Orchidectomy alone versus orchidectomy plus radiotherapy in stage I nonseminomatous testicular cancer: a randomized study by the Danish Testicular Carcinoma Study Group.

All Danish patients with stage I nonseminomatous testicular cancer diagnosed between December 1980 and January 1984 entered a randomized study comparing irradiation of retroperitoneal lymph nodes with surveillance only after orchidectomy. Twenty-four of the seventy-nine patients in the observation-only group have relapsed, three patients relapsing more than 2 years after orchidectomy. Ten of the seventy-three patients receiving irradiation have relapsed, all within 10 months after orchidectomy. The median time to relapse in both groups was 4.5 months. Irradiation prevented retroperitoneal relapses. Thirty-three of the relapsed patients were rendered disease free with chemotherapy, and one is still being treated. Four deaths have occurred, all unrelated to testicular cancer or antineoplastic treatment. Absence of embryonal carcinoma and presence of teratocarcinoma correlated with improved relapse-free survival. Patients with increased serum concentrations of tumour markers before orchidectomy had an increased risk of relapse. Surveillance-only is a reasonable treatment strategy in clinical stage I nonseminomatous testicular cancer. Preferably control and treatment of relapses should take place in specialized centres.

Brief psychiatric rating scale for children: quantitative scoring of medical records.

The utility of the Brief Psychiatric Rating Scale for Children (BPRS-C) for transcribing narrative medical records into standard quantitative form for research purposes is examined. Three clinicians independently read and rated the records of 40 child and adolescent patients. Interrater reliabilities were adequate in most major domains of psychopathology, the one notable exception being symptoms in the anxiety domain. On the basis of these results, the BPRS-C appears appropriate for use in administrative and audit reviews, as well as in epidemiological and program evaluation research in which medical records are the primary sources of information.

Risk of acute nonlymphocytic leukemia and preleukemia in patients treated with cyclophosphamide for non-Hodgkin's lymphomas. Comparison with results obtained in patients treated for Hodgkin's disease and ovarian carcinoma with other alkylating agents.

Of 602 patients treated for non-Hodgkin's lymphomas, 9 developed overt acute nonlymphocytic leukemia or preleukemia with refractory cytopenia and cytogenetic abnormalities of the bone marrow. A Kaplan-Meier estimate of the cumulative probability of leukemic complications was 6.3 +/- 2.6% (mean +/- SE) 7 years after start of treatment. All 9 patients with leukemic complications belong to a major subgroup of 498 patients treated with alkylating agents, predominantly cyclophosphamide. The risk of leukemic complications in this subgroup was compared with the risk in 312 patients treated with other alkylating agents for Hodgkin's disease, and with the risk in 553 patients treated with dihydroxybusulfan for ovarian carcinoma. Cumulative 9-year risks were 8.0 +/- 3.3%, 12.8 +/- 3.5%, and 7.1 +/- 1.9%, respectively. The general risk of secondary leukemia after long-term treatment with alkylating agents ranges from 1% to 1.5% per year from 2 to at least 9 years after start of treatment.

[Contents and patterns of gangliosides in the membranes of human erythrocytes].

10-20ml erythrocytes of 17 healthy subjects were separated from leukocytes by passage through cotton. The erythrocytes were haemolyzed in hypotonic buffer and the ghosts isolated by centifugation. Lipid extraction was performed and the gangliosides purified by molselect chromatography. Fractionation of gangliosides was carried out by thin layer chromatography. The quantitative and qualitative determination was performed by thin layer scanning with a High Speed TLC-Scanner Shimadzu CS 920. The sensitivity of the method was about 100 ng lipid bound neuraminic acids. The concentration of neuraminic acids in the total gangliosides was about 14 microgram/20 ml erythrocytes. The distribution of the 4 different ganglioside fractions which could be detected after staining with Svennerholm reagent were: GM3=16%, GM1=52%, GD1a=22%, GT1=90%.

In vitro stimulation of human neutrophil granulopoiesis by unmasking an effect of human serum. Target cell specificity.

Human bone marrow cells from normal subjects were density-separated in continuous density gradients of the Percoll medium. The distribution of cells showed a linear relationship between morphologic maturation and increasing density. The method was used to characterize the target cell of the growth-stimulating effect of ultrasonically irradiated (UI) serum. All experiments showed that a distinct cell population of higher density contained the target cells, corresponding to the distribution of early myelocytes. Furthermore, the target cell was shown not to contain the Ia like antigen and was destroyed by freezing, both facts supporting the assumption of a relatively mature target cell. It is concluded that the divisions at the myelocytic level might be under the influence of a serum factor, unmasked by UI treatment.

Biochemical markers for testicular germ-cell tumors in relation to histology and stage: some experiences from the Danish testicular cancer (DATECA) Study from 1976 through 1981.

The DATECA Project was started in January 1976 and includes all patients with testicular cancer in Denmark. During the first 6 years, 1246 patients entered the joint protocol for histology, staging, management, and clinical use of tumor markers. Only 746 DATECA patients fulfilled the claims for being ideally monitored with preoperative as well as postoperative serologic marker determinations. The overall prevalence of marker positivity, that is, elevated preoperative values of alpha-fetoprotein (AFP) and/or human chorionic gonadotrophin (HCG) was 8% for patients with seminoma and 63% for patients with nonseminomatous disease. The prevalence of increased marker concentration in serum was correlated to stage (higher prevalence in higher stages) and to prognosis (marker-negative patients had a better prognosis than did marker-positive patients). An increased concentration of AFP or HCG was correlated with the presence of a primary endodermal sinus tumor and choriocarcinoma component, respectively.

Secondary surgery in advanced testicular germ-cell tumors.

Twenty-four patients with residual tumor after intensive chemotherapy for advanced testicular germ-cell tumors were subjected to secondary surgery. Twenty patients had complete resection with the following distribution of histological types: 4 embryonal carcinoma, 9 mature teratoma and 7 fibrous tissue. Eighteen of these patients remain free of disease. Four patients had incomplete resection. Two of these patients with embryonal carcinoma died later despite further treatment; 2 patients with teratoma remain free of disease. Secondary surgery is recommended in cases where residual tumor is found after intensive chemotherapy and where tumor markers are not elevated.

A method for the quantification of cellular composition in lymphoma biopsies.

A method for the classification and quantification of lymphoma cells in non-Hodgkin lymphomas is described. Lymphoma-cell types were defined and used in differential counts of haematoxylin and eosine stained paraffin-embedded sections. The following cell types were distinguished: small lymphoid cells (small lymphocytes and small plasma cells), medium-sized lymphoid cells (round, cleaved and cerebriform lymphocytes, and medium-sized plasma cells), and large lymphoid cells (lymphoblastic, round triple-nucleolated, and pleomorphic types). The intra-observer agreement in the classification of cell types was 43-85%. The agreement in distinguishing small, medium-sized, and large lymphoid cells was 85-92%. In differential counts in contrast to conventional histologic classification all cellular profiles were classified regardless of possible tangential cutting. 92% of the cellular profiles at different sectional planes of the same cells were consistently classified, indicating that tangential cutting of cells only influenced the differential counts to a minor degree. By the quantification of cellular composition in multiple, randomly selected areas, classified as representative at low power magnification, only small intra-lymphoma variations were observed in contrast to marked inter-lymphoma variations. The high reproducibility of lymphoma-cell types in combination with the small intra-lymphoma variation makes the quantifiable method suitable in the histological evaluation of non-Hodgkin lymphomas.

[Communicative performance using hearing aids (author's transl)].

By means of a questionnaire the utilization of hearing aids has been investigated in 550 patients who had received their aids in the way being typical for the Federal Republic of Germany, i.e. after being prescribed by the ENT-specialist the hearing aid is fitted by the hearing aid acoustician. Additionally the study aimed to analyze relevant factors influencing the use of hearing aids. 45% of the patients used their aids always, 52% regularly (on special occasions) and 3,1% never. The most common complaints of patients refusing their hearing aid were disturbing noise and unsatisfactory understanding. A relatively better utilization of hearing aids--as compared with the average use of the whole group--was found in children, in patients with a hearing loss of a higher degree or with a conductive hearing loss and in patients wearing aids on both ears or aids having a PC-circuit or a so-called "dynamic compression". Relatively lower utilization was seen in patients being older than 60 years, in patients wearing the hearing aid on the worse ear and in workmen. Further factors related to less regular use were: mild hearing loss and high-tone deafness, bone conductive earphone, eyeglass hearing aid and AVC-circuit. There was no correlation between the hearing aid use and the monosyllable discrimination score.
