Holter electrocardiographic recordings were performed for 24 hours in 20 patients on chronic hemodialysis to evaluate the effect of changes in serum calcium and potassium during a 5-hour hemodialysis period on the QT interval. Hemodialysis caused an acute increase in serum calcium from a predialysis value of 9.6 +/- 1.3 mg/dl to 11.5 +/- 1.2 mg/dl and a decrease in serum potassium from 4.9 +/- 0.6 mEq/L to 3.5 +/- 0.4 mEq/L. The Q-oTc interval shortened from a predialysis value of 0.240 +/- 0.023 sec to a minimum of 0.215 +/- 0.022 sec during the 5th hour of hemodialysis. This Q-oTc interval shortening was correlated strongly with an increase in serum calcium and was statistically significant only in subjects showing an increase in serum calcium of more than 2 mg/dl (p less than 0.05). The Q-eTc interval increased from a predialysis value of 0.392 +/- 0.030 sec to 0.414 +/- 0.024 sec during the 4th hour of hemodialysis. This Q-eTc interval prolongation was correlated with a decrease in serum potassium. It was concluded that increased serum calcium during hemodialysis caused shortening of the Q-oTc interval, that the degree of shortening was proportional to the magnitude of increase in serum calcium, and that changes in serum calcium and the Q-oTc interval were concurrent. It was also concluded that hemodialysis resulted in prolongation of the Q-eTc interval. This change was possibly caused by a decrease in the level of serum potassium during the procedure.
A case of hepatocellular carcinoma (HCC) complicated with vascular invasion of the portal vein, hepatic vein, and pulmonary artery, accompanied by inferior vena caval and renal vein thrombosis, is reported. The patient was a 38-year-old female, positive in serum HBsAg and anti-HBe. Because portal trunk thrombosis was pointed out at the first diagnosis, incomplete transcatheter hepatic arterial embolization was carried out three times and one-shot therapy of anti-cancer agent given once. Four months after the first therapy, the patient died of oliguria and hypotension of sudden onset. The autopsy revealed not only intrahepatic vascular invasion but also tumor growth in the bilateral pulmonary arteries and thrombosis from the inferior vena cava to both renal veins unaccompanied by right atrial growth. Because the renal vein was thrombosed and HCC was accompanied by vascular invasion, especially pulmonary arterial invasion, the patient was thought to have become oliguric and hypotensive before severe hepatic failure. The metastatic tumor had grown in the pulmonary trunk without intra-atrial expansion because of the histological tight junction between the tumor and the wall of that vessel.

A 10-year-old girl was admitted displaying a medullary thyroid carcinoma, accompanying neurinomas of the tongue, marfanoid habitus, megacolon, and scoliosis. Although her adrenal glands were found to be unremarkable on both CT and ultrasonogram examination an MIBG scintiscan showed a 131-1 uptake, suggesting the presence of medullary hyperplasia. Remarkable nodal involvement with invasive features witnessed in the upper mediastinum during tertiary surgery four years later, following a total thyroidectomy. The TCT and CEA profiling was not sufficient enough to predict a recurrence. At the present state there is difficulty in providing an early diagnosis; through node dissection, however, is considered to be necessary at time of primary thyroid operation in cases with MEN type 2b.
In 1978, the new immunosuppressant, cyclosporin A, was accepted in clinical transplantation by Calne. Cyclosporin A eventually improved the results obtained after transplantation, after which it was widely accepted all over the world. Transplantation of several organs can be accepted as an effective form of treatment for patients with end-stage disease of each organ. Recently, transplantation has proved to be an excellent therapeutic method for cancer, especially in primary liver malignancy. Starzl has obtained good results in cases of fibrolamellar hepatomas. However, conservative surgical treatment should be used, whenever possible. Today, patients with unresectable liver tumor can expect long-term survival and even cure after liver transplantation with adjuvant therapeutic methods. Naturally, organ replacement is the most extensive surgical procedure for cancer treatment. However, early recurrence of tumors has often been observed following this therapy, possibly due to suppression. Adjuvant chemotherapy will thus be needed to improve the results of transplantation for malignancy.

Surgical experience of a rare case of malignant retroperitoneal cyst is reported. A 41-year-old female was admitted on Feb. 26, 1986, complaining of left lower abdominal tumor and mild abdominal pain. She underwent complete removal of an abdominal tumor located at the left flank lateral to the sigmoid colon on March 5. The tumor was well encapsulated, cystic and oval, 12 X 10 X 9 cm in size. Histologic feature of the tumor is classified as mucinous cystadenoma of low grade malignancy by WHO classification.
Clinicohistopathological and immunohistochemical studies on intrapancreatic spread of pancreatic carcinoma were performed on 30 cases with total pancreatectomy accompanied by portal vein resection. In the observation of HE stained tissue sections of 25 cases of carcinoma of head of the pancreas, the intrapancreatic spread from the head to body or tail was observed in 9 out of 25 cases (36%). However, by the immunostaining of CEA, CA19-9 and Dupan 2, small cancer nests surrounded by fibrous tissues could be easily detected and intrapancreatic continuous spread from the head to body or tail was observed in 15 out of 25 cases (60%). The intrapancreatic spread of the carcinoma correlated with portal invasion of carcinoma, hardness of the body and tail, obstruction of main pancreatic duct and irregular pancreaticogram. The intraoperative quick immunostaining on the cryostat sections of the pancreatic tissue, together with the HE staining, is useful to determine the intrapancreatic spread of the carcinoma. The indication of total pancreatectomy for pancreatic carcinoma can be determined by these results.

The effect of interleukin-2 (IL-2), injected directly into the tumor-bearer's spleen, on inhibition of tumor metastasis was evaluated. C3H/HeN mice were inoculated intradermally with 2 X 10(6) X5563 syngeneic tumor cells on day 0, and the tumor was surgically resected on day 10. The operation failed to prevent tumor death of these mice within 3 weeks. Autopsy of these mice revealed that death was due to systemic metastasis of tumor cells to lymphoid organs including the liver although the tumors had been successfully removed without any visible local recurrence. In this model, we administered IL-2 by intrasplenic injection daily for 3 days after operation. Mice treated with an intrasplenic injection of IL-2 showed a significantly prolonged survival time. Histological findings after this treatment revealed lymphoid cell proliferation of the spleen, no metastatic foci were found in the liver. Lymphokine-activated killer (LAK) activity from IL-2 injected spleen was also augmented. Intravenous (i.v.) and subcutaneous (s.c.) administration of IL-2 were not effective. A major difficulty in achieving significant immunologic effect in vivo by IL-2 infusion is the relatively short half-life of IL-2. Therefore IL-2 administered directly into the responding lymphoid organ is theoretically reasonable. In fact, treatment with intrasplenic injection of IL-2 significantly augmented the antitumor activity. Splenic arterial infusion of IL-2 may be an appropriate route of administration for adjuvant immunotherapy in human cancer.
