Angiogenesis Inhibitor O-(Chloroacetyl-carbamoyl) fumagillol (TNP-470) Inhibits Tumor Angiogenesis, Growth and Spontaneous Metastasis of MKL-4 Human Breast Cancer Cells in Female Athymic Nude Mice.

Accumulated knwoledge regarding the important roles played by angiogenesis in solid tumor growth and metastasis has prompted the development of angiogenesis inhibitors for clinical use in cancer therapy. Among these inhibitors, O-(chloroacetyl-carbamoyl) fumagillol (TNP-470) has been reported to have both antitumor and antimetastatic activities in rodent and human tumor models. We recently established a new metastasis model of MKL-4 human breast cancer cells in female nude mice. This cell line is a co-transfectant of the MCF-7 cell line with genes of a potent angiogenic factor, fibroblast growth factor 4, and a genetic marker, bacterial lac ¤ In this paper, we describe the inhibitory effects of TNP-470 on tumor angiogenesis, growth and metastasis in this MKL-4 metastasis model. Subcutaneous injection of 10 or 50 mg/kg of TNP-470 every other day for two weeks obviously inhibited the tumor growth and metastasis of MKL-4 cells in female nude mice. The treatment of TNP-470 at both doses significantly reduced the tumor volumes, respectively, to 28% and 14% of that in the control group(p<0.05 in each comparison). The positive rate of metastasis into the axillary lymph nodes was 100% in the control group, whereas it was only 33% in both of the treatment groups. Distant metastasis into the inguinal lymph nodes, lungs, kidneys and liver also tended to decrease in both of the treatment groups. Immunohistochemical analysis using anti-factor VIII antibody revealed that the number of microvessels in both of the treatment groups was significantly less than that in the control group(p<0.01 in each comparison). To the best of our knowledge, this is the first report describing simultaneous demonstration of the antiangiogenic, antitumor and antimetastatic effects of TNP-470 on human breast cancer cells in nude mice.

Analysis of Factors Associated with Quality of Life in Breast Cancer Patients after Surgery.

The objective of this study was to investigate the factors associated with the quality of life (QOL) in breast cancer patients after surgery. The QOL in 83 primary breast cancer patients after surgery was prospectively assessed using a newly developed Japanese QOL questionnaire: The QOL Questionnaire for Cancer Patients Treated with Anticancer Drugs (QOL-ACD). The demographic and medical factors relating to the QOL score in the subjects were investigated by multiple regression analyses. Factors related to the four categories of the QOL-ACE (activity, physical condition, psychological condition, and social relationship) were also analyzed. The results revealed that only hospitalization had a strong negative relation to the overall QOL score. With regard to the four categories of the QOL-ACE, hospitalization had strong negative relations to the three categories (activity, physical condition and social relationship) and had weak negative relation to the psychological condition. Other demographic and medical factors such as types of surgery or adjuvant therapy had no significant association with the QOL score, except for the performance status which had positive, but not significant, association with the activity. In conclusion, this study yielded useful information concerning the management of the breast cancer patients after surgery. We had better be more concerned with the hospitalized period than other demographic or medical factors such as types of therapy to improve the QOL.

The practice of breast self-examination results in the earlier detection and better clinical course of Japanese women with breast cancer.

Using a questionnaire survey, we analyzed the relationship between the frequency of breast self-examination (BSE) and the clinical stage and course of breast cancer in Japanese patients. BSE had been performed monthly by only 5.4% of the patients (M group), occasionally by 35.4% (O group), and not at all by 59.2% (N group). There was a positive relationship between more frequent BSE and an earlier clinical stage, the percentages of Tis/stage 0 and I for the M, O, and N groups being 83%, 44%, and 36%, respectively (P < 0.05). The mean maximum tumor diameters for the three groups were 1.7 cm, 2.5 cm, and 3.0 cm, respectively. The tumor size in the M and O groups was significantly smaller than that in the N group at P < 0.01 and P < 0.05, respectively. The percentages of patients in the M, O, and N groups who underwent breast-conserving therapy were 42%, 11%, and 19%, respectively, with patients who had performed monthly, BSE more frequently undergoing breast-conserving therapy (P < 0.05). At a median follow-up time of 34 months, 0%, 3.8%, and 7.6% of the patients from the M, O, and N groups, respectively, had died of breast cancer, the overall survival curve of the M group being significantly better than that of the N group (P < 0.01). This retrospective study suggests the positive correlation of BSE frequency with earlier detection, and a more favorable clinical course in Japanese breast cancer patients.

[Recent development of endocrine treatment for breast cancer--new drugs and new treatment methods using tamoxifen].

The characteristics and clinical efficacies of new hormonal drugs and new treatment methods for breast cancer using tamoxifen (TAM) were described. The new anti-estrogens, such as trioxifen, toremifene and droloxifen, have equal or better clinical efficacy than TAM. They showed a high response rate in postmenopausal patients with no previous treatment or estrogen receptor positive tumors. They were occasionally effective for patients who relapsed after TAM treatment. Pure anti-estrogen ICI 164, 384, with fewer estrogenic agonistic properties than TAM, and anti-estrogen TAT-59, which has been developed in Japan, are under clinical investigation. Luteinizing hormone-releasing hormone agonists, such as leuprolide, buserelin, triptrelin and goserelin, are available for premenopausal patients, and showed a 37% response rate on average. The pure aromatase inhibitors, such as 4-hydroxyandrostenedione and CGS 16, 949 A, are available for postmenopausal patients, and showed 30% and 17% response rates on average, respectively. The new pure aromatase inhibitor ZD-1033 is also under clinical investigation. TAM or toremifene as well as the calcium antagonist verapamil were proved to overcome the multiple anticancer drug resistance caused by P-glycoprotein. Clinical studies for breast cancer using high-dose TAM as a potential modulator of drug resistance have already been started in Europe and the United States. Chemoprevention of breast cancer using TAM, which started in the U.S., was described.

Imprint immunocytochemistry of estrogen receptors in breast carcinoma.

To clarify the accuracy of immunocytochemical detection of estrogen receptors (ER) in breast carcinomas using cytological materials, imprint specimens from tumor tissue were compared with frozen tissue sections and tumors analyzed by the dextran coated charcoal (DCC) method and enzyme immunoassay (EIA). Out of 50 cases examined by imprint immunocytochemistry, there were 39 ER positive cases (78.0% positivity). The positivity in the imprint materials agreed with that of the DCC in 36 out of 40 cases (85.0%), with 100% sensitivity and 60.0% specificity. The two methods statistically correlated with each other in their positivity and grade (p less than 0.001). The positivity and grades of imprint and frozen immunocytochemistry as well as those of imprint immunocytochemistry and the EIA agreed almost perfectly with each other. As a result of the present study, we concluded that immunocytochemical detection of ER is indeed reliable, as accurate as other procedures. We recommend that aspiration biopsy cytology (ABC) be used for morphological examination and ER immunocytochemistry when adequate materials are available and that imprint materials be used when ABC materials are inadequate and fresh tissue is available at the time of surgery.

Prognostic value of hormone receptors in breast cancer.

The aim of this study was to establish the role of estrogen receptor (ER) and progesterone receptor (PgR) as prognostic indicators for early recurrence and survival. In all, among breast cancer patients, 166 patients who had undergone radical or extended radical mastectomy were studied. These patients were treated with adjuvant chemotherapy alone for 2-3 years after surgery. No patients had adjuvant endocrine therapy. Local recurrence and/or distant metastases were treated by endocrine therapy and/or chemotherapy. The relapse-free interval was not different between the ER-positive and ER-negative patients. The postrelapse survival curve was significantly different between the two groups. There was no significant difference in the relapse-free interval and the postrelapse survival curve between the PgR-positive and PgR-negative patients. These results suggest that ER is a good predictor of the response to endocrine therapy given after relapse, but not of early recurrence.

Changes in the hormone dependency of DMBA-induced rat mammary tumors with reference to the effect of tamoxifen.

Tamoxifen was administered over a period of 4 weeks to 19 female Sprague-Dawley rats bearing 20 DMBA-induced mammary tumors. Consequently, 13 tumors (65.0 per cent) regressed and showed a reduction in their estradiol receptor (ER) and progesterone receptor (PgR) concentrations. In spite of subsequent treatment given to the rats bearing the regressed tumors, 6 new tumors occurred. The ER and PgR concentrations of these tumors were lower than those of the tumors before tamoxifen therapy, and histochemical study showed these tumors to be composed mostly of ER-negative cells. These results suggest that: (a) the hormone dependency of breast cancer is reduced by tamoxifen therapy; (b) breast cancers recurring during tamoxifen therapy show reduced hormone dependency.

[A case of breast cancer with a chondrosarcomatous appearance].

A case of a breast tumor which showed the appearance of invasive ductal carcinoma and chondrosarcoma is reported. The patient was a 36-year-old female. Simple mastectomy and lymph node dissection were performed for the tumor. Local recurrence and metastases to the contralateral breast, lymph nodes, subcutaneous tissue, and bones arose after the operation. She died of the disease 2 years and 3 months after the operation. It is thought that this tumor had formed as a result of metaplasia of the carcinomatous tissue, since a histological transition between the carcinoma and sarcoma was demonstrated.

[Competitive binding assay with dextran-coated charcoal for measurement of sex hormone-binding globulin in breast cancer].

A simplified method for determination of the binding capacity of the sex hormone binding globulin (SHBG-BC) in human serum and its clinical application are described. This method is based on the estimation of the amount of estradiol (E2) bound with SHBG which possessed binding sites with it. After removing the endogenous steroids from the serum-samples by adsorption with dextran coated charcoal, the value of SHBG-BC increased by 18% compared with that of untreated serum. In 48 patients of various ages with breast cancer, the value of SHBG-BC (34.6 +/- 11.1; mean +/- S.D.), expressed as pico mole per 1 ml serum, did not differ significantly from that of 12 healthy control women (29.6 +/- 6.6). However, in postmenopausal women, the value of SHBG-BC in 24 patients with breast cancer (33.4 +/- 12.2) differed from that of 6 controls (26.1 +/- 4.9; p less than 0.05). In addition, estrogen receptors (ER) were measured by using gel-filtration method in cytosols from the tissue of breast cancer. The value of SHBG-BC in 15 ER-positive cases (40.4 +/- 10.1) was significantly higher than that in 13 ER-negative cases (29.3 +/- 8.7; p less than 0.01), but the value of SHBG-BC and ER did not run parallel completely.

[Clinical study of the measurement of tissue polypeptide antigen in patients with breast cancer].

Tissue polypeptide antigen (TPA) concentrations in patients with breast cancer were measured using a radioimmunoassay (RIA) kit. Although this RIA kit presented some problems in the dilution test of high-concentration samples, it was found to be adequate for clinical use. It was shown that the measurement of serum TPA concentration was useful in monitoring the local recurrence, for the early diagnosis of distant metastasis and for evaluating the therapeutical effect after surgery. TPA concentration showed no correlation with CEA concentration. Therefore, in order to raise the rate of diagnostic success, simultaneous measurement of both TPA and CEA was essential.

[Preservation of the nipple and areola for breast cancer--histopathological study on cancerous involvement of the nipple and areola].

In 141 mastectomy specimens, performed for invasive or non-invasive carcinomas, histopathologic study was performed to assess the extent of nipple-areola involvement by the tumor. In this study, patients were excluded when 1) the tumor was located beneath the areola, 2) nipple and/or areola abnormalities were clinically present. Tumor involvement of the nipple and/or areola was found in 44 of 141 specimens (31%), with intraductal growth in 36 (82%) of 44, stromal invasion in 3 (7%) and ductal & stromal invasion in 5 (11%). Analysis of nipple-areolar involvement with consideration of the different variables indicates that it occurred in association with tumor size, tumor-areola distance and histological type. Such information provides clinically relevant guide lines in decision making for limited breast surgery.

[Stromal sarcoma of the male breast].

A 63-year-old man who had been aware for 5 years of a tumor near his left nipple was admitted complaining of rapid tumor enlargement. He received standard mastectomy under the diagnosis of breast sarcoma. The regional lymph nodes were not involved and the excised tumor was 12.0 cm in diameter it showed a grayish-white cut surface and a sharp border. Histologically we made a diagnosis of stromal sarcoma of the breast showing diffuse proliferation of spindle shaped cells with high cellularity, moderate atypia and scattered mitosis. Postoperatively, he underwent chemotherapy with cyclophosphamide. Two years and five months after the operation, he is alive and well without any sign of recurrence.

[Non-epitherial malignant neoplasms of the breast--a clinicopathological study of 4 cases].

Between 1972 and 1981, 4 women with breast sarcoma underwent treatment at our department; they accounted for 1.1% (4 of 355) of all female breast malignancies. There was one case of malignant lymphoma and liposarcoma each, and 2 cases of malignant cystosarcoma phyllodes. The patients averaged 31.0 years of age. Of the two cases of cystosarcoma phyllodes, one had coexisting lobular carcinoma in situ arising in a fibroadenoma of the contralateral breast, the other had a coexisting lung cancer. Mastectomy was performed in 3 cases, irradiation in one (malignant lymphoma). All patients received concurrent chemotherapy. The regional lymph nodes were not involved, and 2 patients are alive and well without recurrence.

An evaluation of mass screening for breast cancer.

In Tokushima Prefecture, Japan, 41,922 women (26,669 in actual number) were subjected to mass screening with physical examination from 1970-1977. In 42 including 10 of so-called "interval cancer", breast cancer was detected. Those with breast cancer detected by mass screening were in the earlier stage of the disease as compared to those diagnosed through regular medical care at our-patient clinic during the same period. Procedures such as inspection and palpation are readily carried out and early occurrences of breast cancer can be detected.

Therapeutic value of mepitiostane in the treatment of advanced breast cancer.

A new, orally active antiestrogenic steroid, mepitiostane (20 mg/day), was given to 45 patients with advanced breast cancer. The regression rate was 31.1%, or 14 of 45 patients, and a duration of regression of greater than 6 months was obtained in seven patients. Virilizing effects such as hoarseness, hirsutism, and acne were observed relatively often, but there was no evidence of abnormality in the liver function tests or in the serum calcium level.
