Evolutionary conservation in various mammalian species of the human proliferation-associated epitope recognized by the Ki-67 monoclonal antibody.

The human proliferation-associated epitope recognized by the Ki-67 monoclonal antibody (MAb) was detected in proliferating normal and neoplastic cells of many mammalian species (lamb, calf, dog, rabbit, rat) besides human. In contrast, Ki-67 stained proliferating cells from other species weakly (mouse) or not at all (swine, cat, chicken, pigeon). The immunostaining pattern of Ki-67 in animal tissues was identical to that previously described in human: Ki-67 reacted only with cells known to proliferate (e.g., germinal center cells, cortical thymocytes) but not with resting cells (e.g., hepatocytes, brain cells, renal cells); this MAb produced a characteristic nuclear staining pattern (e.g., stronger labeling of nucleoli than of the rest of the nuclei and staining of chromosomes in mitotic figures); and Ki-67 crossreacted with the squamous epithelium in both animal and human tissues. In vitro studies showed that when quiescent (Ki-67-negative) NIH 3T3 fibroblasts or bovine peripheral blood lymphocytes were induced to proliferate, the appearance of Ki-67-positive cells paralleled the induction of cell proliferation caused by addition of fetal calf serum or PHA, respectively, to the cultures, and in both human and rat proliferating cells the Ki-67 expression closely paralleled the incorporation of [3H]-thymidine. These findings indicate that the epitope recognized by the Ki-67 MAb in human and animal species is the same. The widespread evolutionary conservation of the human proliferation-associated epitope recognized by the Ki-67 MAb suggests that it and/or its carrier molecule may play an important role in regulation of cell proliferation.

Cellular localization of laminin gene transcripts in normal and fibrotic human liver.

We have attempted to determine the cell types producing laminin in normal and fibrotic human liver by in situ hybridization to laminin B1 chain gene transcripts with [35S]-labeled RNA probes. In normal liver laminin transcripts were detected in vascular endothelial, in bile duct epithelial, and in portal tract mesenchymal cells as well as in perisinusoidal cells of the lobule. In fibrotic liver, expression of laminin RNA was increased in proliferating bile duct epithelial cells, stromal cells of fibrotic septa, and in perisinusoidal cells of the regenerating nodules. Although recently suggested as a major source of laminin, hepatocytes in none of our samples showed evidence for significant laminin B1 gene transcription. Thus laminin synthesis does not appear to be a function of hepatocytes, but rather of mesenchymal, endothelial, and bile duct epithelial cells in the liver. This pattern of laminin expression underlines the importance of the mesenchyme in conditioning the function and differentiation of the hepatic parenchymal compartment.

BER-H2: a new anti-Ki-1 (CD30) monoclonal antibody directed at a formol-resistant epitope.

The production and characterization of a monoclonal antibody (MoAb) designated Ber-H2, directed against a new epitope of the Ki-1 (CD30) antigen, are described. In comparison with the formerly reported Ki-1 MoAb whose reactivity with Hodgkin and Reed-Sternberg (H-RS) cells in frozen tissue sections is well-documented, the Ber-H2 MoAb showed new, important features: the labeling intensity of the Ber-H2 MoAb was much stronger, and the number of positively labeled cells was higher. Most important, however, was that the Ber-H2 MoAb could be applied in routinely processed, formaldehyde-fixed, paraffin-embedded tissue sections. Therefore, it was possible to investigate an unprecedented number of tumors received as frozen or formaldehyde-fixed material for expression of the CD30 antigen. Beside Hodgkin's disease, the Ber-H2 MoAb labeled a variable number of cells in lymphomatoid papulosis, peripheral T-cell lymphomas, and angoimmunoblastic lymphadenopathy. Among B-cell non-Hodgkin's lymphomas (NHLs), some cases containing large centroblast-like or immunoblast-like cells or displaying plasma-cellular differentiation were positive. This finding was in keeping with the reactivity of the Ber-H2 MoAb with activated B-cell blasts and a subpopulation of plasma cells in paraffin sections of normal lymphoid tissue. The diagnostic value of the Ber-H2 MoAb was most significant for a group of anaplastic large-cell (ALC) lymphomas (formerly frequently referred to as malignant histiocytosis or regressive atypical histiocytosis), of which more than 50 cases could be investigated, owing to applicability in paraffin sections. Although about one third of these ALC lymphomas did not express the leukocyte common (CD45) antigen, they were consistently reactive with the Ber-H2 MoAb in both frozen and paraffin-embedded tissue sections. Using the Ber-H2 MoAb, these Ki-1 lymphomas could be easily distinguished from other nonlymphoid anaplastic large-cell tumors.

Bone and joint infections due to Pseudomonas aeruginosa: clinical aspects and treatment.

The occurrence of nine cases of Pseudomonas septic arthritis and osteomyelitis within a 12-month period may be considered an uncommon experience even in a large, tertiary referral center. These cases are reported here and the various clinical patterns and therapeutic implications associated with Pseudomonas bone infections are discussed. Epidemiologic serotyping of the isolated strains indicated that they do not represent a single nosocomial outbreak.

Lower limb pain and disability following strenuous activity.

One hundred five military recruits were evaluated in our outpatient clinic for pain in the lower limb following prolonged sustained physical effort and strain. Plain x-ray investigations augmented by technetium-99m polyphosphate demonstrated the presence of 80 stress fractures in 52.4% of the patients. None of the patients had any anatomical abnormalities of the lower limbs. Of the stress fractures seen, 82.5% were located in the tibial and femoral diaphyses and 16.2% were in the small bones of the foot. Plain radiography gave false negative results in 87.3% of documented fractures. The remaining 47.6% of these recruits had no evidence of stress fractures. However, 74% of them had various anatomical deformities of the lower limb, mainly of the feet. Correction of these deformities enabled these patients to return to previous strenuous activities of their basic military training. The pathogenesis of stress fractures is still not understood. However, the development of persistent pain in the lower limb following prolonged sustained intensive physical activity needs further investigation. Anatomical deformities may be associated with development of stress fractures.

Detection of viral genomes in the liver by in situ hybridization using 35S-, bromodeoxyuridine-, and biotin-labeled probes.

Methods employing 35S-, biotin-, and bromodeoxyuridine (BrdUrd)-labeled DNA probes were compared for the detection of hepatitis B virus (HBV) and cytomegalovirus (CMV) in the liver. The results demonstrate that: 1) HBV can be detected reliably only by the use of radiolabeled probes, whereas methods employing nonradioactive probes obviously are not sensitive enough for this virus. The use of 35S-labeled probes shortens the exposure times considerably in comparison to tritiated probes. 2) Biotin-labeled probes are of limited value for in situ hybridization on liver tissues because the presence of endogenous avidin-binding activity often leads to false positive results. 3) Brd-Urd-labeled probes are a useful alternative to biotinylated probes for the detection of CMV. In comparison with biotinylated probes, BrdUrd-labeled probes produce a specific signal of similar staining intensity in the absence of background staining in the liver.

Primary and secondary cutaneous Ki-1+ (CD30+) anaplastic large cell lymphomas. Morphologic, immunohistologic, and clinical-characteristics.

Skin biopsies were collected from 15 patients with cutaneous tumors morphologically similar to Ki-1+ anaplastic large cell (ALC) lymphoma of the lymph nodes, including Ki-1+ ALC lymphoma of childhood. The histology and immunophenotype of these cutaneous tumors are reported and follow-up data on the patients are given. The tumorous infiltrates were composed of large, sometimes very bizarre cells with one nucleolus or multiple nucleoli. All tumor cells expressed the lymphoid cell activation antigen Ki-1 (CD30) in conjunction with CD25 and the beta-chain of the T cell receptor. In 11 patients, Ki-1+ cutaneous tumors developed primarily in the skin. In nine patients, these were restricted to the skin in follow-up periods ranging from 3 months to 6 years. Two patients developed lymph node involvement after 2 months and 2 years, indicating the spreading potential of these cutaneous tumors. Morphologic and immunophenotypical identity of the atypical cells found in primary cutaneous ALC lymphoma, in regressing atypical histiocytosis (RAH), and in lymphomatoid papulosis (LyP) of type A, together with the protracted clinical course in all three conditions, suggests that primary cutaneous ALC lymphoma, RAH, and LyP of type A represent clinical variants of the same lymphoma entity. Secondary development of Ki-1+ ALC skin tumors was observed in two patients with cutaneous T cell lymphomas of mycosis fungoides type. These secondary Ki-1+ ALC lymphomas of the skin showed rapid systemic progression similar to the primary lymphonodal Ki-1+ ALC lymphomas. Concomitant or subsequent occurrence of Ki-1+ ALC tumors in cutaneous T cell lymphomas thus may be a bad prognostic sign.

Isolation and antigenic profile of follicular dendritic cells.

Follicular dendritic cells (FDC) were isolated from human tonsils and adenoids. With a simple modification of an isolation method described previously, FDC could be enriched up to 50%. The isolated FDC were characterized immunocytochemically. In contrast to previous studies, CD4 antigen was not detected on freed FDC, thereby suggesting that the CD4 molecule does not play a role in the infection process of FDC in AIDS, as has been demonstrated for T helper cells. Fc receptors could not be found on FDC by the panel of monoclonal antibodies applied, whereas C3 receptors were observed in abundance. Therefore, antigen trapping in germinal centres most probably involves only C3 receptors. This concept is in contrast with the current one (also developed on isolated cells), which postulates the involvement of Fc receptors. The extent to which C3 receptors are involved in the pathogenesis of AIDS will be examined in further studies.

Stress fractures of the sacrum following strenuous activity.

Three military recruits developed stress fractures in the sacral wing during rigorous elite basic training. Stress fractures in the sacrum have previously not been reported in a healthy population. Sacrum, like every bone of the pelvis and the lower limb that participates in weight bearing, may be susceptible to stress fracture following sustained excessive physical activity.

Reactivity of T cells in mycosis fungoides exhibiting marked epidermotropism with the monoclonal antibody HML-1 that defines a membrane molecule on human mucosal lymphocytes.

Twenty-eight cases of cutaneous T cell lymphomas of mycosis fungoides type, 8 of which showed a marked epidermotropism, were investigated for their reactivity with the monoclonal antibody HML-1. This reagent selectively recognizes human benign and malignant intestinal T lymphocytes, but not benign or malignant lymph node T cells. In all cases, the majority of nonepithelium-associated T cells present in the corium were HML-1 negative. In the eight cases with marked epidermotropism, the intraepidermal (IED) T cells were HML-1 positive in three cases, and HML-1 negative in the remaining five cases. The HML-1 positive IED T cells tended to be localized preferentially in the basal layers, whereas the HML-1 negative IED T cells were more diffusely distributed throughout all epidermal cell layers or formed intra-epidermal aggregates. These findings provide further evidence that mycosis fungoides is not a single entity, but represents a group of neoplasms originating from different T cell subsets that have in common their tropism to epidermis and/or dermis, and thus have a similar clinical presentation.

[Open reduction and single screw fixation of acromioclavicular dislocation].

Open reduction of acromioclavicular dislocations and fixation with a single screw provides satisfactory alignment, and results in a pain-free pseudarthrosis. No correlation between subjective complaints and objective findings during prolonged follow-up were found. Arthrotic changes or calcification of the ligaments do not influence movement of the joint. It is pointed out that calcification is part of the normal healing process and does not impair function of the shoulder joint.

Identification of a T cell lymphoma category derived from intestinal-mucosa-associated T cells.

2 cases of precursor T cell lymphoma and 37 cases of peripheral T cell lymphoma were investigated for their reactivity with the monoclonal antibody (mAb) HML-1, which recognises human intestinal T lymphocytes but not lymph-node T cells. In all but one of the lymphomas studied, the tumour cells were unreactive with the mAb HML-1. The HML-1+ lymphoma was the only tumour that was primarily localised in the epithelium and lamina propria of the small intestine, and was associated with ulcerative jejunitis and coeliac disease. This result suggests that the HML-1+ lymphoma was derived from intestinal mucosa T lymphocytes and differs from precursor T cell lymphoblastic lymphomas and nodal and cutaneous peripheral T cell lymphomas.

[Endosonographic findings of benign and malignant lesions in the stomach wall. A prospective comparison with conventional imaging study procedures].

The results of endoscopic ultrasonography in the diagnosis and differentiation of benign and malignant lesions of the stomach was prospectively compared with those obtained by upper endoscopy with biopsy, double-contrast radiography and computed tomography in 15 patients with benign and 26 with malignant gastric lesions. The validity of the methods was established by comparing the results obtained with the long-term course observed clinically in conjunction with an endoscopic-histological follow-up or with the findings on histopathological examination of the operative specimen. Precise diagnosis of the lesion was made by endoscopic ultrasonography in about 90% of cases, a result better than that obtained with radiography or computed tomography. Furthermore, endoscopic ultrasonography had good sensitivity for demonstrating perigastric lymph node enlargement; in this respect it was better than computed tomography, but was not able to distinguish whether the enlargement was benign or malignant: this will only become possible with technical improvement of the instruments.

HIV-related malignant lymphomas. A clinical and pathological study of 13 cases.

The clinical and laboratory records of 13 patients with HIV-related lymphomas diagnosed between 1983 and 1987 are reviewed. 8 cases were classified as high-grade Non-Hodgkin-lymphomas (NHL), 4 as low-grade NHL (including 2 plasmocytomas), and one as Hodgkin's disease (HD). 11 of the 12 NHL were of the B-cell lineage. The mean patient's age was 39 years. At initial staging evaluation extranodal disease was present in 10 patients, in one of them as primary central nervous system (CNS) lymphoma; 9 patients presented with stage III or stage IV involvement. 9 patients were eligible for multidrug chemotherapy. In 5 cases an initial complete remission (CR) was observed, followed, however, by CNS relapse within a few months in 3 patients. 3 patients did not respond to chemotherapy and died within 4 to 6 weeks. Of the 9 patients treated chemotherapeutically, 5 died within 1 to 5 months, 1 is still alive in CR and 3 are still alive in partial remission (PR).
