Squamous cell carcinoma of the maxillary sinus and the oral part of the upper jaw. Comparison of treatment results.

The treatment results were compared in 77 patients with maxillary sinus squamous cell carcinoma (MC) and 53 patients with squamous cell carcinoma arising from the oral part of the upper jaw (OC). Both sets of patients received radiotherapy alone, or radiotherapy combined with surgery and/or chemotherapy. Computerized tomography was useful for the definition of the treatment volume. Intraarterial chemotherapy was given in 89 of 130 patients and in these patients the total radiation dose was reduced by about 10 Gy. No difference was found in the 5-year survival rate between the MC (65%) and the OC (66%) groups. The cumulative incidence of local failure was higher in MC (36%) than in OC (26%), whereas the ultimate incidence of neck node metastasis was higher in OC (43%) than in MC patients (18%). Half of the inoperable patients (9/18) were older than 80 years and had contraindications to anaesthesia and major surgery. The local recurrence rate was high in the inoperable MC patients (6/8). Contralateral sinus cancers occurred in 4 patients in the MC group.

[Long-term renal function after nephrectomy in living related kidney donors].

The purpose of this study is to evaluate the long-term renal function of donors after nephrectomy in living related renal transplantation (n = 100). The survival rate of donors was 98% and causes of death in the other 2% were not associated with the donation. Serum creatinine (Scr) and Creatinine clearance (Ccr) deteriorated just after the donation, but gradually improved. The number of donors with proteinuria increased after donation. In Scr and Ccr there was no significant difference between donors with and without proteinuria. In the donors with hypertension (25.3%) mean urine protein was higher than in those without hypertension. There was a correlation between systolic blood pressure and proteinuria. These results strongly suggest that renal function of donors deteriorates once just after the donation, but gradually improves year by year, and that hypertension is strongly associated with renal dysfunction, especially proteinuria, after the donation.

Brachytherapy for stage I & II oral tongue cancer: an analysis of past cases focusing on control and complications.

An analysis of the therapy used for 370 tongue cancer patients has been made, said patients having been treated by interstitial irradiation alone or by combined external irradiation and brachytherapy (Stage I:90 cases, IIa: 196 cases; and IIb: 84 cases). The neck was followed by close follow-up (304 cases), treated by elective neck irradiation (56 cases), or underwent operation at the time of local recurrence (10 cases). The results have shown that the 5-year survivals for Stages I, IIa, and IIb were 84%, 78%, and 72%, respectively. further, the 5-year primary control was 85% for tumors of the superficial type, 79% for tumors of the exophytic type, and 45% for tumors of the infiltrative type (p < 0.004). In non-electively irradiated patients, a neck metastasis occurred in 31% in Stage I, 41% in Stage IIa, and 51% in Stage IIb. Finally, 110 patients incurred radiation-induced complications (110/291 = 38%) and 11 patients (11/291 = 4%) required a surgical procedure. Brachytherapy for tongue cancer achieved results that are comparable with surgery. The analysis also revealed that the introduction of computer dosimetry and the use of a spacer (a dental guard) in brachytherapy have achieved superior results in the management of a tongue cancer.

Involvement of the fimbria fornix in the initiation but not in the expression of methamphetamine-induced sensitization.

To put forward our previous finding that the lesion of the fimbria fornix, a hippocampo-accumbal pathway, blocked the development of behavioral sensitization induced by repeated methamphetamine (MAP) administrations, we examined the role of the fimbria fornix in the expression of sensitization in this study. After rats had shown locomotor augmentation following repeated drug injections, they received either the fimbria fornix lesion or a sham operation. Both groups of rats still exhibited a similar sensitized locomotor response to MAP as before the surgeries. In addition, we evaluated dopamine metabolism in the nucleus accumbens of these rats following a challenge injection of MAP after the behavioral study. The data obtained correspond to the results of behavioral experiments in that 3-methoxytyramine, one of the dopamine metabolites, increased significantly after MAP challenge only in the groups of sensitized animals. These findings further support the recent concept that there may exist different neural mechanisms in the initiation and expression of sensitization phenomenon.

The efficacy of radiation therapy for a malignant melanoma in the mucosa of the upper jaw: an analytic study.

An analysis has been made of the effect of radiation therapy in 28 patients with a malignant melanoma (Stage I: 18 cases; Stage II: 10 cases) in the mucosa of the upper jaw. Treatment had been provided by one of the following methods: intraoral mold (10 cases), interstitial brachytherapy (two cases), intraoral electron therapy (nine cases), or external irradiation (seven cases). The results have shown that the survival rate for all 28 patients was 25%, and that the survival rate for stage I patients treated by intraoral electron or brachytherapy was 47%. The primary tumor control rate in percentages was 79% (22/28) in all 28 radiotherapy patients; 92% (11/12) for tumors treated by a mold or an interstitial implant; 67% (6/9) for tumors treated by an intraoral cone; and 71% (5/7) for tumors treated by external irradiation with or without surgery. A neck metastasis that was found in 19 patients was treated by surgery, radiotherapy, and/or immunochemotherapy, and the result was successful in nine patients. The major factor in the failure of treatment was a distant, metastatic dissemination. This analysis revealed that radiotherapy has achieved similar or better results than surgery and may be advocated for the management of a localized malignant melanoma in the mucosa of the upper jaw.

Two polymorphic microsatellites in a coding segment of the canine androgen receptor gene.

A 0.6-kb segment of exon 1 of the canine androgen receptor gene contains two polymorphic CAG tandem repeats which encode strings of glutamine homopolymers. The number of CAGs in each tandem repeat was determined by (1) polymerase chain reaction (PCR) amplification of a gene segment containing both repeats, (2) cleavage between repeats with restriction enzyme EcoO109I and (3) fractionation of the restriction fragments containing individual CAG repeats by denaturing polyacrylamide gel electrophoresis (PAGE). Individual genomic DNA samples from 80 unrelated dogs (53 males plus 27 females for a total of 107 X chromosomes) contained 10-12 CAGs in the 5' repeats and 10-13 CAGs in the 3' repeats. Thirteen distinct androgen receptor genotypes were identified. Eleven (or 41%) of the 27 unrelated females were heterozygous in one or both repeat regions, whereas all male samples produced single bands as expected for X chromosome markers. A total of seven distinct haplotypes contributed to the 13 genotypes. The 'polymorphism information content' or 'PIC' for this seven-allele X chromosome marker was 0.67.

Inflammatory esophagogastric polyp and fold in an adolescent.

The case of a 13 year old boy with an inflammatory esophagogastric polyp and ulcerative colitis is described. Endoscopy revealed a typical polyp and gastric fold complex at the esophagogastric junction and a hiatal hernia. Histology of a biopsy specimen confirmed an inflammatory polyp covered by hyperplastic squamous and gastric foveolar epithelium. Continuous 24 hour esophageal manometry suggested gastroesophageal reflux, which may be related to the pathogenesis of the lesion. Follow-up endoscopy showed marked regression of the polyp with medication for reflux esophagitis. This clinical entity is rare in childhood and adolescence, and the manifestations may not be readily recognized. Therefore, endoscopic biopsy is important in children with esophageal polyps. However, polypectomy is unnecessary except when malignancy is suspected or when symptoms persist.

Endoscopic ultrasonography in a child with chronic pancreatitis.

In a case with chronic pancreatitis and pseudocysts related to muscular cytochrome-c oxidase deficiency, endoscopic ultrasonography (EUS) was performed to morphologically examine the pancreas for features that may have been missed by abdominal computed tomography and pancreatography. Conventional abdominal sonography had failed to show valuable diagnostic information of chronic pancreatitis in this case. EUS demonstrated several new findings. Parenchymal atrophy was remarkable, especially in the pancreatic tail, and a dilated pancreatic main or branch duct was clearly found in the region. Moreover, pancreatic stones in the main duct were also distinctly proved. EUS may become a useful new diagnostic method in pediatric gastroenterology, especially when sufficient findings are not obtained by conventional imaging techniques. However, an ultrasonographic endoscope with a shorter rigid portion and smaller caliber should be developed for pediatric patients.

Dehatrine, an antimalarial bisbenzylisoquinoline alkaloid from the Indonesian medicinal plant Beilschmiedia madang, isolated as a mixture of two rotational isomers.

Through bioassay-guided separations of the chemical constituents of the Indonesian medicinal plant Beilschmiedia madang BL. a bisbenzylisoquinoline alkaloid was obtained as the major antimalarial principle. The physicochemical properties of the alkaloid were consistent with the proposed structure of dehatrine. However, the alkaloid isolated by us was shown to be a mixture of two rotational isomers. The X-ray crystallographic analysis of 1 has shown that two rotamers are incorporated in a single crystal in 1:1 ratio. The complex NMR spectrum of 1 has also been defined as a mixture of two rotamers by extensive use of 2D (COSY and COLOC) techniques. Dehatrine has been shown to significantly inhibit the growth of cultured Plasmodium falciparum K1 strain (cholorquine resistant) with similar activity to quinine.

Brachytherapy for non-metastatic squamous cell carcinoma of the buccal mucosa. An analysis of forty-five cases treated with permanent implants.

An analysis has been undertaken of 45 patients with non-metastatic squamous cell carcinoma of the buccal mucosa treated by permanent 198Au or 222Rn implants and in most cases supplementary external irradiation. Eight patients had T1, 30 had T2, and 7 T3 disease. Of the lesions 21 were located in the buccal mucosal surface, 14 in the retromolar region, and 10 in the bucco-alveolar sulci. Seven of the 45 (16%) died of the disease and the actuarial 5-year survival rate was 81%. No obvious differences were noted in survival rate between different stages of the disease or between different subsites of the primary lesion. There were 6 local recurrences occurring from 3 to 33 months after treatment, and only one of these was salvageable by further radiation therapy. Neck node metastasis occurred in 11 cases from 2 to 34 months after treatment, and 8 of these cases responded successfully to a radical neck dissection or radiotherapy.

Efficiency of visual selection in duplex and conjunction conditions in partial report.

Factors that determine the efficiency of visual selection were investigated in two precued partial-report experiments. There were four selection conditions: a color condition (e.g., selecting purple letters among blue letters), a class condition (e.g., selecting letters among digits), a duplex condition (e.g., selecting purple letters among blue digits; that is, the targets differed from the distractors both in color and in alphanumeric class), and a conjunction condition (e.g., selecting purple letters among blue letters and purple digits; that is, the targets differed from the distractors either in color or in alphanumeric class). The efficiency of visual selection was highest in the duplex condition and lowest in the conjunction condition. The difference in performance across the different selection conditions within subjects was accounted for by variation in the value of a single parameter of the fixed-capacity independent race model (FIRM) proposed by Shibuya and Bundesen (1988), which assumes independent parallel processing, limitations in both processing capacity and storage capacity, and time-invariant selectivity. By assuming that the factors that determine the efficiency of selection in the color and class conditions also underlie the efficiency of selection in the duplex and conjunction conditions, an extension of FIRM, FIRM+, accounted for the relationship with respect to efficiency of selection among the different conditions.

Ultrasonography of the sublingual space.

Fifty-three patients were evaluated by ultrasonographic visualization of the sublingual space as part of routine neck sonography. Delineation of the sublingual gland was good or fair in 93% of the cases. The normal gland was 32.06 mm long in the sagittal plane. The submandibular duct was identified in 53% of the cases and measured less than 2.5 mm in diameter. The sublingual vessels were detected in 93% of the cases and had almost the same mean diameter as the duct. Four cases of frequent pathologies in this region were also examined. Ultrasonography is the examination to use for diseases of the sublingual space.

[A simple and rapid method capable of detecting methicillin-resistant Staphylococcus aureus (MRSA) by single culture on an agar plate].

A novel simple agar plate capable of detecting methicillin-resistant Staphylococcus aureus (MRSA) by single culture was developed. The agar plate included 1) mannitol and a pH indicator, and 4-methylumbelliferyl phosphoric acid, both for identification of Staphylococcus aureus by its ability to digest mannitol and produce phosphatase; detection of the former by change in color of pH indicator and the latter by fluorescence of 4-methylumbelliferone, 2) oxacillin at a minimum concentration just to eliminate methicillin-sensitive Staphylococcus aureus (MSSA), and 3) phenylethyl alcohol to inhibit growth of most gram negative rods. The method was simple, sensitive, time-saving, and quite effective in the identification of MRSA directly from clinical specimens. Neither false positive nor false negative results were observed by the present method.

[Magnet-supported chemiluminescent detection for amplified mecA gene of methicillin-resistant Staphylococcus aureus].

A method of chemiluminescent detection for mecA gene of methicillin-resistant S. aureus amplified by polymerase chain reaction, was developed. The senseprimer was biotinylated at 5'-end, and an oligonucleotide probe, complementary to the senseprimer, was labeled with acridinium ester at 5'-end. Template DNA was obtained from colonies of S. aureus cultured on blood agar. Polymerase chain reaction product was hybridized with the probe and was separated using streptavidin-coated magnetic microparticles in magnetic field. The pellet was washed by decantations, and was measured for chemiluminescence in an automatic luminescence reader. For reference, drug sensitivity and presence of mecA gene were tested by dilution method and electrophoresis of polymerase chain reaction product, respectively, for each colony studied. Results showed that methicillin-resistant S. aureus (n = 44) and methicillin-susceptible S. aureus (n = 19) can be successfully differentiated by chemiluminescence measurement. This method is specific, rapid, and suitable for handling a large number of samples.
