Adhesion of Staphylococcus saprophyticus to renal tubular epithelial cells is mediated by an N-acetyl-galactosamine-specific structure.

S. saprophyticus CCM883 and 9325 were found to adhere to the tubular cell line LLC-PK1. An ELISA technique was used to determine adherence of bacteria and inhibition of adherence by various carbohydrates. Only N-acetyl-galactosamine was found to significantly inhibit adhesion (p less than 0.001), which suggests that the surface component mediating adhesion recognizes structures on the target cell that contain this carbohydrate.

[Muscle relaxation using atracurium in porphyria variegata--case notes].

A case is presented involving the use of atracurium (120 mg) as a muscle relaxant in a patient with known variegate porphyria. The concentrations of urinary porphyrins and porphyrin precursors did not increase after this anaesthetic, and no exacerbation of the disease was noted. This single case suggests that atracurium can be used safely in patients with porphyria.

[The diagnostic value of the plain abdominal radiogram in differentiating rotavirus-negative and rotavirus-positive necrotizing enterocolitis].

Clinical data and radiographic findings of 32 newborn suffering from necrotising enterocolitis were analysed [12 patients with rotavirus-positive necrotising enterocolitis (RV + NEC), 20 patients with rotavirus-negative necrotising enterocolitis (RV-NEC)]. The presence and degree of pneumatosis intestinalis, portal venous gas and pneumoperitoneum on abdominal radiographs were graded after Kosloske et al. according to "mild, moderate, and severe". Pneumatosis intestinalis occurred twice as often in the ascending colon in RV-NEC compared to RV + NEC, whereas the transverse colon was involved nearly as frequently as the descending colon in both groups. Portal venous gas was present in 10% of the cases with RV-NEC and was absent in RV + NEC. Pneumoperitoneum only occurred in 8% of RV + NEC but in 20% of RV-NEC. Radiographic findings are helpful in the differentiation between both groups of NEC. Conservative therapy is preferable especially in mainly distal colon distribution of pneumatosis intestinalis without pneumoperitoneum.

[The skeletal changes in premature infants with a copper deficiency].

We describe 5 preterm infants (25th to 30th week of gestation) suffering from alimentary copper deficiency. The diagnosis was confirmed by low serum copper and caeruloplasmin concentrations. Characteristic clinical findings were repeated apnoeic attacks, hypopigmentation of skin and hair, anaemia, neutropenia and leucopenia refractory to other therapy, as well as increasing serum alkaline phosphatase activity in the first month of life. Starting in the 3rd to 12th week of life the radiographic findings were general skeletal osteoporosis and retardation, metaphyseal radiodense lines, irregular metaphyses, cupping and spurring of the metaphyses, followed by multiple fractures and subperiosteal new bone formation and enlarged costochondral junctions. Copper was substituted orally resulting in complete healing of fractures and improvement in both clinical symptoms and laboratory parameters.

Sarcoid heart disease--results of follow-up investigations.

52 patients thought to have sarcoid heart involvement have been followed up. By administration of a standardized glucocorticoid therapy we demonstrated improvement of patients' symptoms, ECG disturbances, thallium scan defects of left heart muscle and there was 60-70% regression of pulmonary sarcoidosis. There was a low death rate. We conclude that our diagnostic measures are reliable, and that glucocorticoid therapy is efficacious in the early stages of the disease.

Differences in the clinical and radiologic patterns of rotavirus and non-rotavirus necrotizing enterocolitis.

We analyzed retrospectively 32 successive infants who developed necrotizing enterocolitis (NEC), 13 with rotavirus (RV) infection (RV+) and 19 RV-negative (RV-). All patients showed at least pneumatosis intestinalis. All patients except one had risk factors for perinatal asphyxia. Our study demonstrated significant differences between RV+ NEC and RV- NEC cases: RV+ NEC infants had a higher birth weight and were born at a later gestational age. Oral feeding was started earlier and symptoms developed later and more insidiously in RV+ patients than in RV- NEC babies. Radiology revealed a less severe and more distal colon involvement in RV+ NEC infants, whereas the RV- NEC patients mostly had small intestinal or ileocecal changes and more frequent complications of pneumoportogram and intestinal perforations. These latter infants often had a rapidly deteriorating clinical course; 84% needed surgical treatment. In conclusion RV may be a cause of NEC in susceptible infants. Historic and clinical data and a more distal colonic pneumatosis allow a differentiation of RV+ NEC from other forms of NEC.

Biosynthesis and desaturation of prokaryotic galactolipids in leaves and isolated chloroplasts from spinach.

Mono- and digalactosyldiacylglycerol (MGDG and DGDG) were isolated from the leaves of sixteen 16:3 plants. In all of these plant species, the sn-2 position of MGDG was more enriched in C(16) fatty acids than sn-2 of DGDG. The molar ratios of prokaryotic MGDG to prokaryotic DGDG ranged from 4 to 10. This suggests that 16:3 plants synthesize more prokaryotic MGDG than prokaryotic DGDG. In the 16:3 plant Spinacia oleracea L. (spinach), the formation of prokaryotic galactolipids was studied both in vivo and in vitro. In intact spinach leaves as well as in chloroplasts isolated from these leaves, radioactivity from [1-(14)C]acetate accumulated 10 times faster in MGDG than in DGDG. After 2 hours of incorporation, most labeled galactolipids from leaves and all labeled galactolipids from isolated chloroplasts were in the prokaryotic configuration. Both in vivo and in vitro, the desaturation of labeled palmitate and oleate to trienoic fatty acids was higher in MGDG than in DGDG. In leaves, palmitate at the sn-2 position was desaturated in MGDG but not in DGDG. In isolated chloroplasts, palmitate at sn-2 similarly was desaturated only in MGDG, but palmitate and oleate at the sn-1 position were desaturated in MGDG as well as in DGDG. Apparently, palmitate desaturase reacts with sn-1 palmitate in either galactolipid, but does not react with the sn-2 fatty acid of DGDG. These results demonstrate that isolated spinach chloroplasts can synthesize and desaturate prokaryotic MGDG and DGDG. The finally accumulating molecular species, MGDG(18:3/16:3) and DGDG(18:3/16:0), are made by the chloroplasts in proportions similar to those found in leaves.

Treatment of bacterial vaginosis with metronidazole or pivampicillin.

In a double-blind randomized controlled trial from general practice, we assessed the efficacy of treatment with either metronidazole or pivampicillin in patients fulfilling the diagnostic criteria of bacterial vaginosis. 50 women were treated with metronidazole 500 mg twice daily and 54 with pivampicillin 700 mg twice daily, both for seven days. Evaluated four weeks after the start of treatment, we found a significantly higher cure rate in the metronidazole group (90%) than in the pivampicillin group (69%) (p = 0.01). Cure was defined as lack of fulfillment of the diagnostic criteria of bacterial vaginosis. Test for confounding according to parity and complaint of vaginal discharge showed no influence. Side-effects were significantly commoner in the pivampicillin group (28%) than in the metronidazole group (8%). We consider that metronidazole is a potent drug in the treatment of bacterial vaginosis, with pivampicillin as a useful alternative.

Obstructive ventilatory impairment. An investigation in general practice.

We estimated the prevalence of obstructive ventilatory impairment and its relationship to sex, age, chronic bronchitis, asthma, and smoking in a cross-sectional epidemiological study. The population consisted of 3119 persons greater than or equal to 16 years listed as patients in our general practice. From this group, 2740 (88%) underwent dynamic spirometry (Vitalograph). The main results showed a high prevalence of obstruction. 26% had MMEF values less than 75%, 8% FEV-1 less than 75% and 5% FEV-1/FVC less than 65%. Obstruction was significantly correlated with old age, chronic bronchitis, asthma, and smoking.

[An etiological study of eosinophilic granuloma].

By means of the proof for the enzyme phenol oxidase (PO; EC 1.14.18.1) was investigated, whether the eosinophilic granuloma correspond to a cell mediated immunological defence reaction. The method allowed to determine the eosinophils rapidly and surely and give an overview on the frequency and distribution of these cells. By electron microscopy was Langerhans-cells identified in close contact to many eosinophils. Their granula shows manifold changes in their structure. The eosinophilic granuloma is a cell mediated defence reaction approximate to the type of a delayed hypersensitivity.

[Non-surgical removal of a knotted Swan-Ganz catheter].

Intracardiac knotting is a rare complication of pulmonary arterial catheterization. This paper describes a 68-year-old male patient who had a 7-F Swan-Ganz catheter inserted via the internal jugular vein. The postoperative chest roentgenogram revealed a catheter knot in the right ventricle. Using fluoroscopy it was possible to pull back the knot up to the indwelling introducer. By pulling the knot against the introducer its size was reduced to a minimum. After this manoeuvre, both the introducer and the small knot could be withdrawn from the jugular vein without further incident. The possible complications of this method and of alternative techniques are described and discussed. Surgical intervention in the case of a knotted Swan-Ganz catheter may only be necessary when intracardiac fixing of the knot is encountered.

[Epidemiology of liver diseases in the GDR, 1979-1989].

From 1979 to 1988 in the GDR the number of patients with alcoholic liver cirrhosis increased from 2,387 to 3,958 (61.2%). An evident decrease of patients with acute virus hepatitis was observed (from 7,037 in 1979 to 1,578 in 1989). In future a further increase of alcoholic liver diseases with the resulting complications (e.g. coma, bleeding varices and withdrawal symptoms) has to be expected.

[Investigation of caries occurrence and occlusal restoration in beginning school children in Erfurt compared to Marburg an der Lahn].

At the end of 1990 in the area of Erfurt 309 and in and around Marburg an der Lahn 348 school beginners were examined by one children's dentist under identical conditions. The results (milk teeth) showed that the caries incidence (dmfs) was comparable in both regions. However, the proportion of restored milk teeth tended to be higher in Erfurt than in Marburg. In addition, the differences in individual caries incidence were more marked. In Marburg there were more caries-free children, but children whose teeth had been damaged by caries were more seriously damaged than those in Erfurt.

[Neurologic complications after bone marrow transplantation].

The retrospective study presented here reports clinical, electrophysiological and CT-data from 21 patients who underwent bone marrow transplantation for leukemia or aplastic anemia between 1980 and 1985. The aim of the investigation, which was performed at the end of 1986, was to elucidate whether antineoplastic drugs and radiation can result in long-term damage of the central nervous system. In 9 patients we observed neurological signs (1 leukencephalopathy, 5 cerebellar symptoms). Seven patients revealed dysrhythmia in the EEG and five had delayed latencies of visual evoked potentials. Eight patients exhibited pathological changes in CT (3 atrophy of supratentorial structures, 3 periventricular lucencies, 2 a combination of both). The results indicate that long-term damage to the CNS may be encountered after successful bone marrow transplantation.
