[Pharmacokinetic studies on the penetration of azlocillin and mezlocillin into bone and tissue fluid (author's transl)].

During and after hip replacement arthroplasty antimicrobial concentrations in serum, bone and tissue-fluid were determined microbiologically using agar diffusion assay technique after 15-min infusion of 5 g 6-[D-2-(2-oxoimidazolidine-1-carboxamido)-2-phenylacetamido] - penicillanic acid (azlocillin) and 5 g 6-[D-2-(3-methylsulfonyl-2-oxoimidazolin-1-ylcarboxamido)-2-phenylacetamido]-penicillanic acid (mezlocillin) and after i.v. bolus injection of 2 g mezlocillin. Serum pharmacokinetic data on the individual patients are computer derived using a modified open two-compartment model. The geometric mean of bone level determinations at defined intervals with deviation factor are presented. These results are related to the organic bone compartment and are expressed in mg/l. From the tissue fluid-concentration curve, peak concentration CP and its time were calculated. The time for which one-fourth of the peak concentration CP 1/4 was maintained in tissue fluid was determined graphically. This concentration is compared with the cumulative minimum inhibitory concentrations of representative bacteria causing bone and joint infections. These results are focused as criteria of valuation of antimicrobial chemotherapy.

[Atypical course of human salmonellosis (author's transl)].

During the last few years atypical forms of human salmonellosis were observed in 27 patients. Most frequent were acute gastroenteritis with bacteraemia or septicaemia, abscess formations in various organs and septic infections with gastroenteritis Salmonellae but no demonstrable gastroenteritis. In addition there were cases of osteomyelitis, peritonitis, one case of infected hip replacement, one of infected adrenal cortical tumour, and one of infected aortic aneurysm. In 24 of the 27 patients there was an underlying disease likely to have favoured the development of such infections. Diabetes mellitus, cholelithiasis and malignant tumours were the most frequent condition. Isolation of the positive organism was obtained from various materials (blood, abscess pus, gallbladder smear, operative specimen). Chemotherapy is definitely indicated in such Salmonella infections. Often additional surgical measures, e.g. to control spread of septic foci, were necessary. Prophylactic chemotherapy is recommended for patients with risk factors in order to prevent bacteraemic-metastatic events.

[Availability of cefotaxime. Pharmacokinetic studies on the distribution in central and various peripheral compartments (author's transl)].

After 2 g as a bolus injection the concentration of Cefotaxime was determined in serum, bone, tissue fluid, peritoneal exudate and bile under clinical conditions in 43 patients with a mean age of 61 years. Pharmacokinetic constants were calculated using a modified two compartment model. The basis for these calculations was a mean serum curve from 299 serum samples. The geometric mean and deviation factors of time-dependent values in tissue and tissue fluid are presented. The results are compared to the minimal inhibitory concentrations of pathogenic bacteria. The clinical success in 121 severe infections at a dose of 2 g every 12 hours support the theoretical results of this study.

[The excretion of methylmethacrylate from bone cement through the lungs (author's transl)].

By using a continuous infra-red monitoring technique, it could be shown that in 42 patients undergoing TEP surgery methylmethacrylate is excreted through the lungs. The amount of monomer eliminated in the exhaled air is less following acetabulum implantation than following femoral shaft implantation. The concentration of monomer in the exhaled air is strongly influenced by measures such as femoral shaft drainage, washing the implantation site with icewater and lengthening the cement mixing time which reduce the transfer of the monomer. Pulmonary elimination of MMA-monomer takes 20 min and is very fast.

On auditory evoked potentials and heart rate in man during whole-body vibration.

Auditory evoked brain potentials and heart rate were recorded from three healthy male subjects during sinusoidal whole-body vibration exposure in the longitudinal (+/- aZ) direction (two intensities: I1 = 0.57 ms-2 r.m.s., I2 = 3.2 ms-2 r.m.s., frequency: 4 HZ) and under no-vibration control conditions according to a change-over design. All conditions were performed at a constant noise level. The part of vibration-synchronous activity contaminating the averaged evoked potentials (AEP) was eliminated by means of a subtraction technique. The AEP amplitude N1--P2 showed a significant decrease during vibration exposure; this decrease was slightly greater during I2 than during I1. Except for a shortening of P2 due to vibration of I1, the peak latencies did not change significantly. The heart rate increased when subjects were exposed to vibration at I1; there were interindividually opposite significant changes under I2 exposure. Time effects and subject effects were also proved. The AEPs are considered to be an informative measure for studying the effect of vibration on central nervous information processing.

The effect of nisin on murein synthesis.

Nisin inhibits murein synthesis with concomitant accumulation of undecaprenyl-pyrophospho-MurHAc(pentapeptide) (lipid intermediate I). This inhibition is caused by the formation of a complex between the antibiotic and lipid intermediate I. Undecaprenyl-pyrophospho-MurNAc(pentapeptide)-GlcNAc (lipid intermediate II) also forms a complex with nisin. However, when murein synthesis is inhibited by nisin, this latter complex is not formed since lipid intermediate II is not longer synthesized.

[Prevention of surgical wound infections by means of ring drapes].

In order to test the efficiency of wound protecting ring drapes and incisional drapes we carried out a prospective study of 283 patients, who underwent larger operations of the gastrointestinal tract from 1975 to 1979. Whilst complications of wound healing following operations of the upper gastrointestinal tract were not reduced, a reduction of the rate of secondary wound healing by 20% was achieved after operations on the large bowel. Overweight did not cause a higher rate of wound sepsis as it is usually the case when wound protecting drapes are not used. A further significant reduction of complicated wound healing could be observed after preoperative systemic application of antibiotics. This effect could not be stated when antibiotic prophylaxis was started after the operation.

[Follow-up studies on the behavior of mineral metabolic parameters with particular reference to the degree of skeletal minearlization in the milch cow in the period 4 weeks ante- to 16 weeks postpartum].

The following parameters were tested in clinically intact dairy cows and those afflicted with parturitional paresis, over a period from four weeks before to 16 weeks after parturition: calcium, inorganic phosphate (PA), magnesium, alkaline phosphatase (AP), glucose, total protein, mineral mass/fat-free dry matter (A/FFT), mineral mass/volume of fresh skeletal bioptate (A/V). Literature data regarding the behaviour patterns of Ca, PA, Mg, AP, and glucose were confirmed. The dynamics of physiological mineral catabolism and mineral anabolism were expressed by the A/V changes more clearly than they had been reflected in A/FFT. The criterion A/V should be included as a function of age in the spectrum of parameters of metabolic monitoring.

[Dynamic behavior of the adaptive control system, hypothalamus--anterior pituitary--adrenal cortex. Animal experimental study of the adequate system dynamics of the direct and indirect parameters of the adaptive control system].

The dynamic response of the adaptive closed-loop control system hypothalamus-anterior pituitary-adrenal cortex after application of short-time disturb pulses were examined experimentally in the pig. Blood sampling was performed biologically non-reactive by chronically implanted vascular catheters in the upper vena cava. A circadian biphase periodical dynamics characterizes the distrubance response of the adaptive control system, respectively, its direct and indirect parameters. After distrubance variables induced initial excess the plasma corticosteriods undershoot the basic level over a long period. Conversely, the dynamics of the eosinophils is determined through an initial eosinopenic phase and a subsequent typical overshoot. Periodical dynamic is absent in the steady state. The controller of the adaptive system does not show any spontaneous activity in the steady state without stress. The absence of an endogenic periodicity of the dependent parameters likewise gives evidence of an active oscillator. On the other hand, the controlling unit of the adaptive system constitutes a control response of circadian periodicty after activation by disturbance variables in pulse shape. The high damping of the control system terminates the transient state within 24 hours, and the corticosteriods and the eosinophils reach the steady state level. This control quality of the adaptive control system gains a high importance in the origination of the spontaneous circadian periodicity by corticosteriods and eosinophils and for the biological compensation of exogenous distrubance variables.

Staffing pattern of a health maintenance organization. A systems approach.

A systems analysis approach was applied to a study of health provider jobs in a health maintenance organization to develop a proposal for an improved staffing pattern and an appropriate training curriculum for the provider. Physicians reported performing 512 ambulatory care tasks; health associates, 372; and health assistants, 284. Overlap was found among each group, especially in data-gathering tasks. This led to reallocation of tasks, with the most complex but smallest number assigned to the physician, and the least complex but highest number assigned to the assistant. In turn, a staffing pattern was suggested whereby a team of two physicians, four associates, and eight assistants would be able to manage the visits of 10,000 patients.
