Fibrinolysis in chronic arteriosclerotic occlusions: intrathrombotic injections of streptokinase. Work in progress.

Forty-seven patients with chronic arteriosclerotic occlusions of iliac and femoropopliteal arteries were treated by intrathrombotic fibrinolysis. The occlusions were 10-65 cm (mean, 22 cm) long and 6 weeks to 2 years (mean, 4.5 months) old. By means of consistent intrathrombotic injections of 2,500 units of streptokinase every 5 minutes, the thrombi were recanalized within 1-7 hours (mean, 2.5 hours). The primary recanalization rate was 75% (35/47), the patency rate after 2 weeks, 68%. In 29 patients (62%), a residual stenosis had to be dilated by balloon angioplasty. Because of the low total dose of streptokinase (mean, 70,000 units), the thrombin time was elevated up to twice the normal value in only one patient. Bleeding that required transfusions was observed in only two patients (4%). Advantages of intrathrombotic fibrinolysis include higher recanalization rate, lower total dose of streptokinase, fewer bleeding complications, and shorter therapy time than previously reported with other treatments.

[Diabetic osteoarthropathy].

The diabetic osteoarthropathy is an important diabetic complication. There is an obligatory connexion of the diabetic osteoarthropathy with the diabetic neuropathy. It seems to be essential for the diagnosis and treatment to separate the neuropathic and arthropathic lesions from angiopathic changes. It should be paid more attention to the diabetic osteoarthropathy in the treatment and training of diabetic patients suffering from diabetic neuropathy.

[Therapeutic embolization with polyvinyl alcohol (Ivalon). Clinical experiences].

Polyvinyl alcohol is rigid foam used in pressed or unpressed form for the purpose of embolisation. It is suitable for both micro-embolisation in the vascular periphery and for central vascular occlusion. Polyvinyl alcohol will result in permanent vascular occlusion. Basing on the experience collected with 71 embolizations, the article reports on the technique applied and on the results obtained with craniofacial, thoracic, abdominal, pelvic and peripheral embolisation.

[Transcatheteral embolization with polyvinyl alcohol--technic and experimental studies].

The article presents hardened polyvinyl alcohol foam (PVA) as embolisation material. PVA is well tolerated by the tissue and produces permanent vascular sealing. It is possible to achieve peripheral embolisation of arteries having a diameter of approx. 100 microns, as well as sealing of vessels with diameter of up to 8 mm. Pressed PVA is used for occluding larger vessels. This PVA re-assumes its original shape and dimensions in liquids, depending on their osmolarity and molecular size. Depending upon the pressure exercised, compression and re-expansion can be fourfold to tenfold.

[B-scan sonography of the carotid artery].

176 sonograms of carotid arteries are compared with the corresponding angiograms to evaluate the diagnostic possibilities of B-mode sonography in detecting arteriosclerotic vessel disease. Ultrasound tomography is proved to be a reliable method to rule out arteriosclerosis; the specificity of the investigation method is 100%. However, the diagnostic value varies concerning the visualization of different arteriosclerotic lesions. B-mode sonography is of minor significance in detecting arteriosclerotic ulcers. Fresh thrombi are often not diagnosed. The sensitivity of this investigation method therefore corresponds 85%.

[Percutaneous transthoracic fine-needle biopsy of the lung. Indications, results and complications in 578 punctures].

578 fine-needle biopsies of lung lesions in 319 patients were performed. This investigation was mainly done by means of Rotex-biopsy instruments. The procedure proved to be efficient in 84%, complicated in 19,7% of the patients. Major complications were seen in 5,6% only. Percutaneous needle biopsy in appropriate for obtaining a cytological diagnosis to clarify the etiology of localized lung lesions.

[Visceral arteriography of hepatic, pancreatic and intraperitoneal tumours during childhood (author's transl)].

The value of visceral arteriography is discussed in relation to four hepatoblastomas, two vascular hamartomas of the liver, one inactive islet cell tumour of the pancreas and two malignant intraperitoneal tumours. Although non-invasive methods, such as computer tomography, sonography and scintigraphy are valuable, angiography is an important part of the pre-operative diagnosis of visceral tumours during childhood.

[Quality criteria - urography (author's transl)].

The technique of urography is discussed on the basis of the pharmacokinetics of contrast media with easy renal passage. The quality characteristics of a technically good urogram are defined. The factors influencing the quality of this method of examination are discussed.

[Clinical diagnosis of normal pressure hydrocephalus (author's transl)].

5 patients are described in which normal pressure hydrocephalus was diagnosed. The criteria on which the diagnosis was based were a cerebrospinal fluid perfusion test and an air study. Pathological verification was additionally obtained in 2 patients. The clinical picture in all patients was characterized by dementia. 4 patients were incontinent and 3 were paraspastic.

Radiological detection of glioblastoma recurrence.

Brain scanning is of great value in detecting glioblastomas. Since there is little stress upon the patient by this method it is specially suitable for follow-up after surgical treatment and radiation therapy of this tumour, thus giving the best chance of an early detection of a recurrence. On the other hand angiography is essential if further surgery is necessary. Diagnostic problems may arise when using arteriography in cases of avascular recurrences and lack of space-occupying symptoms due to the following factors: Position and small size of the recurrence, growth into the operation cavity, and postoperative cerebral atrophy. Avascular mass lesions of other origin may mimic a tumour recurrence in the angiogram. Thus, a combined use of isotope studies and radiological investigations is necessary for an unequivocal detection of glioblastoma recurrences.

[Varicosities of the renal veins, the renal pelvis and the ureteric veins (author's transl)].

Varicosities of the renal veins and of the veins of the upper urinary tract may occur with or without haematuria. The urogram may be negative. Frequently, however, there are single or multiple impressions on the ureters as a result of the venous varicosities. Arteriography, which should always be carried out in the presence of undiagnosed haematuria, is unable to demonstrate the venous abnormalities. Since the symptoms are also not characteristic, it is necessary to carry out renal vein pharmaco-phlebography. The clinical features, radiological appearances and aetiology of this rare condition are illustrated by three cases.
