[Clinical studies on sulbactam/cefoperazone in the field of obstetrics and gynecology].

Sulbactam/cefoperazone (SBT/CPZ) was administered to 8 cases with gynecologic infections including pelvic peritonitis (2 cases), pyometra (2 cases), acute adnexitis, Bartholin abscess, endometritis and infected lymphocyst. The clinical efficacy was assessed as effective in 7 cases and poor in 1 case. The effective rate was 87.5%. No adverse reactions including clinical signs were observed. But slight elevation of GOT was observed in 1 case.

[Usefulness of ECG gated cardiac computed tomography in measurement of left ventricular volume and wall thickness].

Electrocardiogram gated cardiac computed tomography (CT) was performed to evaluate the usefulness in the measurement of left ventricular volume and left ventricular wall thickness in 25 patients; five with angina pectoris, five with old myocardial infarction, eight with hypertrophic cardiomyopathy, four with dilated cardiomyopathy, and three healthy men. The left ventricular volume was calculated as the sum of the volume of each slice, which was area times thickness of the slice. The left ventricular wall thickness was measured in reconstructed short-axis view at the level of the mitral valve and papillary muscle. The left ventricular volume and left ventricular wall thickness determined by CT were compared with that by left ventriculography (LVG), and that by two-dimensional echocardiography (2 DE), respectively. The following results were obtained. There were good relationships between left ventricular volume, end-diastolic volume, end-systolic volume, ejection fraction by CT and those by LVG, but left ventricular volume by CT was smaller by about 20% than that by LVG. In the measurement of stroke volume, no relationship was recognized between CT and LVG. Concerning the measurement of left ventricular wall thickness, more available figures were obtained by CT than by 2 DE, particularly at the apical region, lateral wall, and posterior wall. In the other segments of the left ventricle, CT was also more suitable for the measurement of the wall thickness. Interventricular septal thickness was correlated between reconstructed short-axis view of CT (Y) and transverse view of CT (X), and the equations Y = 0.66X + 2.13 (r = 0.79) at the mitral valve level, and Y = 0.56X + 3.00 (r = 0.81) at the papillary muscle level were obtained. Therefore, reconstructed short-axis view should be used for the measurement of the left ventricular wall thickness not to overestimate the thickness.

Angina Pectoris

[Lipids metabolism of the patients with subarachnoid hemorrhage due to ruptured intracranial aneurysm, in comparison with other cerebrovascular diseases].

Lipids metabolism has been extensively studied in the large number of patients with hypertensive cerebral hemorrhage or cerebral infarction, however, the patients with subarachnoid hemorrhage due to ruptured intracranial aneurysm were customarily excluded from the study. Total cholesterol, LDL-cholesterol, HDL-cholesterol, triglyceride and atherogenic index were determined in 40 cases, who had undergone surgical treatment for ruptured intracranial aneurysm in various locations, and the results were compared with the data obtained from 20 operative cases with hypertensive cerebral hemorrhage, 40 cases with non-operative cases with hypertensive cerebral hemorrhage, 60 cases with cerebral infarction and 20 cases of cerebral lesions other than cerebrovascular disease (non-CVA). Serum levels of LDL-cholesterol and atherogenic index were significantly correlated with the preoperative grading of subarachnoid hemorrhage when compared with non-CVA (p less than 0.05). Total cholesterol, LDL-cholesterol and atherogenic index were well correlated with operative morbidity and active daily life (ADL) in the follow-up study of the patients with intracranial aneurysm, where the higher value of cholesterol indicated the poorer risk of the patients. In our present study, none of the data was found significant, when compared with non-CVA, in the other groups of cerebrovascular diseases except for the HDL-cholesterol, which was found significantly in low level in the group of cerebral infarction (p less than 0.001). Triglyceride was found elevated in the group of hypertensive hemorrhage indicating negative correlation to the severity, and rather similar pattern was seen in the subarachnoid hemorrhage, however, the data were not conclusive.(ABSTRACT TRUNCATED AT 250 WORDS)

Studies on the biosynthesis of bialaphos (SF-1293) Part 3. Production of phosphinic acid derivatives, MP-103, MP-104 and MP-105, by a blocked mutant of Streptomyces hygroscopicus SF-1293 and their roles in the biosynthesis of bialaphos.

During biosynthetic studies on bialaphos to reveal the formation mechanisms of carbon-phosphorous bonds in detail, three new metabolites containing a H-P-C bond structure were isolated from the fermentation broth of a mutant of Streptomyces hygroscopicus SF-1293. Based on the spectroscopic analysis, the structures of these compounds have been determined as shown in Fig. 1. Transformation experiments of these metabolites to bialaphos suggested that the reduction of the phosphorous atom in phosphate will take place at an early biosynthetic stage.

Myocardial visualization on a routine perfusion lung scintigram: relationship to the amount of right-to-left shunt.

Three cases of myocardial visualization on a routine perfusion lung scintigram with 99mTc-macroaggregated albumin were reported in patients with congenital heart diseases; two cases of tetralogy of Fallot and one case of truncus arteriosus type IV. Large right-to-left shunts greater than 39% and marked hypertrophy of the ventricle suggesting the presence of increased coronary blood flow were noted in all cases. In the two patients with tetralogy of Fallot myocardial activity appeared to be located in the hypertrophic right ventricles.

Differentiation-associated changes of glycolipid composition and metabolism in mouse myeloid leukemia cells. Induction of globotriaosylceramide and a galactosyltransferase.

This study was to find out whether induction of special glycolipids or glycosyltransferases for glycolipid synthesis which might be involved in the cell functions occurred during the differentiation. Mouse myeloid leukemia cell line (M1-), the differentiated cells (M1+), and a subcloned cell line (Mm1) were used for this purpose. Gangliotriaosylceramide (GA2) was the major glycolipid component in M1- cells. As a result of differentiation of M1- into M1+ cells, globotriaosylceramide (CTH) was newly induced as the main glycolipid, while GA2 decreased to a minor component. GA2 was found to be the main glycolipid in Mm1 cells but no CTH was recognized. All precursor glycolipids and glycosyltransferases required to complete the biosynthetic pathway glucosylceramide (CMH) leads to lactosylceramide (CDH) leads to GA2 leads to gangliotetraosylceramide (GA1) leads to sialosylgangliotetraosylceramide (GM1b) were found in M1- and also in Mm1 cells. A galactosyltransferase activity for CTH synthesis from CDH increased 10 fold during the differentiation. The induction of CTH in M1+ cells could be attributed to the increase of the galactosyltransferase activity. Both CTH as a surface marker and the galactosyltransferase as an enzyme marker are proposed as valuable markers of differentiation in M1- cells. Besides the galactosyltransferase, N-acetylglucosaminyltransferease involved in the formation of lactotriaosylceramide (amino-CTH) increased up to 3 fold during the differentiation. The increase of the enzyme activity seemed to be responsible for the biosynthesis of lactoneotetraosylceramide (paragloboside) which appeared in M1+ cells.

Studies on biosynthesis of pentalenolactone. V isolation of deoxypentalenylglucuron.

Deoxypentalenylglucuron was isolated from the culture broths of three different strains, such as Streptomyces omiyaensis, S. albofaciens and S. viridifaciens. The structure of deoxypentalenylglucuron has been determined by 1H and 13C NMR, mass spectroscopy and by chemical correlation to be an oxidation product of pentalenene 1. Deoxypentalenylglucuron (1) has some antitumor activity against Sarcoma 180 in mice.

Leptomycins A and B, new antifungal antibiotics. II. Structure elucidation.

The structures of new antifungal antibiotics, leptomycins A and B produced by Streptomyces sp. ATS1287 were determined as described below (Fig. 1) on the basis of their spectral and chemical character. Leptomycins have unique structures which belong to the unsaturated, branched-chain fatty acids with delta-lactone rings at the end.
