Acidemia and brain pH during prolonged cardiopulmonary resuscitation in dogs.

BACKGROUND AND PURPOSE: Cardiopulmonary resuscitation (CPR) generating low perfusion pressures and beginning immediately after cardiac arrest maintains cerebral ATP but not cerebral pH or arterial pH. We tested the hypothesis that preventing severe arterial acidemia prevents cerebral acidosis, whereas augmenting arterial acidemia augments cerebral acidosis. METHODS: In dogs anesthetized with pentobarbital and fentanyl, cerebral pH and ATP were measured with 31P MR spectroscopy and blood flow was measured with radiolabeled microspheres. A pneumatically controlled vest was placed around the thorax, and chest compressions were begun immediately after electrically induced cardiac arrest. Cerebral perfusion pressure was maintained with epinephrine at 30 mm Hg for 90 minutes. The arterial acidemia observed during CPR was untreated in a control group, corrected to a pH of 7.3 with the use of sodium bicarbonate, or maintained below pH 6.5 with intravenous lactic acid after 14 minutes of CPR. RESULTS: At 10 minutes of CPR, cerebral ATP (99 +/- 1.5%, control), blood flow (35 +/- 3 mL/min per 100 g), O2 consumption (4.0 +/- 0.2 mL/min per 100 g), and cerebral pH (7.05 +/- .03) were unchanged from prearrest values (mean +/- SEM). After 10 minutes of CPR in the control group, cerebral pH progressively fell (6.43 +/- 0.10 at 90 minutes) in parallel with cerebral venous pH. In the bicarbonate group cerebral pH was maintained higher (6.91 +/- 0.08). Cerebral blood flow, O2 consumption, and ATP were sustained near prearrest values in both groups. In the lactate group, however, the rate of decrease of cerebral pH was augmented (6.47 +/- 0.06 by 30 minutes), and cerebral blood flow and metabolism were significantly reduced. CONCLUSIONS: Cerebral pH decreased in parallel with blood pH when resuscitation was started immediately upon arrest even when cerebral O2 consumption and blood flow were near normal. Although cerebral metabolism was near normal during the first hour of CPR, systemic bicarbonate administration ameliorated the cerebral acidosis. This finding indicates that the blood-brain pH gradient is important at the subnormal cerebral perfusion pressures seen in CPR.

alpha-Difluoromethylornithine increases the anti-tumor effect of cis-diamminedichloroplatinum in G-XII rat glioma.

The in vivo effects of the single or combined administration of alpha-difluoromethylornithine (DFMO), an inhibitor of polyamine synthesis, and cis-diamminedichloroplatinum (cisplatin) on the growth of G-XII glioma cells inoculated subcutaneously in rats were tested. Treatment with DFMO or cisplatin significantly decreased the bromodeoxyuridine (BrdU) labeling index of tumor tissues. Combined treatment with DFMO and cisplatin achieved a further significant decrease in the BrdU labeling index. All treatments significantly reduced the tissue levels of N1-acetylspermidine and putrescine, and the ornithine decarboxylase (ODC) activity, but little affected the tissue levels of spermidine and spermine, and the activity of spermidine/spermine N1-acetyltransferase. The reduction of ODC activity by cisplatin treatment may be associated with the anti-tumor effect.

[Activation of complement during hemodialysis].

In hemodialysis using 3 types of dialysis membrane materials [regenerated cellulose (RC), cellulose triacetate (CTA), and polysulfone (PS)], activation of the complement, reduction of white blood cells, and variation of vitronectin (VN) were observed. RC membrane caused a significant reduction of white blood cells and elevations of Bb and soluble membrane attack complex (S-MAC), indicating a strong activation of the alternative complement pathway. Especially, the increase of S-MAC persisted for a long time during hemodialysis. Because reduction of VN was transient, it was assumed that the S-MAC escaping removal by VN receptors might have persisted in the circulation. These findings suggested that S-MAC would become useful as an index for evaluating biocompatibility of various artificial organs including dialysis membranes.

[A case of bilateral partial anomalous pulmonary venous connections with the right upper pulmonary vein draining to the high superior vena cava].

A 10-year-old girl with bilateral partial anomalous pulmonary venous connections (PAPVC) was referred to us. The right upper pulmonary vein (PV) drained to the high superior vena cava (SVC) and the left upper PV to the left innominate vein. The left upper PV was directly anastomosed to the right atrial appendage. The SVC was divided distal to the orifice of the right upper PV and anastomosed to the right atrial appendage. A Xenomedica patch was placed within the right atrium to divert the pulmonary venous blood from the SVC orifice across the surgically enlarged ASD to the left atrium. The postoperative course was satisfactory without arrhythmia, and cineangiography revealed no obstruction at the SVC and the right upper PV pathways. We thought that this method is advantageous in preserving the sinus node and its feeding artery, as well as being applicable to the PAPVC to the high SVC.

[Immunologic alterations after splenectomy for trauma].

The effects of splenectomy for trauma patients on immunologic function were studied. Various immunologic indices including humoral and cellular immunity were serially measured up to one year in thirteen adult patients with blunt trauma: All the patients recovered from the initial injury and were good health at 12 month except for one died at ten month from unrelated cause. Complements and immunoglobulins decreased following splenectomy, then rapidly increased to normal or above normal levels and maintained the level throughout the one year period. Contrary to previous reports, Ig-M levels were not decreased after splenectomy. Anti-pneumococcal antibody (Pn-ab) was below effective concentration at one year except one case. Lymphocyte subpopulation was characterized by decreased T (OKT-3) cells and inverted helper/suppressor ratio. Lymphocyte blast formation was suppressed before surgery and remained low throughout the period. This study demonstrated that the humoral immunity was reserved after splenectomy, while cellular immunity such as helper/suppressor ratio and lymphocyte blast formation was impaired even one year after injury in most patients. Increased Ig-M levels after splenectomy in this study might suggest different immune response in Japanese. Decreased Pn-ab would justify the use of anti-pneumococcal vaccine after splenectomy for trauma.

[High resolution MR imaging of the inner ear apparatus using 3D-Fast spin echo sequence].

To evaluate the efficacy of high resolution MR imaging of the inner ear with 3D-Fast spin echo sequence, one volunteer and 12 patients were imaged with a 1.5 T MR scanner. High resolution T2-weighted images were obtained with a head coil in 13 minutes, and the voxel size was 0.39mm x 0.45mm x 1.2mm in most subjects. Original images and maximum intensity projection (MIP) images of the inner ear were evaluated. On original images, the endolymphatic duct was visualized in 72% of subjects, the cochlear aqueduct in 96%, three branches of the cranial nerves in the internal auditory canal in 100%, and flow void of the vessel in the cerebellopontine angle in 100%. On MIP images, more than two cochlear turns were visualized in 92% of subjects, three semicircular canals in 100%, and the anterior and posterior ampulla in 100%. MR imaging of the inner ear with 3D-Fast Spin Echo sequence provides a variety of useful information regarding the anatomy around the inner ear.

[The clinical study on secretory leukoprotease inhibitor (SLPI) in sera of patients with various pulmonary diseases].

It has been reported that secretory leukoprotease inhibitor (SLPI) can be a useful indicator for acute respiratory tract inflammation. In the present study, we attempted to measure automatically the serum concentration of SLPI by enzyme immunoassay (EIA) in patients with various pulmonary diseases. In this automatic measurement of SLPI, by which the results basically well-correlate with the manual method, we could measure many samples easily. Serum levels of SLPI in patients with various pulmonary diseases were significantly higher than those in healthy controls (50.5 +/- 9.8ng/ml). The serum concentration of SLPI in patients with inflammatory lung diseases correlated with that of C-reactive protein (CRP) or interleukin 6 (IL-6) significantly but not strongly. These results suggest that SLPI may be a useful indicator for local inflammation in respiratory tract. The serum concentration of SLPI in patients with lung cancer (71.1 +/- 10.8ng/ml), in particular adenocarcinoma, was significantly higher than that in healthy controls, but not correlated with other inflammatory markers.

Purification, characterization, and inhibition by phosphatidic acid of lysophospholipase transacylase from rat liver.

Lysophospholipase transacylase was purified 214,360-fold to homogeneity from the rat liver 100,000 x g supernatant. After DEAE chromatography, total activity increased 12.9-fold, due to the removal of endogenous inhibitors. The inhibitors were isolated and identified as phosphatidic acid and fatty acid. The final preparation showed a single band on SDS-polyacrylamide electrophoresis with an M(r) of 60,000. Gel filtration through Sephacryl S-200 gave a similar value, suggesting that the enzyme exists as a monomer. Activity was highest at pH 6.0 and was not affected by Ca2+, Mg2+, and EDTA. The enzyme produced glycerophosphocholine (GPC), palmitic acid, and dipalmitoyl-GPC on incubation with 1-palmitoyl-GPC, indicating that the enzyme catalyzed both deacylation and transacylation. The relative rates of deacylation and transacylation were 1:0.3 under standard assay conditions. Km for 1-palmitoyl-GPC and Vmax of hydrolase activity were 91 microM and 12.9 mumol/min/mg, respectively. The enzyme was selective for choline lysophospholipid. Ethanolamine, inositol, and serine lysophospholipids were not good substrates of the enzyme. Phosphatidic acid was a potent, competitive inhibitor of the enzyme with Ki of about 10 microM as determined with 1-stearoyl-2-arachidonoyl glycerophosphate. Although less potent, lysophosphatidic acid, palmitoyl-L-carnitine, and fatty acid were also inhibitory to the enzyme.

Alkaline phosphatase isozymes of serum and urine and urinary protein in young men before and after running 3 km.

Urine samples were collected from seven healthy male students 1 h before (control fraction) and 5-15 min (5-15 min fraction), 1 h (1-h fraction) and 3 h (3-h fraction) after running 3 km at a perceived exertion equal to approximately 15-17 Borg's scale. Venous blood was also collected from the subjects 1 h before and 1 h after the run. Urine was studied by measuring protein excretion, patterns of sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE) of proteins, total alkaline phosphatase (ALP) excretion, intestinal alkaline phosphatase (IAP) excretion and ALP isozyme analysis by PAGE. Serum ALP was studied by measuring the total ALP activity and isozyme analysis by PAGE. The following results were observed: firstly, an increase was seen in the excretion of total protein, total ALP and IAP after the exercise. Statistical significance was seen in the first two of these parameters; secondly, albumin was the only detectable protein band in the control, 5-15 min and 3-h fraction by SDS-PAGE; thirdly, no significant changes were seen in total ALP activity and ALP isozymes in the serum; and fourthly, three to five ALP bands (bands 1, 2, 3, 4 and 5, from the cathodal side) were detected in the zymograms of urinary ALP. Bands 1-3 were high molecular mass ALP. Bands 4 and 5 were intestine-like and liver-like ALP, respectively. An increase of ALP activity of high molecular mass ALP and/or decrease of that of band 5 or appearance of band 4 were seen in the 1-h fraction, in comparison to the control fraction.(ABSTRACT TRUNCATED AT 250 WORDS)

Elevation of N1-acetylspermidine and putrescine in hepatic tissues of patients with fulminant hepatitis and liver cirrhosis.

Hepatic polyamines were assayed in 15 patients with liver diseases, 5 with fulminant hepatitis (FH); 3 with exacerbated liver cirrhosis (ELC); and 7 with liver cirrhosis (LC). Hepatic putrescine and N1-acetylspermidine as percentages of total polyamines, were elevated and spermine was decreased in all 15 patients. The increase in hepatic N1-acetylspermidine levels appeared to be greater in patients with FH and ELC than in those with LC. These results suggest that the production of N1-acetylspermidine in human liver is closely associated with the induction of spermidine/spermine N1-acetyltransferase activity in liver diseases.

Androgen receptors, 5 alpha-reductase activity and androgen-dependent proliferation of vascular smooth muscle cells.

To assess the direct effect of androgen on the development of atherosclerosis, we investigated the effect of androgen and its receptor expression in rat vascular smooth muscle cells (VSMC) isolated from rat aorta. We detected the androgen receptor mRNA in VSMC by reverse transcription of the total RNA coupled with amplification of the resulting cDNA by polymerase chain reaction. Binding studies revealed the presence of a single class of binding sites for testosterone (Kd 7.37 nM, Bmax 10.59 fmol/mg protein) and dihydrotestosterone (DHT; Kd 4.89 nM, Bmax 11.37 fmol/mg protein) in VSMC. Measurement of 5 alpha-reductase activity suggested that testosterone is converted to DHT in VSMC (Km 0.36 microM, Vmax 623 fmol/mg protein/h). Moreover, in the present study, DHT significantly stimulated DNA synthesis of VSMC (120-160% of control). The mitogenic activity of testosterone is much less potent than that of DHT. Considering these results, we concluded that androgen may directly accelerate atherosclerosis by stimulating the proliferation of VSMC.

Noninvasive, continuous blood pressure measurement by arterial tonometry during anesthesia in children.

BACKGROUND: The authors' previous study of arterial tonometry in children demonstrated poor agreement of tonometric blood pressure (TBP) measurements with intraarterial blood pressure (IBP) measurements. The aim of the current study is to evaluate the feasibility of TBP measurements in children aged 1-6 yr using a newly designed pediatric sensor housing, by comparing TBP values with values obtained by IBP measurements. METHODS: Thirty-four children (aged 1-6 yr, ASA physical status 1-2) were studied undergoing elective abdominal or urologic surgery under general anesthesia. A 22- or 24-G cannula was inserted into the right radial artery. A TBP sensor housing was positioned over the left radial artery. TBP and IBP were continuously monitored and data were periodically sampled during and after intubation, during surgery, and during the recovery period. RESULTS: TBP waveforms were similar to those of IBP. Limits of agreement (bias +/- 2 SD) were 12 and -13.3 mmHg for systolic, 11.6 and -10.4 mmHg for mean, and 13.2 and -10.8 mmHg for diastolic pressures for 3,400 paired points. Measurements could not be made in ten children because of insufficient pulse contour. CONCLUSIONS: When pulse contour is sufficient for analysis, TBP monitoring provides an apparently safe and accurate method for the continuous measurement of arterial blood pressure during anesthesia in children aged 1-6 yr.

Carpal tunnel syndrome: evaluation of median nerve circulation with dynamic contrast-enhanced MR imaging.

PURPOSE: To identify mechanism(s) responsible for carpal tunnel syndrome (CTS). MATERIALS AND METHODS: At dynamic gadolinium-enhanced magnetic resonance (MR) imaging, the enhancement pattern of the sagittal section of the median nerve in 10 wrists of nine patients with CTS (two men and seven women, aged 43-60 years [mean, 52 years 10 months]) was studied while each wrist was in a neutral, flexed, and/or extended position. Five asymptomatic volunteers (four men and one woman, aged 23-54 years [mean, 31 years 5 months]) also underwent MR imaging to establish the normal enhancement patterns. RESULTS: Two distinctly abnormal patterns of median-nerve enhancement were revealed: marked or no enhancement. Marked enhancement was attributed to nerve edema, and the lack of enhancement was attributed to ischemia. Marked enhancement changed to no enhancement changed to no enhancement when the wrists were in a flexed or extended position. This change was associated with an aggravation of the symptoms of CTS. CONCLUSIONS: CTS may result from a circulatory disturbance rather than from deformation or compression of the nerve.

Triangular fibrocartilage in asymptomatic subjects: investigation of abnormal MR signal intensity.

PURPOSE: To investigate the prevalence of abnormally high signal intensity of triangular fibrocartilage (TFC) tears on magnetic resonance (MR) images of asymptomatic subjects. MATERIALS AND METHODS: MR images of one wrist in 70 asymptomatic volunteers (age range, 15-78 years) were evaluated for TFC high signal intensity. Findings at MR imaging and topographic parameters of each wrist studied were correlated with the subject's age. Any vertical or horizontal high-intensity slits that reached the joint surface or evidence of a perforation was considered abnormal. RESULTS: Abnormally high TFC signal intensity was seen on images of 35 (50%) wrists. Further, a thin TFC was more often seen in subjects with positive variance, and positive ulnar variance and thin TFC were associated with high signal intensity. Age did not substantially affect the presence or absence of high signal intensity. CONCLUSION: Since TFC high signal intensity was frequently detected in these asymptomatic wrists, MR imaging cannot reliably be used to detect clinically meaningful abnormalities.

Ulnar collateral ligament in the growing elbow: MR imaging of normal development and throwing injuries.

PURPOSE: To study the magnetic resonance (MR) imaging appearance of the ulnar collateral ligament (UCL) and its insertion into the medial epicondyle. MATERIALS AND METHODS: Sixteen normal and 20 symptomatic elbows were examined with a 1.5-T unit. Normal elbows were imaged with axial T2*-weighted three-dimensional Fourier transform sequences. Symptomatic elbows were imaged with coronal T1-, T2-, and T2*-weighted and/or short-inversion-time inversion-recovery sequences. RESULTS: In normal immature elbows, the ulnar periosteum was seen as an extension of the UCL, and its enthesis had signal intensity characteristics that differed from those of the mature ligament. In symptomatic elbows imaged before epiphyseal fusion, segmentation and subchondral bone resorption of the ossification center were seen with or without a capsular tear. After epiphyseal fusion, a full-thickness or a partial UCL tear at the site of its insertion, with or without subcortical bone resorption, was seen. CONCLUSION: The MR imaging characteristics of the developing elbow differ from those of the mature elbow. MR imaging is useful in assessing UCL damage.

HELLP syndrome: CT evaluation.

Patients with the HELLP syndrome (hemolysis, elevated liver enzymes, and low platelet count) require careful observation and expedient delivery. However, those who develop this syndrome peripartum do not always fulfill its diagnostic criteria before labor begins. We investigated the potential usefulness of liver and spleen CT evaluation in identifying such patients. Seven patients with this syndrome were studied by CT scanning on postpartum days 0-3 and again 3-4 weeks after delivery. Although 3 patients with severe thrombocytopenia had a reduced liver-spleen CT number ratio (< 1.1) and subsequent normalization, the remaining 4 with less severe thrombocytopenia demonstrated no CT changes. Thus, antepartum evaluation of liver and spleen by CT may not be sensitive enough to detect patients who develop HELLP syndrome during the peripartum period. Serial examination of platelet counts may be more useful than CT in detecting patients at risk for this peripartum syndrome.

Fulminant hepatitis following exposure to dichlorohydrin--report of two cases.

A 34-year-old man suffered from fulminant hepatitis after cleaning a tank in which there were traces of dichlorohydrin. In spite of daily plasma exchanges, he died 10 days after exposure. A 27-year-old man with much lighter exposure showed only slight liver dysfunction. This is the first report of lethal injury following exposure to dichlorohydrin.
