenuated inversion-recovery (FLAIR) sequences to three patients with acute subarachnoid hemorrhage. Acute subarachnoid hemorrhage could be clearly demonstrated as areas of high signal intensity on FLAIR sequences in all patients. These preliminary results suggest that with FLAIR sequences one could reliably diagnose acute subarachnoid hemorrhage.

[Imipenem/cilastatin sodium alone or combined with amikacin sulfate in respiratory infections].

In the present study, we used the envelope method to divide patients with respiratory infections into two groups: a monotherapy group given imipenem/cilastatin sodium (IPM/CS) and a combination therapy group given imipenem/cilastatin sodium plus amikacin sulfate (AMK). We then compared the clinical efficacy and safety between groups. 1. Safety was evaluated in 83 patients in the IPM/CS group and 88 in the IPM/CS + AMK group while clinical efficacy was evaluated in 77 and 80 patients in the respective groups. 2. The overall efficacy rate was 84.4% in the IPM/CS group. Among the main infections, the efficacy rates were 82.7% in 52 cases of pneumonia (including lung abscess), 100% in cases of infected bronchiectasis, 66.7% in six cases of secondary infection of chronic respiratory disease, and 100% in four cases of chronic bronchitis. The overall efficacy rate was 83.8% in the IPM/CS + AMK group. Among the main infections, the efficacy rates were 88.1% in 59 cases of pneumonia (including lung abscess), 83.3% in 12 cases of infected bronchiectasis, and 60.0% in five cases of secondary infection of chronic respiratory disease. No significant differences in efficacies were seen between groups. 3. In the IPM/CS group, the efficacy rates were 92.3% for patients without prior antibiotic therapy in the IPM/CS group and 68.0% for those with prior therapy; in the IPM/CS + AMK group, the respective rates were 83.7% and 83.9%. In the IPM/CS group, there was a significant difference in the responses of patients with and without prior antibiotic therapy (P < 0.05). 4. Side effects were observed in six patients in the IPM/CS group (7.2%) and two patients in the IPM/CS + AMK group (2.3%). Abnormal laboratory test results were noted in 5 patients in the IPM/CS group (6.0%) and in 10 in the IPM/CS + f1p4group (11.4%). There was no significant difference in the incidence of side effects between groups and no severe adverse reactions in either group. These results indicate that IPM/CS alone produces of good response in moderate to severe respiratory infections while IPM/CS combined with AMK is useful in intractable respiratory infections.
