Levels of fusidic acid in skin blister fluid and serum after repeated administration of two dosages (250 and 500 mg).

The fusidic acid steady-state concentrations in serum (S) and skin suction blister fluid (SBF) after oral doses of 250 and 500 mg administered twice daily for 6 days, as film-coated tablets of sodium fusidate, were studied in eight healthy subjects. The mean peak serum concentrations after the 250- and 500- mg regimens were 39 +/- 5 and 102 +/- 11 mg/l, respectively, obtained between 2 and 3 h after drug intake. The corresponding values for SBF, obtained later, between 2 and 12 h after drug intake, were 21 +/- 5 and 79 +/- 11 mg/l. As measured by the ratio of area under the concentration vs. time curve (SBF/S), fusidic acid penetration is 69-75%, whatever the dose. With either regime, the fusidic acid SBF and serum levels exceed the minimal inhibitory concentration of bacteria usually found in skin infections, especially Staphylococcus species. A dose of 250 mg twice a day appears sufficient to treat these infections and could be tested in clinical studies.

Longterm results of ultrasonic irradiation in Meniere's disease.

Thirty-seven patients with Ménière's disease were treated with ultrasonic irradiation to the lateral and superior semicircular canals. In a longterm follow-up study, lasting alleviation of the vertiginous attacks was found in 24 out of 33 patients (72%). In most of the cases, hearing remained unchanged. It is suggested that the therapeutic effect is associated with a change in the endolymphatic drainage dur to histological alterations in the labyrinth produced by ultrasound.

ABO blood groups in unipolar and bipolar manic-depressive patients.

The authors determined ABO blood groupings for 66 manic-depressive patients diagnosed and divided into bipolar and unipolar groups according to strict symptomatic and course criteria. A significantly higher percentage of bipolar patients than unipolar patients had blood group O, while a significantly higher percentage of unipolar patients than bipolar patients had blood group A. THese findings provide support for the validity of the unipolar-bipolar distinction and are consistent with the concept that vulnerability to manic-depressive disorders may be related to membrane disturbances.

The crystal structure of 3-hydroxy-3-isobutyl-2-pyrrolidone-5-carboxylic acid, lactam of 4-hydroxy-4-isobutylglutamic acid from Reseda odorata L.

The relative configuration of 3-hydroxy-3-isobutyl-2-pyrrolidone-5-carboxylic acid has been determined by an X-ray crystal structure analysis. The compound crystallized in space group P212121 with two molecules in the asymmetric unit. a=16.023(3) A, b=19.349(7) A, c=6.9053(16) A. The structure was solved by direct methods using MULTAN and refined by full-matrix least-squares technique to an R of 0.137 for 645 diffractometer-collected intensities. The absolute configurations of the title compounds were deduced; the configuration of the amino acid and the lactam are 2(S), 4(S) and 3(S), 5(S), respectively.

Histology of nasal polyps of different etiology.

The aim of the present study is to elucidate the correlation between etiology and histology in nasal polyps with special regard to cystic fibrosis (CF). Nasal polyps from 15 children with CF and a control group of non-CF polyps from 15 adult patients were examined by light-microscopy. The histological evaluation was carried out on a blind basis in order to avoid bias. Among the parameters used, the tissue eosinophilia proved to be the most valuable factor in the differentiation between CF and non-CF polyps, as only few eosinophils were found in the CF-polyps. The polyp glands were few and generally pathological. Some characteristics abnormalities in the grandular morphology are apparently more common in CF polyps. It is concluded, that the histological examination of nasal polyps is of importance for the correct classification of the patient, but the diagnosis of CF cannot be made based on microscopy of polyps. Further studies including blinded histological examination of nasal polypous tissue might contribute to a more differentiated diagnose of nasal polyposis.

Bacteriology and antibiotics in acute suppurative otitis media.

One hundred and forty-seven patients with acute suppurative otitis media, were divided into three groups and treated with antibiotics (azidocillin, ampicillin, and cephalexin). The therapeutic effect was assessed bacterioloically by swabbing from the aural discharge and from the nasopharynx on the first, second, third, and seventh day after initiation of treatment. In addition, the concentration of antibiotic in the aural discharge and in the nasopharynx was determined. As compared with other published materials, there was a common occurrence of Haemophilus influenzae and S. Aureus. Hemolytic streptococci are less common than prior to the advent of antibiotics. Pneumococci disappeared in all cases from the aural discharge in the course of the first three days. The effect upon Haemophilus was slower. In the nasopharynx the effect was questionable, and no effect was obtained upon other bacteria. The clinical course could not be correlated to the bacterial findings except that resistant bacteria were found in all cases with persisting discharge.

Replica cytology in cancer of the larynx: an evaluation of a replica method in the diagnosis of laryngeal malignancy.

A series of 38 cytological samples from the larynx, obtained with a replica method, is presented. The method is described in detail. Conclusive material was obtained in 34 cases. Fourteen per cent false negative evaluations were found. However, when more strict criteria were used for accepting the cytological specimen as suitable for evaluation, no false negative was encountered. It is therefore concluded that this replica method for obtaining cytological samples from the larynx could become a valuable tool as a screening method during indirect laryngoscopy in search of laryngeal malignancy provided very strict criteria are used for accepting the cytological specimen as suitable for evaluation.
