Systemic amyloid arthropathy associated with multiple myeloma.

This is the first report of an autopsy case of systemic amyloid arthropathy associated with multiple myeloma in Japan. The patient was a 72-year-old male who had been suffering from multiple myeloma (IgA-lambda) with extensive arthropathy of the systemic joints. Autopsy examination revealed severe deposit of amyloid in the articular cavities of the systemic joints showing a feature of amyloidoma. Furthermore, numerous amyloid deposits were found in the wall of small blood vessels in the major organs. In the bone marrow, there were many atypical plasma cells and foamy cells both showing positive reaction for IgA and lambda light chain. No such atypical cells were observed in the other organs except for the bone marrow.

Indirect coupling to Na+ of p-aminohippuric acid uptake into rat renal basolateral membrane vesicles.

Experiments with basolateral membrane vesicles prepared from rat kidney cortex were performed to study the mechanism by which p-aminohippuric acid (PAH) is taken up across the contraluminal membrane and is concentrated in proximal tubule cells. An inward Na+ gradient failed to stimulate [3H]PAH uptake compared with K+ or Li+ and did not cause intravesicular PAH accumulation above equilibrium distribution. In the absence of Na+, the dicarboxylates glutarate and suberate cis-inhibited and trans-stimulated [3H]PAH uptake, indicating a common transport system. In the presence of Na+, 10 microM glutarate in the incubation medium did not cis-inhibit, but rather stimulated [3H]PAH uptake and caused PAH accumulation above equilibrium distribution ("overshoot"). Li+ diminished this stimulation, but was without effect on [3H]PAH/PAH- and [3H]PAH/glutarate exchange. The data indicate the coexistence of a Na+ -coupled, Li+-sensitive transport system for dicarboxylates and a Li+ -insensitive PAH/dicarboxylate exchanger in the basolateral membrane. We propose that dicarboxylates are cotransported with Na+ into the cell and subsequently exchange for extracellular PAH at the basolateral membrane. PAH uptake is thereby indirectly coupled to Na+ via the Na+/dicarboxylate cotransporter.

Effect of isoproterenol on renal uric acid excretion in rats.

Effect of isoproterenol on renal uric acid excretion was examined in oxonate-loaded and oxonate-nonloaded rats, using a clearance technique. Oxonate loading was performed by continuous infusion into the femoral vein. Isoproterenol (50 micrograms/kg, i.v.) induced antidiuresis and hyperuricemia accompanied with decreases in urinary excretion rate of uric acid, uric acid clearance, inulin clearance, and uric acid clearance/inulin clearance ratio in oxonate-nonloaded rats. These effects of isoproterenol were inhibited by propranolol (1 mg/kg, i.v.). Very similar results were obtained in oxonate-loaded rats; i.e., systemic blood pressure decreased synchronously with uric acid excretion by isoproterenol. In contrast, phenylephrine (100 micrograms/kg, i.v.) induced hypertensive, diuretic and slightly uricosuric results. In allopurinol-pretreated and oxonate-loaded rats, isoproterenol also decreased renal uric acid excretion and showed a less potent hyperuricemic effect than that observed in the animals not pretreated with allopurinol. In addition, in rats with ligated renal vessels, the hyperuricemic effect of isoproterenol was completely inhibited by allopurinol. These results suggest that isoproterenol-induced hyperuricemia is related not only to the stimulation of uric acid production, but also to the depression of renal uric acid excretion.

The clinicopathological significance of changes of ganglioside patterns in the cirrhotic liver: a study of 11 cases.

Unique ganglioside components including GM2 and spots No. 1 and 2 designated tentatively were increased in content in some cases of cirrhotic liver as well as in all cases of hepatocellular carcinoma (HCC). Tissues of cirrhotic liver in which these components were increased were examined histologically. These components were increased in cirrhotic liver showing many foci of dysplastic nodules and in cirrhotic nodules at the matrix of multicentric HCC. These findings, thus, indicate that the unique ganglioside components are probably related to the histological findings of the cirrhotic liver.

Activation of N-nitrosodialkylamines by near-ultraviolet irradiation: formation of directly-acting mutagens and DNA-damaging products.

On near-ultraviolet (UVA) irradiation in a phosphate buffer, N-nitrosomorpholine (NMOR) and N-nitrosopyrrolidine (NPYR) were converted into directly-acting mutagens. The activated NPYR was fractionated, and the active product was isolated. The compound was shown to be identical to alpha-phosphonooxy NPYR on the basis of several properties: retention times in high-performance liquid chromatograms, mutagenic specificity and potency, ultraviolet spectrum, and inactivation by phosphatase treatment. Photoactivation was inhibited by superoxide dismutase, and therefore superoxide is implicated as playing a key role in mutagen formation. N-Nitrosoproline (NPRO) and eighteen other N-nitrosodialkylamines were irradiated with UVA in the presence of phi X174 RFI DNA. The DNA underwent single-strand breaks to give RFII DNA, indicating that N-nitrosodialkylamines in general have this property. DNA chain cleavage was inhibited both by superoxide dismutase and hydroxyl-radical scavengers. These results provide new information on the genotoxic mechanism of action of N-nitrosodialkylamines.

Epidural ketamine for postoperative pain relief after gynecologic operations: a double-blind study and comparison with epidural morphine.

This double-blind study evaluates whether ketamine given epidurally is effective for postoperative pain relief, and compares the effects of epidural ketamine with those of epidural morphine. Sixty-eight patients undergoing abdominal gynecologic surgery were randomly assigned into six groups (control; ketamine 4, 6, and 8 mg in saline; 6 mg in 10% glucose; morphine 3 mg). All patients were anesthetized with thiopental, nitrous oxide, and enflurane, and drugs were administered epidurally at the end of the operation. The duration of analgesia in the ketamine groups did not differ from that in control patients and the difference in diluent had no observable effects. Significantly, none of the patients in the morphine group needed additional analgesics within 24 hr, whereas 85% in the other five groups did. We conclude that ketamine administered epidurally is inadequate for postoperative pain relief after gynecologic operations.

[A case of priapism caused by disseminated intravascular coagulopathy].

A case is presented of priapism resulting from disseminated intravascular coagulopathy (DIC), which was diagnosed by pathological studies of the amputated penis and skin biopsy. To our knowledge, this is the first case reported in Japan. This 72-year-old-man visited a hospital complaining of fever and cough, and was administrated for treatment of bronchitis and liver cirrhosis. A few days after admission, multiple purpura with edema and pain appeared over the skin regions on the bilateral knee joint, foot joint and upper extremities. A week after purpura appeared, priapism began. Regardless of irrigation and aspiration of corpora cavernosa and glans-cavernosa-fistula creation, penile necrosis developed. We had to perform penile amputation. The pathology of the amputated penis and skin, and blood coagulative examination suggested that DIC resulted in priapism. DIC was controllable by the use of FOY and heparin. He was discharged and is an outpatient.

[Neuroblastoma; biology and prognostic factors].

Recently accumulated data on the biology of the tumors of neuroblastoma group are reviewed. Histopathological classification system developed by Shimada et al, elevated levels of serum neuron-specific enolase and ferritin, and gene abnormalities including partial deletion of the short arm of chromosome #1 and N-myc gene amplification are discussed as well as their prognostic significance. Also, the current activities of CCSG (Childrens Cancer Study Group) neuroblastoma studies are briefly reported.

Replication timing: histone genes replicate during early S phase in cleavage-stage embryos of sea urchin.

Newly synthesized DNA was separated from the bulk of the DNA by pulse-labeling with BUdR and centrifugation in an alkaline CsCl buoyant density gradient. The content of histone gene in the newly synthesized DNA was determined by DNA dot hybridization. The gene contents in DNA replicated during the early half of S phase and during the whole S phase were compared. Results showed that histone genes were replicated during the first half of the S phase in embryos in the early cleavage stage.

Specific desensitization in the cytoskeletal assembly of platelet by receptor-dependent activations.

Cytoskeletal assembly induced by receptor-dependent or independent activation of bovine platelets was investigated. When platelets were preactivated with receptor-dependent stimulus, ADP or thrombin, cytoskeletal assembly was not induced repeatedly by the same agonist. However, in the case of receptor-independent stimulus, cryo-activation, the assembly was induced not only by receptor-dependent but also by independent stimulus. The desensitization, therefore, lies in the transmission of stimuli from receptors to cytoskeletal proteins.

The complete nucleotide sequence of the tobacco chloroplast genome: its gene organization and expression.

The complete nucleotide sequence (155 844 bp) of tobacco (Nicotiana tabacum var. Bright Yellow 4) chloroplast DNA has been determined. It contains two copies of an identical 25 339 bp inverted repeat, which are separated by a 86 684 bp and a 18 482 bp single-copy region. The genes for 4 different rRNAs, 30 different tRNAs, 39 different proteins and 11 other predicted protein coding genes have been located. Among them, 15 genes contain introns. Blot hybridization revealed that all rRNA and tRNA genes and 27 protein genes so far analysed are transcribed in the chloroplast and that primary transcripts of the split genes hitherto examined are spliced. Five sequences coding for proteins homologous to components of the respiratory-chain NADH dehydrogenase from human mitochondria have been found. The 30 tRNAs predicted from their genes are sufficient to read all codons if the ;two out of three' and ;U:N wobble' mechanisms operate in the chloroplast. Two sequences which autonomously replicate in yeast have also been mapped. The sequence and expression analyses indicate both prokaryotic and eukaryotic features of the chloroplast genes.
