[Preventive effects of various socks against adhesion of dermatophytes to healthy feet].

We studied the preventive effects of socks against dermatophyte infection. Wearing various socks (cotton socks, nylon stockings, wool socks and "tabi"), a healthy volunteer walked on a bath mat on which a patient with tinea pedis had stepped earlier. The volunteer pressed her right foot with socks onto large agar medium (Foot-press method), then, took off the socks and performed the Foot-press method again. The number of colonies of isolated dermatophytes on the medium was counted. The number of isolated colonies from the sole after taking off the nylon stockings was larger than that from the foot wearing the stockings. Dermatophytes were also isolated from the sole after taking off cotton socks. In contrast, few dermatophytes were isolated from the sole after taking off wool socks or "tabi". On microscopic observation, fibers of the nylon stockings and cotton socks were seen to be loose enough for dermatophytes to pass through. In contrast with those socks, fibers of wool socks and "tabi" were tight or fluffy. In conclusion, the nylon stockings and cotton socks are unsatisfactory in preventing the adhesion of dermatophytes.

[A case of successful management of nonresectable advanced cholangiocellular carcinoma by intermittent hepatic arterial infusion at home].

An 82-year-old female was referred to our hospital because a 16 x 8 cm tumor detected in her liver by abdominal ultrasonography (echo, hereafter) and CT. The patient was diagnosed as having highly advanced cholangiocellular carcinoma (CCC) by abdominal angiography. Since excision of the tumor was impossible due to the patient's age, a reservoir was indwelled for intra-arterial injection into the liver. Continuous injection of 1,000 mg 5-FU over 24 hours was performed every 2 weeks using a portable disposable pump 70 times. The tumor has been markedly reduced since the start of chemotherapy, with a reduction rate (PR) of 98% over the 3 years until the present. Since the frequency of administration was low, only twice a month, the patient had few side effects despite her old age, and injections could be performed in the outpatient department. Usually, the prognosis for CCC is poor. However, the patient has maintained a good QOL with the periodic intra-arterial injection of the carcinostatic into the liver, and this treatment has had a strong antitumor effect. This chemotherapy is thus considered useful for CCC which can not be resected.

[A case of multiple liver metastases from colon cancer successfully operated after intraarterial chemotherapy performed in home].

A 60-year-old male patient had unresectable multiple liver metastases from a sigmoid colon cancer that had been resected, and thus hepatic arterial infusion therapy was planned. A heparin coated catheter was inserted from the left thoracoacromial artery to the proper hepatic artery. 5-fluorouracil (1,000 mg) was administered via the catheter 24 hours/week using an implantable vascular device and a small disposable pump in his home. After 59 weeks, the metastatic liver tumors had decreased remarkably in size, and all tumors could be resected completely through surgery. Three weeks after the operation, the same intraarterial chemotherapy was restarted to prevent the recurrence in the liver. However, the patient died of lymph node recurrences. The intraarterial chemotherapy is thought to be useful for neoadjuvant therapy in patients with inoperable liver metastasis from colon cancer.

[Correlations of histologic classification with clinical features and prognosis in interstitial lung disease associated with collagen disease].

We studied 32 patients with interstitial pneumonia associated with collagen disease. All underwent surgical lung biopsy. Twenty-seven were histologically classified as follows: 8 with usual interstitial pneumonia (UIP), 10 with nonspecific interstitial pneumonia (NSIP), 6 with bronchiolitis obliterans organizing pneumonia (BOOP), and 3 with diffuse alveolar damage (DAD). Twenty-five of the patients were treated with corticosteroid but 4 (3 DAD, 1 NSIP) deteriorated rapidly and died within a month after lung biopsy. At the final follow-up, all 8 UIP patients were alive with residual respiratory impairment, whereas 6 NSIP and 4 BOOP patients had almost completely recovered. These findings suggested that histologic classification can be highly valuable to the therapeutic responsiveness and prognosis in cases of interstitial pneumonia associated with collagen disease.

[A case of effective bisphosphonate therapy of sequential pamidronate and incadronate for bone metastases from breast cancer].

A 55-year-old woman complained of neck pain ten years after undergoing surgery. She was diagnosed as having bone metastases and treated using pamidronate therapy with doxifluridine, tamoxifen and fadrozole, which was assessed as effective. She complained of neck pain 32 months after pamidronate infusion. CT revealed that the sclerosis had continued but new lytic lesions were detected in the cervical vertebra. Thus, we attempted incadronate and medroxyprogesterone acetate (MPA) administration. After 2 months, incadronate therapy with MPA relieved her neck pain. CT revealed lytic lesions of cervical vertebra showing sclerosis 3 months after this treatment and the sclerosis has continued. Bisphosphonate therapy of sequential pamidronate and incadronate with MPA administration is a useful treatment for bone metastasis from breast cancer.

[A case of effective chemotherapy using CAF followed by docetaxel for advanced breast cancer].

A huge mass measuring 13 x 12 cm and wide cutaneous edema were detected in the right breast of a 51-year-old woman. Under a diagnosis of locally advanced breast cancer (T4bN2M1, stage IV) with liver metastases, we attempted sequential neoadjuvant chemotherapy. After three courses of CAF therapy (cyclophosphamide, doxorubicin (DXR), 5-FU), the primary tumor was decreased by 56% and the liver metastases had disappeared. A minor pathologic response was observed. Subsequently, three courses of docetaxel (TXT) administration were carried out. The primary tumor was then decreased by 75% and the axillary metastases had disappeared. Histopathological examination showed gross viable tumor cells in the residual tumor and positive axillary lymph nodes. The only toxic effect was nausea (grade 1) and no major adverse effects were observed. Neoadjuvant chemotherapy with sequential DXR followed by TXT is a useful treatment for locally advanced breast cancer.

[Two bedridden patients with bone metastases from breast cancer effectively treated with pamidronate therapy].

Case 1: A 43-year-old woman underwent mastectomy because of locally advanced breast cancer with multiple bone metastases. She was treated with CMF therapy but developed a compression fracture of a thoracic vertebra after 10 months and received pamidronate therapy. Pamidronate administration relived her back pain after 2 months and she was able to walk again after 3 months. However, she developed a resistance to the treatment, and then refused another treatment. She was found to have hypercalcemia 6 months later and received pamidronate again, but died 9 months after the treatment. Case 2: A 52-year-old woman underwent mastectomy because of breast cancer (T2) and was diagnosed as having multiple bone metastases 24 months after the operation. She could not turn over in bed due to progressing bone pain and received pamidronate therapy with CMF therapy at home 23 months after the diagnosis. After 2 months, pamidronate administration relieved her bone pain and she was free of pain after 4 months. After 5 months, X-rays revealed that lytic lesions showed sclerosis, and the pamidronate therapy was assessed as producing a PR. Pamidronate therapy improved her quality of life and activities of daily living, and she continues to receive it this time as an outpatient. Pamidronate therapy is promising as an effective treatment for bedridden patients with bone metastasis from breast cancer.

[Evaluation by FDG-PET of arterial infusion chemotherapy for unresectable metastatic liver tumor].

We evaluated arterial infusion chemotherapy for unresectable metastatic liver tumor using FDG-PET and CT. A 72-year-old female patient with multiple metastatic liver tumors of rectal cancer was treated by arterial infusion chemotherapy. The tumor size decreased by chemotherapy, but there was a high uptake lesion of FDG. Three months later, tumor ingrowth was detected. FDG produces images of regional glycolytic activity, and it is a useful method of assessment of tumor viability after chemotherapy in patients with cancer.

The vibration analysis of obturator framework designs.

The purpose of this study was to analyze the vibration of obturator framework designs and investigate the vibration effects of connecting parts of the components. Four designs of maxillary obturator framework were used in this study: a partial palatal plate with continuous bar (A), with a part of continuous bar (B), without continuous bar (C) and without continuous bar and indirect retainer (D). In order to investigate the vibration of each framework (total) and some component parts, the measurement points of the retentive parts were divided into 3 groups; anterior, middle and posterior. The framework was activated by a vibration generator. The vibration of each measurement point was detected with a laser doppler vibrometer. The frequency response signals were conducted by an FFT Analyzer between 220-2000 Hz. The maximum displacement (MDP) and the decay rate (DR) of the vibration were analyzed statistically by ANOVA and Fisher's PLSD (p<0.05). The result showed that in the posterior group, MDP of D was higher than those of A, B and C. Regarding the anterior, middle and total group, the DR of D were lower than A, B and C. It was suggested that D might be clinically unfavorable.

Radiotherapy prostheses.

The orofacial region performs the essential functions of mastication and speech, as well as that of appearance, which for better or worse evokes an instant and instinctive response in other people. In this region, tissue loss caused by surgical resection of a malignant tumor has a tremendous negative impact on the patient's quality of life, with deep mental and psychological repercussions. Therefore, from the standpoint of preserving form and function, radiotherapy has a major role to play in the treatment of malignant tumors in the orofacial region. That said, important organs, such as sensory organs, are present in close proximity to each other in this small region. During the irradiation process, therefore, it is important to ensure that the lesion is sufficiently irradiated while simultaneously protecting the surrounding normal tissue. In certain cases of radiotherapy of malignant tumors of the orofacial region, the use of radiotherapy prostheses can help to satisfy a basic principle of radiotherapy: that of delivering a lethal dose to the tumor while minimizing irradiation to normal tissue. In recent years, medical and dental experts have taken a team approach to creating and employing a variety of radiotherapy prostheses, working towards improving the treatment record for malignant tumors as well as reducing complications in surrounding normal tissue. As a result, patients treated with radiotherapy prostheses are now able to receive post-radiotherapy prosthodontic treatment in a dramatically safer and more rapid manner. It is clear that radiotherapy prostheses contribute significantly to the improvement of these patients' quality of life.

The vibratory properties of an obturator prosthesis with a soft lining material.

This study used modal analysis to evaluate the vibratory properties of cast obturator prostheses fitted with soft lining material. Three types of buccal flange bulbs were prepared: a resin (R) type; a 2 mm-thickness relined (RM) type; and a type fully relined with MOLTENO, (M) type. A vibration generator excited the obturator, while a Laser-Doppler Vibrometer detected the vibrations at specified measurement points. Both the excitation and response signals were sent to an FFT analyzer, which calculated the frequency response functions. Then, using DAMPCAL simulation software, the decay rates and the maximum amplitudes of rest and clasp parts were calculated. Statistical analysis was performed by means of one-way ANOVA with Fisher's PLSD test. The results indicated that for impacts simulated on both the defective and non-defective sides, the R type showed a significantly higher decay rate than did the other samples (p<0.0001). When impacts were directed at the non-defective side, the R and M types showed significantly smaller maximum amplitude values than did the RM type (p<0.0001). Only the M type, however, showed significantly smaller values when impacts were directed at the defective side (p<0.0001).

Protein myristoylation in protein-lipid and protein-protein interactions.

Various proteins in signal transduction pathways are myristoylated. Although this modification is often essential for the proper functioning of the modified protein, the mechanism by which the modification exerts its effects is still largely unknown. Here we discuss the roles played by protein myristoylation, in both protein-lipid and protein-protein interactions. Myristoylation is involved in the membrane interactions of various proteins, such as MARCKS and endothelial NO synthase. The intermediate hydrophobic nature of the modification plays an important role in the reversible membrane anchoring of these proteins. The anchoring is strengthened by a basic amphiphilic domain that works as a switch for the reversible binding. Protein myristoylation is also involved in protein-protein interactions, which are regulated by the interplay between protein phosphorylation, calmodulin binding, and membrane phospholipids.

Phosphorylation of the myristoylated protein kinase C substrate MARCKS by the cyclin E-cyclin-dependent kinase 2 complex in vitro.

The myristoylated alanine-rich C-kinase substrate (MARCKS) purified from brain was recently characterized as a proline-directed kinase(s) substrate in vivo [Taniguchi, Manenti, Suzuki and Titani (1994) J. Biol. Chem. 269, 18299-18302]. Here we have investigated the phosphorylation of MARCKS by various cyclin-dependent kinases (Cdks) in vitro. We established that Cdk2, Cdk4 and, to a smaller extent, Cdk1 that have been immunoprecipitated from cellular extracts phosphorylate MARCKS. Comparison of MARCKS phosphorylation by protein kinase C (PKC) and by the purified cyclin E-Cdk2 complex suggested that two residues were phosphorylated by Cdk2 under these conditions. To identify these sites, Cdk2-phosphorylated MARCKS was digested with lysyl endoprotease and analysed by electrospray MS. Comparison with the digests obtained from the unphosphorylated protein demonstrated that two peptides, Gly12-Lys30 and Ala138-Lys152, were phosphorylated by cyclin E-Cdk2. The identity of these peptides was confirmed by automatic Edman degradation. On the basis of the consensus phosphorylation sequence described for Cdk2, and on MS/MS analysis of the Ala138-Lys152 peptide, we concluded that Ser27, one of the phosphorylation sites identified in vivo, and Thr150 were the Cdk2 targets in vitro. None of the other sites described in vivo were phosphorylated in these conditions. Interestingly, a preliminary phosphorylation of MARCKS by PKC improved the initial rate of phosphorylation by Cdk2 without modifying the number of sites concerned. In contrast, phosphorylation of MARCKS by Cdk2 did not significantly affect further phosphorylation by PKC.
