[The clinical value of urinary polyamine analysis in urological disease].

To study the clinical usefulness of the determination of urinary polyamine levels, voluntary urine of several urological diseases including 56 bladder tumor patients was analyzed by high performance liquid chromatography. The obtained values were adjusted by the concentration of urinary creatinine and expressed as the unit of mumol/g creatinine (mumol/g Cr) From the measurement of 8 normal adults, the normal upper limit of each polyamine was decided by mean + 2SD, and the limit for total polyamine was 59.1 mumol/g Cr, putrescine 38.1 mumol/g Cr, spermidine 16.6 mumol/g Cr and spermine 9.2 mumol/g Cr, respectively. In the patients with non-neoplastic benign urological disease, the polyamine levels were statistically not different from those of the normal adults. In the case of bladder tumor, the urinary levels of total polyamine, putrescine and spermine were significantly elevated compared with the control group. The true positive rate of this determination in bladder tumor patients was 26/56 (46%) by total polyamine level, 21/56 (38%), by putrescine level, 11/56 (56%) by spermidine level and 16/56 (29%) by spermine level. Grade or stage of the bladder tumor did not have any significant correlation with the urinary polyamine level. This determination would not be included in routine clinical examinations due to the difficulty of measurement, difference of urine sampling and lack of high sensitivity and specificity.

Reduction of human and rat testicular follicle stimulating hormone receptors by human menopausal gonadotrophin in vivo and in vitro.

Reduction of human and rat testicular FSH receptors by hMG was studied in vivo and in vitro. After a single injection of hMG, the number of high affinity FSH receptors were significantly reduced for 5 and 3 d in human and rat testes, respectively, without affecting their affinity. FSH receptor numbers recovered to pretreatment values by 14 d after the injection. Radioactivity in rat testes found 48 h after a s.c. injection of 125I-labelled human FSH was less than 10% of the maximum found 10 h after the injection, showing that the prolonged reduction of FSH receptors after hMG injection was not due to occupancy of the binding sites. Occupied FSH receptors measured in rat testes accounted for about 15% of all receptors on the day after an injection of hMG and for less than 5% after the third day. In experiments in vitro using organ culture techniques, an exposure to hMG for 24 h induced a dose-related significant loss of the specific FSH binding sites for 7 and 5 d in human and rat testes, respectively. Thereafter, the loss was gradually recovered. These findings suggest that the reduction in FSH receptors in human and rat testes by hMG is mainly due to down-regulation of the binding sites and that the testicular organ culture method used in the present study is useful to study hormonal regulation of testicular function, especially in human testes.

Maximum electrical stimulation for urge incontinence.

Urge incontinence was controlled in 13 (62%) of 21 patients by maximum electrical stimulation which was applied to the anus or the perianal skin. The first session of maximum electrical stimulation was able to determine if this treatment would be successful. This method of patient selection for further stimulation was simple and reliable and achieved clinical success in all of 13 selected patients.

Effect of estrogen treatment on testicular gonadotropin receptors in prostatic cancer patients.

Effects of estrogen treatment on testicular human chorionic gonadotropin (hCG) and follicle-stimulating hormone (FSH) receptors in man were investigated. Ten patients (aged 58 to 75, mean 67 years) with prostatic cancer were treated with diethylstilbestrol diphosphate (DESDP) (300 mg daily, oral administration). They were divided into two groups: 5 of them (aged 58 to 74, mean 67 years) were orchiectomized after 7 days of treatment and the remainder (aged 60-75, mean 67 years) after 6 months of treatment. hCG and FSH receptors in the resected testes of each group were measured and compared with those of age-matched prostatic cancer patients without any treatment (controls). After 7 days and 6 months of DESDP treatment, the number of hCG receptors decreased to approximately 53% and 24%, respectively, of that of the controls. FSH receptors in the testes of the patients treated with DESDP did not differ significantly from those of the controls.

[A case of invasive squamous carcinoma of the urinary bladder perforated intraperitoneally after intra-arterial chemotherapy].

We report a case of invasive squamous cell carcinoma of the urinary bladder, which was perforated intraperitoneally after intra-arterial chemotherapy. The patient was a 67-year-old woman complaining of macrohematuria and body weight loss. After thorough examination, the diagnosis of clinical stage C of squamous cell carcinoma of urinary bladder was made. At first, balloon occluded intra-arterial infusion with 80 mg, cis-dichlorodiamine platinum 10 mg mitomycin C and 1 mg vincristine was performed, followed by radiotherapy and 15 mg bleomycin injection. On the 5th day, the patient fell into shock with abdominal expansion, and bladder perforation into peritoneal cavity was confirmed by rapid cystography. Emergency cystohysterectomy and drainage were performed. Resected bladder had a large defect on its posterior wall, and tumor had fallen into severe necrosis. Since January, 1986 we performed balloon occluded intra-arterial infusion of antineoplastic drugs for 3 other cases. We obtained a partial response for 2 of these cases. The outcome of intra-arterial chemotherapy in our hospital is also reported.

[Renal oncocytoma associated with tuberous sclerosis: report of a case].

A case of a renal oncocytoma associated with tuberous sclerosis in a 31-year-old woman is reported. She displayed many characteristic skin features of tuberous sclerosis but no central nervous symptoms. Although she complained of dysmenorrhea and hypermenorrhea, she had no complaints related to the kidney. A left renal tumor was discovered incidentally during preoperative examination for gynecologic disease and left nephrectomy was performed. Histologically, the tumor was shown to be a typical renal oncocytoma. This is supposed to be the first case of renal oncocytoma associated with tuberous sclerosis in Japan.

[A case of solitary pelvic kidney with vesicoureteral reflux and neurogenic bladder dysfunction].

A case of solitary pelvic kidney with neurogenic bladder dysfunction with vesicoureteral reflux is presented. The patient was a 15-year-old boy with sacral vertebral dysplasia and hare-lip, and he has been complaining of recurrent fever episodes and urinary incontinence since 11 years old. Renal anomaly was confirmed by DIP, CT and angiography, and grade IV vesicoureteral reflux was demonstrated by voiding cystourethrography. On cystometrography, low compliance bladder which had a 70 ml capacity on first desire to void and 90 ml capacity on maximum desire to void was observed. Electromyography of anal sphincter performed with uroflowmetry revealed no relaxation of external sphincter during voiding. To preserve renal function, antireflux surgery was performed by Cohen's method, and a successful result, that is cessation of reflux and no ureteral obstruction, was achieved. After operation, periodic transcutaneous electrical stimulation were applied to the pudendal nerve, as a result bladder capacity increased to 150 ml and dysuria with incontinence improved.

[Reifenstein's syndrome: case report of two siblings].

Two cases of penoscrotal hypospadias and gynecomastias in a 14-year-old and 16-year-old, who were siblings are reported. These cases, with rather small testes and 46, XY karyotypes, were classified as familial incomplete male pseudohermaphroditism, type 1, so called Reifenstein's syndrome. Their serum testosterone and estrogen levels were high, but the luteinizing hormone (LH) and follicle-stimulating hormone levels were within normal limits. LH-releasing hormone tests revealed good response of LH. Urethrographies showed no prostatic utricles. They had urethroplasties at the ages of 5 and 6. Anti-estrogen therapy had some effect on the gynecomastias.

Transrectal longitudinal ultrasonography of prostatic abscess.

A definite diagnosis of prostatic abscess sometimes is difficult to make. We report 2 cases of prostatic abscess diagnosed with the aid of transrectal longitudinal ultrasonography by electronic linear scanning. Transperineal aspiration of the abscesses was performed easily and correctly with this echographic technique.

[A long ureteral polyp with ureteropelvic junction obstruction: report of a case].

A case of long ureteral polyp with ureteropelvic junction obstruction in a 29-year-old man is reported. He had complained of occasional left flank pain since several years earlier. Excretory urography showed left giant hydronephrosis and no urolithiasis. Retrograde pyelography revealed a mass lesion of the left upper ureter. At exploration, the left ureteropelvic junction was found to be obstructed by an aberrant lower pole renal vessel. At just distal of the junction, a finger-like ureteral polyp, 5 cm in maximal length, was present and accompanied with several little polyps. Partial ureterectomy and pyeloplasty improved the left hydronephrosis. The patient remains symptomless for one year after the operation.

Sclerosing lipogranuloma of the penis: chemical analysis of lipid from the lesional tissue.

The origin of paraffin from penile granuloma tissue was determined first by field desorption mass spectrometry. A case of penile granuloma with histopathological findings of sclerosing lipogranuloma is presented. Chemical composition of the lesional lipid by elemental analysis and infrared absorption spectroscopy revealed paraffin hydrocarbon. Field desorption mass spectrometric analysis showed the paraffin to be a mixture of saturated hydrocarbons containing substantially 24 to 40 carbon atoms. On the basis of the chemical structure of naturally occurring lipids we concluded that this paraffin was exogenous and not endogenous material that was produced in the tissue following lipid degeneration.

[A case of Werner's syndrome with neurogenic bladder].

The present report is Werner's-syndrome associated with a neurogenic bladder. The patient visited our clinic with the chief complaint of retention of urine and urinary tract infections. The diagnosis of Werner's syndrome was made because the patient had such clinical features as bird-like or "masked" face, short stature, graying of the hair in youth, cataracts in youth, skin changes like scleroderma principally on limbs, early menopause. After trans-urethral-resection of bladder neck and administration of distigmine bromide, her bladder function was improved, she could urinate by herself and she was free from urinary tract infections.

[Management of vesicoureteral reflux in children with neurogenic bladder--retrospective studies of antireflux surgery].

During the last 3.5 years, 18 children and 9 adults with vesicoureteral reflux and neurogenic bladder dysfunction were seen at our hospital, and 11 children and 4 adults required antireflux surgery. The criteria of repair were progressive renal deterioration and/or uncontrollable urinary infection. Surgery was applied mainly to the conditions of detrusor hyperreflexia and/or lower urinary tract obstruction. The combined technique of Politano-Leadbetter and Glenn-Anderson was used in 8 children and 3 adults, and the Cohen technique was used in 3 children and 1 adult. Surgery was successful that is, cessation of reflux and no obstruction, was achieved in all but one patient, who had received radical hysterectomy previously. Though recurrent episodes of lower urinary tract infection persisted in some cases after operation, over-all usage of antibiotics was reduced. Seven children with reflux and uninhibited bladder, though some of them with high grade reflux, were managed successfully by anticholinergic drug or transcutaneous electrical stimulation.

[Results of the Burch colposuspension operation for female stress incontinence].

Of 48 patients with radiologically proven cystocele, 13 patients were treated with the Burch colposuspension procedure from October, 1980 to August, 1984 in our clinic. A satisfactory result with good urinary control was obtained in 13 cases and the procedure was a failure in one. Partial or complete urinary retention was evident postoperatively in 8 patients (62%), but all of the patients voided satisfactorily within 4 weeks. Radiological diagnosis of the cystocele and advantage of Burch colposuspension procedure are discussed.
