Three years' experience with large ovarian cysts diagnosed in utero.

Thirteen fetuses with abdominal cystic tumors were diagnosed by routine prenatal ultrasonography between the 28th and 36th week of gestation. Postnatal ultrasonography of the full-term newborns confirmed the findings. Laparotomies were performed in all cases except one. Patients who were operated on had large ovarian pseudocysts with volumes between 24 and 120 cc. In seven patients (54%), the cysts arose from the left ovary; this included four cases in which the postnatal ultrasound was interpreted as showing the cysts in the right abdomen. Very thin cystic walls threatening perforation were found in 91%. In 33%, we found salpingotorsion on the affected side. Small contralateral ovarian cysts were found in 66% of the patients. One infant required resection of 30 cm of necrotic jejunum because adhesions to the cyst had caused bowel volvulus. Histology of the cysts showed hemorrhage and calcifications, but ovarian stroma was absent in all but one patient. Serum estradiol-17 beta, progesterone follicle-stimulating hormone, and luteinizing hormone were normal in all cases, and similar levels were found in the cyst fluid. These results show that large abdominal cystic masses in full-term infant girls with normal gonadotrophin levels and normal serum estradiol-17 beta and progesterone levels are very likely to require surgery; this is in contrast to preterm neonates with elevated gonadotrophins who can be treated with medroxyprogesterone acetate in the absence of clinical signs necessitating surgery.

A biography is presented in commemoration of the 70th anniversary of the death of Max Wilms, whose name is almost exclusively known in association with the mixed tissue tumours of the kidney. Describing his curriculum vitae, we intended to place particular emphasis on the significance of his surgical work at the University of Heidelberg.

Differences between a supervised and independent strength and conditioning program with chronic low back syndromes.

The purpose of this study was to evaluate changes in physiologic parameters seen in a group of patients with chronic low back syndrome assigned to supervised and independent strength and conditioning programs. Forty patients with chronic low back syndrome were assigned either to a control group (independent exercise) or to an experimental group (supervised exercise). All subjects underwent pre-testing for aerobic fitness, strength and responses to visual analog pain rating scales. Twenty control subjects were given predesigned exercise programs and told to exercise four times per week for 6 months. Twenty experimental subjects were given predesigned exercise programs but were monitored by a strength and conditioning specialist for the same period. Statistically significant results were seen for increases in aerobic fitness and strength, decreases in reported pain, and percent body fat in the experimental group. Since the experimental group completed 90.75 sessions out of 96, compared with 31.95 for the control group, it could be concluded that supervision increases chances for compliance and success as measured by these parameters.

[The effect of endogenous and exogenous factors on psoriasis vulgaris. A clinical study].

Using computerized data collected from 390 patients we investigated the impact of "endogeneous" factors (infections, menarche, menopause, gravidity, hormonal contraception, stress, nutrition, alcohol consumption, drugs) and "exogenous" factors (seasons, climate, sun exposition, Koebner phenomenon) on the course of psoriasis. We recommend certain rules of behaviour based up on our data and the reviewed literature. It is of importance to inform patients about their skin disease and positive as well as negative modifications of its course by endo- und exogenous factors. This is the only way to enable patients for an active selfcare.

Low-fat diet and regular, supervised physical exercise in patients with symptomatic coronary artery disease: reduction of stress-induced myocardial ischemia.

The effects of physical exercise and normalization of serum lipoproteins on stress-induced myocardial ischemia were studied in 18 patients with coronary artery disease, stable angina pectoris, and mild hypercholesterolemia (total serum cholesterol 242 +/- 32 mg/dl). These patients underwent a combined regimen of low-fat/low-cholesterol diet and regular, supervised physical exercise at high intensity for 12 months. At 1 year serum lipoproteins has been lowered to ideal levels (serum cholesterol 202 +/- 31 mg/dl, low-density lipoproteins 130 +/- 30 mg/dl, very low-density lipoproteins 22 +/- 15 mg/dl, serum triglycerides 105 [69 to 304] mg/dl) and physical work capacity was improved by 21% (p less than .01). No significant effect was noted on high-density lipoproteins, probably as a result of the low-fat/high-carbohydrate diet. Stress-induced myocardial ischemia, as assessed by thallium-201 scintigraphy, was decreased by 54% (p less than .05) despite higher myocardial oxygen consumption. Eighteen patients matched for age and severity of coronary artery disease served as a control group and "usual medical care" was rendered by their private physicians. No significant changes with respect to serum lipoproteins, physical work capacity, maximal rate-pressure product, or stress-induced myocardial ischemia were observed in this group. These data indicate that regular physical exercise at high intensity, lowered body weight, and normalization of serum lipoproteins may alleviate compromised myocardial perfusion during stress.

[Alcohol and the cardiovascular system].

Because of the high frequency of cardiovascular diseases and a steadily increasing consumption of alcohol the potentially causal relationship between alcohol and cardiovascular diseases gains great interest for public health policy. Alcohol and its metabolites induce a toxic damage of myocardial metabolism with an injury of electromechanic coupling. As a consequence of acute alcoholic intake cardiac arrhythmias and a reduced contractility of the myocardium are found not only for chronic alcoholics but also in healthy non-drinkers. Chronic abuse of alcoholic beverages for many years can be the cause of alcoholic cardiomyopathy in a small percentage of patients, who have a bad prognosis. Atria and ventricles are dilated, light and electron microscopic changes of the myocardium are unspecific. The pathogenesis of alcoholic cardiomyopathy is unknown, modulations of cardiomyocytic membranes are discussed in the course of a toxic damage. In the genesis of atherosclerosis alcohol can approach from different sites: Changings on thrombocytes and an increase of HDL-cholesterin can be protective, however an increase in blood pressure support the process of atherosclerosis. In numerous investigations a smaller degree of atherosclerosis was found for little or moderate alcohol intake, while in chronic heavy abuse of alcohol a higher extent of atherosclerosis was observed. As the amount of alcohol, assumed to be protective against the development of atherosclerosis, is consumed already by the majority of the population, there is no reason to propagate a regulate consume of moderate amount of alcoholic beverages.

[Effect of dithranol therapy on membrane, basement membrane and nuclear markers in psoriasis lesions].

We investigated the effects of anti-psoriatic therapy with dithranol (1/20-1%) in salicylic acid (0.5%) in white petrolatum on lesional skin. FITC-labeled lectins and pemphigus vulgaris antibodies (PV) served as analytical means to study the glycocalyx. Antibodies of bullous pemphigoid (BP) were used as basal membrane markers. Nuclear antigens were recorded according to the binding of speckled, anti-nuclear antibodies (ANA) as well as antibodies to dsDNA. With some lectins, dithranol therapy resulted in pronounced fluorescence of the lower parts of the basal cells. ConA was fixed by the basal cell layer. To a lesser degree, ANA were fixed by nuclei of keratinocytes. PV antibodies were not fixed at all.

Tumour-induced intraluminal stenoses of the cervical trachea--tumour excision and tracheoplasty.

Three children with stenoses of the cervical trachea caused by tumours are reported on. The first was a 13-year-old girl with a sarcoma of the tracheal wall 20 cm down. We performed partial resection of the trachea and turned a skin flap according to Denecke into the defect. In the second case a capillary haemangioma had caused severe stenosis, requiring resection of the tumour together with the tracheal mucosa. A haemangioma had caused tracheal stenosis in the third case, too; in this child partial tracheal resection and skin flap plasty were performed. The postoperative course was uneventful in all three children. So far, there is no recurrence of the sarcoma 4 1/2 years after operation.

Optic disc cupping as clinically estimated from photographs.

Cup-to-disc ratios of 204 right eyes were evaluated by two clinicians and by a trained grader using a standardized grading scheme. Mean measurements by the grader were greater than estimates made by either clinician. The measurement scheme used by the trained grader has a high degree of reliability and validity. The standard protocol provides an objective assessment of optic disc cupping and could be easily adapted for office use.

[Ovarian pseudocysts in female newborn infants: prenatal ultrasonic diagnosis and surgical consequences].

During the last year we diagnosed within the scope of preventive medical care of pregnant women in about the 30th pregnancy week, seven foetuses with abdominal cystic tumours by ultrasound examination. Postpartal ultrasound controls of the mature newborn confirmed the findings. After laparotomy we found ovarian pseudocysts with a volume up to 90 ccm. Histologically we could not identify ovarian tissue. In 66% of the cases we found small cysts on the contralateral ovary. Differential diagnosis and the possible complications must be considered. A laparotomy should be performed and only by high steroid serum level a conservative therapy with MPA should be considered. This is the case in premature newborn with an immature endocrine regulation mechanism.

[Cantrell's syndrome].

Congenital diaphragmatic hernia is associated with a high mortality, especially when combined with other defects. The unusual repeated combination of the same congenital abnormalities by a number of patients and the fact that during embryonic life these organs develop from the mesoderm implies the existence of a specific syndrome. This article summarises the available clinical data and reports on three cases with Cantrell's syndrome, showing different pattern of congenital defects such as: 1) supraumbilical abdominal wall defects; 2) a defect of the lower sternum; 3) a deficiency of the anterior diaphragm; 4) a defect in the diaphragmatic pericardium; 5) congenital intracardiac defects.

[Arterial blood supply of the upper pole of the spleen in main artery occlusion].

Vascular dissections of the splenic-, gastric- and pancreatic region, corrosion specimens of the upper splenic pole and arteriographies of the superior polar artery after trunc lienal artery occlusion in human served as an answer to the question of the arterial supply of the upper lienal pole. The present results point out the importance of the upper polar artery and the existence of intersegmental anastomosis. The latter gain importance in case of trunc splenic artery occlusion. The short gastric vessels are of subordinate value.

Collateral Circulation

[Gastrointestinal perforations in neonatal surgery].

From 1970 until 1987 we treated 69 newborns with gastro-intestinal perforations. Immediate measures in form of laparotomy were undoubtedly necessary. Regarding our casuistic the authors present the clinical findings, necessary preoperative diagnostic measures and intra-operative and postoperative treatment. The localisation and the origin of the perforation are pointed out. The most frequent localisation was the small bowel (58%), followed by the colon (35%). Etiologically we found extensive NEC, atresias, meconium ileus and Hirschsprung's disease. In 4 cases iatrogenic rectum perforations, in 3 cases spontaneous gastric and duodenal perforations and twice an appendicular perforation were seen. Our operative procedure depends on the localisation, the degree of peritonitis, on intestinal necrosis and trophic alterations of the gut following ileus. In spite of intensive care, the mortality rate is still very high indicated with 33%. Furthermore more than half premature newborns died (18 out of 33). Based on these facts we propose quick diagnostic measures, interdisciplinary cooperation, an emergency operation followed by intensive care to ameliorate the results. Nevertheless we must keep in mind that newborns, who some years ago were condemned to die, nowadays can be operated, thus changing the mortality of gastro-intestinal perforations.
