The just provision of health care: a reply to Elizabeth Telfer.

Dr Hillel Steiner in this reply to Elizabeth Telfer takes each of her arguments for different arrangements of a health service and examines them--'four positions which can be located on a linear ideological spectrum'--and adds a fifth which could have the effect of 'turning the alleged linear spectrum into a circle'. Underlying both Elizabeth Telfer's article and Dr Steiner's reply, the base is inescapably a 'political' one, but cannot be abandoned in favour of purely philosophical concepts. Whatever the attitude of mind of the reader of these two papers to the provision of a health service, the stimulus to more careful assessments of our own National Health Service and its problems can only be good.

[Clinical trial of the antibacterial combination sulfamoxole/trimethoprim (CN 3123)/Fourth communication: Clinical results with CN 3123 in infections of the gastrointestinal tract, the skin and gynecological adnexes (author's transl)].

The antibacterial activity of N1-(4,5-dimethyl-2-oxazolyl)-sulfanilamide (sulfamoxole) and 2,4-diamino-5-(3,4,5-trimethoxy-benzyl)-pyrimidine (trimethoprim) (CN 3123, Nevin, Supristol) was studied in various bacterial infections in the fields of internal medicine, dermatology and gynecology. It was found that: 1. CN 3123 exhibits antibacterial activity in bacterial infections of the intestinal tract, particularly in salmonellosis and shigellosis. 2. CN 3123 is effective against bacterial infections of the female genital organs. 3. CN 3123 is very effective in patients with bacterial infections of the skin. 4. The antibacterial activity of CN 3123 in these indications is comparable to its action on bacterial infections of the urinary organs and respiratory tract and in ENT infections.

[Comparison of labour induction with intravenous prostaglandin E2 and intravenous oxytocin (author's transl)].

Based on our results we are able to attribute to the PGE2 a special importance during the beginning of cervical dilatation. A cervical dilatation up to 3 cm is faster achieved by cervical relaxation effect and a direct effect on myometrial tissue independant from exterior parameters. The earlier the cervix has reached such a dilatation, the earlier regular contractions are registered at inductions with PGE2. These contractions are furthermore more effective and led to a shorter induction delivery interval under a minimal physiologic stress. This procedure is combined to a lower intrauterine pressure and shows no negative sideeffects on child and mother.

The catalytic mechanism of carbonic anhydrase. Hydrogen-isotope effects on the kinetic parameters of the human C isoenzyme.

1. The steady-state kinetics of the interconversion of CO2 and HCO3 catalyzed by human carbonic anhydrase C was studied using 1H2O and 2H2O as solvents. The pH-independent parts of the parameters k(cat) and Km are 3-4 times larger in 1H2O than in 2H2O for both directions of the reaction, while the ratios k(cat)/Km show much smaller isotope effects. With either CO2 or HCO3 as substrate the major pH dependence is observed in k(cat), while Km appears independent of pH. The pKa value characterizing the pH-rate profiles is approximately 0.5 unit larger in 2H2O than in 1H2O. 2. The hydrolysis of p-nitrophenyl acetate catalyzed by human carbonic anhudrase C is approximately 35% faster in 2H2O than in 1H2O. In both solvents the pKa values of the pH-rate profiles are similar to those observed for the CO2-HCO3 interconversion. 3. It is tentatively proposed that the rate-limiting step at saturating concentrations of CO2 or HCO3 is an intramolecular proton transfer between two ionizing groups in the active site. It cannot be decided whether the transformation between enzyme-bound CO2 and HCO3 involves a proton trnasfer or not.

Perinatal cardiac arrest. Quality of the survivors.

Steiner, H., and Neligan, G. (1975). Archives of Disease in Childhood, 50, 696. Perinatal cardiac arrest: quality of the survivors. Twenty-two consecutive survivors of perinatal cardiac arrest have been followed to a mean age of 4 1/4 years, using methods of neurological and developmental assessment appropriate to their ages. 4 showed evidence of gross, diffuse brain-damage (2 of these died before the age of 3 years). These were the only 4 survivors of the first month of life who took more than 30 minutes to establish regular, active respiration after their heartbeat had been restored. The arrest in these cases had occurred during or within 15 minutes of delivery, and followed antepartum haemorrhage, breech delivery, or prolapsed cord. The remaining 18 were free of any evidence of brain damage. In the majority of these the arrest had occurred during shoulder dystocia or exchange transfusion, or was unexplained; the heartbeat had been restored within 5 minutes in most cases, and regular, active respiration had been established within 30 minutes thereafter in all cases.
