Renal microangiography and correlated histopathological observation of cows with nephropathy.

Seven Holstein-Friesian cows showing chronic nephropathy were studied by renal microangiography and its correlated histopathology. In cases of pyelonephritis associated with severe pathological lesions such as thickening of arterial walls, narrowing of the arterial and arteriolar lumen, and interstitial inflammation and abscess formation, patchy loss of the peritubular capillary plexus from the cortex to the medulla was clearly demonstrated by microangiography. Interlobular arteries were tortuous and attenuated or truncated. Opacification in the vasa rectae and interstitial capillaries was increased. Extensive non-perfused regions could be detected in the cortex. In cases of mild interstitial nephritis and moderate pyelonephritis, microangiography showed focal changes in the renal vasculature. Microangiography is thus shown to clearly demonstrate changes in the renal vasculature corresponding to the severity of the histopathological lesions.

Identification of effective component from a traditional herbal medicine and the inhibitory effects on experimental glomerular lesion in mice.

Effective constituents for renal disease were partially purified from a herbal prescription. The efficacy on renal diseases was examined using an experimental model system of glomerular lesion in mice induced by Agkistrodon acutus venom (Ac1-P). The extract of boiling water of the herbal prescription (P-3) was first fractionated by ether into an acidic fraction and a mixture of basic and neutral fractions. The acidic fraction was proved to be more effective and then further examined by thin layer chromatography and spectrophotometry. One of the main components was confirmed to be caffeic acid which had inhibitory effect on renal failure in mice by Ac1-P. This effect was considered to be caused by caffeic acid inhibiting the proteolytic enzyme activity of Ac1-P. Caffeic acid should thus have prophylactic or inhibitory effect on glomerular disease in which proteolytic enzymes may have pathogenic roles.

Expression of N-myc and c-src protooncogenes correlating to the undifferentiated phenotype and prognosis of primary neuroblastomas.

Genomic amplification of the N-myc protooncogene in neuroblastomas correctly predicts poor outcome for the patients. However, the prognosis for neuroblastomas with a single copy of N-myc is also poor in cases diagnosed after 1 year of age but good in infantile cases. To elucidate the different prognoses depending upon the age of the patients with neuroblastoma, we performed an analysis of the expression of protooncogenes related to neural differentiation. We examined the genomic amplification of N-myc in 26 specimens of neuroblastomas and further analyzed 22 of the 26 cases for expression of N-myc, c-src, c-Ha-ras, and c-fos. Consequently, we observed frequent overexpression of N-myc in undifferentiated neuroblastomas and enhanced expression of c-src and c-Ha-ras in infantile neuroblastomas with favorable prognosis and in neuroblastomas differentiated by chemotherapy. These findings suggest that c-src and c-Ha-ras play important roles in the neural differentiation of infantile neuroblastomas.

Detection of human papillomavirus in head and neck tumors with DNA hybridization and immunohistochemical analysis.

The presence of human papillomavirus (HPV) DNA in oral, sinus, pharynx, and larynx lesions of Japanese patients was studied by Southern blot hybridization under less stringent (25% formamide, 42 degrees C) and stringent (50% formamide, 42 degrees C) conditions. Three samples from 10 benign tumors, and 3 of 30 malignant tumors, contained HPV DNA or HPV-related sequences. The HPV DNAs harbored in three laryngeal papillomas were HPV-11, -6, and -6 or -11, respectively. The HPV DNA and viral capsid antigens were easily detected by in situ hybridization, Western blotting, and peroxidase-antiperoxidase staining. However, neither the typical restriction pattern of HPV DNA nor viral antigen was identified in the malignant tumors, suggesting that subgenomic fragments remained integrated in the host cell DNA.

Quantitative detection of spliced E6-E7 transcripts of human papillomavirus type 16 in cervical premalignant lesions.

The splicing patterns of E6-E7 transcripts of human papillomavirus type 16 (HPV16) in cervical premalignant lesions were quantitatively analyzed by S1 nuclease protection assay. The major E6-E7 transcripts in HPV16-containing cervical lesions (four cervical intraepithelial neoplasias and one invasive carcinoma) were from spliced E6*I/E7 mRNA. The unspliced E6/E7 mRNA, which can encode the full-length zinc finger protein E6, is expressed as 8 to 15% of E6-E7 transcripts. The spliced E6*II/E7 mRNAs were expressed as 14 to 24% of E6-E7 transcripts in most tissues. However, in HPV 16-containing cell lines, the expression levels of spliced and unspliced E6-E7 transcripts were variable.

Human papillomavirus type 6 and 11 E4 gene products in condyloma acuminata.

The human papillomavirus type 6 (HPV-6) E4 gene was expressed in Escherichia coli as a fusion protein with E. coli beta-galactosidase (E4-beta-Gal), and rabbit antibody against the E4-beta-Gal was prepared. By Western blotting with this antibody, we detected E4 gene products in six out of 18 condyloma acuminata specimens. In four specimens (C-1, C-13, C-14 and C-19), the E4 protein was found as a 10K/11K doublet, but in other specimens (C-8 and C-23), only the 11K protein was detected. By Southern blot analysis, it was found that C-13 harboured HPV-6 DNA but that C-1 and C-8 harboured HPV-11 DNA, indicating that the E4 proteins of HPV-6 and -11 have cross-reactive antigenicity. After incubation at 37 degrees C of the C-23 tissue specimen, the 10K protein was clearly detected. These results suggest that the 10K protein may be derived from the 11K protein by a modification such as proteolytic cleavage before and/or after specimens were taken.

A pathologic study on the inhibitory effects of a herbal medicine against the glomerular lesion induced by Agkistrodon venom in mice.

The inhibitory effects of a prescription of herbal medicine, tentatively named P-3, were studied pathologically in an experimental model of the glomerular lesion induced by purified snake Agkistrodon acutus venom proteinase (Ac1-P) in mice. Ac1-P was intravenously inoculated at a single LD50 dose. In the treated group, mice were intraperitoneally injected with an extract of P-3 at a designated time once every two days from 2 days before to 1 week after Ac1-P inoculation. The control group mice were injected with saline instead of P-3. In the control group, the main pathologic changes within 48 hrs after inoculation were pulmonary and gastrointestinal tract hemorrhage and renal petechiae with hematuria. The kidney microscopically showed cystic transformation of the glomerular capillary tufts. followed by occlusive thrombosis. One week after inoculation, the glomerular lesions were mostly replaced by proliferative or proliferative-sclerosing changes with occasional crescent formation. Early signs of tubular atrophy accompanying the glomerular changes were observed. In the P-3 treated mice surviving 48 hrs and 1 week, the changes observed in the controls were markedly inhibited, although P-3 treated mice dying earlier than 30 hrs exhibited hemorrhagic changes similar to controls. This indicated that the herbal medicine had efficacy against the tissue injuries induced by Ac1-P as a proteolytic enzyme.

[The relationship between the anti-HCV titer and the state of chronic hepatitis C].

By dilution of the serum, we quantitatively analyzed anti-HCV to examine the relation between the anti-HCV titer and the state of chronic hepatitis C, and determined the effectiveness of interferon therapy. The anti-HCV titer was related with the state of chronic hepatitis C, but not as closely as GPT. Cases with blood transfusion had a lower titer of anti-HCV, weaker histological finding and better response to interferon than those without blood transfusion. The anti-HCV titer was not useful for determining the effectiveness of interferon therapy.

[Transforming activity of human papillomavirus 16 DNA].

A HPV16DNA integrated in a human cancer cell line of the uterine cervix (QG-U) was isolated and cloned. Three kinds of cells (mouse Balb/3T3, Detroit 551 and human keratinocyte) were co-transfected with HPV16DNA and neo DNA with calcium phosphate co-precipitates. Selection of the Transformants and their characteristics were examined. Transformants of mouse Balb/3T3 cells were able to be obtained about 2 weeks after co-transfection. HPV16DNA was integrated in the transformants. The most characteristic change observed in the transformants was the ability to proliferate in serum-free medium. In contrast, two kinds of human diploid cells (Detroit 551 and human keratinocyte) were not transformed by transfection, and ceased growing at almost the same time as nontransfected cells. These results indicate that HPV-DNA transformants are easily selected in cell lines rodents but selection of transformants from human diploid cells is relatively difficult.

[Fundamental studies on differentiation inducers using a human embryonal carcinoma model].

A human embryonal carcinoma (EC) cell line, NEC14, was pluripotent and the tumors formed in nude mice contained differentiated somatic and extra-embryonic elements in addition to EC. The NEC14 cells could be induced to differentiate with 5 differentiation inducers in vitro, and N,N'-hexamethylene bisacetamide (HMBA) showed the most potent differentiator induction among them. All of the NEC14 cells were morphologically differentiated when exposed to 10(-2) M HMBA for 3 days. The differentiated derivatives ceased to proliferate in vitro and became non-tumorigenic in nude mouse. Several differentiation markers appeared in the differentiated derivatives. When the tumor-bearing nude mice were injected intraperitoneally with HMBA (10mg, twice a week), the tumor growth was inhibited. These results show that human EC is a useful model of differentiation-induction in cancer therapy.

Crystal-storing histiocytosis and crystalline tissue deposition in multiple myeloma.

Systemic distribution of crystal-storing histiocytes, increasing in number, and widespread crystalline tissue deposition were found in a 75-year-old man with a 5-year history of IgG-kappa-type multiple myeloma associated with corneal opacity and chronic renal failure. Characteristic crystalline inclusions were present not only in myeloma cells but also in cornea, epithelial cells of glomeruli, tubuli, Bowman's capsules, and choroid plexus. Histiocytes had particularly infiltrated the renal interstitium. These inclusions were positive by immunofluorescence for kappa light and gamma heavy chains. By electron microscopy, the inclusions were filled with fine crystalline hexagonal columns, each possessing a core structure. Of various factors generally considered responsible for renal failure in multiple myeloma, marked infiltration of histiocytes and the nephrotoxic effects of light chain appeared most relevant in the present case.

Application of renal microangiography to normal and diseased kidneys of cattle and mice.

Use of microangiography is now essential for the study of microcirculation in various organs. Renal microangiographic studies have been reported in rats, rabbits, dogs, human beings, and mice. However, we could not find any report on use of the technique in cattle, despite high incidence of renal disease in that species. The perfusion technique used in mice was improved over that of our previous report, and was applied to normal and diseased bovine kidneys. For the microangiographic technique, composition of the contrast medium, pressure of the injection, duration of perfusion, and washing of kidneys with heparinized saline solution before perfusion are important. In cattle, 1- to 2-mm-thick sections of the kidneys were generally necessary to observe renal vasculature: arcuate and interlobular arteries, afferent arterioles, and glomerular capillaries. In normal bovine kidneys, the angiographic and microangiographic findings were easily recognized as normal, compared with those of normal mice. In affected bovine kidneys, which histologically represented glomerulonephritis and pyelonephritis, angiography and microangiography revealed corresponding findings.

Alterations in Fc receptor activity in sinusoidal endothelial cells and Kupffer cells during D-galactosamine (GalN)-induced liver injury in rats. A histological study.

Fc receptors in sinusoidal cells and immune complex uptake were studied histologically in D-galactosamine HCl (GalN)-induced liver injury in rats. Kupffer cells and monocytes were distinguished from sinusoidal endothelial cells and from each other by endogenous peroxidase staining. Fc receptors were found along the sinusoidal endothelium throughout the lobules in normal livers. In acute injury caused by 300 or 750 mg/kg of GalN, Fc receptors were preserved within necrotic foci until the foci were infiltrated by inflammatory cells. The endothelial Fc receptor activity altered, as demonstrated by their capacity to bind immune complexes, after GalN injection. The activity decreased from 24 h after injection in the periportal areas in both dose groups, and increased transiently with dose-dependence in the remaining areas. Kupffer cell numbers also showed a transient dose-dependent increase, except in the periphery of lobules where they generally decreased. In chronic injury with 400 mg/kg, Fc receptors were lost and Kupffer cells decreased in the periportal areas. Circulating immune complexes were ingested by Kupffer cells and endothelial cells in normal and injured livers, showing the the same distribution as that of Fc receptors except that the complexes decreased gradually towards the centrilobular zones.

Defect of sinusoidal Fc receptors and immune complex uptake in CCl4-induced liver cirrhosis in rats.

The purpose of this paper is to provide a histopathologic basis for abnormalities in immune-complex clearance in liver disease. Fc receptors in CCl4-induced liver cirrhosis in rats were studied by applying peroxidase-antiperoxidase immunoglobulin G complex as a ligand to the frozen sections. Intravenous injection of bovine serum albumin-antibovine serum albumin complexes or colloidal carbon was combined with histological staining for endogenous peroxidase, fibronectin, laminin, or a lectin, Bandeiraea simplicifolia agglutinin I. In the cirrhotic process, sinusoidal Fc receptors showed a weakened reactivity to the ligand with focal absence, and the length of the Fc receptor-positive portion of the sinusoids in unit area decreased to about 50% of the normal value in the advanced cirrhosis. Fibronectin and the lectin showed the presence of sinusoids where Fc receptors were absent. The endothelium in Fc receptor-negative areas did not take up either immune complexes or carbon, and Kupffer cells were absent in these areas. A disturbed immune-complex metabolism was thus suggested to occur in association with the defect of sinusoidal Fc receptors in liver cirrhosis. These abnormalities appeared to not be directly related to perisinusoidal laminin deposition, i.e., capillarization of the sinusoid.

[An aggressive angiomyxoma of the vulva].

A case of an aggressive angiomyxoma of the vulva in a 38-year-old lady is reported. The tumor was gelatinous and 12 x 8 x 7 cm in size. Histologically, spindle or stellate-shaped tumor cells and blood vessels were found distributed in a myxoid stroma. It was found that the tumor cells were immunoreactive to vimentin but negative to desmin and smooth muscle specific actin. Ultrastructurally the tumor cells had an intercellular junction, an abundant, rough endemic reticulum, and intracytoplasmic filaments, but no focal densities of the filaments and the basal lamina. These findings have revealed that this tumor consisted of myofibroblastic cells resembling fibroblasts rather than myofibroblasts.

A new therapeutic trial of secretin in the treatment of intrahepatic cholestasis.

Many animal experiments have been studied on the choleretic effects of secretin. We intended to estimate secretin choleresis in human (15 patients) who had received PTCD or T-tube insertion into the common bile duct. Based upon these data of secretin and choleresis, secretin was administered to 11 patients with prolonged jaundice due to intrahepatic cholestasis in order to evaluate this as a new therapy for intrahepatic jaundice. As controls, eleven patients with intrahepatic cholestasis treated with steroid hormones and/or phenobarbital were used. In all cases with biliary drainage, secretin produced a remarkable choleretic effect with a high concentration of bicarbonate. In 9 out of 11 patients with intrahepatic cholestasis who were treated with secretin, levels of serum bilirubin decreased linearly and other liver function tests returned to the normal range. The mean values of T1/2 (number of days required for reduction by half) of serum bilirubin in 9 effective cases to secretin was 10.8 days. On the other hand, that in 11 effective cases treated with steroid hormones and/or phenobarbital was 23.2 days. These results suggest that secretin therapy may be an effective treatment for intrahepatic cholestasis.
