Spontaneous rupture of the quadriceps tendon in patients on maintenance hemodialysis--report of three cases with clinicopathological observations.

Three patients receiving maintenance hemodialysis therapy for end-stage renal failure presented with spontaneous rupture of the quadriceps tendon(s). Biochemical data and the skeletal roentgenograms were compatible with secondary hyperparathyroidism. Histological examination of the excised quadriceps tendon specimens suggested that repeated minor avulsion fractures of the bone cortex at the tendon insertion site had preceded the final total tendon rupture and that osteitis fibrosa was responsible for these minor fractures. Serum alkaline phosphatase level had been increasing continuously for approximately five years prior to the tendon rupture in all three patients, indicating that uncontrolled osteitis fibrosa due to secondary hyperparathyroidism over these years preceded the tendon rupture.

[Experimental and clinical assessment of myocardial protective effect of a prostacyclin analogue (OP 41483)].

The beneficial effects of a stable prostacyclin analogue (PGI2-A), OP 41483 against myocardial ischemic injury were experimentally and clinically evaluated. In 31P-NMR study, the preischemic treatment with PGI2-A prevented myocardial ATP depletion of rat hearts which were subjected to 20 min global ischemia. In patients with congenital heart disease, the PGI2-A of 300 ng/ml was added to the glucose-insulin-potassium cardioplegia (PGI2-group), and this group was compared to control group without PGI2-A in terms of postoperative maximal leakage of CPK-MB (maxCPK-MB). In patients under 1 yr or patients over 1 yr with aortic cross clamp time exceeded 120 min, the maxCPK-MB was significantly (p < 0.05) reduced as compared to the control. This stable PGI2 analogue has shown significant myocardial protective effects experimentally and also in clinical setting.

Familial multiple neurilemoma. A case report.

A case of familial multiple neurilemoma is reported. A 59-year-old man with five neurilemomas was treated. Fifteen of his 22 family members had had soft-tissue tumors. Ten neurilemomas had been diagnosed microscopically in five members of the family. There were no symptoms related to von Recklinghausen's disease. This case seems to be a forme fruste or related form of von Recklinghausen's disease.

Radioimmunodetection of human pancreatic tumor xenografts using DU-PAN II monoclonal antibody.

The potential of DU-PAN II, monoclonal antibody (IgM), which was raised against the human tumor cell line, was evaluated for radioimmunodetection of human pancreatic tumors (PAN-5-JCK and EXP-58) grown in nude mice. 125I-labeled DU-PAN II was accumulated into PAN-5-JCK producing DU-PAN II antigen with a tumor-to-blood ratio of 2.72 +/- 3.00, but it did not localize in EXP-58 because of insufficient DU-PAN II. There was no significant uptake of 125I-nonimmunized IgM in PAN-5-JCK. These facts indicated the specific tumor uptake of DU-PAN II. Excellent images of the tumor PAN-5-JCK were obtained 3 days after the injection of 125I-DU-PAN II. Gel chromatography was also investigated with respect to the plasma taken from mice injected with antibody, or incubated with antibody in vitro. The results indicate that circulating antigen affected the tumor uptake of DU-PAN II: The more the tumor grew, the higher the amount of antigen excreted into the blood, leading to the degradation of DU-PAN II before it reached the tumor sites. Consequently, the immunoscintigram of the small tumor was remarkably clear. The catabolism and the radiolysis of the labeled IgM injected are critical points in applying immunoscintigraphy.

NMR-invisible ATP in rat heart and its change in ischemia.

The subcellular compartmentalization of adenosine 5'-triphosphate (ATP) in isolated perfused rat heart and its relation to energy depletion in ischemia were examined by 31P nuclear magnetic resonance (31P-NMR) spectroscopy and chemical analyses. The signal intensities of the beta-phosphate of ATP and creatine phosphate in the 31P-NMR were standardized by the intracellular volume ratio measured with 23Na-NMR to determine the actual content of each. During aerobic perfusion the ATP content determined by NMR (13.7 +/- 2.2 mumol/g dry weight) was significantly lower than that found by chemical analysis (22.4 +/- 0.7 mumol/g dry weight), while the creatine phosphate contents determined by the two methods were the same. During ischemia at 33 degrees C, the signal of the beta-phosphate of ATP in the 31P-NMR spectrum decreased progressively, disappearing completely after 16 min. But at this time 5.7 +/- 1.7 mumol/g dry weight of myocardial ATP was still detected by chemical analysis. These results indicated that there were two different compartments of intracellular ATP in the heart, only one of which is detectable by 31P-NMR spectroscopy, and that during ischemia the ATP that is detectable, which seems to be the free ATP in the cytosol, decreased more rapidly than the ATP in the other compartment.

Experimental and clinical study of crystalloid cardioplegic solution in neonatal period and early infancy. Effects of calcium and prostacyclin analogue.

The effects of calcium and a prostacyclin (PGI2) analogue in the glucose-insulin-potassium (GIK) cardioplegic solution for the neonatal period and early infancy were evaluated. The assessment was based mainly on semiquantitative scoring of mitochondrial damage and intracellular edema in postreperfusion biopsies. Experimentally, 45 isolated perfused newborn rabbit hearts (age, 0-2 days) underwent 2 hours of global ischemia at 15 degrees C with a single dose of GIK cardioplegic solution and were subsequently assigned to three groups: Group 1 hearts (n = 15) were infused with basic GIK cardioplegic solution alone (no added calcium, but measured at 0.1-0.2 mM/l); Group 2 hearts (n = 15) received GIK cardioplegic solution with calcium (1.2 mM); and Group 3 hearts (n = 15) received GIK cardioplegic solution with calcium and a PGI2 analogue (OP-41483, 300 micrograms/l). Group 3 hearts showed significantly lower mitochondrial damage and intracellular edema scores than did Group 1 and Group 2 hearts (p less than 0.05). Hemodynamic measurement (aortic flow and coronary flow) results after reperfusion were also better in Group 3 hearts than in the hearts of the other two groups (p less than 0.05). In the clinical study with 18 infants who were less than 3 months old, the same three cardioplegic solutions were used. Group 2 (n = 6) and Group 3 (n = 5) infants showed significantly lower mitochondrial damage scores than did Group 1 (n = 7) infants. Group 3 infants also showed significantly lower intracellular edema scores than did Group 1 and Group 2 infants.(ABSTRACT TRUNCATED AT 250 WORDS)

Monoclonal antibodies against porcine platelet membrane glycoproteins Ib and IIb/IIIa.

We prepared murine monoclonal antibodies to porcine platelet membrane glycoprotein (GP) Ib and GP IIb/IIIa for further study of the porcine hemostatic mechanism. One monoclonal antibody, designated PP3-4C, blocked Ristocetin-induced platelet agglutination and caused 80% inhibition of Ristocetin-induced 125I-von Willebrand factor (vWF) binding to porcine platelets at a concentration of greater than or equal to 12 micrograms IgG/mL. PP3-4C did not affect adenosine diphosphate (ADP)- or collagen-induced platelet aggregation. Binding of 125I-Fab fragments of PP3-4C to platelets was saturable at 3.7 x 10(4) +/- 0.8 x 10(4) molecules per platelet. Another monoclonal antibody, designated PP3-3A, blocked ADP- or collagen-induced platelet aggregation at 6 micrograms IgG/mL. At a concentration of 10 micrograms IgG/mL, PP3-3A completely inhibited binding either of 125I-fibrinogen or of 125I-vWF to ADP-stimulated platelets. PP3-3A did not affect Ristocetin-induced platelet agglutination nor 125I-vWF binding to platelets in the presence of Ristocetin. Binding of 125I-Fab' fragments of PP3-3A to platelets was saturable at 9.8 x 10(4) +/- 1.2 x 10(4) molecules per platelet. PP3-4C antibody (anti-GP Ib) did not bind to human platelets; however, PP3-3A antibody (anti-GP IIb-IIIa) had partial cross-reactivity with human platelets. Immunoaffinity chromatography of solubilized surface-radiolabeled porcine platelets and subsequent sodium dodecyl sulfate (SDS)-polyacrylamide gel electrophoresis demonstrated that PP3-4C recognized a GP with an apparent molecular weight of 160,000 (nonreduced), and 140,000 (reduced). PP3-3A recognized GPs with apparent molecular weights of 130,000 and 80,000 (nonreduced), and 115,000 and 95,000 (reduced). These monoclonal antibodies to porcine platelet membrane GPs, which are structural and functional analogues of human GP Ib and GP IIb/IIIa, will be useful for in vitro and in vivo studies of the mammalian hemostatic mechanism.

[An operated case of multiple endocrine neoplasia type IIb and review of the Japanese literature].

Multiple endocrine neoplasia, type IIb (MEN IIb) is a rare syndrome characterized by the occurrence of medullary thyroid carcinoma (MTC), pheochromocytoma and mucosal neuroma. A 35-year-old male patient with MEM IIb having megacolon, marfanoid habitus and no family history of the disease underwent surgery. Because MTC was present in both lobes, total thyroidectomy and modified neck dissection were performed. Pheochromocytoma was found bilaterally and bilateral adrenalectomy with adrenal autotransplantation in the rectus abdominis muscle was carried out. Postoperative course was satisfactory except for transient hypocalcemia and mild ileus. After the slow corticosteroid weaning process, his adrenocortical function was at the lower level within a normal range. In August 1986 (24 postoperative months), he was maintained by the administration of 10mg of hydrocortisone every three days, and calcitonin and CEA levels in sera were normal. We collected 15 cases reported in Japanese literatures. MTC and mucosal neuroma were found in all cases, whereas pheochromocytoma was present in 9 cases. Bilateral and multicentric occurrences were usual, and total thyroidectomy and bilateral adrenalectomy were, warranted. We believe that autotransplantation following bilateral adrenalectomy is a worthy alternative.

[Serological test of carbohydrate antigen CA50 in patients with cancer of the digestive tract].

In order to determine the clinical usefulness of carbohydrate antigen CA50 as a marker for cancers of the digestive tract, an attempt was carried out to measure the CA50 levels (normal level less than or equal to 18.1 U/ml) in sera of patients with various diseases of the digestive tract including pancreatic cancer. In patients with pancreatic cancer, the frequency of elevation of CA50 was 85.7% (6/35), which was the highest frequency of elevation, and a serum level of over 1,000 U/ml was observed in 6 (17.1%) of these 35 pancreatic cancer patients. Furthermore, patients with gall bladder carcinoma, hepatocellular carcinoma and gallstones showed relatively high frequencies of elevation. On the other hand, the maximum level of CA50 in serum was 41 U/ml in patients with benign pancreatic disease. Therefore, it was thought that CA50 was able to differentiate pancreatic cancer from benign pancreatic disease in patients with pancreatic disease with extremely high levels of CA50. The serum level of CA50 showed significant correlation with that of CA19-9. This study suggested that CA50 might be a potentially useful marker.

[Tumor markers in human breast cancer].

We described tumor markers which are considered to be useful for the detection of recurrence, index of the efficacy of treatment and assessment for the prognosis of the patients with breast cancer. CEA and CA 15-3 are relatively useful markers among various serological ones. However, although accuracy of the diagnosis with CA 15-3 and CEA is generally thought to be not superior to those of imaging method, CA 15-3 and CEA seems to be clinically useful tools for making diagnosis because of its simplicity and less cost. Estrogen and progesterone receptors shows good responses to endocrine therapy. Response rate was 70 to 80% in both receptor-positive patients. Presence of ER shows higher rate of recurrence and shorter survival of the patients than those with absence of ER. Recent attention has been focused on EGF and proto-oncogene, suggesting relationship of those to prognosis.

Ultrastructural assessment of the infant myocardium receiving crystalloid cardioplegia.

The effectiveness of cold crystalloid potassium cardioplegia was evaluated in 26 infants (age 27 days to 17 months, 7.5 +/- 5.2 months, mean +/- SD) who underwent intracardiac repair for various cardiac lesions. A myocardial biopsy sample was obtained before aortic cross-clamping (AXC) and 20 min after release of AXC (AXC time 22 to 161 min, mean 68 +/- 37 min), and semiquantitative assessment of the mitochondrial structure was made by scoring. The post-AXC score was significantly higher than the pre-AXC score (1.1 +/- 0.6 vs 0.4 +/- 0.4, p less than .001) for the whole group. Patients receiving preoperative catecholamine support had higher pre- and post-AXC scores than those who did not (pre-AXC score 0.7 +/- 0.4 vs 0.2 +/- 0.3, p less than .01; post-AXC score 1.4 +/- 0.5 vs 1.0 +/- 0.5, p less than .05). Infants less than 3 months old (n = 7) and those 3 to 12 months old (n = 11) had higher pre AXC scores than infants over 12 months old (n = 8). With respect to post-AXC score, only those less than 3 months old had significantly higher values than the other infants. These results indicate that the myocardial injury was not fully prevented by crystalloid potassium cardioplegia in infants, and that infants with preoperative heart failure and less than 3 months old appear to have increased myocardial susceptibility to ischemic injury under cardioplegia.

Effect of fibronectin and von Willebrand factor on the adhesion of human fixed washed platelets to collagen immobilized beads.

The adhesion of human fixed washed platelets (FWP) to collagen was measured using collagen immobilized beads. The addition of normal plasma or severe von Willebrand disease (VWD) plasma to FWP decreased the adhesion, suggesting the presence of some inhibitors of platelet adhesion in human plasma. Although the adhesion of FWP in severe VWD plasma was not different from that of FWP in normal plasma, the addition of purified von Willebrand factor (vWF, 1-2 mu/ml ristocetin cofactor) to FWP in buffer increased the FWP adhesion at higher flow rates, and the percent of adhesion in the absence of vWF was 10% (collagen 500 micrograms) and 30% (collagen 1,000 micrograms) of that in the presence of vWF at 10 ml/min. The enhancing effect of the vWF on FWP adhesion was also observed by pretreatment of the collagen column with vWF suggesting the important role of bound vWF to the collagen; adhesion 72% to the collagen column (1,600 micrograms) treated with vWF and 16% to the collagen column without the pretreatment at 10 ml/min. The promoting effect of vWF was also present in some commercial factor VIII preparations which had no large or intermediate multimers of vWF antigen. The adhesion of FWP was inhibited by fibronectin (FN) and the binding of ristocetin cofactor (vWF:RCo) to collagen fiber was also inhibited by FN; bound vWF:RCo to 50 micrograms/ml collagen in the absence or presence of 125 micrograms/ml FN were 60% and 8% respectively. It is suggested that vWF, even small multimer of vWF:Ag, is involved in the initial platelet-collagen interaction at high flow rates, while plasma FN acts as one of anti-adhesion factor.
