[MESA (microsurgical epididymal sperm aspiration) and IVF (in vitro fertilization). A therapy concept in treatment of male infertility].

Microsurgical epididymal sperm aspiration (MESA), or sperm microaspiration retrieval technique (SMART), in conjunction with in vitro fertilization is a successful therapy in male infertility. From November 1991 to March 1994 a total of 29 attempts at MESA with subsequent IVF were made. Of 48 aspirations, 37 were successful and 13 attempts at IVF were possible, 6 of which were successful with 10 subsequent embryo transfers. In all, 3 pregnancies were achieved and 1 boy was born. In conclusion, microsurgical spermaspiration in conjunction with in vitro fertilization is a way of treating male infertility with a chance of achieving paternity with the partner's own sperm; the chances are probably better with intracytoplasmatic sperm injection.

[The thrombin-antithrombin III complex ELISA test in diagnosis of surgery-induced blood coagulation activation].

In addition to measuring the routine blood coagulation parameters (Quick, PTT, TT, AT III, Fibrinogen, PLT), the Thrombin-Antithrombin III Complex (TAT) Test was performed preoperatively on 20 patients treated because of nodular goiter in the euthyreoid state and ten patients with symptomatic cholecystolithiasis. While there were no changes in the routine blood coagulation parameters, significant alterations of the TAT occurred in both patient groups. This indicates that TAT makes operation-induced activation of the blood coagulation system, which is impossible by just measuring the routine blood coagulation parameters alone, detectable.

Patellar instability: treatment by arthroscopic electrosurgical lateral release.

We reviewed the results of arthroscopic lateral release using electrosurgery in 39 patients (45 knees) with a history of recurrent patellar subluxation or dislocation. The average follow-up time was 28 months (range, 24-36). At follow-up, the patients had decreased swelling, instability, and pain. There was an improvement in flexion activities, sports participation, and overall functional ability. Only 20% of the knees had completely normal physical findings: 11.1% of the knees were rated as excellent; 64.4% were improved; and 24.5% were poor. Dislocators had more frequent poor results. The complication rate was 4.4%. There were no postoperative hemathroses. One patient was considered a surgical failure. The technique yields results comparable with those of open extensor realignment procedures and avoids the complications inherent to lateral release in general. The place for this procedure in patellar instability is well-documented.

Computerized tomography and adenocarcinoma of the esophagus.

A case of adenocarcinoma of the stomach spreading to the esophagus is described. It was well seen on computerized tomography and appeared more ominous than the esophagram. The latter illustrated an appearance simulating narrowing secondary to peptic esophagitis; the former demonstrated a relatively large encompassing mass. The definitive diagnosis was made during endoscopy and confirmed by biopsy. Although computerized tomography is not considered the method of choice for establishing an abnormality in this organ, it can occasionally help. It can usually show the total extent of the tumor better. Additional cases are presented.

Adenocarcinoma

Megakaryocyte polyploidization in May-Hegglin anomaly.

Polyploidization of megakaryocytes was studied in bone marrow aspirates from 3 patients with May-Hegglin anomaly by combined application of cytophotometric determination of the DNA content and autoradiography with 3H-TdR labeling in vitro. A marked elevation of the influx of progenitor cells into the megakaryocytic cell system as well as a decreased maturation capacity from type II to type III megakaryocytes was observed possibly contributing to the pathological platelet sequestration. The polyploidization activity as assessed by 3H-TdR labeling and nuclear DNA content was normal.
