Prostacyclin does not influence placental blood pool in vivo.

In 20 healthy pregnant women placental blood pool was estimated by means of placental scintigrams before, during and after infusion of either prostacyclin (PGI2) or placebo for 10 min to study the effect of PGI2 on the local regulation of uterine blood flow. There was no difference in the mean number of counts between the PGI2- and the placebo-treated group. Therefore no effect of PGI2 on uteroplacental blood pool could be detected in healthy gravidae.

Irradiation depresses prostacyclin generation upon stimulation with the platelet-derived growth factor.

Experimental and clinical findings demonstrated rather severe arterial lesions occurring in irradiated arteries. It is suggested that a local haemostatic imbalance might be one causative factor. Liberation of PDGF causes smooth-muscle cell proliferation and PGI2 formation by the arterial wall cells, inhibiting in turn further release of PDGF from the alpha granules of the platelets. A decreased ability of irradiated vascular segments to generate PGI2 upon PDGF stimulation described here might cause haemostatic imbalance and subsequent radiation-induced lesions.

Possibilities and pitfalls of indium-111 platelet scintigraphy in the monitoring of renal transplant recipients.

During the last four years, we have performed two studies on the clinical application of 111In-platelet scintigraphy after renal transplantation. In the first study, we collected 75 patients who were treated with prednisolone and azathioprine. The platelet deposition in the graft was expressed as a platelet-uptake index (PUI). The results suggested that the platelet scan was a valuable tool for early detection of acute graft rejection, which was indicated by an increase in PUI. Quite different results were obtained in 50 patients immunosuppressed with cyclosporin A (CSA) and prednisolone. Acute interstitial rejection escaped detection by this diagnostic procedure. Significant elevations of PUI were observed in acute vascular rejection and microvascular CSA nephrotoxicity, resembling the haemolytic uraemic syndrome (HUS) only. Therefore, platelet deposition in the graft can be regarded as a non-specific phenomenon, occurring in two entities which require completely different therapeutic approaches. In spite of the monitoring by means of the platelet scan, percutaneous biopsy is still necessary for differential diagnosis of graft dysfunction.

[A semiquantitative method of determining alkaline phosphatase activity in human arterial wall].

A new method of semi-quantification of the alkaline phosphatase enzyme activity in human vascular tissue is described. An index is derived from the number of alkaline positive cells and the different levels of intensity of the staining reaction. In the autopsy and surgical material of male and female subjects aged between 40 and 60 a, we evaluated the alkaline phosphatase enzyme reaction of normal and atherosclerotic arteries after various incubation intervals. Tissue samples were obtained from aorta, common carotid artery, renal artery, splenic and anterior tibial artery, and Sparks prosthetic graft.

Prostacyclin-synthesis stimulating plasma factor and platelet sensitivity in preeclampsia.

Hemostatic regulation is partly maintained by antiaggregatory and vasodilative prostaglandins (PGI2, PGE1, PGD2) and by the proaggregatory and vasoconstrictive Thromboxane A2. PGI2-Synthesis is regulated by a plasma factor (PF). This plasma factor and the platelet sensitivity (PS) to PGI2 and PGE1 decrease during pregnancy towards term. In preeclampsia PF and PS is more decreased than in healthy pregnant women. Both parameters increase significantly after delivery. The decreased values of PF and PS in preeclampsia demonstrate a dysregulation of hemostasis in preeclampsia.

[Therapy with interferon (recombinant IFN-alpha-2C) in myeloproliferative diseases with severe thrombocytoses].

15 patients with myeloproliferative diseases and thrombocythaemia (7 Polycythaemia vera, 5 essential thrombocythaemia, 3 chronic myelogenous leukaemia) were assigned to a therapy with recombinant interferon (recombinant IFN-alpha-2C) in a prospective, controlled trial. Under therapy all patients showed a significant decrease in thrombocyte values. 51% of the patients revealed thrombocyte values within the normal range after 3 months of IFN therapy. Noted side effects of IFN therapy were dose-dependent and clinically well tolerated by the patients.

Detection of aneurysms by gamma-camera imaging after injection of autologous labelled platelets.

Autologous platelet labelling was used to calculate platelet half-life in 860 patients with symptoms of coronary heart disease and/or peripheral vascular disease. Abnormal platelet deposition indicating an abdominal aortic aneurysm was found in 21 by gamma-camera imaging after reinjection of autologous indium-111 labelled platelets. Aneurysms detected by imaging were confirmed by angiography, computer tomography, and at subsequent surgery.

Parathyroid hormone does not inhibit platelet aggregation.

The suggestion that parathyroid hormone (PTH) is a major uraemic toxin was examined by testing the effects of synthetic human PTH fragments and synthetic bovine PTH on ADP-induced and collagen-induced platelet aggregation. Whereas the bovine parathyroid-gland extracts inhibited platelet aggregation in a dose-dependent manner, none of the synthetic compounds was effective even at high concentrations. It is suggested that the inhibition of platelet aggregation by extracts of bovine parathyroid glands is not caused by PTH fragments and is probably an effect of other constituents contained in the extract. These findings argue against a role of PTH in the pathogenesis of platelet dysfunction and bleeding tendency in uraemia and were supported by platelet-aggregation studies in 6 patients with primary hyperparathyroidism. Platelet aggregation was normal before and unchanged after surgery of the parathyroid glands.

[Scintigraphy using 111-indium-oxine-sulfate-labeled autologous granulocytes compared to scintigraphy using 99-Tc-pyrophosphate in inflammations of the jaw region].

The present paper compares scintigraphy using leucocytes labelled with 111 indium oxine sulphate with conventional 99Tc pyrophosphate scintigraphy for the investigation of inflammatory processes in the region of the jaw bones. It also tries to establish whether progress can be made in the field of differential diagnosis using a combination of these two methods of investigation. It is shown that conventional scintigraphy using 99Tc pyrophosphate represents an approved method of investigation, whereas scintigraphy using autologous leucocytes does not as yet fulfil the expectations that have been placed in it.
