[Dissecting aneurysm of distal posterior inferior cerebellar artery--case report and review of the literature].

A rare case of dissecting aneurysm of distal posterior inferior cerebellar artery (PICA) is reported. A 51-year-old woman was admitted to our hospital complaining of severe headache and nausea. CT scan revealed subarachnoid hemorrhage which was thicker in the posterior fossa. The vertebral angiography demonstrated an aneurysm on the telovelotonsillar segment (Lister's classification) of the left PICA. On the third day, the left suboccipital craniotomy was performed and the fusiform aneurysm was resected. The postoperative course was uneventful. Histological examination of the resected aneurysm showed a dissection between the ruptured elastic lamina and the tunica media. Dissecting aneurysm of distal PICA is still belong to a rare entity. In all three cases found in the literature, the dissecting aneurysms are sited in the anterior medullary segment of PICA. Probably, this is the first report described a dissecting aneurysm on the more distal part-telovelotonsillar segment of PICA. The clinical features, pathogenesis and treatment of intracranial dissecting aneurysms are briefly discussed with reviewing the literature.

Effects of interleukin 4 on stromal cell-associated bone marrow culture.

We examined the effect of murine interleukin 4 (IL-4) on in vitro hemopoiesis associated with an endothelial-adipose cell line derived from a murine bone marrow stroma and termed 14F1.1. The addition of IL-4 to the co-culture led to the rapid disappearance of hemopoietic progenitors, and when greater than or equal to 20 U/ml IL-4 was added, this effect became remarkable. The stimulation of 14F1.1 cells by IL-4 before the start of co-culture accelerated the disappearance of hemopoietic progenitors, although continuous stimulation by IL-4 was needed. When IL-4 was added to the co-culture 2 weeks after initial incubation, the myeloid lineage progenitor cells disappeared within 1 week. When we examined whether the inhibitory activity on hemopoiesis was induced by 14F1.1 cells stimulated with IL-4, we found no distinct inhibitory activity in the 14F1.1-cell supernatant, but there was a slight increase in colony-stimulating activity. Colony formation of bone marrow cells seeded on top of 14F1.1 cells but separated by a thin 'empty' agar layer was enhanced by IL-4-treated 14F1.1 cells. We also noted the disappearance of the 'cobblestone' area, which had been the site of active hemopoiesis in this culture, together with the disappearance of any granulocyte-macrophage colony-forming units (CFU-GM), thereby implying that IL-4 alters the interaction between hemopoietic progenitors and stromal cells.

[Biventricular assist support using centrifugal pump in cardiogenic shock following coronary bypass grafting--a case report of long survival].

Biventricular assist support (BVAS) with centrifugal pump was used for the treatment of 48 y/o male patient with cardiogenic shock following coronary bypass surgery. Assist flow was 2 to 4 l/min in LVAS and 2 to 3 l/min in RVAS. RVAS was weaned in the fourth postoperative day and LVAS in the sixth. Cerebral infarction developed in the weaning period of LVAS. Post operative examination revealed good right and left ventricular function. Patient was well and returned to full work in a two years and two months after surgery. BVAS may be useful for the treatment of severe biventricular failure associated with stunned myocardium.

Identification of the hyperpolarization-activated inward current in young embryonic chick heart myocytes.

Whole-cell voltage-clamp experiments were performed to examine the underlying currents flowing during the pacemaker potential of spontaneously-beating embryonic chick ventricles. The holding potential was -30 mV. Long-duration (3 s) hyperpolarizing pulses were applied to -40 to -120 mV, in increments of 10 mV. A marked hyperpolarization-activated inward current (If) was produced. In cells from 3-day-old hearts, the threshold potential for the inward current was -50 to -60 mV. In 17-day-old cells, there was almost no If current. At -120 mV, the inward current was -93.8 +/- 6.3 pA (n = 5) in 3-day-old cells and -15.7 +/- 2.8 pA (n = 5) in 17-day-old cells. The average capacitances were 10.1 +/- 2.0 pF (n = 17) in 3-day-old cells, and 6.9 +/- 1.2 pF (n = 14) in 17-day-old cells. The reduction of If paralleled the decrease in spontaneous activity. In the presence of 3 mM CsCl, the inward current was blocked completely, and the tail current was reduced. In addition, 3 mM CsCl depressed the spontaneous action potentials and had a negative chronotropic effect. These results indicate that the hyperpolarization-activated inward If current exists in young embryonic chick heart cells, and decreases during development. This If current may contribute somewhat to the electrogenesis of the pacemaker potential.

[Aneurysms of the distal posterior inferior cerebellar artery; the report of 10 cases].

Non mycotic and non traumatic distal posterior inferior cerebellar artery (PICA) aneurysms are rare, but eleven aneurysms in ten cases were reported. They all originated from subarachnoid hemorrhage due to rupture of these aneurysms. The patients in these cases were all admitted within 5 days after the onset. The neurological state of four cases on admission was grade 4 or 5 in Hunt and Kosnik's grading system. Two patients of grade 5 died within 24 hours after the onset. The CT scan on admission revealed heavy subarachnoid hemorrhage mainly in the posterior fossa. In severe cases, ventricle hematoma, cerebellar hematoma and/or subdural hematoma were evident in the posterior fossa. The locations of these eleven aneurysms were as follows: On the bifurcation of the Telovelotonsillar segment in six cases. In the cortical segment in three cases. In the anterior medullary and tonsillomedullary segment in one case. Six saccular aneurysms were situated on bifurcations of parent arteries but three saccular aneurysms did not arise from bifurcations. Two of them were not from turning points of the arteries. One fusiform aneurysm situated on the bifurcation of telovelotonsillar segment was excised, and histologically shown to be a dissecting aneurysm with hypoplasia of elastic lamina and tunica media in the parent artery. Three cases were associated with small AVM located on the superior surface of the cerebellar vermis and fed mainly by the superior cerebellar artery (SCA) in two cases, and by SCA and PICA in one case. In these cases the hemodynamic stress on PICA did not seem to increase so remarkably.(ABSTRACT TRUNCATED AT 250 WORDS)

[Assessment of left ventricular contractile characteristics following coronary artery bypass grafting].

The present study was designed to assess a left ventricular function, particularly contractile characteristics following coronary artery bypass grafting (CABG). The subject was 29 post-CABG patient consisting of 4 groups; 6 patients with complete revascularization (CR) and no myocardial infarction (MI) (CR.MI(-) group), with CR and MI (CR.MI(+) group), 5 with incomplete revascularization (IR) and no MI (IR.MI(-) group) and 8 with IR and MI (IR.MI(+) group). Ejection fraction (EF), systolic pressure/end-systolic volume index (SP/ESVI) and end-systolic volume index (EDVI) were evaluated utilizing radionuclide angiography at rest and during exercise (Ex). Increase of SP/ESVI and EF and no change of EDVI during Ex were observed in CR.MI(-) group. Increase of SP/ESVI and no change of EF and EDVI in CR.MI(+) group, no change of SP/ESVI, EF and EDVI in IR.MI(-) group and no change of SP/ESVI and EDVI and decrease of EF in IR.MI(+) group were observed. The SP/ESVI and EF of CR.MI(-) group were respectively highest among 4 groups and showed no difference compared with control group during Ex. Ex-induced increase ratio of SP/ESVI had linear correlations with Ex-induced increment of EF (r = 0.81, p less than 0.001) and Ex-induced increase ratio of EFVI (r = 0.54, p less than 0.005). It is concluded that both CR and no MI lead to normalization of EF and SP/ESVI during Ex. Furthermore, in the case of poor contractile reserve, systolic function may deteriorate in spite of increased preload during Ex.

[Superior petrosal sinus dural arteriovenous malformation with varix indenting brain stem--report of a case and review of the literature].

A rare case of the dural AVM mainly around the left petrosal sinus was reported. A 64 years old man was admitted just after the sudden onset of severe headache and nausea. The CT scan revealed subarachnoid hemorrhage in the left ambient cisterns. A small hematoma was also found in the left cerebellar peduncle. External carotid angiogram showed a dural AVM which nidus was located adjacent to the left superior petrosal sinus. Its feeding arteries were as follows; the middle meningeal artery, the artery of foramen rotundum, the accessory meningeal artery, the dural branch of occipital artery and the ascending pharyngeal artery. The voluminous petrosal vein and the dilated cortical veins were identified as drainers and, the portion of the latter appeared as "varix" embedded in the pons, which was clearly delineated by MRI. In the venous phase, stenotic straight sinus and residual Falcine sinus were illustrated. Superselective embolization of the feeding arteries was employed followed by the direct clippings of draining vein. Postoperative course was uneventful. The present case should be classified into the tentorial dural AVM. Only 26 cases of this rarely encountered entity was reported in the literature. Based on both the present and the previously reported cases, the clinical features, treatment and pathogenesis of this disease were briefly discussed.

[Evaluation of myocarditis using T1-201 myocardial scintigraphy with bull's eye functional map].

In order to evaluate myocardial damage in a patient with myocarditis, rest thallium-201 myocardial single photon emission computed tomography (SPECT) was performed in 15 patients with myocarditis. For qualitative and semiquantitative analysis, Bull's eye functional maps were made up with SPECT images. In the functional map, the abnormal area, where T1 uptake is less than mean-2SD of the T1 uptake of normal subjects, is generally distributed in the myocarditis group. But focal and sequential abnormal areas were recognized more often in the clinically severe cases. Abnormal area tended to be observed commonly at the antero-septal wall, but it was uncommon at the lateral wall. Extent score, i.e. degree of extension of abnormal area, and severity score, i.e. degree of abnormality, were in good negative correlation with left ventricular ejection fraction (r = 0.6, r = 0.7). Furthermore, existence of abnormal area was in good correlation with the left ventricular regional wall motion. Abnormal area existed 100% in the akinetic region, 71% in the region of severe hypokinesis, and 27% in the region of hypokinesis. Abnormal area occupied 30% of the normokinetic region in the myocarditis group, which was a higher rate than in the normal control group (p less than 0.05). It was suggested that latent myocardial damage existed in the normokinetic myocardium with myocarditis. Thus, rest T1-201 SPECT with Bull's eye map is useful for clinical diagnosis in patients with myocarditis.

Transfer of a normal human chromosome 11 suppresses tumorigenicity of some but not all tumor cell lines.

The complete suppression of tumorigenicity of a human cervical cancer cell (HeLa) and a Wilms' tumor cell line (G401) following the introduction via microcell fusion of a single chromosome t(X;11) has been demonstrated by Stanbridge and co-workers. To determine whether other tumor cell lines are suppressed by chromosome 11, we performed chromosome transfer experiments via microcell fusion into various human tumor cell lines, including a uterine cervical carcinoma (SiHa), a rhabdomyosarcoma (A204), a uterine endometrial carcinoma (HHUA), a renal cell carcinoma (YCR-1), and a rat ENU-induced nephroblastoma (ENU-T1). We first isolated a mouse A9 cell containing a single human chromosome 11 with integrated pSV2-neo plasmid DNA. Following microcell fusion of the neo-marked chromosome 11 with the various tumors mentioned above, we isolated clones that were resistant to G418 and performed karyotypic analyses and chromosomal in situ hybridization to ensure the transfer of the marked chromosome. Whereas the parental cells of each cell line were highly tumorigenic, SiHa and A204 microcell hybrid clones at early passages were nontumorigenic in nude mice and HHUA was moderately tumorigenic. On the other hand, YCR-1 and ENU-T1 microcell hybrid clones were still highly tumorigenic following the introduction of chromosome 11. Thus, the introduction of a normal chromosome 11 suppresses the tumorigenicity of some but not all tumors, suggesting that the function of the putative suppressor gene(s) on chromosome 11 is effective only in specific tumors.

Effects of zinc L-carnosine on gastric mucosal and cell damage caused by ethanol in rats. Correlation with endogenous prostaglandin E2.

The effects of zinc L-carnosine on ethanol-induced damage and the correlation of these effects with endogenous prostaglandin E2 were evaluated in rat gastric mucosa in vivo and in vitro. When given either intragastrically or intraperitoneally, zinc L-carnosine (10 or 30 mg/kg) prevented gross visible damage to gastric mucosa caused by ethanol without affecting the mucosal prostaglandin E2 level. This protective effect of zinc L-carnosine was not inhibited by indomethacin. Histological assessment showed that zinc L-carnosine inhibited deep mucosal necrosis, as did 16,16-dimethyl prostaglandin E2. Zinc L-carnosine (10(-6) or 10(-5) M) inhibited the damage caused by ethanol to gastric cells isolated from rat gastric mucosa in vitro; this effect was not inhibited by indomethacin. The results suggested that zinc L-carnosine protects the gastric mucosa and enhances cellular resistance to ethanol without the mediation of endogenous prostaglandins.

Effects of a new 1,3-thiazole derivative ZSY-39 on force of contraction and cyclic AMP content in canine ventricular muscle.

A newly synthesized 1,3-thiazole derivative ZSY-39 increased the force of contraction in a concentration-dependent manner in association with elevation of tissue cyclic AMP levels in the isolated canine ventricular trabeculae electrically driven at 0.5 Hz at 37 degrees C. ZSY-39 shortened the duration of isometric contractions mainly by abbreviation of the relaxation time. The maximal response to and EC50 of ZSY-39 were 0.7 (isoproterenol = 1.0) and 4.6 x 10(-5) M. Bupranolol (3 x 10(-7) M) did not affect the positive inotropic effect of ZSY-39. The time course of increases in the force of contraction induced by ZSY-39 (10(-4) M) coincided with that of cyclic AMP accumulation. The concentration-response curve for the increase in the force of contraction produced by ZSY-39 was superimposable on that of the elevation of cyclic AMP levels. Carbachol (3 x 10(-6) M) shifted the concentration-response curve for the increase in force by ZSY-39 to the right and downward, and decreased the accumulation of cyclic AMP induced by ZSY-39 (10(-4) M). ZSY-39 (10(-5) M) enhanced significantly the positive inotropic effect of isoproterenol. The relationship between the force of contraction and cyclic AMP levels after the administration of ZSY-39 was not modified by the addition of carbachol or isoproterenol. These findings indicate that cyclic AMP plays an important role in the positive inotropic effect of ZSY-39 on canine ventricular muscle.

Right ventricular myocardial infarction and late cardiac tamponade due to right coronary artery aneurysm--a case report.

Coronary artery aneurysm is a relatively rare disease, which may cause angina, myocardial infarction, or sudden unexpected death due to thrombosis, embolization or rupture. This report describes a case of a 46 year old male who suffered an inferior myocardial infarction with right ventricular involvement, third degree atrioventricular block, cardiogenic shock and late cardiac tamponade, all caused by a right coronary artery aneurysm. He was successfully treated with emergency coronary artery bypass grafting. A review of the literature is also given to emphasize the importance of prompt recognition and correct management of the coronary artery aneurysm.

Simultaneous coronary and abdominal aortic surgery--a report of two cases.

Myocardial infarction is a major cause of operative mortality following abdominal aortic surgery. For this reason, routine coronary angiography and, if indicated, coronary revascularization prior to aneurysm repair is recommended. Nevertheless, some controversy still exists concerning the risks of aneurysm rupture and myocardial ischemic events when repair or coronary surgery is staged. This report describes two successful cases of simultaneous surgery for an expanding aneurysm and severe coronary artery disease, emphasizing the usefulness of gastroepiploic artery grafting in such cases.

Comparison of prostacyclin production of human gastroepiploic artery and saphenous vein.

The prostacyclin production of the gastroepiploic artery (GEA) and saphenous vein (SV) were studied in 5 patients undergoing coronary artery revascularization. The GEA produced 90.0 +/- 11.9, 132.4 +/- 13.7, and 191.1 +/- 21.8 pg/mg tissue (mean +/- standard error of the mean) of 6-keto-prostaglandin F1 alpha (prostacyclin metabolite) after 2.5, 5.0, and 10.0 minutes, respectively, of incubation in Krebs-Henseleit buffer at 37 degrees C. The SV produced 39.8 +/- 7.0, 66.7 +/- 9.1, and 123.6 +/- 15.1 pg/mg tissue of 6-keto prostaglandin F1 alpha after 2.5, 5.0, and 10.0 minutes, respectively, of incubation. The GEA produced significantly more 6-keto-prostaglandin F1 alpha than SV at all three sampling times up to ten minutes of incubation (p less than 0.01). Prostacyclin is a potent vasodilator and an inhibitor of platelet aggregation. Prostacyclin production by the internal mammary artery was reported to be much higher than that of SV, and the patency rate of internal mammary artery grafts is reported to be better than that of SV grafts in coronary artery revascularization. Therefore, our results suggest that the patency rate of GEA grafts may be better than that of SV grafts in coronary artery revascularization. The GEA is a promising and excellent graft from the biochemical point of view.

Free arterial graft as internal mammary artery-Y complex.

Because of excellent surgical results obtained with in situ internal mammary artery grafts, recent efforts have been made to extend the use of arterial conduits. In this study 2 cases of coronary revascularization using free internal mammary artery and gastroepiploic artery grafts as an internal mammary artery-Y complex are described. We believe that complete revascularization with only arterial conduit can be achieved more widely by using this technique.

Pharmacological response of internal mammary artery and gastroepiploic artery.

Pharmacological response of coronary artery bypass conduit is of great importance. This study was designed to clarify the contractile properties of internal mammary artery and gastroepiploic artery obtained from coronary revascularization. The response to ergonovine, serotonin, and phenylephrine was examined by isometric contraction recording apparatus. The concentration-response relation of both internal mammary artery and gastroepiploic artery to ergonovine, serotonin, and phenylephrine showed similar sigmoid curves. There were no significant differences in developed tension between internal mammary artery and gastroepiploic artery at any concentration for any agent. There were no significant differences in the 50% effective dose value for any agent between internal mammary artery and gastroepiploic artery. Internal mammary artery and gastroepiploic artery are reported to be similar in terms of size, flow capacity, and freedom from atherosclerosis. This study shows their equivalence from a pharmacological viewpoint.

Dose and dose-splitting effects of X-rays on lung tumour induction in mice.

This paper reports lung-tumour induction 12 months after single or split doses of X-rays in C3H/He male mice. The early proliferative response of lung cells after doses which induced lung tumours was also examined after single X-irradiation. The lung-tumour incidence tended to increase with increasing dose after a single irradiation and peaked at 5 Gy. At more than 10 Gy it decreased sharply to the control level. The mean tumour diameters tended to increase with doses up to 7.5 Gy and then decreased beyond 10 Gy. These results suggested that suppression of tumour growth reduced the tumour incidence at doses of over 10 Gy. The lung-tumour incidence decreased with increasing intervals between two equal doses of 2.5 or 5 Gy. The decrease was thought to be caused by the repair of the tumorigenic injury. However, the tumour incidence after two 2.5 Gy irradiations at 1 day intervals or two 5 Gy irradiations at 6 h intervals was higher than that observed after a single dose. This phenomenon was regarded as a progression of the tumorigenic injury. The labelling indices of the lung cells, determined using tritiated thymidine after a single irradiation, were higher than those of non-irradiated control cells. This response was delayed as the dose increased. The responses versus days after irradiation could be classified into three patterns on the basis of their peaks. The possibility that the larger delay after higher doses had some relation to the reduction of target cells for tumour incidence is suggested.

Haloperidol delays visually evoked cortical potentials but not electroretinograms in mice.

Visually evoked cortical potentials (VECPs) and electroretinograms (ERG) to flash stimulation were studied in one ICR strain mouse and 23 ICR mice, respectively. The effect of a dopamine receptor blocker, haloperidol, on both responses was investigated. Significant peak latency prolongation was found in VECPs following injection of haloperidol, while the amplitude reduction of the b-wave was found in ERG. No effect of anesthetics only on ERG was found. It has been assumed that haloperidol affects VECPs related function mainly the higher visual pathway. However, the slight decrease of the b-wave amplitude of ERG could not exclude the possibility of the toxic effect on the retina.
