Antibodies against dietary antigens in rheumatoid arthritis patients treated with fasting and a one-year vegetarian diet.

OBJECTIVE: To compare serum antibody activity against dietary antigens in patients with rheumatoid arthritis (RA) and healthy controls, and to examine whether anti-food antibody activity fluctuated with disease activity during a trial of fasting followed by a one-year vegetarian diet. METHODS: Serum IgG, IgA and IgM antibody activity against several food antigens was measured by an enzyme immunoassay. Abnormally high antibody activity was defined as values above the 90th percentile of the measurements in 30 healthy controls. Serum IgE antibody activity was measured by a radioallergosorbent test. RESULTS: During the trial 10 of 27 patients suspected that certain food items aggravated their arthritis symptoms. Elevated antibody activity against one or more of the dietary antigens was found in all RA patients, but these measurements could not be used to predict which food would aggravate the symptoms. Elevated IgG and IgA antibody activity against alpha-lactalbumin was found in a significantly larger number of RA patients than in controls. With the exception of one patient, there was no concordance between the clinical course and antibody activity against the various dietary antigens. CONCLUSION: The results indicate that a systemic humoral immune response against food items is probably not involved in the pathogenesis of RA.

Crocodile bites and traditional beliefs in Korogwe District, Tanzania.

OBJECTIVES: To investigate why fatal crocodile bites are increasing in a Tanzanian district and the importance of traditional beliefs and superstitions in determining the residents' response to the crocodiles. DESIGN: Information about beliefs was obtained by interview of Korogwe residents. Human and crocodile fatality statistics were obtained from the Korogwe Department of Natural Resources. SETTING: Villages within Korogwe District. SUBJECTS: Population of Korogwe District. RESULTS: Crocodiles have been responsible for 51 deaths in the 52 months from January 1990 to April 1994. Of these, 18 deaths occurred in the first four months of 1994. CONCLUSIONS: Local beliefs and superstitions about crocodiles include those about the taming of animals, with implications concerning the choice of victim and the penalties that may ensue if a crocodile is killed. The recent rise in human fatalities is thought to relate to increasing river pollution reducing the fish supply, together with a change in social mores at the riverside which has increased the crocodiles' displeasure. A reliable pumped water supply would reduce the need to draw water and bathe in the river, and eradication of superstition would empower the villagers in the fight against a common enemy.

Competitive athletes and injuries: a rehabilitation performance perspective. Interview by Charles A Maher and Mary Faut Rodts.

The injured competitive athlete can be considered from a perspective that we term a rehabilitation performance perspective. Through consideration of the injured competitive athlete in this way, the health care provider can be guided in discovering and deciding how to formulate assessment protocols and plan for effective interventions. Aspects of our rehabilitation performance perspective and its seven psychologic states are illustrated with selected comments from an interview with Haley Scott. Scott, a competitive swimmer and student athlete from the University of Notre Dame, who had been paralyzed from a bus accident, has now returned to competition, school, and a full life.

ACTH and TSH producing ectopic suprasellar pituitary adenoma of the hypothalamic region: case report.

A 34-year-old woman with a long-term history of amenorrhoea, headache and visual disturbances was operated for a hypothalamic tumor which could be completely removed. Postoperatively the patient developed a transient SIADH-syndrome and deep vein thrombosis; otherwise the clinical course was uneventful. There has been no sign of tumor recurrence at a follow-up period of fifteen months. Histological examination of the tumor revealed an ectopic pituitary adenoma with production of ACTH and TSH shown by immunohistochemistry.

FHSAs and family health services.

Family health services and their management consume a large portion of the NHS budget, but the key features of the services, and how they are organised, are not widely known. In fact the national framework for family health services predates the NHS as a whole by more than 30 years.
