[Evaluation of the effects of coronary artery bypass grafting on left ventricular function in patients with previous myocardial infarction].

The present study was designed to assess a left ventricular function following coronary artery bypass grafting (CABG) in patients with previous myocardial infarction (MI). The subject was consisted of 10 patients with MI (MI(+) group) and 6 without it (MI(-) group). Both groups underwent complete revascularization. Stroke index (SI), ejection fraction (EF), systolic pressure/end-systolic volume index (SP/ESVI) and end-diastolic volume index (EDVI) were evaluated utilizing radionuclide angiography at rest and during exercise (Ex) before and after CABG. The influence of size of myocardial infarction on left ventricular function were also analyzed. Preoperatively during Ex, EF showed decrease, SP/ESVI no change and EDVI increase in both MI(+) and MI(-) group compared with those at rest. Postoperatively during Ex, SI, EF and EDVI showed no change and SP/ESVI increase in both MI(+) and MI(-) group compared with those at rest. Ejection fraction and SP/ESVI during Ex in MI(+) group had significant differences compared with those in both MI(-) group and control group. Ejection fraction and SP/ESVI during Ex in MI(-) showed no difference compared with those in control group. The size of MI suggested by ECG scoring system (proposed by Wagner GS et al, Circulation '82) had negative correlations with both Ex-induced increment of postoperative EF and Ex-induced increase ratio of postoperative SP/ESVI. These data indicated that complete revascularization improved EF and SP/ESVI in patients with MI, and moreover normalized those in patients without MI. But in the case of extensive MI, even complete revascularization might not recover poor functional reserve during Ex.

[Results of sequential aorto-coronary bypass grafting using saphenous vein graft].

Seventy patients underwent sequential coronary artery bypass grafting with saphenous vein during 28 months period. Seventy eight sequential grafts comprising 165 distal anastomoses were performed. Early death was 2.9%. Postoperative angiographic evaluation within 6 months showed 86% patency of the 65 sequential grafts and 89% patency of the 136 distal anastomoses. The patency of anastomosis to left anterior descending artery (LAD), diagonal branch (Dx), obtuse marginal branch (OM), posterior lateral branch (PL), and posterior descending branch (PD) was 87% (13/15), 92% (36/39), 91% (40/44), 81% (26/32) and 100% (6/6), respectively. The patency of Dx-LAD, Dx-OM and OM-PL was 86% (12/14), 94% (15/16) and 76% (13/17), respectively. There was no significant difference in the patency rate according to the site of anastomosis and the inner diameter of the coronary artery. The patency of side to side anastomoses was 92% (65/71) which was not significantly different from that of the end to side anastomoses; 86% (56/65). These results showed sequential aorto-coronary vein grafting means to be effective for the complete coronary artery revascularization .

[Basic and clinical studies on aztreonam in obstetric and gynecologic infections during the perinatal period].

The basic and clinical efficacies of aztreonam (AZT) were evaluated in obstetric and gynecologic infections during the perinatal period. The results obtained are summarized below. 1. Concentrations of AZT in umbilical venous blood and amniotic fluid showed a good placental transfer of AZT. The rate of placental transfer of AZT was very similar to rates for cephalosporins. 2. Since the clinical efficacy rate of AZT in the treatment of Gram-negative bacterial infections was 80.0%, AZT appears to be a useful drug against Gram-negative bacterial infections.

EM-523, an erythromycin derivative, and motilin show similar contractile activity in isolated rabbit intestine.

The effect of EM-523 [de(N-methyl)-N-ethyl-8,9-anhydroerythromycin A 6,9-hemiacetal], an erythromycin derivative, on preparations of isolated intestine of rabbits, rats and guinea pigs was investigated and compared to the effects of motilin and prostaglandin F2 alpha (PGF2 alpha). EM-523 and motilin induced contractions in the rabbit intestinal preparations but not in the rat or guinea pig preparations. In contrast, PGF2 alpha induced contractions in all three intestinal preparations. The rabbit duodenum and jejunum were more sensitive than the ileum to EM-523 and motilin, but no selectivity was observed with PGF2 alpha. The contractile responses induced by EM-523 and by motilin were not influenced by pretreatment with tetrodotoxin, atropine, naloxone or mepyramine but were greatly suppressed by pretreatment with verapamil or removal of calcium ions from the medium, suggesting that both agents induce intestinal contractions by acting directly on smooth muscle and that their actions depend largely on extracellular calcium. The contractile response to EM-523 or motilin was reduced markedly after treatment of the preparation with a high concentration of either agent, and cross-tachyphylaxis between EM-523 and motilin was also observed. These findings indicate that the contractile activity of EM-523 is very similar to that of motilin and that EM-523 and motilin may share the same site of action.

Anti-human T-cell lymphotrophic virus type I antibody-positive adult T-cell leukemia/lymphoma with no monoclonal proviral DNA. A clinicopathologic and immunologic study.

Proviral DNA of adult T-cell leukemia virus (HTLV-I) was examined by the standard Southern blotting method in lymph nodes of 45 patients with anti-HTLV-I antibody (ATLA)-positive adult T-cell leukemia/lymphoma (ATLL). Six of these patients revealed no monoclonal proviral HTLV-I DNA in tumor cells. These six patients showed typical flower cells in peripheral blood; they comprised five cases of the smoldering type and one of lymphoma type. They showed a longer clinical course than ATLL patients with integrated proviral HTLV-I DNA. Five of the six patients were alive from 8 to 36 months after onset; the other patient died 9 months after onset. Histologically, they exhibited features of T-cell malignancy but with absence of the typical cerebriform giant cells that are usually present in ATLL. The tumor cells represented T-cell markers, usually CD4, but CD25 was negative. Rearrangement of the T-cell receptor gene C beta was found in four of the six cases. On the basis of these results, cases of ATLL with no monoclonal proviral HTLV-I DNA should be clinicopathologically differentiated from those with integrated proviral DNA.

[A young adult male with primary carcinoma of the jejunum].

A 37-year-old male was admitted to our hospital because of epigastric pain and weight loss. He was in a stable state of health until 2 months earlier but gradually worsened and lost weight about 13 kg during 1 month. After 1 month, he consulted our clinic. Upper GI barium studies revealed malignant stenosis in about 50 cm from the ligament of Treitz. Radical surgical resection was performed including the segment in 25 cm of intestine containing the primary tumor. Lymph node and liver metastases were not seen. Pathologic findings revealed "well differentiated adenocarcinoma". Thirteen months after the operation, he is well and has gained weight to state of health.

Epidermal growth factor as a regulatory hormone maintaining a low pH microclimate in the rat small intestine.

This study was designed to determine the effect of epidermal growth factor (EGF) in the lumen on the pH of the intestinal surface in the rat jejunum, which is referred to as "microclimate-pH". In the control experiment, a significant pH gradient was observed between the mucosal surface (approximately pH 6.8) and the bulk phase (approximately pH 7.3). The microclimate-pH was decreased by 0.2-0.6 pH units after addition of higher concentrations of EGF (3-100 nM) to the lumen. The microclimate-pH thus decreased recovers to the control value by replacing EGF with TES buffer, suggesting that the EGF effect is reversible. Considering that the Na+-H+ exchanger exists on the luminal membrane of the intestinal cells, the decrease in the microclimate-pH which was induced by EGF added to the luminal side may be due to the activation of Na+-H+ exchanger.

Normal human chromosome 11 suppresses tumorigenicity of human cervical tumor cell line SiHa.

We examined the ability of human chromosome 11 derived from normal fibroblast cells to suppress the tumorigenicity of SiHa cells, a human cervical tumor cell line. Using DNA transfection, the human chromosome was tagged with a selectable marker (the pSV2neo gene, which encodes resistance to the antibiotic G418), transferred to mouse A9 cells by cell hybridization and microcell transfer techniques, and then transferred to SiHa cells by microcell transfer. These procedures resulted in the appearance of 15 independent, G418-resistant clones, 5 of which had one or two extra copies of an intact human chromosome 11. In situ chromosomal hybridization of these clones with the pSV2neo plasmid revealed the presence of a neo-tagged human chromosome 11 in all of the five SiHa-microcell hybrids. Two SiHa-microcell hybrids that contained a single copy of neo-tagged human chromosome 12 were also isolated by the same methods. The tumorigenicities of SiHa clones with one or two extra copies of chromosome 11 (SiHa-11) were suppressed; four of the five SiHa-11 clones formed no tumors in nude mice, whereas both parental SiHa cells and SiHa cells with an extra chromosome 12 formed tumors within 30 d. One SiHa-11 cell clone formed a single tumor 90 d after injection. This rare tumor had lost one copy of chromosome 11 and rapidly formed tumors when reinjected. These results indicate that the introduction of a single copy of normal human chromosome 11, but not chromosome 12, suppresses the tumorigenicity of SiHa cells, indicating the presence on human chromosome 11 of a putative tumor-suppressor gene (or genes) for human cervical tumors.

Electrophysiological actions of A23187 and X-537A in spontaneously beating and in voltage-clamped rabbit sino-atrial node preparations.

Electrophysiological effects of calcium ionophores, A23187 and X-537A, on spontaneously beating and voltage-clamped rabbit sino-atrial node preparations were examined, using the voltage-clamp technique with two microelectrodes. (1) A23187 (administered cumulatively) increased the cycle length significantly at 3 x 10(-6) and 10(-5) mol/l, and X-537 only at 10(-5) mol/l. Other action potential parameters were unaffected in the presence of these concentrations of either agent. At 2 x 10(-5) mol/l, either agent prolonged the cycle length significantly, but increased the amplitude and the duration of the action potentials and the maximum diastolic potential not to any significant extent. Both X-537A and A23187, at 2 x 10(-5)mol/l, induced a dysrhythmia, which in the former was probably due to delayed afterdepolarizations. (2) In voltage-clamped sino-atrial node cells, the holding current was shifted outwardly, to a greater extent in the presence of X-537A than A23187 at the same concentration (2 x 10(-5) mol/l). The ionophores initially increased the slow inward current and then decreased it. The steady outward current was inhibited, and its activation curve was shifted to a more negative voltage range. X-537A caused a transient inward current and an inward tail current on repolarization to the holding potential. (3) At concentrations of 10 and 18 mmol/l [Ca2+]o or in the presence of isoprenaline 10(-7) mol/l, these ionophores induced a more severe dysrhythmia. Conversely in the nominal absence of [Ca2+]o the regular rhythm was resumed.(ABSTRACT TRUNCATED AT 250 WORDS)

Absence of effect of 16,16-dimethyl prostaglandin E2 on reduction of gastric mucosal blood flow caused by indomethacin in rats.

The effects of 16,16-dimethyl prostaglandin E2 PGE2 on gastric mucosal blood flow and gastric mucosal damage were tested in rats given indomethacin. Blood flow was measured by hydrogen gas clearance. Indomethacin given intragastrically reduced the blood flow in nonlesion areas and the levels of PGE2 and 6-keto-PGF1 alpha in the gastric mucosa and caused mucosal damage in a dose-related way. Indomethacin (20 mg/kg) significantly reduced the blood flow 30 min after administration, and the reduction increased until 90 min. Then the flow plateaued until 240 min. Gastric mucosal damage caused by 20 mg/kg of indomethacin and evaluated by only gross observations, began at 60 min and developed with time until 240 min after administration. 16,16-Dimethyl PGE2 (5 micrograms/kg) did not affect the reduction of blood flow caused by indomethacin during 240 min of measurements, but it significantly inhibited the indomethacin-induced mucosal damage evaluated by gross observations. These results suggest that prevention by 16,16-dimethyl PGE2 of grossly observed gastric mucosal damage caused by indomethacin was not related by preservation of the gastric mucosal blood flow in the areas without lesions.

A case of a postoperative aneurysm of the common hepatic artery which ruptured into the remnant stomach after a radical gastrectomy.

A rare case of an iatrogenic aneurysm of the common hepatic artery which ruptured into the remnant stomach is described herein. The patient had undergone a radical gastrectomy with regional lymphadenectomy one month previously. The iatrogenic surgical trauma to the common hepatic artery incurred during the regional lymphadenectomy with the resection of the perivascular lymphatics and neural plexuses, was considered as the main cause responsible for the postoperative aneurysmal development.

On the characteristics of the inward tail current induced by calcium overload.

The characterization of an inward tail current (Iex) induced by calcium overload and often associated with the oscillatory current (Ios) was studied by means of a two-microelectrode voltage clamp method in sheep cardiac Purkinje fibers. It was found that: (1) the inward tail current requires a prior depolarization to about -25 mV (or less negative values) in order to appear; (2) after depolarization to those values, the membrane needs to be repolarized to about -45 mV (or to more negative values) for the inward tail current to be initiated; (3) the inward tail current becomes larger after larger or longer clamp steps and increases progressively at more negative potentials; (4) the inward tail current can be separated from other known currents, notably from the pacemaker current; (5) the inward tail current is reduced by quinacrine, a blocker of Na-Ca exchange; (6) the inward tail current does not reverse at less negative potentials; (7) the membrane conductance does not increase during the inward tail current. Thus, the inward tail current shows similarities and dissimilarities with the oscillatory current, probably in relation to a common final pathway and to a different mode of onset of the two currents, respectively.

Endothelin: a potent vasoconstrictor associated with coronary vasospasm.

Endothelin, administered into the coronary arteries of anesthetized dogs, produced a profound and long-lasting reduction in coronary blood flow with electrocardiographical evidence of myocardial ischemia. Coronary angiography revealed delayed filling of the distal branches and, in some cases, cessation of the blood flow distal to the epicardial portions of coronary arteries. The coronary vasoconstriction induced by endothelin subsided after intracoronary administration of nitroglycerin. Pretreatment with the Ca2+-channel antagonist, nitrendipine, suppressed endothelin-induced vasoconstriction. These findings suggest that endothelin, produced by vascular endothelial cells, may contribute to the pathogenesis of coronary vasospasm.

Stressor predictability and rat brain noradrenaline metabolism.

This study examined the effects of stressor predictability on regional rat brain noradrenaline (NA) turnover, by measuring levels of a principal metabolite of NA (3-methoxy-4-hydroxyphenylethyleneglycol sulfate, MHPG-SO4). Male Wistar rats were exposed to one of three shock conditions for 19 hr: nonshock, signalled, and unsignalled shocks. Rats in the shock conditions received shock (1.2 mA intensity, 2 sec duration) on a 2.5 min variable time (VT) either preceded by a 12-sec, 10-W light signal (signal-shock interval of 10 sec) or not preceded by this signal. The tail electrodes for these rats were in series, so that the shock received by all rats was of exactly the same number and duration. After 19 hr in a VT-2.5 min shock session, the rats exposed to unsignalled shock (unpredictable group) showed significantly greater increases in MHPG-SO4 levels in the hypothalamus, amygdala, midbrain, cerebral cortex, thalamus and locus coeruleus, as well as in plasma corticosterone levels. Rats exposed to signalled shock (predictable group) showed significant increases in MHPG-SO4 levels in the first four of these regions, as compared to the nonshocked rats. Moreover, the unpredictably shocked rats exhibited greater elevations in MHPG-SO4 levels in the hypothalamus, amygdala, and thalamus, as well as in plasma corticosterone levels, when compared to the predictably shocked rats. These results are consistent with previous reports showing that unsignalled shock induced extensive somatic effects in comparison to signalled shock. The present study suggests that the presence of a signal attenuates the extent of NA release in some brain regions resulting from irregular inescapable shock stress.

Poorly expressed CD2 antigen on the leukemic cells of adult T-cell leukemia and chronic lymphocytic leukemia of T-cell lineage.

Adult T-cell leukemia (ATL) and T-cell chronic lymphocytic leukemia (T-CLL) are hematologic neoplasms in differentiated stages of peripheral mature T cells. This is suggested by the presence of CD2 (E rosette receptor) and mature and/or pan-T cell membrane surface antigens on their leukemic cells. We recently encountered one patient with ATL and another with T-CLL; their leukemic cells poorly expressed CD2 antigens, but clinical presentation, morphology of the leukemic cells and other marker studies were characteristic of either ATL or CLL. The clinical significance of the poor expression of CD2 remains to be further studied. The two patients reported here died of severe complications 4 and 9 weeks after diagnosis. The poor expression of CD2 in the peripheral T cells in these neoplasms is likely to indicate an aggressive nature of the disease.

Insulin-like growth factor-I, GH, insulin and glucagon concentrations in bovine colostrum and in plasma of dairy cows and neonatal calves around parturition.

1. The IGF-I concentrations in colostrum on days 1 and 2 after parturition were higher than those in cow and neonate plasma. 2. The modest increase in GH concentrations in cow plasma around parturition would not be enough to stimulate IGF-I release by tissues. 3. The concentrations of insulin, GH and glucagon in colostrum were substantially lower than those in plasma.
