[Surgical voice restoration after pharyngolaryngoesophagectomy].

Although pharyngolaryngoesophagectomy (PLE) offers the only reliable possibility for cure in patients with advanced carcinomas of the cervical esophagus, the majority must reconcile themselves to subsequent voicelessness. Various surgical methods for voice restoration have been devised to solve this problem. Among these methods, the most common following PLE are the tracheogastric shunt, double tracheo-esophagogastric shunt, elephant trunk-shaped tracheo-esophageal shunt, tracheo-jejunal shunt, and tracheo-ileocecal shunt. The basic premise of these procedures is to introduce expiratory air from the trachea into the reconstructed esophagus to produce voice without aspiration of liquid or food during deglutition. The resultant speech is acceptable in most patients.

[A case of intra-peritoneal recurrence of colon carcinoma that responded remarkably to combined therapy of low-dose leucovorin and 5-fluorouracil].

We treated a patient with intra-peritoneal recurrent tumor from colon cancer who responded completely to chemotherapy of combined low-dose Leucovorin (LV) and 5-fluorouracil (5-FU). The patient was a 75-year-old man. He underwent resection of the transverse colon, sigmoid colon and distal stomach for colon and gastric cancers. Nine months after the operation, his CEA level increased to 39.5 ng/ml and a CT scan revealed an intra-peritoneal tumor measuring about 5 cm. He received chemotherapy of 30 mg/day of LV that was injected in a bolus and 500 mg/day of 5-FU that was given i.v. by continuous infusion for 10 days. At the end of 2 cycles of this regimen, CT scan demonstrated complete tumor remission and the patient's CEA level decreased to normal level. After an additional cycle of this regimen, he received modulated chemotherapy combined with l-Leucovorin and 5-FU as an outpatient. However, after 3 months of treatment, a recurrent tumor was detected in the same portion and the first regimen was re-started for 5 days. After 4 cycles of treatment the tumor disappeared completely from a CT scan. It is important to investigate effective regimens that do not reduce the quality of life of the patient. This clinical experience suggests that a low-dose LV/5-FU therapy may be beneficial to patients with recurrent colon cancer. Further investigation is necessary to establish an effective regimen that can be given for a long period without adverse effects on quality of life.

[Results of treatment based on endocrine therapy for bone metastasis from breast cancer].

Breast cancer patients with bone metastases at times survive for comparatively long periods, though complete cure is rare. The aim of the present study is to assess the value of endocrine therapy in the management of metastatic breast cancer patients. Of the 18 patients, the response rate (CR + PR) was 39% and the fifty percent-survival was 35 months. Improvement in symptoms and performance status was seen in 78% and 33% of the patients, respectively satisfactory palliation could be achieved with endocrine therapy for patients with metastatic breast cancer.

Synthesis and biological evaluation of tetrademethyl isocolchicine derivatives as inhibitors of DNA topoisomerase action in vitro.

Four tetrademethyl isocolchicine analogs were prepared and evaluated as inhibitors of mammalian DNA topoisomerase in vitro. All compounds inhibited topoisomerase II-dependent DNA unknotting by a mechanism which did not involve "cleavable-complex" formation. N-Deacetylation as well as N-substitution with the (3',4',5'-trihydroxybenzoyl)-group afforded compounds which were less selective, based on their added ability to inhibit topoisomerase I-mediated DNA relaxation.

Cytotoxic principles of Securinega virosa: virosecurinine and viroallosecurinine and related derivatives.

Virosecurinine (1) and viroallosecurinine (2) were isolated as two cytotoxic alkaloids from the leaves of Securinega virosa. A comparison of the cytotoxicity of 1 and several of its derivatives indicates that an alpha,beta- and a gamma,delta-unsaturated lactone located in a strained ring system, such as rings -B, -C, and -D of 1, is structurally required for significant cytotoxicity.

Analysis of amlodipine in serum by a sensitive high-performance liquid chromatographic method with amperometric detection.

An analytical method for a new calcium channel blocking agent, amlodipine, has been developed using high-performance liquid chromatography with electrochemical detection. No compound modification is required for detection and the calibration curve in spiked sera is linear and reproducible over the range 0.2-2.0 ng ml-1. The method has been applied successfully to pharmacokinetics studies in rats and also can be used for other dihydropyridine compounds such as nifedipine and nicardipine.

Analysis of discrepancies between VCG and ECG interpretation of anterior wall myocardial infarction.

In 247 cases in which an old anterior myocardial infarction (MI) was suspected and in which SVEC III-lead system vectorcardiograms (VCGs) and matching conventional 12-lead electrocardiograms (ECGs) were available, records were analyzed to investigate the diagnostic agreement or discrepancy between the two techniques. The frequency of agreement (155 cases) exceeded the frequency of disagreement (92 cases), but the frequency of disagreement even with lumped subgroups was high. Eighty percent of this disagreement was between positive VCG and negative ECG diagnostic consensus. The positive MI group by ECG diagnosis showed smaller R waves and larger S waves and larger S waves in leads V2 and V3 and smaller R/S ratios in leads V2 and V4 than the negative MI groupe by ECG, while small R waves or QS patterns and large S waves in just lead V1 were the only positive finding in the positive MI group by VCG. The important VCG characteristic for positive MI diagnosis was abnormal posterior deviation of the initial QRS vectors. The high incidence of discrepancies shown by positive VCG and negative ECG diagnoses resulted from the fact that, in spite of the presence of a fairly well developed R wave in precordial leads, the initial QRS vectors were displaced posteriorly. In 8 of 18 cases classified into negative VCG and positive ECG diagnostic group, poor R wave progression did not necessarily result in the posterior displacement of the initial QRS vectors. In contrast to the difficulties in applying ECG criteria for anterior MI (variability of QRS patterns in precordial leads), an orthogenal VCG (representing the phasic changes in the depolarization process) seems to clarify the equivocal situation in the ECG diagnosis of MI.

Induction of Furano-terpene Production and Formation of the Enzyme System from Mevalonate to Isopentenyl Pyrophosphate in Sweet Potato Root Tissue Injured by Ceratocystis fimbriata and by Toxic Chemicals.

When sweet potato (Ipomoea batatas) root tissue was infected by Ceratocystis fimbriata, activity of the enzyme system from mevalonate to isopentenyl pyrophosphate, especially of pyrophosphomevalonate decarboxylase (EC 4.1.1.33), was increased in the noninfected tissue adjacent to the infected region, preceding the furano-terpene production in the infected region. Cutting and incubation of sweet potato slices did not produce furano-terpenes, and only slightly increased the activity of the enzyme system from mevalonate to isopentenyl pyrophosphate. The enzymic activity in diseased tissue was localized in the soluble fraction, and was higher in the tissue from the surface to a depth of about 5 mm with gradual decrease toward the inner part.Mercuric chloride (0.1%, w/v) and sodium dodecyl sulfate (1.0%, w/v) were utilized as model inducers of furano-terpenes and pyrophosphomevalonate decarboxylase. The mercuric chloride- or sodium dodecyl sulfate-induced response was inhibited by administration of cycloheximide to the discs together with the inducer immediately after disc preparation. When cycloheximide or blasticidin S was applied together with the inducer, to the discs 9 hours or more after disc preparation, the induction was not inhibited but rather stimulated.
