A long term clinical, immunological, and histological follow-up study of patients with goitrous chronic lymphocytic thyroiditis.

Goiter size, thyroid function, antimicrosomal and antithyroglobulin antibodies, and thyroid histology were compared in 43 patients with goitrous chronic lymphocytic thyroiditis (Hashimoto's disease) confirmed by biopsy before and after an interval of 10-20 yr. Although all were given thyroid hormone therapy, 13 patients took it infrequently, and are considered untreated. Among these 13 patients, 5 (38%) became hypothyroid, and 1 became thyrotoxic. Goiter size decreased in 17 (57%) of 30 patients during therapy and in 3 (23%) of 13 untreated patients, and the incidence of reduction was greater in treated than in untreated patients (P less than 0.05). However, in 13 (43%) of the patients, goiter size did not decrease even during treatment. Titers of antithyroid antibodies decreased in only 8 (21%) of 38 patients. There was no significant correlation between changes in antibodies and the size of goiter, and treatment with thyroid hormone did not decrease the titers of antibodies. In patients whose goiters were initially diffusely enlarged and regressed significantly in size during the follow-up period, the initial biopsies showed histological changes in diffuse lymphocytic infiltration and diffuse epithelial changes. Comparative studies of histological differences between patients who were treated with thyroid hormone and those who were not were made in the patients who had no change in goiter size during the observation period, which was greater than 10 yr. The histological changes were similar in both groups at the beginning of the study, and no significant histological changes during the period of greater than 10 yr were found in either group. In one patient who developed thyrotoxicosis after 20 yr and was rebiopsied, the histological picture was that of thyroid hyperplasia and lymphocytic thyroiditis. These data indicate that goitrous chronic lymphocytic thyroiditis changes little as a function of time in many patients regardless of whether thyroid hormone is administered.

Extramedullary plasmacytoma of the thyroid gland producing gamma heavy chain.

We encountered a 65-year-old man with gamma heavy chain disease associated with extramedullary plasmacytoma of the thyroid gland. Serum electrophoresis revealed an abnormal fast gamma band that cross-reacted with anti-IgG (Fc gamma) sera immunoelectrophoretically. Intracytoplasmic monoclonal immunoglobulin, IgG (Fc gamma), was demonstrated in thyroid tissue sections using an indirect immunofluorescence method. After surgery, the serum abnormal fast gamma band disappeared. At the time of writing, the patient has survived 18 months post-surgery.

[A factor analytic study of the items for the personality description based on the principle of the three traits theory for the work curve of addition of the Uchida-Kraepelin psychodiagnostic test].

An inventory form for the personality description based on the principle of the three traits theory for the work curve of the Uchida-Kraepelin psychodiagnostic test was investigated from the factor-analytic point of view. Three kinds of analyses were performed. First of all, the 66 items of the present form were administrated and a tentative orthogonal factor solution was obtained. Secondly, the 20 items for a simplified pattern in the sense of "simple structure" were selected based on the result of the first factor rotation, and the further orthogonal factor rotation was applied to the data based on the selected items so that the assumption for the three traits, primacy (A), variability (B), and, recency (C) were confirmed factor-analytically. Finally, in order to increase the number of items for providing an extended form for academic and practical use, more 10 items were added to the 20 items of the second analysis after applying the third orthogonal factor rotation so that the new form consisting of 30 items was obtained. Some relationships between the present work and the one of Eysenck and Eysenck (1968) were discussed.

[The study of urinary tract infection with a possible cause of fungi and evaluation of the efficacy of 5-FC].

Prior to clinical investigations, in vitro activities of 5-fluorocytosine (5-FC), ofloxacin (OFLX), and the combination on a strain of E. coli and of C. albicans were tested. The two drugs showed highly active on each target strain without any interaction. In clinical studies, 5 female adult patients with persistent uncomplicated UTI (3 of cystitis, 2 of pyelonephritis) caused by polymicrobial organisms (including bacteria and fungi) were treated in combination of 5-FC and oral chemotherapeutic agent. The results were both bacteriologically and symptomatically successful in all of 5 cases. Those had been poor by other antibiotic drugs in single use or 5-FC alone. Consequently, 5-FC and oral antibacterial drug combination was useful in therapy on even uncomplicated UTI due to polymicrobial organisms including fungi.

[A case of left pelvic kidney with non-functioning hydronephrosis].

A 19-year-old male was admitted with left lower abdominal pain and dysuria. Excretory urograms showed only a right kidney. V.C.G. showed a cystic mass lesion over the bladder. Computerized tomography revealed a huge retroperitoneal cystic mass located near the bladder. It seemed to be a huge bladder diverticulum, but the diagnosis was not clearly confirmed before operation. Through a lower abdominal midline incision, the cystic mass structure was removed. The specimen was a left ectopic pelvic kidney with non-functioning hydronephrosis. The removed mass was 91 X 85 X 78 mm in size, and 120 g in weight. The intrarenal fluid was 350 ml. A brief discussion and review are made. To our knowledge, only 3 such case have been described in the Japanese literature.

[Clinical evaluation of sisomicin in the treatment of complicated chronic urinary tract infections administered by intravenous infusion].

The pharmacokinetics of sisomicin (SISO) were determined in 3 patients including a case with mild renal dysfunction after intravenous infusion over 1 hour of a single dose of 50 mg. The peak serum concentration was ranged from 3.3 to 3.9 micrograms/ml with about 2 hours of half-life in patients with normal renal function and 4.5 hours in a patient with renal impaired function. This result suggested that dosage regimen should be adjusted in patients with renal impaired function. Clinical response was evaluated in treatment of 16 cases with chronic complicated UTI. SISO was administered intravenously over 1 or 1.5 hours at a daily dose of 100 or 150 mg. An overall excellent or moderate effect was seen in 68.8% of treatment cases with the evaluation by the UTI committee's criteria. As for clinical laboratory abnormal values, a slight but reversible increase of BUN was observed in 1 case. In conclusion, intravenous administration of SISO appeared to be effective and useful method for treatment of complicated UTI.

[The measurement of dializable free testosterone].

Non-protein bound testosterone is considered to represent an active androgen in blood, and it reflects in androgenicity. We measured the dializable free testosterone (AFT) in various disorders. The values of plasma testosterone (T), fractional free testosterone (%FT) and AFT were 560 +/- 151 (mean +/- SD) ng/dl 1.9 +/- 0.4% and 10.7 +/- 3.3 ng/dl in normal males (n = 43, aged 25-58), 51 +/- 12 ng/dl, 1.2 +/- 0.3% and 0.6 +/- 0.2 ng/dl in normal females (n = 30, aged 21-41), and 63 +/- 26 ng/dl, 0.6 +/- 0.3% and 0.4 +/- 0.2 ng/dl in pregnancy (n = 20, second and third trimester), respectively. Those values in normal females and pregnancy were significantly lower than those in normal males (p less than 0.001). In male, the concentrations of T and AFT in patients with primary (n = 43, aged 18-50) and secondary hypogonadism (n = 16, aged 22-66) were significantly decreased (p less than 0.001 vs normal males), respectively. The plasma concentration of total testosterone (T) was increased 1032 +/- 269 ng/dl in chronic hepatitis (p less than 0.001) (n = 21, aged 28-52) and 1455 +/- 366 ng/dl in hyperthyroidism (p less than 0.001) (n = 44, aged, 18-52). However, %FT in those disorders was significantly decreased 1.2 +/- 0.3% in chronic hepatitis (p less than 0.001) and 0.8 +/- 0.3% in hyperthyroidism (p less than 0.001), and plasma levels of AFT in both disorders were not different as compared with that in normal males.(ABSTRACT TRUNCATED AT 250 WORDS)

Gastrin-releasing peptide immunoreactivity in medullary thyroid carcinoma.

Two cases of gastrin releasing peptide (GRP)-producing medullary thyroid carcinoma are presented. Immunohistochemical examination revealed the presence of GRP-like immunoreactivity (IR-GRP) in the primary tumor tissues. High concentration of IR-GRP was also demonstrated in extracts of the primary tumors by radioimmunologic means with use of a GRP-specific antiserum. Chromatographic analysis showed that the immunoreactivity was composed of at least two molecular forms: one behaved as synthetic porcine GRP on Sephadex G-50 gel filtration and the other as porcine GRP (14-27), a C-terminal active fragment of GRP. The IR-GRP was shown not to be attributed to bombesin-like immunoreactivity. Substance P-like immunoreactivity was not detected in the tumor tissues by either immunohistochemical or radioimmunologic means. This is, as far as the authors are aware, the first finding of IR-GRP as an ectopic product in medullary carcinoma.

[A long-term follow-up study of patients with chronic lymphocytic thyroiditis--with special reference to histological findings].

It is generally believed that in the long-term observation of chronic thyroiditis, the goiter decreases in size with thyroid hormone therapy and the thyroid function drops gradually. On the other hand, the histological changes in so-called Hashimoto's thyroiditis have been recognized to show progressive loss of epithelium and increased fibrosis. In this study, goiter size, thyroid function, thyroid microsomal and thyroglobulin antibodies and histology in needle biopsy were investigated in 75 patients with chronic thyroiditis during an interval of more than ten years. Among 75 cases, 8 (11%) were hypothyroid at the first medical examination. Among 21 cases who received no treatment, 7 (33%) became hypothyroid during the period of more than ten years. Among 54 cases with thyroid hormone therapy, 16 (30%) showed a remarkable reduction in size of goiter, but among 21 cases without thyroid hormone therapy only 3 (14%) showed a remarkable reduction. This paper discusses changes in titers of thyroidal antibodies in 47 cases. Among these 47 cases, 21 increased titer of thyroglobulin antibody during the period of more than ten years. 10 (48%) out of these 21 cases showed a remarkable reduction in size of goiter. But among 11 cases with a decrease of titer of thyroglobulin antibody, only one (9%) showed a remarkable reduction in size of goiter. On the other hand, titer of thyroid microsomal antibody increased in 33 cases. 9 (27%) out of these 33 cases showed a remarkable reduction in size of goiter. Only one (13%) of 8 cases, which decreased titer of thyroid microsomal antibody, showed a remarkable reduction in size of goiter.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical results and safety of ceftazidime, a new injectable cephalosporin, in long-term administration of the treatment of infections in urology].

CAZ was administered to 34 patients in the field of urology for treatment or prevention of serious infections with many complicated factors. The duration of treatment ranged from 9 to 30 days, the most frequent being 14 days, which was the duration originally set as the standard. As the results CAZ was considered to be one of the drugs of choice in the cases requiring long-term treatment, from the viewpoint of both efficacy and usefulness.

[Hemangiopericytoma of the prostate: a case report].

A case of hemangiopericytoma arising in the prostate is presented. The patient, a 67-year-old male, was admitted to the hospital for difficulty in urination and underwent transurethral resection of the prostate. The pathological diagnosis was "well-differentiated adenocarcinoma and focal hemangiopericytoma". To the best of our knowledge, the only four cases of hemangiopericytoma of the prostate have been reported in the English literature so far and none in the Japanese literature.

[Laboratory and clinical study of ofloxacin in the treatment of bacterial prostatitis].

Ofloxacin (OFLX), a new pyridonecarboxylic acid derivative for oral use, was administered in the treatment of 22 cases of bacterial prostatitis (acute 4, chronic 18). In advance of the clinical trial, the concentration of OFLX in prostatic fluid (PF) was determined following administration of the drug to 3 patients with chronic bacterial prostatitis at a stationary phase. Assays revealed antibiotic concentrations of a mean level of 1.34 micrograms/ml in 3 PF samples. The ratios to the plasma antibiotic level (R value) averaged approximately 1.10. In clinical use, the drug was given at a daily dosage of 300-600 mg for 5-21 days. The bacteriological response of the infections caused by gram negative bacteria, in all eight patients (100%) was complete eradication, whereas that in 14 cases (including 5 mild infections) caused by gram positive cocci, was excellent or moderate in 10 (71%) and poor in 4 cases. Adverse reactions were observed in 2 cases; complaint of diarrhea (drug administration was discontinued in 1 case). No abnormal values such as of kidney or liver functions or of peripheral hematology were encountered. From the data obtained ofloxacin was concluded to be highly effective, safe and useful in treatment of bacterial prostatitis.

A study of thyroid function after subtotal thyroidectomy for Graves' disease: particularly on TRH tests, T3 suppression tests and antithyroid antibodies in euthyroid patients.

Of 305 patients who underwent subtotal thyroidectomy for Graves' disease between 1969 and 1975, recurrent hyperthyroidism was found in 31 (10.2%) and hypothyroidism in 18 (5.9%). The remaining 256 patients were clinically euthyroid, but an elevated serum TSH level was found in 104 (34.1%) and an elevated serum T3 level in 19 (6.28%). In 57 of 133 clinically and biochemically euthyroid patients, a TRH test, T3 suppression test and measurement of antithyroid antibodies were performed. Twenty-nine of the 57 patients (50.9%) showed an abnormal response to TRH. Eight of these (14.0%) showed an impaired or absent response. The T3 suppression test showed that 15 of the 57 patients (26.3%) were non-suppressible. Positive antithyroid antibodies, especially antimicrosomal antibodies, were more frequent in non-suppressible and TRH-non-responsive patients than in suppressible and TRH-responsive patients. It is suggested that after operation for Graves' disease: 1) only half of the clinically euthyroid patients were biochemically euthyroid, 2) of the clinically and biochemically euthyroid patients, there were many with abnormalities in TRH responsiveness and T3 suppressibility, and 3) thyroid functional status is unstable and long careful follow-up is important after operation for Graves' disease.
