[Pulmonary endometriosis with recurrent catamenial hemoptysis].

A 32-year-old woman with a history of dilation and curettage for missed abortion at 31 years old had sudden hemoptysis during menstruation on May 25, 1993. She had catamenial hemoptysis again, and was admitted to our hospital on July 21, 1993. After admission, catamenial hemoptysis occurred, and a nodular shadow was noted in the right S4 area on chest roentgenogram and computed tomogram. A few days after menstruation, hemoptysis and the abnormal shadow on chest roentgenogram disappeared. Pulmonary endometriosis was diagnosed by clinical course, past history, and chest roentgenographic findings. During the next menstruation, hemoptysis and a nodular shadow in the right S4 area on chest roentgenogram recurred, so transbronchial lung biopsy and laparoscopy were done. TBLB specimens revealed macrophages with phagocytosed hemosiderin, and laparoscopy revealed pelvic endometriosis. She was treated with buserelin acetate. Catamenial hemoptysis and the nodular shadow on the chest roentgenogram disappeared after treatment.

[Simultaneous production of parathyroid hormone-related protein (PTHrP) and granulocyte colony-stimulating factor (G-CSF) in lung cancer patients with hypercalcemia and leukocytosis].

Hypercalcemia and leukocytosis are often associated with primary lung cancer as a paraneoplastic syndrome. Recently, parathyroid hormone-related protein (PTHrP) and granulocyte colony stimulating factor (G-CSF) have been identified as major causative peptides for hypercalcemia and leukocytosis, respectively. We studied four men with advanced primary lung cancer (stages from IIIA to IV) who presented with hypercalcemia (corrected serum calcium levels: 10.5 mg/dl) and leukocytosis (WBC > 10,000 per mm3). The age of the patients ranged from 59 to 79 years old. The pathological subtypes were squamous cell carcinoma in three and adenocarcinoma in one. The mean serum calcium levels and leukocyte counts were 15.8 +/- 1.4 mg/dl (mean +/- SE) and 24,800 +/- 3,253 cells/mm3 (mean +/- SE), respectively. Abnormally high serum levels of PTHrP and G-CSF were found in three patients (mean +/- SE: 137 +/- 68 pg/ml; normal range in human serum, < 16 pg/ml), and in all four (mean +/- SE: 72 +/- 7.7 pg/ml; normal range in human serum, < 20 pg/ml), respectively. Immuno-histochemical examination of cancerous tissue obtained from these patients showed positive staining for both PTHrP and G-CSF within the cytoplasm of all the cancerous tissue. These results suggest that the association of hypercalcemia and leukocytosis in patients with advanced primary lung cancer is caused by production of both PTHrP and G-CSF by cancerous tissue.

[Clinical features of subacute interstitial pneumonia--clinico-pathological study based on open lung biopsy findings].

A clinico-pathological study was done to elucidate characteristic features of subacute interstitial pneumonia. The patients were four men and mine women, with a mean age of 60 years. In ten patients, the disease was idiopathic, three had collagen vascular disease, (and one was undergoing gold therapy for rheumatoid arthritis). The time interval between onset of symptoms and open lung biopsy was 80 +/- 40 days. Eleven patients had progressive dyspnea, seven had coughing, and only one complained of fever. Fine crakles were heard in ten patients. Mild increases in CRP were observed in all cases. Mild increases in total serum IgG concentration were observed in five of eight cases. Multiple patchy infiltration or diffuse interstitial shadows, located predominantly in the lower fields of both lungs were the characteristic chest roentgenographic findings. The average %VC was 62.7 +/- 17% and the average PaO2 was 68.3 +/- 10 Torr. Bronchoalveolar lavage was done in nine patients, and the mean total cell count was 16.5 +/- 10.2 x 10(4)/ml. A moderate increase in lymphocytes (30.8 +/- 18.6%) with a low CD4/8 ratio (0.48 +/- 0.57), a mild increase in neutrophils (6.2 +/- 9.1%), and a mild increase in eosinophils (2.3 +/- 3.7%) were observed. Pathologically, interstitial cellulo-fibrous changes associated with alveolar space closure due to organization of exudate were the main features. Patients were given steroid pulse therapy or oral steroids. The results were mild to marked improvements in chest roentgenographic findings and lung function.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of angiogenesis inhibitor TNP-470 to suppress the recurrence in the liver remnant after partial hepatectomy for liver metastases in rabbits].

We investigated the effects of an angiogenesis inhibitor, TNP-470, in suppressing the recurrence in the liver remnant after partial hepatectomy for VX2 liver metastases model of rabbits. After inoculation of 1 x 10(6) VX2 tumor cells via the portal vein, the medial lobe of the liver was resected. Then, TNP-470 (7 mg/body/day) was infused (Group 1), or distilled water (Group 2), continuously via the mesenteric vein for a week. The control group received only infused distilled water via the mesenteric vein. Fourteen days after the tumor inoculation, there were 66.0 +/- 20.2 (mean +/- SD) metastatic colonies on the liver surface of control group, against 23.3 +/- 24.0 and 123.7 +/- 47.9 colonies in Group 1 and Group 2, respectively. There were significant differences between control group and Group 2, and between Group 1 and Group 2. We consider that the liver regeneration following hepatectomy might provoke tumor growth of occult metastases in the liver remnants. TNP-470 suppressed these growth by its anti-neovascular activity. And this effect reduced the number of metastatic colonies. Fourteen days after hepatectomy, the weights of the liver were no different among 3 groups. The BrdU Labeling Index was also no different between Group 1 and Group 2. TNP-470 did not suppress the regeneration of the liver remnant. We consider that the systemic side effects of TNP-470 might be made minimal, because this agent affects only on endothelial cells. These results suggest that the angiogenesis inhibitor TNP-470 is beneficial to suppress hepatic recurrence after partial hepatectomy for liver metastases without suppression of liver regeneration.

[Hepatic arterial infusion therapy for gastric liver metastasis using implanted reservoir].

Gastric liver metastasis is a fatal disease, and there is no golden standard as to its therapeutic mode. We experienced 58 such cases. Liver resections were performed in six cases (Group 1). Hepatic arterial infusion (HAI) chemotherapy, with 500 mg 5-FU and 100 mg of carboplatin as one shot weekly, was performed in fifteen cases (Group 2). The 50% survival was 656 days for Group 1, and 565 days for Group 2. Another nine cases, implanted with arterial catheter and reservoir did not qualify for HAI due to subsequent extra-hepatic spread of the disease (Group 3). For the rest of the patients (Group 4, n = 28), HAI could not be applied, due to existing extrahepatic spread of gastric cancer, which exhausted these patients. The 50% survival was 104 days and 93 days for Group 3 and Group 4, respectively. HAI therapy showed considerably improved survival for gastric liver metastasis. However, its indication needs to be clarified further.

[Effect of dosing schedule on efficacy of corticosteroid inhalation in chronic asthma].

A randomized study was conducted for 4 weeks to evaluate the effect of twice daily inhalation of beclomethasone dipropionate (BDP) inhalation (group A) as compared to four times a day inhalation (group B) in chronic asthma. Patients were randomly allocated to receive BDP at a dosage of eight puffs twice daily (800 micrograms/day, group A) or four puffs four times daily (800 micrograms/day, group B). Forty four patients entered the study but eleven were excluded because of their insufficient records or unfitness to eligibility criteria. There was no significant difference in patients' characteristics such as types, and severity of diseases between the two groups. Daily keeping of symptom scores, twice daily measurement of morning and night peak expiratory flow (PEF) and checking of the drug consumption were performed throughout the study. There was no significant difference in the mean %PEF either at 2 and 4 weeks at the study between the two groups. Symptom scores, asthmatic scores, bronchial hyperresponsiveness, pulmonary function tests (FVC, FEV1, FEV1%) and serum cortisol levels also showed no significant difference between the two groups. These results indicate that twice daily inhalation of BDP (800 micrograms/day) for four weeks caused the same effects on the patients with chronic bronchial asthma as 4 times daily inhalation did. Therefore, twice daily inhalation therapy with BDP is recommended for chronic bronchial asthma patients.

Determination of the spleen-blood partition coefficient for water with oxygen-15-water and oxygen-15-carbon dioxide dynamic PET steady-state methods.

METHODS: Regional splenic blood flow (SBF) was quantified by PET using a steady-state method with 15O-carbon dioxide. SBFs were determined using 104 tomographic planes obtained from 49 patients. RESULTS: When the spleen-blood partition coefficient for water (p) was > or = 0.85, significant correlations (p < 0.005) were found between SBF values determined by the steady-state and dynamic methods. The best correlation between SBFs determined by the two methods (r = 0.571) was found when p = 0.93. The best regression line, however, was thought to be the line when p = 0.93. The regression line between SBF calculated by the steady-state method (y) and SBF determined by the dynamic method (x) was y = 0.57 x + 0.03 with an F ratio of 48.75 (d.f. = 103, p = 5.0 x 10(-8)%, by ANOVA) when p = 0.92. CONCLUSION: A quick evaluation of SBF can be made by using the newly defined regression line.

[Diagnosis of acute respiratory distress syndrome: analysis of bronchoalveolar lavage fluid].

To better understand the pathogenesis of acute respiratory distress syndrome (ARDS), we analyzed bronchoalveolar lavage fluid (BALF) from patients with ARDS (n = 89, survival rate = 56.2%), who were admitted to our intensive care units over the past 7 years. ARDS was diagnosed when the lung injury score proposed by Murray et al was greater than 2.5. The BALF had very high centrations of albumin, a marker of permeability edema, along with remarkably high neutrophil counts, percent neutrophils, neutrophil-elastase, and interleukin-8, markers of neutrophil-related lung injury. In addition, the level of IL-8 in BALF was higher in non-survivors than in survivors. Levels of thrombin-antithrombin complex fibrin degenerative product and soluble thrombomodulin (recently recognized as a natural anticoagulant combined with vascular endothelial cells) were very high in BALF from patients with ARDS. Moreover, the level of soluble thrombomodulin in BALF was higher in non-survivors than in survivors. There were significant relationships between these neutrophil-related markers and markers of abnormal coagulation. The results of the BALF analysis suggest that accumulation and activation of neutrophils can affect thrombomodulin on vascular endothelial cells, which can activate thrombin and cause the coagulopathy seen in ARDS.

Structural studies of tRNAs by capillary high performance liquid chromatography/electrospray mass spectrometry.

Capillary high performance liquid chromatography/ electrospray mass spectrometry (LC/ ESI/MS) was applied to studies of nucleic acids. Since the presence of salt adducts obscures the molecular peaks and hinders the precise determination of molecular mass, conditions were sought in which effective online desalting and efficient ionization could be achieved simultaneously. In the presence of triethylamine, which is routinely used for the reversed-phase separation of nucleic acid, small oligo-nucleotides could be measured satisfactorily, while RNA tended to show more pronounced adduct peaks. When tributylamine was used instead of the shorter analogue, the adduct peak formation was well suppressed, and the hydrophobic nature of the counter ion made the nucleotides binding to the column tighter, resulting in the better chromatographic separation. The efficient ionization and suppression of the ion adducts make it possible to determine precisely the molecular mass of large nucleotides such as tRNA.

A substrate of ecto-protein kinase is microtubule-associated protein 1B in cortical cell cultures undergoing synaptogenesis.

Synapse formation between cultured rat cortical neurons is inhibited by the continuous application of K-252b, an ecto-protein kinase inhibitor, which cannot permeate the cell membrane. In order to identify the phosphorylated membrane proteins which are necessary for synapse formation, endogenous substrates for ecto-protein kinase activity were investigated. To detect phosphorylation of proteins containing extracellular domains, [gamma-33P]ATP was applied to the medium for brief periods. Proteins were then separated by SDS polyacrylamide gel electrophoresis and detected by autoradiography. Some bands showed immediate phosphorylation and this phosphorylation was suppressed by the addition of K-252b to the medium. We examined partial amino acid sequences of these substrates. The band with the highest molecular weight, whose phosphorylation was strongly inhibited by K-252b, was identified as microtubule-associated protein (MAP) 1B. These results suggest the possibility that the phosphorylation of extracellular domains of MAP1B is involved in synaptogenesis between cortical neurons.

A mass spectrometric study on the in vivo posttranslational modification of GAP-43.

GAP-43 isolated from calf brain was analyzed by the electrospray mass spectrometry. The mass spectrum of the intact protein showed two species with a mass difference of 80 Da, suggesting that the isolated GAP-43 contains phosphorylated species. To establish the in vivo phosphorylation sites, the protein was digested with trypsin, and analyzed by the liquid chromatography/mass spectrometry technique, in which a capillary reversed-phase chromatography column was connected on line to an electrospray mass spectrometer. Two pairs of peptides with a mass difference of 80 Da were observed. From the tandem mass spectrometry, two novel phosphorylation sites (Thr-87 and Ser-152) were identified. The novel phosphorylation sites contain proline immediately after the phosphorylated serines. No phosphorylated peptide was detected corresponding to the protein kinase C or casein kinase II phosphorylation sites. A peptide corresponding to the acetylated N-terminal peptide was also identified. The mass of the peptide suggests that the 2 cysteinyl residues are not palmitoylated but form a disulfide bridge.

Myristoylated alanine-rich C kinase substrate (MARCKS), a major protein kinase C substrate, is an in vivo substrate of proline-directed protein kinase(s). A mass spectroscopic analysis of the post-translational modifications.

Myristoylated alanine-rich C kinase substrate (MARCKS), a major in vivo substrate protein of protein kinase C, isolated from calf brain contains various species phosphorylated to different degrees. To establish the in vivo phosphorylation sites, the protein was digested with lysyl endoprotease, and the digests were analyzed by the capillary high performance liquid chromatography interfaced on-line to an electrospray mass spectrometer. Six out of 17 peptides were found to be phosphorylated. Of the 7 phosphorylated serine residues identified by Edman degradation, only 1 was within the known phosphorylation domain by protein kinase C. All the other phosphorylated serine residues originated from the N-terminal half of the molecule and were immediately followed by proline. Therefore, MARCKS, a major in vivo substrate of protein kinase C, is also an in vivo substrate of proline-directed protein kinase(s) such as mitogen-activated protein kinase or Cdk5 kinase.

Expression of c-myc mRNA as an aid in histologic differentiation of adenoma from well differentiated adenocarcinoma in the stomach.

BACKGROUND: Using prospective follow-up studies, the authors examined the value of the expression of c-myc mRNA in biopsy specimens obtained from elevated gastric lesions as an aid in the differentiating between adenomas and well differentiated elevated-type adenocarcinomas. METHODS: By in situ hybridization, the authors determined the expression of c-myc mRNA in biopsy specimens of elevated gastric lesions. The 31 patients who had borderline lesions with and without overexpression of c-myc were followed with repeated endoscopic examinations and gastric biopsies. RESULTS: Endoscopic follow-up examinations showed that gastric cancer was detected in 5 of 11 patients (46%) with elevated lesions that stained positively for c-myc mRNA during an observation period of 15 months (range, 2-32 months). The cancers were confirmed histologically in gastrectomy specimens. No cancers were found in patients with elevated lesions that stained negatively. The difference was statistically significant (P < 0.01). CONCLUSIONS: The overexpression of c-myc mRNA may present a new tool for distinguishing between adenomas and well differentiated adenocarcinomas of the elevated type.

Demyristoylation of the major substrate of protein kinase C (MARCKS) by the cytoplasmic fraction of brain synaptosomes.

Myristoylated alanine-rich C kinase substrate (MARCKS) is a major substrate of protein kinase C, whose interaction with the plasma membrane is dependent on its phosphorylation by protein kinase C and on its N-terminal myristoylation. We describe a hitherto undescribed demyristoylation activity of the protein in cytoplasmic fraction of synaptosomes from bovine brain. The activity is dependent on ATP but independent from calcium. The formation of the demyristoylated form, characterized by an increased mobility on SDS gel (70 kDa instead of 85 kDa), was confirmed by mass spectrometry and amino acid sequencing. The molecular mass of the demyristoylated protein as well as the incorporation of radioactive phosphate from [gamma-32P]ATP indicated that one phosphoryl group was incorporated during the demyristoylation process. Calmodulin, which binds to the protein kinase C phosphorylation domain of MARCKS, inhibited the reaction in a calcium-dependent manner. These data suggest that the demyristoylation is regulated by the signal transduction pathways and that the two conserved domains of the protein, namely the N-terminal myristoylated region and the phosphorylation site domain, are functionally interdependent. The localization of MARCKS in the cells may be regulated not only by its phosphorylation with protein kinase C but also by a reversible myristoylation of the protein in situ.

Monoamine oxidase B inhibitor enhances experimental carcinogenesis in rat colon induced by azoxymethane.

The effects of prolonged administration of the monoamine oxidase (MAO)-A inhibitor N-methyl-N-propargyl-3-(2,4-dichlorophenoxy)propylamine (clorgyline) and the MAO-B inhibitor N-methyl-N-2-propynyl-benzylamine (pargyline) on the incidence, number and histology of colon tumors induced by azoxymethane (AOM), and on the norepinephrine (NE) concentration in the colon wall and the labeling index of colon mucosa were investigated in Wistar rats. Rats were treated s.c. with 7.4 mg/kg body weight of AOM once a week for 10 weeks, and also s.c. with 5 mg/kg body weight of clorgyline or 50 mg/kg body weight of pargyline in 0.9% NaCl until the end of the experiment. Treatment with pargyline significantly increased the incidence of colon tumors in week 35. However, it did not influence the histological appearance of the colon tumors or the histological types and depth of involvement of colon adenocarcinomas. Furthermore, it significantly increased the NE concentration in the colon wall and the labeling index of the colon mucosa during and after AOM-treatment. In contrast, clorgyline had no influence on the development or histological appearance of colon tumors. These findings indicate that the MAO-B inhibitor, but not the MAO-A inhibitor, enhanced colon carcinogenesis, and that its effect may be related to its effect in increasing the NE concentration in the colon wall and subsequently increasing the proliferation of colon epithelial cells.

Clear cell adenocarcinoma of the vagina in a young female, treated by combination chemotherapy (local and systemic chemotherapy), complicated with chromosomal abnormality.

Although cancer of the vagina, a rare form of genital cancer, is usually squamous cell carcinoma in women over the age of 50 years, vaginal clear cell adenocarcinomas (CCA) are exceedingly rare. The pathogenesis of clear cell adenocarcinoma is uncertain, but as a clinical entity, it is most commonly associated with diethylstilbestrol exposure in utero. We report a rare case of primary vaginal CCA in a 17-year-old female who was not exposed to diethylstilbesterol in utero. The case, which was complicated with chromosomal abnormality, was treated with combination chemotherapy. To our knowledge, this is the first reported case of vaginal CCA in a woman with chromosomal abnormality and bicornate uterus.
