Cortisol and beta-endorphin response in alcoholics and alcohol abusers following a high naloxone dosage.

The course of plasma cortisol and beta-endorphin-like immunoreactivity (beta-EP-IR) was determined following a single i.v. administration of 20 mg naloxone. The test subjects included 20 male alcoholics (medication-free), investigated one to three days and four weeks after the onset of abstinence, as well as 10 short-time abstinent alcohol abusers and 10 healthy control subjects. The mean baseline values of cortisol and beta-EP-IR remained within normal limits in all groups. The significant decrease in the plasma cortisol baseline values in the alcoholics after 4 weeks abstinence may indicate a lower level of the regulation of the hypothalamic-pituitary-adrenal axis (HPA) under conditions of abstinence. After naloxone administration an increase in plasma cortisol and beta-EP-IR was observed in all groups. The multivariate trend analysis showed significant differences in the time course of plasma cortisol between the three groups, however not in the course of beta-EP-IR. The changes in the dynamic regulation of the HPA axis, resulting from chronic alcohol consumption, appears to be irrespective of whether the drinking pattern is dependent or abusive. In alcoholics these changes could still be identified following a 4-week abstinence period.

[Fibrin glue injection--a hemostatic technic after laparoscopic liver biopsy].

Fibrin glue was injected into or near the site of needle biopsy puncture in 33 patients (38 laparoscopically guided liver biopsies), 21 with histologically confirmed liver cirrhosis, to arrest post-puncture bleeding. A special double-lumen needle was introduced, suitable for multiple injections. Fibrin-glue injection was successful in 37 instances, but in one case it failed because the injection needle could not be optimally placed. The method thus seems to be a reliable and sensible means of obtaining haemostasis if compression and electrocoagulation have failed.

Cellular aspects of retention and transport of inhaled soluble and insoluble actinide compounds in the rat lung.

The binding of 241Am-hydroxide polymers (as models for readily soluble actinide compounds) to the cell components of rat lung was investigated using differential centrifugation, density gradient centrifugation, gel chromatography, carrier-free electrophoresis and electron microscopic autoradiography (with 241Pu). Irregularly shaped and spherical mixed (U, Pu)O2 particles (as models for insoluble actinide compounds) were administered to rats by inhalation and intratracheal installation and the lung and organ retention was determined. Electron microscopic studies were performed with rat lung and with rat and bovine alveolar macrophages exposed to the actinide compounds in vitro. In the case of the mixed (U, Pu)O2 particles the lung retention was independent of the particle shape and route of administration. Whereas 241Am administered as a hydroxide polymer was transferred rapidly from lung to skeleton and liver, only a few percent of the initial alveolar deposit was found in these organs after mixed oxide inhalation. It is concluded that all types of particles are stored primarily within phagolysosomes of alveolar macrophages. In the case of readily soluble compounds, the actinides are solubilized within these lysosomes and become bound to cytosolic ferritin in the alveolar macrophages. They are then released from the macrophages and probably cross the alveolar membranes as transferrin or as low-molecular-weight forms. Insoluble compounds remain within the lysosomes of alveolar macrophages, but there are indications of chemical damage to the lysosomal membranes, causing the particles to lie free in the cytoplasm.

Behaviour of spherical and irregular (U,Pu)O2 particles after inhalation or intratracheal instillation in rat lung and during in vitro culture with bovine alveolar macrophages.

Spherical and irregularly shaped mixed (U,Pu) oxide particles were administered to rats by inhalation and by intratracheal instillation. The lung retention of the particles was independent of particle shape and of the route of administration. Only a small percentage of the administered radioactivity was found in other organs. Detailed electron microscopic studies showed particles within membrane-enclosed vacuoles as well as lying free in the cytoplasm.

Incorporation of N-acetyl-[14C]D-glucosamine and [3H]L-leucine by isolated pig gastric mucosal cells.

Glycoprotein and protein production of isolated pig gastric mucosal cells were determined by the incorporation of N-acetyl-[14C]D-glucosamine ([14C]GlcNAc) and [3H]L-leucine ([3H]Leu) into acid-insoluble macromolecules (AIM). In four cell fractions (F1-F4), obtained by counterflow centrifugation, specific [14C]GlcNAc incorporation was greatest in the mucous cell-enriched F2. Tracer incorporation by F2 cells, proceeded linearly up to 20 h, was inhibited by cycloheximide or incubation at 0 degree C, and enhanced by PGE2 1 mumol/l. Gel chromatography of released AIM revealed that PGE2-stimulated [14C]GlcNAc incorporation was predominantly directed into high molecular weight (2 X 10(6) daltons) glycoproteins, whereas [3H]Leu incorporation was mainly related to proteins of albumin-like molecular weight. We conclude that incorporation of [14C]GlcNAc by enriched pig gastric mucous cells (F2), further analyzed by gel chromatography, is a suitable probe to study the production of high molecular weight gastric mucous glycoproteins in vitro.

[Initial results of genetic carrier diagnosis in risk pedigrees with hemophilia A and B in East Germany].

In 7 families at risk for hemophilia A 42 individuals were evaluated by the Taq I polymorphism of the extragenic probe St 14.1 and the Bcl I as well the Hind III polymorphism of the intragenic probe F8e16-19. 15 out of 20 females of the core families were identified as carriers, under them all of the obligate heterozygotes. 5 individuals were excluded as carriers. The heterozygosity for each of the RFLPs was found to be between 30% and 79%. Combining the single data in 96% heterozygosity was found under the individuals tested. A linkage disequilibrium was found between the Bcl I and Hind III polymorphismus of the probe F8e16-19. A family at risk for hemophilia B including 5 individuals was studied using the Taq I and Xmn I polymorphisms of the probe P1. In one of two females the carrier state could be excluded in the other one confirmed.

[Injuries of the large vessels caused by laparoscopy].

Laparoscopy is used in surgery, internal medicine and gynecology for diagnostic as well as for therapeutic purpose. Frequency of complications is minimal. By means of 4 cases it will be demonstrated that severe traumata of the retroperitoneal vessels are possible using this method. We could prove by anatomic studies that the possibility of vascular injury should not be underestimated because of the mostly bifurcation. Diagnostic and therapeutic consequences have been discussed.

[Massive hemorrhage in Crohn disease--a rare complication].

Acute massive intestinal bleeding is a rare complication in Crohn's disease. Its occurrence has been quoted between one and 1.4 per cent in the literature, and it has been about two per cent among the authors' own patients. Reported in this paper are three of these cases with acute bleeding as indicator of the disease. In one of them the presence of Crohn's disease had been known for some time. In the second bleeding occurred as a complication to toxic megacolon. In the third patient bleeding was the first manifestation of Crohn's disease.

[Ileocecal Crohn disease with sigmoid involvement].

This report concerns 14 patients with Morbus Crohn and inflammatory conglomerat tumors between ileo-coecum and sigma. In the case of primary affliction of the ileo-coecum and secondary involvement of the sigma (group A), it is generally sufficient to perform an ileo-coecal resection and suture over the sigma, if a fistula is present. In case of Morbus Crohn afflicting primarily the sigma (group B) or simultaneous involvement of ileo-coecum and sigma (group C), a double resection is necessary. It was always possible to demonstrate the presence of an ileo-sigmoidal fistula by means of barium enema and mostly possible with barium meal or enteroclysis. We consider preoperative colonoscopy with graduated biopsies to be absolutely necessary to determine the exact stage of the illness. Fistulas can be more accurately demonstrated by radiogram than endoscopically.

[Age-dependent accumulation of lead in teeth].

Teeth from people aged 5-73 years old, who differ in their occupational type of exposure to lead and who live in different areas of Dortmund, were collected in 1980/81 and were investigated by means of flameless atomic absorption spectrometry. The major result obtained from the 163 permanent and 22 deciduous teeth studied was a statistically significant dependence of the lead content of the permanent teeth on the donor's age. This was superior to all other influences, showing an increase of 3.4% per year of life. There were no sex specific differences, and no correlations with height and body weight were found. To find different lead concentrations within samples of teeth of various living areas and various occupational types of exposure would have required more variation in these variables than was available in the population of our study. It must be emphasized that the reliability of the measured values of dental-lead concentration greatly depends upon the methodical and analytical procedures used, especially because of the overall lead contamination. The analytical method we finally used is described. The factors which influence the dental lead concentration are discussed. The investigation of whole teeth is tolerable. The age-dependent values of the lead content of teeth could be a useful indicator of the overall lead uptake especially in continuous low level lead exposure. Under the conditions described, at least 150 permanent teeth are needed to compare samples of different regions or periods.

Substrate specificity of two bacteriophage-associated endo-N-acetylneuraminidases.

For Escherichia coli Bos12 (O16:K92:H-), a bacteriophage (phi 92) has been isolated which carries a depolymerase active on the K92 capsular polysaccharide. As seen under the electron microscope, phi 92 belongs to Bradley's morphology group A and is different from the phage phi 1.2 previously described (Kwiatkowski et al., J. Virol. 43:697-704, 1982), which grows on E. coli K235 (O1:K1:H-), depolymerizes colominic acid, and belongs to morphology group C. The specificity of the phi 1.2- and phi 92-associated endo-N-acetylneuraminidases has been studied with respect to the following substrates (all alkali treated, and where NeuNAc represents N-acetylneuraminic acid): (i) [-alpha-NeuNAc-(2 leads to 8)-]n (colominic acid), (ii) [-alpha-NeuNAc-(2 leads to 8)-alpha-NeuNAc-(2 leads to 9)-]n (E. coli K92 polysaccharide), and (iii) [-alpha-NeuNAc-(2 leads to 9)-]n (Neisseria meningitidis type C capsular polysaccharide). The increase in periodate consumption of these glycans upon incubation with purified phi 1.2 or phi 92 particles was measured, and the split products obtained from all substrates after exhaustive degradation were analyzed by gel chromatography. It was found that the Neisseria polysaccharide is not appreciably affected by either virus enzyme and that phi 1.2 only depolymerizes a small fraction of the K92 glycan. Colominic acid, however, is completely degraded by both agents, phi 92 yielding smaller fragments (one to six NeuNAc residues) than phi 1.2 (two to seven). Phage phi 92 additionally depolymerizes the K92 glycan, essentially to oligosaccharides of two, four, and six residues. The size distribution of these K92 oligosaccharides indicates that the phi 92 enzyme predominantly cleaves the alpha(2 leads to 8) linkages in this polymer.

[Extra-anatomic bypass in injuries of the subclavian artery].

11 cases of subclavian vessel injuries are reported. The problems of operative approach are discussed. Two out of 11 cases have been operated on by extraanatomical bypass from the common carotid artery to the subclavian or axillary artery. There resulted no problems by temporarily clamping the common carotid artery, no postoperative carotid steal syndrome was seen. In our opinion carotid-subclavian bypass - well known as standard procedure in chronical disease of the proximal subclavian artery - can also be applied in case of acute trauma of the subclavian artery. Thus the direct riskfull approach for these often polytraumatized patients can be avoided.

[Surgical therapy of enterovaginal fistulae following gynecologic primary procedures (author's transl)].

This is a report on 14 enterovaginal fistulae following gynecologic operations. The fistulae were caused by a hysterectomy in 8 cases. There were 3 other enterovaginal fistulae following exploratory laparotomy for ovarian tumors, which proved to be pseudotumors caused by diverticulitis of the sigmoid colon. Rare causes for fistulae were cesarian section, episiotomy, and incision of an abscess of Bartholins' gland. The time interval between operation and manifestation of the fistula was usually about 10 days. The operative management required a two-or three-step procedure. Only 2 of 14 patients were treated by a colostomy, and a bowel resection was performed in 4 cases. After dissection of the fistula either peritoneum or omentum was interposed in 8 patients. No recurrences were observed.
