Successful extended hypothermic cardiopulmonary preservation for heart-lung transplantation.

The inability to obtain sufficiently extended hypothermic organ preservation is a major restriction on clinical heart-lung transplantation. We used core cooling, nonrecirculating retrograde heart perfusion, and lung immersion with liposomal recombinant human superoxide dismutase in an attempt to provide effective 12-hour cardiopulmonary preservation. Donor dogs supported by cardiopulmonary bypass were rapidly cooled to 15 degrees C with cardioplegic arrest, and heterotopic heart and unilateral left lung transplantations were performed. In control dogs (n = 7), hearts and lungs, harvested after core cooling and cardioplegic arrest, were transplanted with a total mean ischemic time of 88 +/- 5 minutes. In group II (n = 7), heart-lung blocks were similarly excised but preserved at 4 degrees C for 12 hours (756 +/- 30 minutes) and then transplanted. During preservation, the lungs were immersed in hyperosmolar extracellular solution. For the heart, retrograde coronary sinus perfusion was performed with intracellular solution containing perfluorochemicals at a temperature of 4 degrees C and a rate of 30 ml/hr for 12 hours. In group III (n = 7), donor organs were similarly excised and preserved for 12 hours (726 +/- 39 minutes), except that liposomal recombinant human superoxide dismutase was administered during harvest, preservation, and reperfusion. Myocardial function, assessed by the ratio of end-systolic pressure to end-systolic dimension, after the 12-hour preservation period in both experimental groups was similar to that of the control group 4 and 6 hours after transplantation. The mean arterial oxygen capacity of the transplanted left lung during ventilation with an inspired oxygen concentration of 40% was also similar in each group. In contrast, the 12-hour preservation of pulmonary function assessed by pulmonary vascular resistance, the accumulation of extravascular lung water, and histologic evidence of alveolar wall injury, interstitial edema, and perivascular hemorrhage were significantly impaired in the absence of liposal recombinant human superoxide dismutase. These findings suggest that successful extended cardiopulmonary preservation for heart-lung transplantation is possible with core cooling, nonrecirculating retrograde heart perfusion, and hypothermic lung immersion incorporating liposomal recombinant human superoxide dismutase.

[High energy phosphates and mitochondrial respiratory function after 24 hours cardiac preservation--comparison between simple hypothermic immersion and continuous hypothermic perfusion].

Cardiac preservation is one of the most important step during heart transplantation. Prolonged preservation period is necessary to get the greater donor pool. To get safe prolonged preservation two methods, simple hypothermic immersion method (group A) and continuous hypothermic perfusion method (group B), were compared in this report by evaluating the myocardiac high energy phosphate level and mitochondrial respiratory function. The values of ATP, TAN and Energy charge were significantly higher in group B than group A (p less than 0.001). Mitochondrial respiratory function such as state 3 QO2, state 4 QO2, RCI and ADP/O also showed significantly higher values in group B than group A (p less than 0.001). These date indicate that continuous hypothermic perfusion method provides better preservation than simple hypothermic immersion method for 24 hours cardiac preservation.

The nuclear matrix from rat liver is capable of phosphorylating exogenous tyrosine-containing substrates.

The nuclear matrix isolated from rat liver phosphorylated exogenous tyrosine-containing substrates angiotensin II and synthetic polymer (Glu, Tyr; 4:1). The phosphorylation reaction was dependent on Mn2+ or Mg2+, but the former was the preferred ion. Km values for poly(Glu,Tyr; 4:1) and ATP were 0.2 mM and 4 microM, respectively. Angiotensin II showed a lower affinity for the kinase than poly(Glu,Tyr; 4:1). The isoflavone genistein, a specific inhibitor for tyrosine phosphorylation, inhibited the tyrosine kinase activity in the nuclear matrix.

Complete purification and immunochemical analysis of S-adenosylmethionine synthetase from bovine brain.

We have purified S-adenosylmethionine (AdoMet) synthetase about 3000-fold from bovine brain extract. The Km values of the enzyme for L-methionine and ATP were 10 and 50 microM, respectively. An apparent molecular mass of the enzyme was estimated to be 160 kDa by gel filtration on a Sephacryl S-200 column. Sucrose density gradient centrifugation gave a sedimentation coefficient of 8 S. Polyacrylamide gel electrophoresis of the purified enzyme in native system revealed a single protein band, whereas two polypeptide bands with molecular masses of 48 kDa (p48) and 38 kDa (p38) were observed in sodium dodecyl sulfate-polyacrylamide gel electrophoresis of the purified enzyme. Antibody against bovine brain AdoMet synthetase was prepared by injecting the purified enzyme into a rabbit. Immunoblot analysis revealed that the antibody recognized both p48 and p38 in the impure enzyme preparations from bovine brain as well as in the purified enzyme. Specific antibodies against p48 and p38 were separated from the immunoglobulin fraction by an affinity purification, both of which inhibited the enzyme activity. These results indicate that AdoMet synthetase from bovine brain consists of two different polypeptides, p48 and p38.

Intracellular Ca2+ antagonist TMB-8 blocks catecholamine secretion evoked by caffeine and acetylcholine from perfused cat adrenal glands in the absence of extracellular Ca2+.

Unlike acetylcholine, caffeine was much more effective in releasing catecholamine in the absence of extracellular Ca2+ than in its presence in perfused cat adrenal glands. The intracellular Ca2+ antagonist, TMB-8 (10(-4) M), inhibited reversibly the catecholamine secretion evoked by caffeine (40 mM) and that induced by acetylcholine (10(-4) M) in the presence of hexamethonium (10(-3) M) during perfusion with Ca2+-free Locke solution containing EGTA (10(-5) M). These results support our view that muscarinic receptor activation causes catecholamine secretion by mobilizing Ca2+ from an intracellular pool just as caffeine does.

Insulin resistance by unprocessed insulin proreceptors point mutation at the cleavage site.

Failure to cleave the interconnecting site between alpha- and beta-subunit produced insulin proreceptors in the plasma membranes which had markedly low affinity to insulin, leading to extreme insulin resistance in a patient. We performed cDNA sequence analysis of the cleavage site of the insulin proreceptor from the patient. Polymerase chain reaction was used to obtain large amount of cDNA coding for the region including the interconnecting site. A thermostable DNA polymerase, Taq polymerase, successfully produced enough amount of cDNA of the region to be sequenced. The results showed AGG (Arg) to AGT (Ser) point mutation, resulting in the change of interconnecting sequence of the two subunits from -Arg-Lys-Arg-Arg- to -Arg-Lys-Arg-Ser-. These results suggest that the tertial structure change of the cleavage site leads to production of unprocessed insulin proreceptors.

Core-cooling, heart-perfusion, lung-immersion technique provides successful cardiopulmonary preservation for heart-lung transplantation.

Mongrel dogs underwent heterotopic heart-orthotopic left lung transplantation. In Group I (N = 6), donor organs procured following core cooling to 15 degrees C on cardiopulmonary bypass (CPB) with cardioplegic arrest were immediately transplanted. In Group II (N = 6), following cardioplegic arrest without CPB core-cooling, the pulmonary artery was flushed with modified Collins' solution. Heart-lung blocks were immersed in extracellular solution for 6 hours and then transplanted. In Groups III and IV (N = 6 each), following CPB core-cooling to 15 degrees C and cardioplegic arrest, the organ blocks were immersed in extracellular solution (Group III) and the heart was perfused with oxygenated extracellular solution (Group IV). Evaluation of lung function using differences in arterial oxygen tension between the left and right atria demonstrated no differences between groups. However, extravascular lung water and pulmonary vascular resistance were significantly elevated in Group II. Cardiac function assessed by the ratio of end-systolic pressure to end-systolic dimension was significantly better in Group IV than in Groups II and III. Thus, adequate 6-hour hypothermic cardiopulmonary preservation with core cooling plus heart perfusion can be achieved for heart-lung transplantation.

Catecholamines and dopamine-beta-hydroxylase secretion from perfused goat adrenal glands.

Splanchnic nerve stimulation caused a frequency-dependent increase in catecholamine (CA) secretion which attained a maximum at 10 Hz in perfused goat adrenal glands. Acetylcholine (ACh) and nicotine also caused a dose-dependent increase in CA secretion. The percentage of three catecholamines released (adrenaline, 51-56%; noradrenaline, 42-47%; dopamine, 1.3-2%) were not significantly different among three different stimuli. These values were almost the same as those found in the extract of adrenal gland and in purified chromaffin granules. The response to ACh was completely abolished by hexamethonium alone. Pilocarpine was ineffective in increasing CA secretion. Dopamine-beta-hydroxylase (DBH) activity increased with the increase in CA secretion in response to splanchnic nerve stimulation and ACh, though the declining phase of DBH was much slower than that of CA. The ratios of CA to DBH activity released for 25 min during and after splanchnic nerve stimulation and ACh infusion were 0.40 +/- 0.07 and 0.50 +/- 0.07, respectively. The ratio was 0.41 +/- 0.06 in chromaffin granules. The results indicate that only nicotinic receptors are involved in CA secretion and that DBH may be useful as an index of exocytosis in goat perfused adrenal glands.

[Long-term chemotherapy in patients with stage IV gastric cancer after surgical treatment--a randomized comparative study of FT-207 and UFT].

A prospective randomized and comparative study of FT-207 and UFT was performed for cases of Stage IV gastric cancer involving long-term cancer chemotherapy after surgery. Immediately after the surgery, the subjects were randomly divided into two groups: A group treated by MMC and FT-207: and B group treated by MMC and UFT. From a total of 54 cases, 8 cases were excluded, so that A group consisted of 24 subjects and B group of 22 subjects. There were no differences in background factors between the two groups. In a comparison of all the cases, B group revealed a significantly higher survival rate than A group. These results indicated that the simultaneous use of MMC and UFT was effective as a long-term cancer chemotherapy after surgery for patients with Stage IV gastric cancer.

[Surgical problems encountered in cases of S0 (serosal invasion-free) gastric cancers].

In this paper we have studied 649 resected cases of S0 (macroscopically negative serosal invasion) gastric cancer, this number of S0 gastric cancer resections experienced from a total number of 1692 patients treated at our hospital during the previous 16 years. The five-year survival rate of S0 cancers was 94% for patients given curative resections. Most of S0 cancers were histologically restricted within the pm layer but a few had reached the ss gamma or se layer. Non-curative factors in S0 cancers we recomposed of cancer positive stumps in the IIb-like margin, liver metastasis in the differentiated type carcinomas with ss invasion, and surgically left lymph node metastasis in the advanced carcinomas. As for lymph node dissections, our study reveals that an R2 dissection in the C and M regions, and a R3 or R2 + No.12 dissection for the A region are recommended. Modes of recurrence were characterized by liver or other hematogeneous metastasis in pm cancers or lymph node-positive cancers at the antrum, and by a peritoneal recurrence at the corpus in cases of advanced cancers. Histological ps(-) cancers showed a different survival result than did the S0 cancers and S(+) cancers.

Production of human thyroid-stimulating hormone in Chinese hamster ovary cells.

Human thyroid-stimulating hormone (hTSH) has been produced in Chinese hamster ovary (CHO) cells co-transformed with two plasmids: one carrying the alpha subunit cDNA with mouse dihydrofolate reductase gene and the other carrying hTSH beta subunit cDNA. Each cDNA was driven to expression under the control of SV40 early promoter. hTSH and its alpha subunit were secreted into culture media, and their secretion increased with exposure of the cells to increasing concentrations of methotrexate. Gel filtration analysis revealed that the molecular size of the hTSH was the same as that of natural hTSH. Furthermore, the CHO cell-produced hTSH elevated the cyclic AMP level in the rat thyroid cell line FRTL-5 in the same manner as natural hTSH does.

Anomalous behavior of human leukocyte interferon subtypes on polyacrylamide gel electrophoresis in the presence of dodecyl sulfate.

35S-labeled human leukocyte interferon (IFN) subtypes produced in a cell-free system derived from Escherichia coli were analyzed by polyacrylamide gel electrophoresis in the presence of SDS (SDS-PAGE). Some IFN subtypes anomalously showed lower electrophoretic mobilities than those expected from their formula molecular masses. The results with hybrid IFNs and esterification suggest that this anomaly of IFN subtypes on SDS-PAGE is due to the introduction of one or two negative charges in the middle of the molecule.

Detection in situ of active polypeptides of mammalian and T4 DNA kinases after sodium dodecyl sulfate/polyacrylamide gel electrophoresis.

DNA kinase activity of rat liver nuclei was detected in situ after electrophoresis in sodium dodecyl sulfate/polyacrylamide gel containing 5'-hydroxyl nicked DNA as DNA substrate. After renaturation of polypeptides, the gel was incubated with [gamma-32P]ATP and Mg2+. An active polypeptide corresponding to Mr 61,000 was observed as a radioactive band by autoradiography. The intensity of the band was proportional to the amount of the enzyme applied. The active band common to various tissues of rat was observed with the nuclear extracts, indicating that DNA kinase for rat tissue is composed of a single polypeptide of Mr 61,000. In contrast, T4 polynucleotide kinase (Mr = 140,000) showed an active polypeptide band corresponding to the subunit of Mr 33,000.

Microcomputer-based local area network system for controlling information on perinatal medicine.

We present an outline of our microcomputer-based Local Area Network (LAN) system designed to process information related to perinatal events. This system enables any of the medical personnel to use a microcomputer terminal installed at the bedside and also to process, in a real-time manner, new input-information together with past information, thereby correlating the processing of medical information to actual clinical care. In addition, using the utility subsystem, a more desired form of data-output can be substituted for conventional hand-prepared clinical charts. Over a 3-month test period for clinical use, it became evident that this could serve as a suitable prototype system capable of covering the entire scope of information required by perinatologists. This system also enables any form of statistical assessment to be made of events, such as calculating the perinatal mortality rate and the incidence of preeclampsia. Problems arising from the clinical application of this system are given immediate attention and appropriate steps taken.
