[Surgical treatment for impending rupture of distal aortic arch aneurysm: a case report using "elephant trunk" prosthesis].

A 75-year-old man with impending rupture of distal aortic arch aneurysm was operated on using the "elephant trunk" prosthesis. A long graft was anastomosed only to the proximal aortic stump and its distal free portion was inserted through the exit of the aneurysm into the descending aorta. Postoperative CT scan and MR image showed complete thrombosis formation in the dead space between the graft and the aneurysmal wall, which precluded necessity for further operation. However the graft was looped and kinked in the aneurysm. Furthermore the patient suffered from incomplete paraplegia, probably caused by the extremely long graft. In terms of promoting thrombosis formation in the aneurysmal inner space the longer prosthesis is feasible, but one should keep it in mind that an excessively long graft can lead to the kinking of the graft and paraplegia.

Combination therapy with rhGM-CSF and rhEpo for two patients with refractory anemia and aplastic anemia.

Because GM-CSF possesses burst-promoting activity (BPA) and megakaryocyte colony-stimulating activity (Meg-CSF) as well as stimulating activity on granulocyte-macrophage progenitors, and erythropoietin (Epo) has thrombopoietin-like activity, the combination therapy of GM-CSF and Epo seems to be more effective for stimulating erythropoiesis and thrombocytopoiesis in patients with pancytopenia. For this reason, the combination therapy of recombinant human GM-CSF (rhGM-CSF) and rhEpo was performed in two patients with refractory anemia (RA) and aplastic anemia (AA). Epo-unresponsive anemia was remarkably improved by adding rhGM-CSF to Epo and the effect lasted for 1 1/2 months in a patient with RA, but severe anemia occurred again immediately after the discontinuation of Epo. The neutralizing antibodies against GM-CSF were not demonstrated at the phase when anemia re-progressed in this patient. In a patient with AA, anemia and thrombocytopenia, which were refractory to previous administration of rhGM-CSF, responded to the combined administration of GM-CSF and Epo. Although the effects were maintained for 3 1/2 months, the anemia and thrombocytopenia became worse again after the administration of rhGM-CSF was changed from daily to every other day. These findings suggest the usefulness of combination therapy of GM-CSF and Epo for patients with pancytopenia.

[Epidemiological study on sexually transmitted diseases in Saitama Prefecture--mainly study of male urethritis].

Epidemiological studies on sexually transmitted diseases (STDs) in Saitama Prefecture, from 1989 through 1991, were performed and the following results were obtained. The number of patients was 1,833 (male: 1,532, female: 30) cases in 1989, 1,294 (male: 1,163, female: 131) cases in 1990, and 1,672 (male: 1,478, female: 149) cases in 1991. Male patients in their twenties or thirties were often affected by STDs. The order of frequency of STDs was roughly as follows; non-gonococcal and non-chlamydial urethritis, gonococcal urethritis, chlamydial rethritis, condyloma acuminatum, genital herpeic infection and syphilis. Concerning the criteria of the diagnosis on chlamydial urethritis, cases diagnosed by only clinical findings decreased markedly from 1989 through 1991, cases diagnosed by the combination of the antibody of C. trachomatis and clinical findings increased, and cases diagnosed by the detection of C. trachomatis were always about 70%. Male patients were mostly infected from prostitutes. Many patients with STDs were often infected in Taiwan, the Philippines, the Kingdom of Thailand and Republic of Korea.

[Studies on the fragments of FDP in 3 non-DIC patients with increased FDP levels in the sera].

We previously studied fibrinolysis and fibrinogenolysis by analyzing fragments of fibrin/fibrinogen degradation products (FDP) employing sodium dodecyl sulfate-polyacrylamide gel electrophoresis and immunoblotting. In this report, we characterized the fragments of FDP in three patients with increased serum FDP, that were caused by various diseases. In the patient suffering from tuberculous constrictive pericarditis (case 1), the most part of the FDP fragments were DD and D. In the patient suffering from infection in addition to liver cirrhosis (case 2), the most part of the FDP fragments were high molecular weight (HMW) and D. In case 1 and 2, serum FDP levels were increased in parallel with the elevations of CRP levels. Although DD and HMW fragments were remarkably increased in case 1 and 2 with our immunoblotting analysis, DD levels assayed with LPIA system were much lower than FDP levels. The reason this discrepancy was explained by the observation that affinities of the monoclonal antibody used in LPIA system with DD and HMW fragment were markedly lower than that to DD-E fragment. In the patient suffering from deep vein thrombosis probably caused by steroid therapy of nephrotic syndrome (case 3), the most part of detected FDP fragments were DD and HMW in the period when APTT was shorter than normal, whereas D was mainly observed in the period when APTT was normal. In case 3, FDP and DD levels were increased in parallel with the shortening of APTT. In these non-DIC patients, increased serum FDP levels were induced by the presence of ascites and/or pleural effusion plus infection.(ABSTRACT TRUNCATED AT 250 WORDS)

[Studies on the fragments of FDP in 3 patients with DIC associated with acute promyelocytic leukemia].

We previously reported a study on fibrinolysis and fibrinogenolysis that analyzed fragments of fibrin/fibrinogen degradation products (FDP) using sodium dodecyl sulfate-polyacrylamide gel electrophoresis and immunoblotting. In this report, we characterized the fragments of FDP in three patients with disseminated intravascular coagulation (DIC) associating with acute promyelocytic leukemia (APL). D, Y, DD, DY/X and high molecular weight fragments were observed in sera of all the patients obtained at the onset of APL. These results showed that various degrees of fibrinogenolysis, concomitant with fibrinolysis, was occurring in APL patients presenting DIC. However, changes in the patterns of the FDP fragments during anticoagulation therapy were apparently different among three patients. Namely, fibrinogenolysis was dominant in case 1, while fibrinolysis was dominant in case 2. Interestingly, fibrinogenolysis and fibrinolysis were almost equivalent from the onset to the end of DIC in case 3. In case 3, FDP and FDP-D dimer were remarkably elevated about two months before the onset of APL, although their elevation was not complicated with DIC but with bone marrow necrosis. At that time, serum LDH levels and plasma polymorphonuclear elastase (PMN-Ela) were increased presumably due to the release of these enzymes from necrotic bone marrow, and the levels of CRP and plasma fibrinogen were increased probably due to an infectious complication. In non-DIC period of case 3, FDP and FDP-D dimer were spontaneously decreased without reduction of PMN-Ela levels, after three weeks of chemotherapy for microbial agents. Taken together, characteristics of FDP fragments were unique to each case of APL-DIC, probably because many factors differently affected the degradation of fibrin and/or fibrinogen.

[Propranolol for prophylaxis of first hemorrhage in cirrhotic patients with esophageal varices--a controlled study comparing with sclerotherapy].

To compare the efficacy of oral propranolol and sclerotherapy in the prevention of first hemorrhage from esophageal varices, 65 cirrhotic patients with moderate to large esophageal varices and no history of bleeding were included in the prospective controlled trial. After randomization, 33 patients received propranolol at a does reducing the heart rate by 25%; 32 patients were treated with intra-variceal and extra-variceal injection of ethanolamine oleate. On entry to the trial, the two groups were comparable in terms of clinical and biological parameters. The patients were observed for up to 60 months, with an average of 31 months. Nine patients bled (5 in propranolol and 4 in sclerotherapy) during follow-up. No significant difference were observed between propranolol and sclerotherapy in the cumulative bleeding rate and survival. The multivariate Cox model indicated that drug compliance in the propranolol group and high portal pressure in the sclerotherapy group were factors predictive of the first hemorrhage. These data support that propranolol and sclerotherapy are of comparable value in preventing the first hemorrhage in cirrhotic patients with esophageal varices.

[Upshaw-Schulman syndrome].

Upshaw-Schulman syndrome is a rare congenital disease, with thrombocytopenia and microangiopathic hemolytic anemia, which can be temporarily and repeatedly corrected by the transfusions of plasma from normal donors. Although the pathogenesis of this congenital disorder remains unknown, it appears that patients with this syndrome lack an unidentified plasma factor, which is normally present in plasma and is genetically determined.

[In vitro chemosensitivity assay using a double-layer-agar system: human tumor cloning assay and thymidine incorporation assay].

Human tumor cloning assay (HTCA) and thymidine incorporation assay (TIA) were both performed using a double-layer-agar system with continuous exposure of cells to standard anticancer drugs. The rate of evaluable assays was 44% (280 of 638 tumor samples) in HTCA and 48% (216 of 452) in TIA. When the tumor samples were restricted to the gastric and colorectal cancers, it was significantly higher in TIA than HTCA (p < 0.01). HTCA was 95% reliable for predicting in vivo resistance and 52% reliable for in vivo sensitivity, whereas TIA was 88% reliable for in vivo resistance and 44% for in vivo sensitivity. These results seem to suggest that it is unnecessary to select just clonogenic tumor cells among whole tumor cell population in assessing chemosensitivity of human tumors. The current study also indicates that chemotherapy with in vitro sensitive drugs assessed by TIA produced longer survival of the patients with stage III or IV gastric cancer. Most of clinical correlation trials including this study, however, have been performed retrospectively. The prospective study is necessary to determine whether drug selection by in vitro assays is superior to that by an experienced oncologist. In addition, further studies on pharmacokinetics of anticancer drugs are required to solve some other problems inherent to the current chemosensitivity assays.

[Crosstalks onto photopeak windows of 99mTc and other radionuclide (201Tl, 123I or 111In) in simultaneous acquisitions using a scinticamera--a fundamental study through phantom experiments].

In simultaneous acquisitions of both 99mTc and other radionuclide, there are crosstalks onto those photopeak windows. Therefore, to quantify the organ uptake in scintigraphic imaging, it is important to correct the crosstalk counts. The purpose of this study is to estimate the crosstalk fractions onto each photopeak window from other radionuclide. Those crosstalk fractions were determined from pulse height spectra, which were measured by inputting signals from a scinticamera into a multi-channel (2048 ch) pulse height analyzer. Four types of collimators, which are HR (high resolution), AP (all purpose) for low energy, ME (medium energy) and HE (high energy), as well as cuboid phantoms (10 x 10 x 10 cm3) were used in this experiment. The crosstalk fractions have a tendency to show small increases at the window width of 10 to 20%, excepting at 30%, a small change for the source thickness from 1 to 7 cm, and the least with the ME-collimator. The crosstalk fractions using the ME-collimator were obtained as follows, when the source thickness and the window width were 5 cm and 15%, respectively. 99mTc and 201Tl: 9.4% onto 201Tl window and 7.3% onto 99mTc window, 99mTc and 123I: 36.1% onto 99mTc window and 14.8% onto 123I window, 99mTc and 111In: 32.6% onto 99mTc window and 6.1% onto 111In window.

[Anomalous origin of the left coronary artery from the pulmonary artery: a case report in an adult].

A 37-year-old female with anomalous origin of the left coronary artery from the pulmonary artery was surgically treated by creation of intrapulmonary aorto-coronary tunnel. Postoperative cardiac catheterization showed a minimal pulmonary stenosis at the baffle. But the remarkable preoperative ST-depression in a treadmill exercise test disappeared completely after operation and the normograde blood flow of the left coronary artery was detected from the ascending aorta.

[Successful repair of total anomalous pulmonary venous connection and incomplete endocardial cushion defect associated with left isomerism].

Successful repair of a 8-month-old girl with polysplenia was reported. The cardiovascular anomalies were TAPVC (II b), incomplete ECD, interruption of inferior vena cava with hemiazygos continuation, bilateral superior vena cava, and left superior vena cava draining into the coronary sinus. Cardiopulmonary bypass was established with ascending aortic perfusion and caval cannulation. A left superior vena cava was directly cannulated after establishing partial bypass. In this case the left pulmonary vein drained into the right atrium near the orifice of the coronary sinus, so the atrial septal flap was made and sutured between the orifice of the left pulmonary vein and the coronary sinus in order to avoid late pulmonary vein obstruction. Then, atrium was separated by an intraatrial baffle which was sutured to the atrial septal flap. Recently, it becomes possible to surgical repair of polysplenia syndrome according to the advancements of the diagnostic methods, cardiopulmonary bypass, and the technique of the open heart surgery.

Carrier-mediated transport system for choline and its related quaternary ammonium compounds on rat intestinal brush-border membrane.

The characteristics of the intestinal transport system for choline were investigated using isolated brush-border membrane vesicles from rat small intestine. In spite of the diminutive lipid solubility, the uptake of choline by membrane vesicles reflected smooth permeation into intravesicular space rather than the binding to the membrane surface. Physiological conditions, present in the intact intestine, such as an inward-directed Na+ or H+ gradient and inside negative membrane potentials, didn't directly involve in choline transport across the brush-border membrane. Moreover, an outward-directed H+ gradient had no significant effect on the time course of choline transport. However, in the absence of a driving-force, the initial uptake of choline exhibited a saturable manner. A kinetic analysis of the initial uptake rate gave an apparent Km of 159 microM. Furthermore, unlabeled choline caused both cis-inhibition and trans-stimulation for labeled choline transport, suggesting the existence of a carrier-mediated transport system for choline, classified as so-called 'facilitated diffusion'. Since tetramethylammonium, acetylcholine, and N1-methylnicotinamide caused both cis-inhibition and trans-stimulation, they appear to be accepted as the substrate of choline carrier. On the other hand, quaternary ammonium compounds (QACs) such as those which possessed hydrophobic parts in their molecules exhibited only cis-inhibition. They also inhibited Na(+)-dependent D-glucose transport, indicating that they influenced various carrier-mediated transport systems non-specifically due to interaction with the membrane. These findings strongly suggest that the choline transport system on the brush-border membrane of rat intestine recognizes only small molecular QACs as its substrate.

CENP-C, an autoantigen in scleroderma, is a component of the human inner kinetochore plate.

We have isolated and characterized a set of overlapping cDNA clones that encode the human centromere autoantigen centromere protein C (CENP-C). The identity of these clones has been established using several criteria. First, they were shown to encode a polypeptide that migrates at the expected position for CENP-C on SDS-polyacrylamide gel electrophoresis. Second, we have demonstrated that this polypeptide shares at least two epitopes with human CENP-C. Polyclonal antibodies were raised to fusion proteins encoded by nonoverlapping regions of the cDNA clones. These antibodies were shown to recognize a protein at a position appropriate for CENP-C on immunoblots of human chromosomal proteins. In addition, we used indirect immunofluorescence to demonstrate that these antibodies recognize centromeres of HeLa chromosomes in the expected pattern for CENP-C. Localization of CENP-C by immunoelectron microscopy reveals that this protein is a component of the inner kinetochore plate.

Uptake characteristics of polyamines into rat intestinal brush-border membrane.

The uptake characteristics of polyamines, such as spermine, spermidine and putrescine, have been investigated using brush-border membrane vesicles isolated from the small intestine of rats. The uptake of these polyamines into the membrane vesicles was high and the order of uptake was spermine greater than spermidine greater than putrescine at medium pH 7.5, respectively. The medium pH considerably affected the uptake of these polyamines and the amount of uptake increased remarkably with an increase of the medium pH (pH 7.5 or 8.0 greater than pH 5.5). An inward Na+ gradient did not stimulate the uptake rate of any of these polyamines. We have also examined the binding behaviour to the membrane lipid, phospholipids and total lipid, and there was a good correlation in the binding properties, pH-dependency and uptake activity, between the liposomes and brush-border membrane vesicles. These results suggest that the uptake of the polyamine into the vesicles consisted of rapid binding to the outside intestinal surface and slower binding to the inside membrane after permeation. Furthermore, findings from experiments concerning the mutual inhibition among these polyamines and concerning the effect of other polycations, having 2-5 amines in number, on the uptake of spermine, suggest that the number of amino groups in the polyamine molecules plays an important role in the uptake process into the brush-border membrane vesicles.

A case of cardiac foreign bodies associated with four types of tachycardias.

A case of cardiac foreign bodies leading to development of four varieties of automatic tachycardia is reported. A 51-year-old male with aortic regurgitation was admitted to our hospital because of palpitations. An electrocardiogram revealed junctional tachycardia with or without left bundle branch block, and two types of fascicular tachycardias. Computed tomography showed metallic foreign bodies from a fractured guidewire in the membranous portion of the interventricular septum, which was inadvertently retained when he underwent diagnostic cardiac catheterization at the age of 27.

The pH dependent uptake of enoxacin by rat intestinal brush-border membrane vesicles.

The mechanism of the intestinal transport of enoxacin, an orally active fluoroquinolone antibiotic, has been investigated using brush-border membrane vesicles isolated from rat small intestine. The initial rate and time-course of enoxacin uptake were considerably dependent upon the medium pH (pH 5.5 greater than pH 7.5) and upon the percent ionization of the carboxyl group (pKa 6.2, anionic charge), namely, the degree of uptake of cationic form was higher than that of the zwitterionic form. There was evidence of transport into the intravesicular space as shown by the effect of extravesicular medium osmolarity on enoxacin uptake at steady state (30 min). This transport across the brush-border membrane was stimulated by the valinomycin-induced K(+)-diffusion potential (interior negative) and an outward H(+)-diffusion potential. Furthermore, changing the pH of the medium from 5.5 to 7.5 significantly decreased the effect of valinomycin-induced K(+)-diffusion potential on the enoxacin uptake. These results suggest that the uptake behaviour of the cationic form of enoxacin plays an important role in the intestinal absorption process of enoxacin.
