Rapid detection of trisomy 21 by homologous gene quantitative PCR (HGQ-PCR).

Down's syndrome results from the production of three copies of chromosome 21 within a cell. We have devised a method termed the homologous gene quantitative polymerase chain reaction (HGQ-PCR), which uses one pair of primers and which can directly identify the additional copy of chromosome 21 by simultaneously amplifying two highly homologous genes of the human liver-type phosphofructokinase located on chromosome 21 (PFKL-CH21) and the human muscle-type phosphofructokinase located on chromosome 1 (PFKM-CH1) for self-detecting determination. On analysis of 34 cases of Down's syndrome, including two cases of unbalanced translocation 46, XY, der (14; 21) (q10; q10), + 21, and 100 normal individuals, the relative ratio of the PFKM-CH1/PFKL-CH21 product was 1.33 +/- 0.323 (mean +/- SD) and 0.40 +/- 0.16 (mean +/- SD) for disomy DNA and trisomy DNA, respectively. The difference between these two groups was highly significant (P < 0.001). These results indicate that this quantitative method is practical and may be used for the prenatal diagnosis of Down's syndrome caused by trisomy 21.

Color Doppler ultrasound, pregnancy-induced hypertension and small-for-gestational-age fetuses.

OBJECTIVE: The nomogram of blood velocity flow resistance of the spiral arteries was built at 13-25 gestational weeks. Thereafter, by using the nomogram we tried to assess the results of the color Doppler examination of the uteroplacental circulation at the second trimester to predict pregnancy-induced hypertension (PIH) and small-for-gestational-age (SGA). METHODS: Two groups of patients were studied. Group 1, for the establishment of the nomogram, included 175 uncomplicated pregnancies with gestational ages ranging from 13-25 weeks. The Doppler flow waveforms of the spiral arteries were measured once for each pregnancy in the studies. Group 2 consisted of 305 singletons selected consecutively for prospective study to confirm the occurrence of PIH or SGA. They were scanned twice for the measurements of the spiral artery waveforms at 13-19 and 20-25 weeks, respectively to test which gestational weeks interval in the nomogram is most sensitive in predicting PIH and SGA. RESULTS: The 5th, 50th and 95th percentiles of the pulsatility index (PI) values of the nomogram at the second trimester were used as the cut-off points to predict pregnancies complicated with SGA or PIH at delivery. Using the receiver operator curve, the 50th percentiles of the PI values of the nomograms were chosen as predictives for the development of PIH and SGA. At 13-19 gestational weeks, the specificities in predicting PIH and SGA were 50.71% and 49.82%, respectively, and the sensitivities were 52.00% and 50.00%, respectively. The calculated Cohen's Kappa statistics were 0.008 and 0.001, respectively in predicting PIH and SGA. At 20-25 gestational weeks, the specificities in predicting PIH and SGA were 49.64% and 49.46%, respectively, and the sensitivities were 56.00% and 57.14%, respectively. The calculated Cohen's Kappa statistics were 0.017 and 0.022, respectively in predicting PIH and SGA. CONCLUSION: The measurements of uteroplacental blood flow velocity waveforms at the second trimester are not sensitive enough to be an early screening tool for PIH and SGA in the low risk, non-selected pregnancy population. The fact suggests that in most gravidas complicated with PIH and SGA, the physiological process of trophoblastic invasion in the spiral artery was not prevented before the 25th gestational week.

Ovarian metastasis from cervical carcinoma.

OBJECTIVE: To study the prognosis and metastatic route of cervical carcinoma with ovarian metastasis. METHOD: From 1980 to 1993, 10 of the 1507 patients with cervical carcinoma operated and who had ovarian metastasis were analyzed. RESULTS: Six patients had squamous cell carcinomas and 4 patients had adenocarcinomas. Their mean age was 45 years. Six of 9 patients undergoing pelvic lymphadenectomy had nodal metastasis. One patient did not have nodal dissection in the treatment course. Five of 10 patients had involvement of corpus: 3 were accompanied with nodal metastasis, 1 was not and 1 other was unknown. None of our cases survived more than 5 years. Their mean survival time was 20.8 months for squamous cell carcinomas and 29 months for adenocarcinomas. CONCLUSIONS: (1) Ovarian metastasis is histologically one of the ominous signs of cervical carcinomas regardless of stage. The prognosis of patients with ovarian metastasis from cervical squamous cell carcinoma from our data is not different from those from cervical adenocarcinoma. (2) Lymphatic spread and transtubal implantation are possible pathways of cervical cancer metastasizing to ovary, and involvement of the corpus may potentiate this mechanism.

Cytokeratin-19 associated with apoptosis and chemosensitivity in human cervical cancer cells.

Cytokeratins are one group of intermediate filament proteins responsible for the integrity of cell structure, and have been recently reported to play a role in conferring a drug resistance phenotype. MAb Cx-99 is a monoclonal antibody exhibiting the specificity toward its corresponding antigen which was recently identified as the cytokeratin-19 protein. In the present study, we found that the level of cytokeratin-19 in cervical cancer cells could be decreased by incubation of cancer cells with MAb Cx-99. The reduction of cytokeratin-19 level had a killing effect on cervical carcinoma SIHA and HeLa S3 cell lines. The DNA ladder pattern, convoluted nuclei and blebbing morphology were observed with these cells after exposure to MAb Cx-99 for 72 h, suggesting that the cytotoxic mechanism of reduced cytokeratin-19 was mediated by induction of apoptosis. Moreover, the MAb Cx-99 treatment could increase the cytotoxicities of cancer chemotherapeutic agents such as cisplatin and vinblastine to both cervical carcinoma cell lines. The LD80 values were at least 15-fold reduced when cancer cells were treated with cisplatin or vinblastine in the presence of MAb Cx-99. These results suggest that the functional role of cytokeratin-19 was associated with the apoptosis prevention and drug resistance of cervical cancer cells.

Primary malignant peritoneal mesothelioma: a report of two cases.

Two cases of primary malignant peritoneal mesothelioma, a rather rare neoplasm, are reported. It is often mistaken by gynecologists for other, more common neoplasms such as ovarian cancer. The diagnosis is mainly established by laparotomy for tissue proof. There is no uniform agreement on the most appropriate therapeutic modalities to manage this tumor. Surgical resection is too technically difficult to achieve a satisfactory result. The response rate to chemotherapy or radiation is poor. The prognosis for primary malignant peritoneal mesothelioma is poor, with median survival from the time of diagnosis of less than 1 year, despite of aggressive treatment. A review of the literature is also presented.

Severe bleeding from a pregnancy tumor. A case report.

BACKGROUND: Hyperplastic gingivitis and gingival hyperplasia accompanying gestation have been termed "pregnancy gingivitis" and "pregnancy tumor." The condition is benign but, rarely, is complicated by severe bleeding that is difficult to manage. A single case of a pregnancy tumor is reported. CASE: A 28-year-old woman in the third trimester was evaluated for treatment of a pregnancy granuloma with recurrent episodes of severe bleeding for two weeks. Conservative management by firm pressure on gauze packs was applied to control the bleeding, but in vain. Induction of labor was conducted at 37 weeks partially because of term pregnancy and uncontrollable bleeding from the gingiva. An emergency cesarean section was done because of acute fetal distress during induction of labor. A healthy infant was delivered. The gingival bleeding stopped spontaneously five days afterwards. The patient was given thorough dental prophylaxis and oral hygiene instructions. The buccal granulomatous tumor was decreased in size four weeks postpartum. CONCLUSION: Careful oral dental hygiene, removal of dental plaque and debris, and use of soft toothbrushes are important during pregnancy to avoid occurrence of a pregnancy tumor. If uncontrolled bleeding occurs, management should be based on the individual condition and should range from supportive therapy--such as desiccation of bleeders; local, firm compression and oral hygiene to blood transfusion--as well as medication to accelerate fetal lung maturity or even termination of pregnancy to save the patient's life, as with treatment of uncontrollable eclampsia.

Color Doppler ultrasound of spiral arteries in normal second-trimester pregnancies.

BACKGROUND: Blood flow resistance of the subplacental spiral arteries in second-trimester pregnancies has not been previously reported. A reference range of blood flow resistance of the subplacental spiral arteries at 13-25 gestational weeks was designed in the hope the reference data could provide a basis for Doppler studies of pathologic disorders in second-trimester pregnancies. METHODS: A cross-sectional study was performed in 175 uncomplicated pregnancies at 13-25 gestational weeks. Doppler flow examinations of the subplacental spiral arteries were done. RESULTS: In the 175 normal pregnancies, both the predicted systolic/diastolic ratios(S/ D) (Y = 1.96258-0.02061x gestational age (GA), adjusted R2 = 0.03773, p = 0.003) and resistance indices (RI) (Y = 0.52131-0.00871 x GA, adjusted R2 = 0.03797, p = 0.003) of the subplacental spiral arteries decreased progressively with advancing gestational age. The predicted S/D value of the subplacental spiral artery decreased from 1.695 at the 13th week's gestation (5th% = 1.221, 95th% = 2.169) to 1.468 at the 24th week's gestation (5th% = 1.057, 95th% = 1.878). The predicted RI value of the subplacental spiral artery also decreased from 0.408 at the 13th week's gestation (5th% = 0.193, 95th% = 0.623) to 0.312 at the 24th week's gestation (5th% = 0.148, 95th% = 0.477). CONCLUSIONS: Normal blood flow resistance of the subplacental spiral arteries at 13-25 gestational weeks decreases progressively with advancing gestational age. The fact suggests that trophoblastic invasion of the spiral arteries occurred continuously throughout normal second-trimester pregnancies.

Linkage analysis of families with autosomal dominant polycystic kidney disease by KG8-CA marker.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common genetic diseases of human. Traditionally, ADPKD is diagnosed by ultrasonography, computed tomography (CT) or magnetic resonance imaging (MRI) of kidneys for the presence of renal cysts. Individuals who carry the defective gene but have not yet developed cysts in kidney may not be diagnosed. Genetic analysis reveals it to be caused mostly by a single-gene disorder of a genetic locus, designated PKD1. Recently, the genetic locus involving PKD1 has been identified on chromosome 16p13.3, and has been cloned and completely sequenced. METHODS: A pair of primers, KG8-CA, located between D16S84 and D16S125, was selected and synthesized for the polymerase chain reaction (PCR) to identify individuals who may carry the defective locus. The sequence of KG8-CA primers, was 5'-CTCCCAGGGTGGAGGAAGGTG-3' and 5'-GCAGGCACAGCCAGCTCCGAG-3'. PCR products were analyzed in denaturing condition, using gel containing 8% acrylamide and 7M urea. Autoradiography was carried out to interpret the results. RESULTS: Four Chinese families with history of ADPKD showed different DNA patterns in individuals with ADPKD and in normal individuals. Among the members in four families with history of ADPKD, every individual shared a common DNA band, suggesting that this band was derived from normal PKD1 allele. On the other hand, individuals diagnosed to have ADPKD showed one or two additional DNA bands which migrated differently from the common DNA band and should therefore be derived from defective ADPKD allele. Previous studies have shown that the ADPKD allele is highly polymorphic, as was evident in these family studies. CONCLUSIONS: Among the members from these four families, some were clinically normal and had DNA pattern that was typical to patients with ADPKD. These individuals might carry the defective PKD1 allele but have not yet developed the ADPKD symptoms. Therefore, the method described in this study has diagnostic values for pre-symptomatic individuals as well as for patients already diagnosed with ADPKD.

Squamous cell carcinoma of the cervix after laparoscopic surgery. A case report.

BACKGROUND: Although more than 50 cases of recurrence at the sites of cannula insertion after laparoscopy for malignant tumors have been reported in the literature, the majority were adenocarcinoma. We report a case of intraperitoneal carcinomatosis and abdominal wall metastases of the trocar site after laparoscopically assisted radical vaginal hysterectomy for the treatment of squamous cell carcinoma of the cervix. CASE: A 47-year-old woman underwent curative, laparoscopically assisted radical vaginal hysterectomy and pelvic lymphadenectomy for treating squamous cell carcinoma of the cervix, stage IB. The postoperative course was uneventful until two months later when abdominal recurrences at the trocar site and diffuse peritoneal carcinomatosis were noted. Pathology proved recurrent intraperitoneal squamous cell carcinoma with invasion of the abdominal port site. CONCLUSION: This case further alerts gynecologic oncologists of the potential risks of laparoscopic surgery for potentially curable gynecologic malignancies.

The evaluation of a new 7-day gonadotropin-releasing hormone agonist protocol in the controlled ovarian hyperstimulation for in vitro fertilization.

OBJECTIVE: Gonadotropin-releasing hormone agonist (GnRHa) was used in the controlled ovarian hyperstimulation (COH) for the in vitro fertilization program. However, the traditional long protocol demanded more human menopause gonadotropin (hMG) and sometimes causes unnecessary delay in the procedure. A new 7-day GnRHa/hMG protocol required to conserve cost and time is thus evaluated for better outcome. METHODS: Sixty consecutive IVF candidates less than 40 years of age were recruited for the study. Cases with severe male factor or polycystic ovarian disease were excluded. The perspective candidates were divided into two groups, one received the traditional, GnRHa2hMG protocol and the other received the new 7-day regimen. RESULTS: When comparing the results in pregnancy rate (33.3% vs 30%), cleavage rate (75.7% vs 75.5%), and the number of oocyte obtained (5.96 +/- 0.91 vs 6.63 +/- 0.90), the 7-day GnRHa/hMG protocol is as good as those of the traditional regimen. The amount of hMG used nevertheless was significantly less (21.48 +/- 0.78 vs 50.59 +/- 2.07). CONCLUSIONS: The new regimen will surely reduce the cost to relieve patient's financial burden and to increase patient's comfort.

Direct molecular diagnosis of CYP21 mutations in congenital adrenal hyperplasia.

The majority of congenital adrenal hyperplasia (CAH) cases arise from mutations in the steroid 21-hydroxylase (CYP21) gene. Without reliance on HLA gene linkage analysis, we have developed primers for differential polymerase chain reaction (PCR) amplification of the CYP21 gene and the non-functional CYP21P gene. Using the amplification created restriction site (ACRS) approach for direct mutational detection, a secondary PCR was then performed using a panel of primers specific for each of the 11 known mutations associated with CAH. Subsequent restriction analysis allowed not only the detection but also the determination of the zygosity of the mutations analysed. Existing deletion of the CYP21 gene could also be detected. In the analysis of 20 independent chromosomes in 11 families of CAH patients in Taiwan, four CYP21 mutation types, besides deletion, were detected. Interestingly, in five different alleles, the CYP21P pseudogene contained some polymorphisms generally associated with the CYP21 gene. These results suggest gene conversion events that are occurring in both CYP21P and CYP21 genes. Our combined differential PCR-ACRS protocol is simple and direct and is applicable for prenatal diagnosis of CAH using chorionic villi or amniotic cells.

A randomized comparative study of the effect of leuprorelin acetate depot and danazol in the treatment of endometriosis.

BACKGROUND: Administration of superactive agonistic analog of gonadotropin-releasing hormone (GnRH) has shown to induce a paradoxic and reversible suppression of gonadotropins, so that gonadal steroid concentrations are suppressed and hypoestrogenemia is induced. In order to compare the efficacy and safety of leuprorelin acetate depot (LA) and danazol in the treatment of endometriosis, we conducted this study. METHODS: A total of forty-five patients with pelvic endometriosis of different severity at laparoscopy with biopsy of peritoneal implants (n = 33) and surgical procedure (enucleation of ovarian chocolate cysts, cystectomy, or fulguration, n = 12) were included in the study and followed during the 20 weeks of treatment. LA 3.75 mg was injected subcutaneously every 28 days, while the daily oral dose of danazol was 800 mg. RESULTS: Both treatments were associated with a lowering of severity score. There was a consistent decrease in women with stage IV disease in the LA group and an increase in patients with stage I disease in the danazol group, but no difference was found between both treatments. Hot flushing was the most common side effect reported in 97% of LA but occurred only in 13% of danazol-treated patients. In contrast, the androgenic, anabolic side effects of weight gain and myalgia were common in those receiving danazol. The CA-125 levels were significantly lower in both treatment groups compared with their pretreatment levels (p < 0.01). During the five months of the LA treatment, biochemical evaluation revealed no pathologic alternation. Only total protein albumin, alkaline phosphatase, total bilirubin, total cholesterol, high density lipoprotein-cholesterol, low density lipoprotein-cholesterol, uric acid and calcium increased significantly. Danazol was also associated with adverse metabolic effects and significantly increased low-density lipoprotein-cholesterol concentrations (p < 0.05). In the fourth week, serum estradiol (E2) levels decreased to nearly castrated levels (13.87 +/- 1.63 pg/ml) in all patients treated with LA, and remained at this level thereafter. On the contrary, a slight but significant increase of the serum E2 levels was noted during the danazol treatment. After five months of treatment with LA, no significant changes in bone density were observed at the femoral neck or the anteroposterior (AP) view of lumbar spine, but there was a significant loss in bone density at the lateral view of lumbar spine (-7.1%, p < 0.05). CONCLUSIONS: Both LA and danazol are effective in the treatment of endometriosis. The hypoestrogenic side effects of LA may be better tolerated than the androgenic, anabolic effects of danazol. However, the danazol treatment costs less than LA and has no significant side effect with osteoporosis.

The efficacy and safety of a 19 nor-steroid in the treatment of endometriosis.

BACKGROUND: An oral synthetic 19 norsteroid (gestrinone) characterized by antigonadotropic activity and antioestrogenic properties provides its use in the treatment of oestrogen depending pathologic conditions. The purpose of this open study is to evaluate the efficacy and tolerance of gestrinone in the treatment of endometriosis. METHODS: Twenty premenopausal women with laparoscopically confirmed endometriosis received 2.5 mg gestrinone two times per week for six months. Laparoscopy was performed before treatment, and clinical responses were determined by second laparoscopy after six months. Serum CA-125 level, plasma lipid and bone density measurements during and after therapy were compared with baseline. RESULTS: The mean endometriosis score, defined by the revised American Fertility Society scoring system, decreased from 20.16 to 4.68 (p < 0.01). The mean serum level of CA-125 also declined from 44.04 U/ml to 18.64 U/ml (p < 0.01). Metabolic studies showed a significant decrease of cholesterol in high-density lipoprotein (p < 0.05), a moderate increase in low-density lipoprotein (p > 0.05), and a slight increase in both triglyceride and total cholesterol levels (p > 0.05). Bone density in this study also showed no significant difference in either spine or femur neck for a duration of six months treatment. Hot flush occurred in less frequency in gestrinonetreated group than in gonadotropin releasing hormone agonist (GnRHa-treated group), where the side effects of weight gain and acne were less often than in the danazol group well. CONCLUSIONS: Gestrinone 2.5 mg given twice a week for six months provides an effective and safe treatment for those women suffering from endometriosis.

Epithelial ovarian tumors of borderline malignancy.

BACKGROUND: Epithelial ovarian tumors of borderline malignancy are different from benign tumors and malignant neoplasms. They exist with relatively benign clinical course, younger age and better prognosis as compared with invasive malignant carcinomas. Most of them are discovered at early stage, for example, stage Ia. This retrospective review evaluates the clinical features, treatments and prognosis of 48 patients with borderline malignancy of ovarian tumors. METHODS: Forty-eight patients with ovarian tumors of borderline malignancy, aged from 14 to 69 years (mean: 39.2 years; median: 36 years), were retrospectively studied. The histopathologic diagnosis was based on the morphologic criteria published by Tazelaar et al. in 1985. All cases, including 16 cases diagnosed before 1985, were pathologically reviewed. All information of clinical stage, surgical intervention and prognosis was achieved by reviewing hospital record or contacting patients by telephone. Two patients were lost to follow up. One patient died of sepsis resulting from another operation for another gynecological cancer. Totally forty-five patients were included for evaluation. RESULTS: Thirty-nine of the 48 patients (81.3%) were at stage Ia, 6 cases (12.5%) were at stage Ib, 2 cases (4.1%) were at stage Ic, and the remaining one patient (2.1%) was at state IIIc. Thirty-four patients (71%) were with mucinous cystadenoma of borderline malignancy, 11 cases (23%) were of serous type, and 3 patients (6%) were of mixed serous and mucinous type. Twenty-two patients underwent total abdominal hysterectomy and bilateral salpingo-oophorectomy (TAH and BSO), but one of them remained partial ovary due to young age (27 y/o). Twelve patients were treated with unilateral oophorectomy or unilateral salpingo-oophorectomy (USO). Twelve patients underwent USO and wedge resection of contralateral ovary. One case underwent debulking surgery. One patient underwent enucleation of ovarian tumor and biopsy of contralateral ovary. Eighteen patients were treated with chemotherapy after operation. One patient developed recurrence 4 months after the primary operation. Excluding two cases lost to follow up and one case with surgical mortality for another gynecological cancer, forty-five patients were alive and were followed from 9 months to 165 months. (median: 48 months; mean: 46 months) CONCLUSIONS: Most of the patients were at the early stage of disease when first diagnosed, 81.3% were at stage Ia and only one case was at stage IIIc. Sixty-three percent of our patients underwent surgical treatment alone while the rest of them (37%) had post-operative chemotherapy with either alkeran or PAC. The use of adjuvant chemotherapy seemed unwarranted as there was no difference in survival between those with and without it. (P > 0.05) The low recurrent rate of 2% in our patients again confirmed the 9 P relative benign clinical course of this disease.

Heterotopic pregnancy after GIFT managed with expectancy: a case report.

A 29-year-old female patient visited our out-patient department (OPD) due to primary infertility in March 1993. Hysterosalpingography revealed cervical canal stricture. Gamete intra-Fallopian transfer (GIFT) was performed on Apr. 10, 1993 after ovulation induction. Three oocytes were placed into each Fallopian tube, then the patient was afflicted with lower abdominal discomfort and fullness 2 weeks later. Ovarian hyperstimulation syndrome (OHSS) was diagnosed. Ultrasonography showed intrauterine twin pregnancy and bilateral tubal pregnancy. During admission, supportive care for OHSS and expectant management only for ectopic pregnancies were given. OHSS resolved gradually. After a series of sonographic follow-up, disappearance of fetal heart beat (FHB) in the left ectopic gestational sac, resolution of bilateral ectopic gestational sacs and normal growth of intrauterine pregnancies were noted. On Dec. 16, 1993, the patient received cesarean section (C/S) due to twin pregnancy with vertex and breech presentation at the 37th gestational week. Twin A was female. Birth weight was 2590 gm. Apgar scores at 1 and 5 min. were 7 and 9. Twin B was also female. Birth weight was 2930 gm. Apgar scores at 1 and 5 main. were 8 and 9. Urinary bladder was injured accidentally at C/S. Repairment was done. The patient and her twins were discharged one week later in stable condition. Literature on heterotopic pregnancy after assisted reproductive technique (ART) was reviewed and discussed in this article.

An evaluation of chemotherapy in patients with cancer of the cervix and lymph node metastases.

During the past twelve years, out of the 288 cervical cancer patients who had lymph node involvement, the recurrence rate in the lymph node, was 26.6% if one or two nodes were involved and 50.8% if three or more nodes were involved (P < 0.0001). The recurrence rate of patients who received eight intravenous treatments (at monthly intervals) of multi-agent chemotherapy was lower than those who did not receive treatment. Of patients with one or two positive lymph nodes, 114 received treatment while 50 did not; their respective recurrence rates were 19.3% and 42.0%, a statistically significant difference (P < 0.025). Of patients with three or more positive lymph nodes, 97 received treatment and 27 did not, their respective recurrence rates, were 46% and 66% (P = 0.30) and the difference is not statistically significant. It would seem that multi-agent chemotherapy may reduce the recurrence rate in patients with cancer of the cervix and positive lymph nodes.

Clinical evaluations of a new ovarian cancer marker, COX-1.

A monoclonal antibody was generated against an ovarian cancer cell line, OC-3-VGH and was shown to recognize a unique tumor-associated antigen, COX-1 found in ovarian or cervical tumors. COX-1 can be detected in the cultured shed medium of several ovarian/cervical cancer cell lines, and in the sera of ovarian or cervical cancer patients when assayed by a sandwich immunoassay kit employing this monoclonal antibody. Multi-medical centers clinical trials have been conducted since 1989 to evaluate the efficacy of using this immunoassay procedure for the cancer monitoring. In the case of ovarian cancer, COX-1 immunoassay kit has the sensitivity of 68% as compared to 71% for the corresponding CA 125 kit. When both COX-1 and CA 125 kits were combined for the simultaneous monitoring of cancer patients, the sensitivity could be increased to as high as 87%. Further analysis revealed that serum COX-1 levels in cancer patients are correlated with stages of tumor progression. Following the surgical removal of tumors, greater than 50% of the cancer patients showed dramatic and significant decrease of serum levels of COX-1 antigen. The results of these multi-center clinical studies clearly suggested that COX-1 is a suitable marker for the monitoring and diagnosis of ovarian cancer patients, when used alone or in combination with CA 125.

Modified pelvic anterior exenteration.

During a 20-year time period, 117 patients with various histologic types of cancer underwent pelvic exentereation. Six of the 107 patients who needed anterior exenteration with urinary diversion received partial excision of the urinary bladder and modification surgery, of which three had uretero-cystostomy, two had mobilization of bladder and uretero-cystostomy and one had uretero-uterostomy. These six patients, except for two, died of disease in 1 year. Two patients are alive and happy, with quality of life because they can urinate normally. We suggest that anterior exenteration should be modified whenever possible to preserve the bladder and not to do an urinary diversion.
