Long-term survivors with nonresectable advanced carcinoma of the rectum.

Two patients with advanced rectal cancer, women aged 73 and 49 years, have survived more than nine years with chemotherapy only and no surgical removal of the disease foci. In one patient, the tumor completely disappeared, and in the other the tumor stopped growing and its size remained unchanged. The reason for this long-term survival appears to be a high susceptibility to anticancer agents and persistence of enhanced host immunity.

Polyps in the acid-secreting area of the stomach.

The relation of gastric polyps to acid-secreting mucosa was examined in 120 patients with gastric polyps. Studies were made by the endoscopic Congo red test developed in this clinic. In 23 patients (19.1%) gastric polyps were located in acid-secreting mucosa and may have arisen from normal oxyntic mucosa. These polyps were of two histological types: oxyntic gland polyps containing many parietal cells, and nonoxyntic gland polyps without parietal cells. Only the former type appeared to secrete acid, judging endoscopically from the change of Congo red from red to blue-black on the surface of the polyps after administration of gastrin.

Plasmid deoxyribonucleic acid and translucent-to-opaque variation in Mycobacterium intracellulare 103.

After treatment with mitomycin D and other antibacterial agents, a translucent, smooth-colony-forming mycobacterium, isolated from sputum and designated as Mycobacterium intracellulare strain 103, gave rise to variants forming opaque colonies. These opaque variants were more sensitive streptomycin, kanamycin, viomycin, and rifampin than were the wild-type translucent variants. Plasmid deoxyribonucleic acids taken from translucent strain cells and from cells of certain opaque variants were analyzed by agarose gel electrophoresis. Two plasmids of molecular weights of approximately 2 x 10(6) and 50 x 10(6), respectively, were found in the wild-type translucent cells; one of them, the 2 x 10(6)-molecular-weight plasmid, was always missing from deoxyribonucleic acids of the opaque variant cells. The results suggested that translucent colonial appearance and antibiotic resistance of the strain are plasmid-determined functions.

Potentiation of neurovirulence of canine distemper virus in guinea pigs by sensitization with neural antigen.

Establishment of animal model of virus-induced encephalomyelitis was attempted in strain 13 guinea pigs sensitized with the homologous spinal cord antigen. Intracerebral inoculation of canine distemper virus alone or sensitization with the neural antigen alone did not induce significant clinical signs. Mild histological lesions were found in both the meninges and parenchyma of the central nervous system (CNS) of virus-infected animals, and in the meninges of the sensitized ones. In contrast, combination of virus infection and sensitization resulted in development of neurological signs of CNS disease as well as of marked histological lesions involving both the meninges and parenchyma. The potentiated CNS disease was successfully transferred by the lymph node cells of the sensitized animals into virus-infected ones. These results suggested that the virus-induced lesions in the CNS were potentiated by the lymphocytes sensitized with neural antigen. Depending on the time schedule of the sensitization and virus infection, different courses of CNS diseases including acute, subacute, and/or recurrent ones were induced, indicating the usefulness of this animal model for immunological and virological analysis of virus-induced CNS diseases.

[The maintenance of insulin secretory response of long-term cultured pancreatic islets to glucose, acetylcholine and epinephrine (author's transl)].

The present study was performed to investigate whether long-term cultured rat pancreatic islets possess a postcultural insulin secretory response to hormones and neurotransmitters in spite of their lack of stimulation during the culture period. We also investigated the method of maintaining the insulin secretory response of islets cultured in a physiological concentration of glucose. The tissue culture media were TCM 199 supplemented with 5.5 mM glucose (A medium), 5.5 mM glucose plus 1 mM adenosine (B medium), 16.7 mM glucose (C medium), and 16.7 mM glucose plus 1 mM adenosine (D medium). Short-term incubation after the culture period of 14 days showed that the islets cultured in B, C and D media maintained the same insulin secretory responsiveness to 8.3 mM glucose and/or 5 microM acetylcholine and also to 1 microM epinephrine as did non-cultured islets. A similar response was found among the islets maintained in B, C and D media. An insulin secretory response to epinephrine and phentolamine was deficient in islets cultured in A medium, whereas it was maintained in those cultured in C medium. The responsiveness of the islets cultured in C medium to the concomitant stimulation by epinephrine and phentolamine was not different from that of the non-cultured islets. It was thus concluded that the addition of adenosine in the culture medium containing the physiological concentration of glucose was as effective in amintaining the insulin secretory ability of the islets as was the culture medium containing a high concentration of glucose, and it was suggested that even the pancreatic islets cultured in these media, though separated from the innervation might preserve acetylcholine and adrenergic receptors similar to freshly isolated islets. Considering the action of adenosine, the necessity of enhancing ATP and C-AMP concentrations in B cells was also suggested in order to maintain the insulin secretory ability of cultured pancreatic islets.

Gastric hamartomatous polyps in the absence of familial polyposis coli.

Hamartomatous polyps in the stomach have been described as gastric lesions of familial polyposis coli. Four cases of multiple gastric hamartomatous polyps not associated with polyposis coli were encountered. Histologically, all these polyps consisted of normal oxyntic glands with numerous cystic dilatations of the glandular lumens of various sizes. Endoscopically, the polyps were all located in the acid secreting area and had an acid secreting function, which was revealed by the endoscopic Congo red test.

Uptake of 3H-GABA (gamma-aminobutyric acid) and 3H-leucine in the pancreatic islets and substantia nigra of the rat.

Isolated pancreatic islets and thin slices of substantia nigra (SN) of the rat were incubated in a medium containing 3H-GABA or 3H-leucine to test the activity of both tissues in the uptake of those substances. Pancreatic islets showed a low uptake of both 3H-GABA and 3H-leucine, but SN had a high activity in the uptake of 3H-GABA, though not for 3H-leucine. This suggests that GABA contained at high levels in the pancreatic islets some functional role other than in neurotransmission as in the central nervous system (CNS).

Diffuse leiomyomatosis of the esophagus.

A case of diffuse leiomyomatosis of the esophagus which was successfully resected is presented. The patient was a 44-year-old man who had an abnormal filling defect, which was incidentally discovered during esophagography, and was observed for two years. Although he remained asymptomatic, an operation was performed because of increase in the tumor's size. The lesion consisted of many confluent myomatous nodules throughout the thoracic portion of the esophagus, mostly seen in the circular layer of the muscularis, and some in the muscularis mucosa. Histologically, these nodules were leiomyomas. This is a very rare condition, and only a dozen similar cases have been reported so far. A brief discussion is made on the definition of diffuse leiomyomatosis of the esophagus.

Role of glucagon and somatostatin in insulin release.

The role of endogenous glucagon and somatostatin in insulin release was studied. Islets isolated from adult rats were incubated for 60 min in 3.3, 8,3 and 16.7 mM glucose with anti-glucagon antiserum (AGA) or anti-somatostatin antiserum (ASA) which was produced in rabbits. For the control experiments the antiserum was replaced by normal rabbit serum (NRS). Insulin release from islets treated with AGA was suppressed in 3.3 and 8.3 mM glucose as compared to islets treated with NRS, while insulin release was not affected in 16.7 mM glucose. On the other hand, insulin release from islets treated with ASA was enhanced in 3.3 and 8.3 mM glucose as compared to that from islets treated with NRS, whereas in 16.7 mM glucose it was not different from the control. These observations indicate that endogenous glucagon and somatostatin play a physiological role in the regulation of insulin release, but their regulation of insulin release is deranged at non-physiologically high glucose concentrations.

Lymphocyte transformation during pregnancy: an analysis using whole-blood culture.

In an effort to provide information on maternal immunocompetence, a study was undertaken to measure lectin-induced transformation of maternal lymphocytes. Whole-blood culture was used. The phytohemagglutinin (PHA)-induced lymphocyte transformation of 137 pregnant women was significantly (P less than 0.05 to 0.001) diminished until 21 weeks' gestation, and concanavalin A (Con A)-induced lymphocyte transformation of 116 pregnant women was also significantly (P less than 0.01 to 0.001) diminished at all stages of gestation, as compared with those of 39 nonpregnant women. In addition, the Con A-induced lymphocyte transformation of ovarian venous blood and uterine venous blood from a pregnant woman was prominently diminished, as compared with that of basilic venous blood. It is suggested that there are maternal T-cell subpopulations that have different reactivities to PHA and Con A.
