Inhibition of hepatic cholesterol biosynthesis by a 3-hydroxy-3-methylglutaryl coenzyme A synthase inhibitor, 1233A, in mice.

We have studied the in vivo inhibition of hepatic sterol biosynthesis by 1233A, a specific inhibitor of the enzyme 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) synthase. Administration of the compound orally to mice resulted in a dose-dependent inhibition of [14C]acetate incorporation into sterols in liver, but did not exert any significant effect on [14C]mevalonate incorporation. The results indicate that the in vivo inhibition of sterol synthesis occurs only at pre-mevalonate enzymatic steps in the sterol biosynthetic pathway, thus being compatible with the presumed site of inhibition, HMG-CoA synthase. Moreover, owing to irreversible inactivation of the enzyme by 1233A, it was possible to detect in vivo effect on the enzyme by assays of its activity in cell-free extracts from livers; the drug-treatment also resulted in a similarly dose-dependent inhibition of HMG-CoA synthase activity. In contrast, acetoacetyl-CoA thiolase and HMG-CoA reductase, the enzymes also responsible for mevalonate synthesis in the pathway, did not show any significant change in activity. These results clearly demonstrate that the inhibition of hepatic sterol synthesis caused by 1233A is indeed due to selective inhibition of HMG-CoA synthase in the tissues.

Coronary thrombolysis and endothelin-1 release.

The relation between intracoronary thrombus and endothelin-1 (ET-1) was studied. In a canine model, acute myocardial infarction (MI) was induced by coronary occlusive thrombus produced at a mock atheromatous plaque. Blood samples were collected from the aorta (A) and coronary vein (V). Twenty-eight open-chest dogs divided into three groups were studied. Group I (n = 15): acute MI was induced by coronary occlusive thrombus, and thrombolysis was obtained by urokinase two hours after MI. Group II (n = 8): nonocclusive thrombus was produced without inducing MI. Group III (n = 5): coronary artery was ligated for two hours and reperfused by release of ligation. In Group I, ET-1 was significantly increased after MI in A and V, and ET-1 in V was significantly more elevated than in A during thrombolysis, suggesting ET-1 production in the coronary vessels by thrombolysis. In Group II, ET-1 increased slightly during thrombus formation, but there was no difference in A and V. In Group III, ET-1 was elevated significantly after MI without A and V difference. These results indicate that there is no detectable ET-1 production with coronary thrombus formation, whereas coronary ET-1 production is detected during thrombolysis, most probably because resolved thrombus releases a more potent stimulus to ET-1 production.

Disseminated herpes zoster with multifocal neurologic involvement in an HTLV-1 carrier.

A 56-year-old previously healthy man suffered from an attack of disseminated herpes zoster, followed by multifocal neurologic involvement. The patient was a human T-cell lymphotropic virus type 1 (HTLV-1) carrier. HTLV-1 infection, even though normally asymptomatic, has been suggested to be one of the risk factors for severe herpes virus infection and its associated neurologic disorders.

Diolmycins, new anticoccidial agents produced by Streptomyces sp. II. Structure elucidation of diolmycins A1, A2, B1 and B2, and synthesis of diolmycin A1.

The structures of diolmycins A1, A2, B1 and B2, novel anticoccidial agents, were determined by spectroscopic analyses. Diolmycins A1 and A2 are stereoisomers with the structure of 1-(3-indolyl)-4-(p-hydroxyphenyl)-2,3-butanediol. From the chemical synthesis of the erythro-isomer, the relative configurations of diolmycins A1 and A2 were determined to be the erythro- and threo-isomers, respectively. The stereoisomers, diolmycins B1 and B2, were also deduced to be erythro- and threo-1,4-di-(p-hydroxyphenyl)-2,3-butanediol, respectively.

Effects of pravastatin on lipid transfer protein and lecithin cholesterol acyltransferase in heterozygous familial hypercholesterolemia.

The effects of HMG CoA reductase inhibitor PRAVASTATIN on plasma lipoprotein levels, lipid transfer protein (LTP) activity and lecithin cholesterol acyltransferase (LCAT) activity were investigated in 20 patients with heterozygous familial hypercholesterolemia. Plasma total cholesterol decreased from 299.2 +/- 45.2 to 245.9 +/- 45.1 (mean +/- SD in mg/dl) (p < 0.0001), low density lipoprotein (LDL) cholesterol from 221.1 +/- 47.6 to 164.9 +/- 48.1 (p < 0.0001) and LDL-triglycerides from 38.0 +/- 10.2 to 30.2 +/- 8.8 (p < 0.0001). High density lipoprotein (HDL) cholesterol increased from 60.1 +/- 13.9 to 63.1 +/- 17.0 (p = 0.0264). The HDL-cholesterol: HDL-triglycerides ratio increased from 3.91 +/- 1.14 to 4.91 +/- 1.65 (p = 0.0029). Lipid transfer protein (LTP) activity (as % of transferred cholesteryl ester (CE)) decreased from 4.68 +/- 2.47 to 3.70 +/- 2.31 (p = 0.0024) and LCAT activity decreased from 113.1 +/- 29.9 (U/L) to 97.0 +/- 22.7. There was a negative correlation between LTP/LCAT ratio and HDL-C levels. These results suggested that the changes in plasma HDL-cholesterol concentrations during treatment with pravastatin may be related to the decreases in LTP and LCAT activity due to this drug.

Lipid metabolism inhibitors of microbial origin.

Thiotetromycin is isolated from Streptomyces sp. OM-674 discovered in the course of the screening for antibacterial antibiotics. The antibiotic was found to be a specific inhibitor of type II fatty acid synthase. In order to search acyl-CoA synthetase inhibitors, a kind of yeast, Candida lipolytica, was utilized as test organism for the primary screening. Four structurally related compounds named triacsin were isolated from Streptomyces sp. SK-1894 as a specific inhibitor of acyl-CoA synthetase I of C. lipolytica. Further biochemical studies revealed that triacsins inhibit acyl-CoA synthetases from widely different sources. The inhibition is competitive with respect to long chain fatty acids. The common N-hydroxytriazene moiety of triacsins is essential for inhibition. To discover inhibitors of mevalonate biosynthesis, Vero cells were used as test organism for the screening. A beta-lactone 1233A(F-244) isolated from Scopulariopsis sp. F-244 was demonstrated to inhibit mevalonate biosynthesis with assays using cell and enzyme systems. Further studies demonstrated that the compound inhibits 3-hydroxy-3-methylglutaryl (HMG)-CoA synthase specifically and irreversibly. The geometry of the (2R, 3R)-beta-lactone ring in the structure is essential for specific inhibition against the enzyme.

[An adult patient with Krabbe's disease--the first case reported in Japan].

A 31-year-old woman, a child of consanguineous parents, who had shown mental retardation and slowness in running ability since childhood, complained of progressive spastic gait for one and a half years prior to admission. On admission, neurological examination revealed severe spastic paraplegia, mental retardation, visual disturbance, cerebellar ataxia, disturbance of deep sensation, sphincter disturbance, pes cavus and pes equinovarus. Protein content in the cerebrospinal fluid was increased to 86.1 mg/dl. Markedly reduced activity of galactosylceramidase in the leukocytes and the cultured fibroblasts confirmed the diagnosis of Krabbe's disease. T2-weighted MR images disclosed high signal areas in the deep white matter of the bilateral parieto-occipital lobes. There were also abnormal high signal bands along the bilateral pyramidal tracts from the corona radiata to the cerebral peduncle and the posterior part of the corpus callosum. The nerve conduction studies showed markedly decreased motor and sensory conduction velocities with decreased amplitudes. Visual evoked potentials showed prolongation of the right P100 latency. Auditory brainstem responses showed prolonged interpeak latencies between I-V peaks. Somatosensory evoked potentials showed prolongation of the N20 latencies. The morphometric histopathological analysis of the sural nerve revealed a decrease in the density of myelinated fibers and the presence of myelinated fibers with thin myelin sheath relative to the size of transverse area of axon. There were linear inclusion bodies in the cytoplasm of Schwann cells on electron microscopic examination. This is the first report in Japan of a patient with Krabbe's disease who survived after adolescence.

[Pharmacodynamic study on the effectiveness of UFT against cancer of gastro-intestinal (GI) tract].

Preoperative oral administration of either 5-FU 200 mg/day (15 patients) or UFT (tegafur plus uracil at a 1:4 molar ratio) 400 mg/day (22 patients) were carried out was carried out against the patients cancer of GI-tract 3 consecutive days till the morning of the operation. Serial blood samples of peripheral and portal veins were obtained for the measurement of concentration of 5-FU. The Concentration of 5-FU in normal and tumor tissues was measured in 13 patients given UFT pre-operatively. Thymidylate synthase inhibition rate were also measured in 5 of them. Furthermore, AgNOR count of tumor tissue before and after administration of UFT were examined in 11 patients. Following conclusions were obtained. (1) The Concentration of 5-FU in peripheral venous blood of UFT group was significantly higher than that of 5-FU group during 1-5 hrs after the last dose (p < 0.05). (2) The concentration of 5-FU in portal venous blood of 5-FU group was higher than that of UFT group during initial 2 hrs after the last dose. However, during 3 to 7 hrs after the last dose, UFT group showed a higher concentration. (3) In UFT group, the concentration of 5-FU in tumor tissue was significantly higher than that in normal tissue (p < 0.01). Consequently, preoperative administration of UFT can be effective for prevention of intraoperative prevention of hematogenous metastasis via portal system in cancer of gastrointestinal tract.

Enhanced production of platelet-activating factor in stimulated rat leukocytes pretreated with triacsin C, A novel acyl-coA synthetase inhibitor.

Triacsin C, a product of Streptmyces sp. SK-1894, was previously reported as an inhibitor of long chain acyl-CoA synthetase. Pretreatment with triacsin C (500 nM) for 1h enhanced production of platelet-activating factor in rat neutrophils, followed by stimulation with A23187 or fMLP. Amount of lyso-PAF was also augumented. Triacsin C alone did not increase PAF content and did not modulate enzymatic activities of acytransferase, cholinephosphotransferase, acetylhydrolase, acetyltransferase or phospholipase A2. These results suggest that triacsin C might enhance supply of substrate for PAF synthesis, i.e. accumulation of lyso-PAF by interfering reacylation pathway.

Partial hepatic resection under intermittent hepatic inflow occlusion in patients with chronic liver disease.

A partial hepatic resection was performed in 13 patients with chronic liver disease using intermittent hepatic inflow occlusion. Eleven patients had liver cirrhosis and two had chronic hepatitis. Seven patients were classified as Child's grade A and six as Child's grade B before operation. Dissection of the hepatic parenchyma was performed during intermittent inflow occlusion. The time of clamping and declamping was 10-20 min and 5-8 min, respectively. Postoperative data on liver function showed recovery to preoperative levels by about 10 days after operation. There were no life-threatening complications. These results indicate that intermittent hepatic inflow occlusion can be achieved easily and safely to allow non-anatomical resection in patients with chronic liver disease.
