A 51-year-old woman was admitted with acute chest and back pain. A CT scan (2 hours after onset) was demonstrated acute type A aortic dissection. The dissecting lumen was already closed with thrombus. A DSA was performed. It showed no intimal tear and a normal aortic figure. Repeat CT (5 hours after onset) was performed. It showed reduction of dissecting lumen compared with previous CT. Although medical treatment was continued, she recomplained intermittent slight chest and back pain. On the sixteenth day after her admission, repeat CT revealed recrudescent dissection and opened dissecting lumen of ascending aorta. Graft replacement was successfully performed using circulatory arrest and retrograde cerebral perfusion. She was discharged in good health.

Two rare cases of early gastric carcinoma that followed gastrojejunostomy without gastrectomy are reported. The time intervals between the initial gastrojejunostomy and resection of the distal stomach with the formation of a gastrojejunal stoma were 30 yr and 33 yr, respectively. The Japanese classification confirmed both lesions as type IIc type mucosal early gastric cancer. In these two cases, with coexisting gastric ulcers, atrophic gastritis and diffuse intestinal metaplasia were observed in the lesions of the carcinomas. The potential correlation between the exposure of tissue to duodenal juice and the subsequent development of a gastric carcinoma is discussed. Literature regarding gastric carcinoma after gastrojejunostomy is reviewed.
The herbal medicine shakuyaku-kanzo-to has been used for the treatment of dysmenorrhea. We attempted to establish the effects of shakuyaku-kanzo-to on prostaglandin (PG) levels and the turnover of arachidonic acid in endometrial cells. Human endometrial cells isolated from proliferative-phase endometria were examined. Arachidonic acid reacylation activity was determined by [3H]arachidonic acid incorporation into endometrial cell phospholipid. Its deacylation rate was determined by both 3H-liberation from [3H]-arachidonic acid-labelled phospholipids and production of lyso-phospholipid. [3H]-PG levels were also analyzed. Shakuyaku-kanzo-to enhanced the rate and maximal level of arachidonic acid incorporation by 1.5-fold into phosphatidyl-ethanolamine and phosphatidylcholine. Degradation of the phospholipids to liberate arachidonic acid and lyso-derivatives was not affected by the shakuyaku-kanzo-to. Shakuyaku-kanzo-to decreased PG levels by approximately 50% of control. Because shakuyaku-kanzo-to stimulates arachidonic acid esterification to phospholipids, it seems likely that shakuyaku-kanzo-to depletes cellular free arachidonic acid with a consequent suppression of prostaglandin synthesis. Shakuyaku-kanzo-to may exert its action against dysmenorrhea through preventing prostaglandin production.
Autotransplantation of the parathyroid glands by mincing method has been performed to maintain parathyroidal function in patients with thyroid cancer, following total thyroidectomy and parathyroidectomy. The functional recovery of grafted parathyroid glands was studied in matured mongrel dogs histologically and by determining the PTH level and serum P, Ca, and compared three different forms of implant; whole gland, minced parathyroid tissue, and thick slices. The minced parathyroid tissue and thick slices were found to be far superior histologically, and endocrinologically, to the whole gland type of implant, with successful takes. And between two types of implants minced tissue and thick slices, no difference was observed histologically and endocrinologically. On the other hand, it is necessary for shortening the time to prepare implant tissue to get successful reimplantation. The time to prepare minced tissue is very short in comparison with thick slices. Thus our method (minced tissue) is very useful for successful reimplantation of the parathyroids.

Between 1973 and 1992, 300 patients underwent parathyroidectomy for secondary hyperparathyroidism due to chronic renal failure in our departments. Using parathyroid glands obtained at operation, histopathological studies were performed, and to estimate pathophysiology DNA analysis of parathyroid cell nuclei and calcium-regulated parathyroid hormone (PTH) secretion in vitro were estimated. PTH mRNA expression was evaluated by in situ hybridization. The typical histopathological findings were asymmetric enlargement, nodularities and increased number of oxyphil cells. Secondary hyperplasia was divided into 2 types: diffuse and nodular type hyperplasia. In the histopathological study nodular hyperplasia indicated more aggressive proliferation. In DNA analysis the relative number of scattered cells in the DNA synthesis phase was significantly greater in nodular than in diffuse hyperplasia. The half of the maximal inhibition of PTH secretion for calcium (the set-point) in the cells from nodular hyperplasia was higher than in the cells obtained from diffuse hyperplasia. However, there was no difference in expression of PTH mRNA in nodular and diffuse hyperplasia. These data suggested that nodular hyperplasia was more progressively hyperplastic, had more aggressive proliferative activities and showed more abnormal regulation of PTH secretion. These results imply that to prevent graft-dependent recurrent hyperparathyroidism after parathyroidectomy, the nodular hyperplastic tissue should not be autografted.
