Areae gastricae and gastritis in double contrast barium meal.

The areae gastricae were studied in patients with different forms of gastritis and with normal gastric mucosa. The material consisted of 57 patients. First gastroscopy was done and biopsies were taken. The stomach was studied on the following day by the double contrast barium meal. The material was divided according to the gastroscopic and biopsy findings into the following groups: normal, superficial gastritis, mixed gastritis, and atrophic gastritis. In the analysis of the surface pattern, special attention was paid to the shape, size and defects of areae gastricae. Corpus and antrum were dealt with separately. In the normal material the areae gastricae were regular in shape, and the size was usually 1--3 mm. Irregular shape, bigger size and more frequent defect areas seemed to point to gastritis. Such changes were especially clear in atrophic gastritis. The significance of the areae gastricae in practice is discussed.

Localization of occult insulin secreting tumors of the pancreas.

Percutaneous transhepatic portal venography (PTP) and selective portal blood sample collection for immunoreactive insulin (IRI) analyses was done in four patients who had clinical evidence of the presence of an insulin secreting tumor, but selective arteriogrphy of the pancreas did not visualize any insulinomas. In all patients the clinical diagnosis was confirmed and the localization of the tumors could be calculated with the aid of the PTP and the IRI values detected in different parts of the portal trees. Because no tumor was found at the operation in two patients, despite careful exploration of the pancreas, blind distal pancreatectomy was performed to the point suggested with the help of the investigations, and insulinomas were found close to the resection lines. In the two other patients the proposed localization of the tumor preoperatively was confirmed. There have been no postoperative hypoglycemic symptoms.

The value of simethicone in abdominal preparation.

The effectiveness of simethicone in reducing the amount of gastrointestinal gas was investigated with abdominal radiographs in a double-blind study of 169 patients receiving either simethicone or a placebo randomly. There was no statistically significant difference between the two groups. Although simethicone may relieve clinical symptoms and accelerate the transit time of gas, it does not reduce the amount of gas in the gastrointestinal tract. This study indicates that simethicone is not helpful in preparing the patient for radiological examination of the abdomen.

Double contrast examination of the stomach compared with endoscopy.

The results of the double examination of the stomach in a series of 102 patients were compared with the findings at endoscopy. Nasogastric intubation was used to introduce the air into the stomach. Hypotonia was achieved mainly with glucagon. The radiologic error rate was 10 per cent, consisting of 7 per cent false negative and 3 per cent false positive findings. The diagnostic advantages of the double contrast technique over those of the conventional barium examination are discussed.

Radiologic demonstration of Meckel's diverticulum.

A case of bleeding Meckel's diverticulum with a positive barium meal examination, selective mesenteric arteriography and 99mTc-pertechnetate scanning is presented. Complementary radiologic examinations are needed in the visualization of Meckel's diverticulum due to difficulties in making the correct diagnosis.

Calcification in cervical arteries in life and at autopsy.

In the clinical part of the study routine neck PA radiographs taken with the mouth open were compared with soft tissue films with the mouth closed for detection of calcification in the carotid arteries. Only 5.5% of 90 hospitalized patients showed foci of calcification in routine films. But foci of calcification were seen in 22% of the soft tissue radiographs of the whole series. In the autopsy part of the study large foci of calcification were found at the carotid bifurcations in 12 out of 20 cadavers by radiography. In an additional five cases contact radiographs also showed smaller foci of calcification. The vascular endothelium over the calcification was often ulcerated. The significance of the large arterial foci of calcification demonstrated in the neck radiographs as a sign of atherosclerosis and as a source of cerebral thromboembolism is discussed.

Tumor detection in extremities of man with 99mTc-tetracycline.

Twelve patients with tumors and three with osteitis of the extremities were studied using 99mTc-labelled tetracycline in order to image the process with gamma camera. The diagnosis of all cases were verified histologically. The labelled tetracycline was found to give positive images for the most malignant tumors and active osteitis which together comprised about half of the studied cases.
