Endogenous prostaglandin E2 metabolite levels, renin-angiotensin system and catecholamines versus acute hemodynamic response to captopril in chronic congestive heart failure.

Hemodynamic and hormonal responses to captopril were measured in 10 patients with severe chronic heart failure poorly controlled by digitalis and diuretics. After administration of a 25-mg dose, stroke volume (SV) increased from 53 +/- 7 to 63 +/- 9 ml (p less than 0.05), while pulmonary wedge pressure (PWP) decreased from 20 +/- 2 to 14 +/- 2 mm Hg (p less than 0.01). The hemodynamic changes were associated with increases in plasma renin activity (PRA; p less than 0.05) and in plasma levels of a novel bicyclo-prostaglandin E2 metabolite (bicyclo-PGE-m; p less than 0.01), whereas norepinephrine (NE) showed a falling tendency. In general, basal hemodynamic and basal hormonal levels did not correlate. Captopril-induced changes in mean artery pressure (MAP) and mean pulmonary artery pressure (mPAP) were positively correlated to pre-captopril PRA (r = 0.74, p less than 0.01; r = 0.64, p less than 0.05) and to changes in PRA (r = 0.85, p less than 0.01; r = 0.80, p less than 0.01) with a similar trend for angiotensin II (AII); decreases of systemic vascular resistance were more pronounced in patients with higher control NE levels (r = 0.62, p less than 0.05), the reduction of NE levels being highest in patients with higher basal concentrations (p less than 0.001); the captopril-induced decreases of mPAP and PWP were inversely related to basal bicyclo-PGE-m levels (r = 0.60, p less than 0.05; r = 0.61, p less than 0.05), and changes in mPAP were closely related to basal ratios of AII/bicyclo-PGE-m (r = 0.67, p less than 0.01). Thus, captopril exerts its acute beneficial hemodynamic effect by inhibiting the generation of AII, associated with toning down of sympathetic stimulation and increased production of vasodilating prostaglandins, such as PGE2. The relation between AII and PGE2-counteracting substances-might determine the hemodynamic response to captopril in the patients.

Radiolabelled platelets and prostacyclin in diagnosis and treatment of transplant rejection.

The trapping of 111Indium-oxine labelled autologous platelets by the transplant was recorded by means of a computerised gamma camera and expressed as platelet uptake index (PUI). Eighty-five patients were studied. The PUI increased in acute rejection from 1.13 +/- 0.11 to 1.74 +/- 0.17, and decreased again when rejection was reversed. In chronic rejection PUI was significantly lower than in acute rejection, but still higher than in long term stable grafts. Prostacyclin infusions were given to six cases of acute rejection and 12 of chronic rejection. In the majority of patients an improvement in transplant function and a decrease in platelet trapping could be demonstrated.

[Dose-dependent increase in prostacyclin synthesis from various vascular tissues by dipyridamole].

Due to the more detailed knowledge of prostaglandin metabolism in the recent past increasing interest was focused onto dipyridamole acting as platelet active substance. It was the goal of this study to investigate whether dipyridamole is able to exert any effect on vascular PGI2-synthesis. It is demonstrated that dipyridamole has a dose-dependent (0.1-100 microM) increasing effect on PGI2-synthesis or 6-oxo-PGF1 alpha-levels, respectively (the stable metabolite of prostacyclin). At a dose range of more than 10 microM this increase reaches the level of significance. Assuming that PGI2 acts as a local hormone in the vascular system this effect on PGI2-synthesis stimulation might contribute to the knowledge of the in vivo action of the substance.

Reduced umbilical artery prostacyclin formation in complicated pregnancies.

Prostacyclin synthesis is severely depressed in the umbilical arteries of neonates of mothers with gestational diabetes mellitus, juvenile-onset diabetes mellitus, or preeclamptic symptoms, and of mothers who smoke during pregnancy and diabetic mothers who smoke. The reduced production of prostacyclin might be responsible for an increased abortion rate and a higher rate of intrauterine fetal death because of a decrease in placental perfusion and umbilical blood flow.

[Plasma factors regulating prostacyclin synthesis during pregnancy].

Prostacyclin is the most potent endogenous inhibitor of platelet aggregation known so far, produced by endothelial and smooth muscle cells. This substance is assumed to play a key role in maintaining a sufficient blood flow in maternal and fetal circulation. There is now some evidence that there is a non-cellular component in human blood, which is able to influence the prostacyclin formation to a certain degree, the so-called "prostacyclin synthesis stimulating plasma factor". A change in the plasma factor activity in patients with a disturbance of PGI2 metabolism has been observed. We examined the activity of this plasma factor during pregnancy and one year after delivery. During the first and second trimenon increased values were observed, whereas at the end of pregnancy the values were significantly decreased. The values for plasma factor activity one year after delivery were comparable to those of the control group. In patients with diabetes mellitus decreased activity of the plasma factor was observed, but its course was similar to that seen in normal pregnancy.

[Prostacyclin synthesis in human umbilical vessels].

Umbilical arteries exhibit the highest capacity to synthesize PGI2 of all human blood vessels. This capacity is not only severely suppressed in adults, but also in umbilical arteries in mothers suffering from diseases accompanied by vessel wall alterations such as diabetes mellitus, hypertension and with smoking, for example. In parallel, severe morphological changes of endothelium and smooth muscle cells of umbilical arteries can be observed. It is assumed that a decrease in PGI2 formation might cause a disturbance in local haemodynamics.

[Platelet half-life in patients with primary hyperlipoproteinemia type IIa, IIb, and IV according to Fredrickson with and without clinical signs of atherosclerosis].

It is generally accepted that platelet half-life is shortened in atherosclerotic vascular disease. Concerning changes due to hyperlipoproteinemia (HLP), however, there exist only few data. Therefore, we examined the platelet-half life in 60 patients with recently discovered HLP type IIa, IIb and IV according to Fredrickson before treatment in comparison to 60 controls. 33 of the HLP-patients had no clinical symptoms of angiopathy, 27 patients suffered from peripheral vascular disease or from coronary heart disease as verified by angiography. The labelling of autologous platelets was performed with 100 microCi of 111Indium-oxine-sulfate at 37 degrees C for 5 minutes. The mean labelling efficiency was 90%, the recovery after 2 hours about 70%. Serum lipoproteins were estimated by means of ultracentrifugation and polyanionprecipitation according to Lipid Research Clinic Methods. In the patients with HLP platelet half-life was significantly shortened in comparison to the control group (p less than 0.01). These changes were most pronounced in patients with HLP-type IIa and with atherosclerotic lesions, respectively. In patients with HLP-type IIa a very close correlation could be demonstrated between platelet half-life and LDL-cholesterol (r = -0.72; p less than 0.001) as well as total cholesterol (r = -0.73; p less than 0.001). These data prove that in HLP in-vivo platelet function as measured by platelet survival is significantly influenced even before the occurrence of clinically relevant symptoms of atherosclerosis.

Radiation induced alterations in rabbit aortic prostacyclin formation.

Earlier studies have demonstrated very severe vascular lesions occuring after irradiation in human and experimental animals as well. We examined the rabbit aortic PGI2-formation using the platelet bioassay technique in one year aged rabbits after different irradiation doses and a time course. The findings demonstrate a significant increase in prostacyclin formation, which might be due to the damage of endothelial cells. This stage is followed by a long lasting severe depression. This behaviour in addition demonstrates a dose dependent manner, The findings of a long lasting decrease in vascular PGI2-formation might contribute to the understanding of the high incidence of vascular lesions being found at the site of irradiation.

Hypertension and beneficial treatment with beta-blocking agents does not change the platelet sensitivity to the antiaggregatory prostaglandins.

Recent publications have described increased vascular prostacyclin synthesis in vascular tissue from both spontaneous and experimental hypertensive animals and hypertensive humans. The only paper dealing with the platelet sensitivity reported that the cells are not abnormally sensitive to PGI2 in spontaneously hypertensive rats. In 22 patients with essential hypertension the platelet sensitivity to the antiaggregatory prostaglandins PGI2, PGE1, and PGD2 was studied on admission and two weeks after successful treatment with a beta-blocking agent. In all the age groups and in both sexes no differences in platelet sensitivity could be detected. These findings suggest that increased vascular PGI-2 synthesis is not counterbalanced by a change in platelet sensitivity in men.

Radiography and bone scintigraphy in multiple myeloma: a comparative analysis.

The sensitivity of radionuclide imaging for detecting skeletal lesions was compared with that of radiography by evaluating 573 different anatomical sites in 41 patients with multiple myeloma. Radiography revealed a significantly greater number of myeloma-related bone lesions than did radionuclide imaging. Of the 179 myeloma-related bone lesions detected when both techniques were applied, 163 were seen by radiography and 82 by radionuclide imaging. Ninety-seven lesions were detected by radiography alone and 16 lesions seen by scintiscanning only, yielding a sensitivity of 91% for the former and of 46% for the latter technique. Radionuclide imaging proved superior to radiography only occasionally in the rib cage, and rarely in other anatomical sites. These findings suggest that radiography is the method of first choice in obtaining a skeletal survey in patients with multiple myeloma. In cases with continued pain, unexplained by standard radiography, the skeletal survey should be supplemented by tomography and radionuclide imaging.

[Age dependency of platelet half life].

In the last few years many reports have been published concerning shortened platelet half-life in different diseases such as atherosclerosis and hematological disorders. Up to the present moment, however, there is no information available concerning age and sex dependency in healthy people. Therefore, we evaluated platelet half-life in 106 patients, who had been examined because of a suspected atherosclerotic or hematological disorder having been excluded in the sequel. In all the patients we found negative bicycle ergometry and negative sonography of the carotid arteries and the legs. Platelets were labelled with 100 muCi of 111Indium-oxine-sulfate at 37 degrees C for 5 minutes. The mean labelling efficiency was 90%, the recovery about 70% after two hours. We found a statistically significant correlation (r = -0,5395; p less than 0,001 between age and platelet half-life. In male patients we observed a trend towards shortened platelet half-life, however, the differences were not significant.
