[The optional usage of blood cardioplegia through eleven-year experience of open-heart surgery].

We propose two clinical studies in this paper for the optimal usage of cold blood cardioplegia (CBC) to evaluate 1) the effects of increasing the volume of CBC, and 2) the optimal use of verapamil, on postischemic myocardial recovery. First, patients with valvular heart surgery were divided into three groups according to the initial dose of CBC: Group I, 5 ml/kg in 368 patients; Group II, 10 ml/kg in 41; and Group III, 12 ml/kg in 86. In all groups CBC was infused every 25 to 30 minutes. Although the preoperative conditions of Group III patients tended to be more serious compared to Groups I and II, there was no difference in cardiac-related hospital mortality among the three groups. Second, 45 patients who had undergone valvular or coronary heart surgery were divided into three groups according to the usage of verapamil: Group A, infusion of verapamil of 1 mg/L into CBC; Group B, infusion of verapamil of 0.2 mg/kg into a perfusate; and Group C, the same method as Group B with additional administration of verapamil of 0.1 mg/kg into a perfusate at 60 minutes after the commencement of cardiopulmonary bypass and a continuous drop of 0.5 micrograms/kg/min for the succeeding 24 hours. Each group consisted of nine patients who had valvular surgery and six patients who had aorto-coronary bypass surgery. There was no difference in the severity of the patients' conditions between the groups. The optimal concentration during and after the operation and good clinical results were obtained in Group C. We conclude that the initial optimal dose of CBC is 12 ml/kg, and that the combination of an infusion of verapamil into a perfusate and a succeeding continuous drop is the appropriate method.

[Membranous obliteration of the inferior vena cava in the hepatic portion--postmortem study of a rapidly deteriorated aged case].

A 67-year-old female of Budd-Chiari syndrome was rapidly deteriorated and died. Autopsy revealed membranous obliteration of the inferior vena cava in the hepatic portion with a small hole. The inferior vena cava and right hepatic vein contained fresh and organized thrombi. The left hepatic vein was patent and opened above the membrane. The onset and the clinical course of this case seem to be atypical, probably because of the patent left hepatic vein and a small hole of the membrane. We call attention such a rare case in managing Budd-Chiari syndrome.

[Intravascular hemolysis after prosthetic valve replacement].

Two kinds of mechanical valve, St. Jude Medical (SJM) and Björk-Shiley (B-S), in patients with single valve replacement have been evaluated on a view point of intravascular hemolysis. Cases were consisted of 78 SJM and 32 B-S for the mitral and 12 SJM and 36 B-S for the aortic positions. Serum LDH, indirect bilirubin, GOT, hemoglobin levels and reticulocyte count were pursued in a given schedule, especially on the 21st postoperative day. A follow-up study on cases with abnormally high level of LDH (over 800 W.U.) at the time of hospital discharge was also performed. Paravalvular leakage detectable on the echocardiographical study was denied in all cases. The case with SJM valve replacement on the mitral position revealed a higher level of LDH, GOT and a large count of reticulocyte than those in the B-S valve. Insignificantly was found, however, in the same study with replaced valve on the aortic position. There was no correlation between the severity of hemolysis and the size of prostheses. The level of LDH, remained in high on discharge declined gradually thereafter during postoperative observation. As a whole, the result of this study supports the conventional valve selection and usage in our clinic. The patient with subclinical hemolysis after valve replacement should be placed on a schedule of close observation.

[Surgical treatment of descending thoracic aortic aneurysm under temporary bypass with antithrombogenic tube and bio-medicus centrifugal pump].

Four patients with descending thoracic aortic aneurysm were successfully operated on under temporary bypass with an antithrombogenic tube and a Bio-Medicus centrifugal pump. The bypass flow ranged from 1.0 to 2.4 l/min with the mean femoral artery pressure of 50 to 70 mmHg. No complications such as paraplegia, hepatic dysfunction or renal failure were encountered in all the patients. Temporary increment of the serum amylase level occurred in all the patients, but any apparent clinical symptoms were not present. The temporary bypass method with an antithrombogenic tube and a centrifugal pump is useful and reliable for surgical treatment of descending thoracic aortic aneurysm.

[Aorto-caval fistula due to a ruptured abdominal aortic aneurysm--an emergency operation following echoic diagnosis].

A successful emergency operation for a 75-year-old man with aorto-caval fistula secondary to rupture of the abdominal aortic aneurysm is reported. A definite diagnosis of aorto-caval fistula was made by echography with characteristic engorgement of the caval vein. Clinical signs and symptoms characterized by lung edema, sudden onset, and circulatory collapse were also noticeable. In the operation, the fistula was closed via inside of the aneurysm with several mattress sutures following control of aortic flow and opening of the aneurysm. Back-flow of the blood through the fistula was readily controlled by finger tip. The aneurysm was replaced conventionally by a vascular prosthesis. There might be several suitable surgical selection available properly to individual case. No delay in surgical treatment depending on definite diagnosis by echography in such urgent condition should be stressed.

[Peripheral neuroepithelioma in the retroperitoneum; a case report and a review of the literature].

A 30-year-old man with peripheral neuroepithelioma which was present in the retroperitoneum underwent surgical treatment under the diagnosis of germ cell tumor. Subsequently he died of local recurrence and multiple metastases to the liver seven months after surgery. To our knowledge, this is the second case of peripheral neuroepithelioma which was observed in the retroperitoneal space in the literature of the world. We also reviewed 20 cases of peripheral neuroepithelioma reported in Japan.

[A comparative study of experimental models for cardiac preservation through orthotopic transplantation].

Various kinds of models for canine heart preservation were evaluated through orthotopic transplantation. Following warm ischemic (WIT) and preservation times (PT), donor hearts were orthotopically transplanted to recipients and observed for 2 hours. The study was divided into four groups. A heart-lung preparation (Group A) was preserved by coronary perfusion with diluted blood for an hour following 15-minute WIT in A-1 (n = 33), and 2 hours following 30-minute WIT in A-2 (n = 19). Following 30-minute WIT, the heart (Group B) was preserved for an hour by coronary perfusion with diluted blood in B-1 (n = 20), a modified Krebs solution in B-2 (n = 14), and a EL-solution in B-2 (n = 14). Following 30-minute WIT, the heart (Group C) was preserved for 2 hours by a Langendorff's model with coronary perfusion using perfluorochemical in C-1 (n = 5) and Hydroxyethyl starch in C-2 (n = 1). In Group D, electromechanical arrest of the heart and coronary vascular washout were performed in D-1 (n = 8) with K+ cardioplegia, D-2 (n = 8) with K+-verapamil cardioplegia, and D-3 (n = 5) with Collins M-verapamil cardioplegia. The heart was then removed and suspended in the same solution. The graft was preserved at 4 degrees C for 6 hours in D-1 and D-2, and 24 hours in D-3. Fourty-eight % in A-1 and 26% in A-2 were successfully transplanted.(ABSTRACT TRUNCATED AT 250 WORDS)

[Detection of a shunt flow through a defect in secundum atrial septal defect by right parasternal approach using pulsed Doppler echocardiography].

Pulsed Doppler echocardiography combined with two-dimensional echocardiographic (2-DE) system was performed to detect a shunt flow through a defect in the interatrial septum (IAS) in patients with secundum atrial septal defect (ASD) utilizing right parasternal approach (RPA). RPA is a method which provides an accurate evaluation of a defect in the IAS on a 2-DE image by placing a transducer on the right of the sternum because the ultrasonic beam passes in a plane almost perpendicular to the IAS. The subjects consisted of 20 patients with ASD (25 +/- 22 yrs) diagnosed by cardiac catheterization or 2-DE with peripheral contrast material injection, and 10 cases of healthy subjects (34 +/- 18 yrs). Defects in the IAS were clearly visible in 19 patients with ASD on 2-DE images by RPA. By placing the sample volume in the center of the defect, Doppler flows could be obtained in 17 of them. In 13 with sinus rhythm, except a case of Eisenmenger syndrome, Doppler signals showed mainly a laminar flow toward the transducer (positive flow), but a transient flow of low velocity away from the transducer (negative flow) was also observed. The positive flow had its peaks in late systole and atrial systole and occasionally in mid-systole, early diastole and mid-diastole. The negative flow occurred in early systole, mid-diastole and late diastole. In a patient with Eisenmenger syndrome and tricuspid regurgitation (TR), a negative turbulent flow was observed from early systole to mid-diastole. In three patients with atrial fibrillation, Doppler signals were variable. The Doppler echogram of a patient with atrial fibrillation but no complication showed a laminar positive flow with its peaks in late systole and mid-diastole, and a negative flow in early systole. One patient with atrial fibrillation and TR had a systolic negative turbulent flow and a mid-diastolic laminar positive flow. In a patient with TR and mitral regurgitation, the Doppler echocardiogram showed a laminar positive flow throughout the entire cardiac cycle with its peaks in early diastole and mid-diastole. Doppler signals from the left atrium (LA) to the right atrium (RA) through a defect coincided in timing with the appearance of negative contrast echo from the defect to the RA on 2-DE image and signals from the RA to the LA coincided with the appearance of the contrast echo into the LA from the RA through the defect. Doppler signals disappeared after the closure of the defect in all six patients examined.(ABSTRACT TRUNCATED AT 400 WORDS)

Surgical treatment of abdominal aortic aneurysm in patients over seventy years old.

Aneurysmectomy is generally the first choice as treatment in most patients with abdominal aortic aneurysm. However, justification of surgical procedures in the aged has been questioned, because of the risk which increases with chronologic age. In the present study, our experience with patients over 70 who underwent abdominal aortic aneurysmectomy is discussed. Forty patients with abdominal aortic aneurysm underwent surgery in our institution up to January, 1983. Ten of these patients were 70 years or older. Operation was performed electively in 6 and as an emergency in 4. The one operative death (10 per cent) was the case of a 74-year old woman with a ruptured aneurysm. The operative mortality was 17.5 per cent in patients under 70 years of age. It would appear that advanced age, per se, is not a contraindication for abdominal aneurysmectomy.

[Myocardial protection with cold blood potassium cardioplegia: experimental and clinical studies].

One hundred and eighty-one patients (101 men and 80 women) underwent 61 congenital and 120 acquired heart operations using cold blood potassium cardioplegia as the method of myocardial protection at Kagoshima University from August, 1978 to August, 1981. Hospital deaths occurred in 18 patients (9.8%), and 4 cases were late death (2.2%). Multivariate analysis revealed no significant relationship between type of heart disease and operative mortality or the occurrence of post-operative low output syndrome. To evaluate the superiority of cold blood cardioplegia for myocardial protection, we carried out 20 orthotopic heart transplants in dog; 16 transplanted grafts could maintain the circulation of recipient without cardiopulmonary bypass. Cardiac contractility following 90-minute ischemic time showed 97 to 100% of the control values without any inotropic drugs. These clinical and experimental findings suggest that cold blood potassium cardioplegia provides excellent myocardial protection during the ischemia of the heart.
