Local immune response in infertile patients with minimal endometriosis.

OBJECTIVE: To evaluate the role of local immunity in women with minimal endometriosis. METHODS: Uterine endometrium and endometrial implants were obtained simultaneously from 30 infertile women with minimal endometriosis and examined immunohistochemically using antibodies of T cell, B cell, macrophage, Langerhans cell, immunoglobulin (Ig)G, and complement (C) 3d. Serum IgG, IgA, IgM, C3, C4, antinuclear antibody and anti-DNA antibody were also examined in 24 of the women. Data from uterine endometrium and serum were compared with 10 fertile women without endometriosis as a control. RESULTS: Microscopic examination revealed that the endometrial implants were divided into two groups: group 1 (n = 13) showed lymphocytic infiltration in the endometrial implants and group 2 (n = 17) showed no or slight lymphocytic infiltration. The endometrial implants of group 1 showed significantly more dense T-cell infiltration than those of group 2. Other types of infiltrating cells and deposits of IgG and C3d revealed no significant differences between groups 1 and 2. The immunohistochemical examination of the uterine endometrium and the serum data revealed no significant differences among all three groups. Cumulative pregnancy rates showed no significant difference between groups 1 and 2. CONCLUSION: The difference of local immune response in endometrial implants did not affect systemic immunity.

[Effectiveness of magnetic resonance angiography for mass screening of unruptured intracranial aneurysms].

Magnetic resonance angiography (MRA) is widely used in screening for unruptured intracranial aneurysm (UIA) in the prevention of onset of subarachnoid hemorrhage (SAH) in Japan. However, the clinical efficacy of mass screening of UIA using MRA has not been established. This study aims to establish the clinical efficacy of mass screening of UIA using MRA by decision-making. The decision tree is constructed simulating a flow of population (100,000 persons, aged 50-59) starting from the MRA screening until the final outcomes of quality adjusted life or death in ten years. In order to estimate the numerical values various parameters were used as follows, prevalence rate: 1%; sensitivity of MRA: 74%; specificity of MRA: 76%; death rate by intracranial angiography: 0.1%; rupture rate of UIA: 2%/year; death rate of SAH: 57%; and death rate for surgery for UIA: 0.0%. The following results were obtained: 1. The number of lives saved by the MRA screening was 760, while the number saved without MRA was 79. 2. The number of deaths in those receiving the MRA screening was 52, while the number of deaths without MRA was 104. 3. The level of quality adjusted life with the MRA screening was higher than that without MRA in all cases. In conclusion, mass screening of UIA using MRA appears to be effective.

Diurnal variations in resting-active cycles in full-term fetal heart rate changes.

To elucidate the mechanism of the resting-active cycles (RAC) of fetal heart rates (FHR), in the resting and non-resting phases (RP and NRP), 24-h FHR recordings were made on 16 normal full-term pregnant women. RP, NRP, RAC-1 (NRP-NRP cycle), and RAC-2 (RP-RP cycle) were defined based on the criteria of Nijhuis et al. Frequency distributions were plotted separately for the entire 24-h period as well as for the day-time (07:00-21:00 h) and night-time (21:00-07:00 h), and were compared using Kolmogorov-Smirnov two-sample tests. The mean durations (+/- S.D.) (min) of RP, NRP, RAC-1, and RAC-2 were 22.7 +/- 11.2, 67.3 +/- 47.2, 90.0 +/- 47.6, and 89.9 +/- 48.6 during 24-h periods, 20.1 +/- 7.7, 68.3 +/- 52.3, 88.6 +/- 53.1, and 88.4 +/- 53.0 during the day-time, and 25.4 +/- 13.2, 66.2 +/- 41.2, 91.4 +/- 41.0, and 91.5 +/- 43.4 during the night-time. Length of RP was the only factor significantly different during the day and night (P < 0.05). We propose that there are different mechanisms controlling RP and NRP.

Growth suppression of a cervical cancer cell line (TMCC-1) by the human wild-type p53 gene.

To investigate the effects of human wild-type p53 expression on the proliferation of cervical carcinoma cells, a plasmid, pMO7-hp53, which contains a full-length cDNA of the human wild-type p53 (wt-p53) gene, was transfected into a cell line (TMCC-1) derived from an endocervical type, human papilloma virus-positive adenocarcinoma of the uterine cervix. The exogenous wt-p53 expression induced growth suppression, morphological changes, and loss of anchorage-independent growth of the tumor cells. As the wt-p53 gene apparently plays a negative role in growth regulation of cervical carcinoma cells, this gene may possibly be of some use for treating subjects with a cervical carcinoma.

Mild hyperglycemia and insulin treatment in experimental cerebral ischemia in rats.

We investigated the effects of mild hyperglycemia and insulin treatment on the metabolism of the ischemic brain in spontaneously hypertensive rats with acute hyperglycemia (n = 9), acute hyperglycemia treated with insulin during ischemia (n = 10), and normoglycemia (n = 10). Cerebral blood flow was measured by the H2 clearance method. Cerebral ischemia induced by occlusion of the bilateral carotid arteries. Cerebral glycolytic metabolites were measured enzymatically. Blood glucose levels were significantly higher in hyperglycemic animals (11.8 to 13.7 mM/l) than in normoglycemic animals (6.0 mM/l). At 60 min of ischemia, the blood flow to the parietal cortex was decreased to 3% of the resting value in all groups. Blood glucose levels at 60 min of ischemia in the hyperglycemic rats were 1.9--3 times higher than the treated hyperglycemic rats and normo-glycemic rats. Glucose concentrations were significantly and positively correlated with the ATP level (p < .0001) but not with the lactate levels in the ischemic brain. Our results suggest that mild hyperglycemia may preserve glucose metabolism in the presence of ischemic insult.

Human papilloma virus and P53 overexpression in carcinomas of the uterine cervix, lower uterine segment and endometrium.

Inactivation of the wild type p53 protein through complexing of protein synthesized by specific subtypes of human papilloma virus (HPV) or mutation in the p53 gene in considered to play an important role in cervical carcinogenesis. The association between p53 overexpression and the presence of HPV 16 and 18 DNA was assessed in 29 cervical carcinomas, 15 carcinomas of the lower uterine segment and 30 endometrial carcinomas. In 29 cervical carcinomas (21 adenocarcinomas and 8 adenosquamous carcinomas), 7 cases were positive for HPV 16 DNA while 5 were positive for HPV 18 DNA. Nine cases (31%) showed p53 overexpression. An inverse association was seen between the presence of HPV DNA and the p53 overexpression. In 15 carcinomas of the lower uterine segment, one case was positive for HPV 16 DNA while 2 were positive for HPV 18 DNA. Overexpression of p53 was seen in 8 (53%) carcinomas of the lower uterine segment. Two of 3 HPV DNA positive carcinomas of the lower uterine segment revealed p53 overexpression. HPV DNA was not detected in endometrial carcinomas and p53 overexpression was shown in 12 (40%) cases. The association between HPV DNA and p53 overexpression differs among the tumors arising in the cervix, lower uterine segment and endometrium.

Simplified model of krypton laser-induced thrombotic distal middle cerebral artery occlusion in spontaneously hypertensive rats.

BACKGROUND AND PURPOSE: The effects of thrombotic occlusion of the middle cerebral artery on compromised ischemic tissue may be different from and more severe than those of cerebral ischemia induced by mechanical occlusion of the artery. Photothrombosis, which is based on photochemical damage to the endothelium and subsequent platelet aggregation, is an efficient method to induce thrombosis in vivo. This study aimed to improve and simplify this unique method for an ischemia model of middle cerebral artery occlusion in rats. METHODS: Male spontaneously hypertensive rats (5 to 6 months old, 300 to 450 g) were anesthetized with halothane, endotracheally intubated, and mechanically ventilated. A krypton laser operating at 568 nm was used to irradiate the exposed distal middle cerebral artery with an intact dura above the rhinal fissure. The photosensitizing dye rose bengal (20 mg/kg body wt) was administered intravenously over 90 seconds starting simultaneously with 4 minutes of laser irradiation at a power of 20 mW to cause thrombotic occlusion of this artery. RESULTS: The irradiated middle cerebral artery was completely occluded by intraluminal thrombi within 3 minutes after simultaneous laser irradiation and rose bengal infusion. Thrombosed materials were not stained by phosphotungstic acid-hematoxylin stain (ie, aggregated platelets lacked apparent fibrin). The mean volume of 3-day-old infarction, indicated by the lack of staining with 2,3,5-triphenyltetrazolium chloride, was 84.8 +/- 17.4 mm3 (mean +/- SD, n = 6). CONCLUSIONS: We demonstrated a reproducible and minimally traumatic model of brain infarction induced by the thrombotic distal middle cerebral artery occlusion in rats.

Cytologic changes in two cervical carcinoma cell lines after transfection of the wild-type p53 gene.

BACKGROUND: To examine morphological changes in cervical carcinoma cells after induction of overexpression of wild-type (wt) p53. METHOD: A dexamethazone-inducible wt-p53 cDNA was introduced into two cervical carcinoma cell lines (TMCC-1 and ME180) and morphological changes were examined under a phase contrast microscope and following Papanicolaou staining. RESULTS: TMCC-1 clones obtained by transfection with wt-p53 gene had an altered morphology especially after induction of p53 expression by treatment with dexamethazone. These changes were characterized by cell flattening, multinucleation, micronucleation, and huge giant cells, yet the parental TMCC-1 had none of these morphological changes even after treatment with dexamethazone. Similar cellular changes were observed in ME180 clones obtained by transfection of wt-p53 gene. CONCLUSION: On the basis of our observations, we propose that the growth inhibition induced by expression of wt-p53 in these cervical carcinoma cell lines carrying HPV DNA sequences is not the result of G1 arrest but rather relates to a blockade in the M phase of the cell cycle.

Do human papillomaviruses have a role in the pathogenesis of bladder carcinoma?

PURPOSE: Since little is known of the associations between bladder carcinoma and human papillomaviruses (HPVs), data on the role of HPV in bladder carcinogenesis are controversial. We attempted to clarify whether HPVs are present in bladder carcinomas. MATERIALS AND METHODS: We examined 36 specimens of bladder carcinoma for HPV positivity by the polymerase chain reaction method. RESULTS: HPV-16 deoxyribonucleic acid was detected in 1 specimen (3%) of a transitional cell carcinoma from a 37-year-old woman who had concomitant squamous cell carcinoma of the uterine cervix with positive para-aortic lymph node metastasis. The cervical tumor, bladder tumor and para-aortic lymph node metastasis were all positive for the same type of HPV. CONCLUSIONS: On the basis of this low rate of HPV detection (3%), HPVs are not likely to have a prominent role in carcinogenesis of the bladder.

[Significance of high normal blood pressure in occupational health care].

In order to elucidate the significance of high normal blood pressure in occupational health care, a 5 year follow up survey was performed on 874 men with normal blood pressure and 225 men with high normal blood pressure. The major results of this survey were as follows. 1) Sixty-seven out of 874 with normal blood pressure (7.7%), and 78 out of 225 with high normal blood pressure (34.6%) became hypertensive in 5 years. 2) High normals started with a higher frequency of obesity, hypercholesterolemia, hyper gamma-Glutamyl transpeptidasemia compared with normal blood pressure at the start of the survey. 3) Logistic regression analysis showed that age, SBP, DBP were risk factors of developing hypertension from normal blood pressure, while only the amount of alcohol drinking applied for high normal blood pressure. 4) Logistic regression showed that high normal blood pressure, drinking, obesity and were significantly related to the development of hypertension. These results suggested that interventive activities for high normal blood pressure should be included in occupational health care because of a high tendency of underlying poor life style and a high risk of getting hypertension.

[A case-control study of ovarian cancer to identify its risk factors].

To analyze the association between the potential risk factors and ovarian cancer, we conducted a case-control study from October, 1994 to January, 1996 in northern Kyushu. We interviewed 78 patients whose ovarian cancer was histologically confirmed, and 346 controls, selected from women at mass screening, who had not ever suffered from a cancer or an ovarian disorder. An odds ratio (OR) and its 95% confidence interval (95% CI) were estimated by the conditional logistic regression method. As a result, it was found that the risk was significantly increased for a family history of ovarian cancer in a mother or a sister (OR = 2.85, 95% CI 1.01-8.08), for heavier maximum body weight in the past (trend, OR = 1.31, 95% CI 1.06-1.63), and for larger maximum body mass index (trend, OR = 1.30, 95% CI 1.06-1.60). Conversely, the risk was significantly decreased with the number of parities (trend, OR = 0.57, 95% CI 0.39-0.83), and with experience in having ever taken oral contraceptive pills (OR could not be calculated, p < 0.05). The positive relationship of maximum body weight or maximum body mass index to the ovarian cancer risk may in part explain the recent increase in the incidence of ovarian cancer in Japan.

[Recurrent aseptic meningitis caused by rupture of a pineal cyst].

A 40-year-old woman experienced three attacks of aseptic meningitis in 19 months. Each attack was characterized by sudden onset of meningeal irritation followed by spontaneous remission in several days. The patient was free of neurological symptoms between the attacks. In the acute phase of each attack, her cerebrospinal fluid (CSF) showed mononuclear pleocytosis and increased protein concentration. Epithelial cell clusters in the CSF were evident during the third episode. No pathogenic microorganisms were identified in CSF or blood cultures. Brain imaging revealed a pineal cyst, 1 cm in diameter, and we suspected that the epithelial cell clusters represented the ruptured cystic wall and that the recurrent aseptic meningitis was triggered by spontaneous rupture of the cyst. Recently several new etiologies for the benign recurrent aseptic meningitis (Mollaret's meningitis) have been reported, but its existence as a clinical entity is still in dispute. Our case appears to support "spontaneous rupture of central nervous system epidermoid cysts" as one of the etiologies of recurrent aseptic meningitis.

The alpha 1-adrenoceptor antagonist, doxazosin, modulates the lower limit of autoregulation of cerebral blood flow during hemorrhagic hypotension in anesthetized hypertensive rats.

The objective of the present study was to examine the effects of administration of an alpha 1-adrenoceptor antagonist, doxazosin, for 7 days on cerebral blood flow and the autoregulatory response to hypotension in anesthetized spontaneously hypertensive rats. We determined the cerebral blood flow at rest and during hemorrhagic hypotension in 14 rats of each group using laser-Doppler flowmetry, and at the same time, the absolute baseline cerebral blood flow values at the parietal cortex were also quantified in some of the rats with the hydrogen clearance method. Baseline mean arterial pressure was significantly lowered, by 41 mm Hg, in the doxazosin-treated group, while the baseline cerebral blood flow was 31 +/- 4 ml/100 g/min(mean +/- S.D.) which was almost the same as the 32 +/- 5 ml/100 G/min in the control group. The lower limits of cerebral blood flow autoregulation were 139 +/- 9 mm Hg in the control group and 96 +/- 12 mm Hg in the treated group; the difference was significant (P < 0.001). The present results demonstrated that the lower limit of cerebral blood flow autoregulation shifts to a lower level after chronic treatment with doxazosin, an effect which is favorable for the maintenance of cerebral blood flow under hypotensive conditions.

Simultaneous detection by consensus multiplex PCR of high- and low-risk and other types of human papilloma virus in clinical samples.

A consensus multiplex PCR (CM-PCR) technique was developed to detect high-risk (HPV 16/18), low-risk (HPV 6/11), and over 40 other types of human papillomavirus (HPV), separately but simultaneously, by mixing three pairs of consensus primers in the same PCR mixture, for gene amplification. Simultaneous detection of three groups of HPV DNA provides valuable information for clinical practice and this procedure is simple and convenient for routine laboratory examinations. We detected HPV DNA sequences in plasmid HPV DNA and DNA extracted from tissues of condyloma acuminata and cervical carcinoma and from exfoliated cells of the lower genital tract of healthy Chinese women living in the People's Republic of China. We confirmed that this simple, convenient, and cost-beneficial CM-PCR technique is reliable for the detection of HPV DNA sequences.
