Doppler and B-mode ultrasound for avascular nephrotomy.

Doppler sonography for intraoperative localization of the intrarenal arteries combined with B-scan sonography for intraoperative visualization of stones allows complete stone removal via small radial nephrotomies for which clamping of the renal artery is no longer necessary. Since September 1980 we used this technique on 35 patients with staghorn or recurrent calculi. The main advantages of this technique are exact and quick stone localization, minimal loss of renal function owing to preservation of the intrarenal vascular system, and no need for renal ischemia and cooling.

[Computed tomography of renal angiomyolipomas].

22 cases of angiomyolipoma were studied by computerized tomography. Characteristics of the tumours were density values less than -25 HU in 21 patients; one patient having tuberous sclerosis showed higher attenuation values. In 6 of 22 patients angiomyolipomas were associated with tuberous sclerosis. Two large tumours presented with spontaneous rupture and massive retroperitoneal hemorrhage. CT findings are sufficiently typical for angiomyolipomas and therefore suitable for distinguishing the benign lesion from renal cell carcinoma.

Doppler sonography and B-scanning in renal surgery.

The combination of intraoperative localization of intrarenal arteries and stones by Doppler and B-scan sonography allows the performance of multiple radial avascular nephrotomies and makes renal stone surgery possible without clamping and cooling.

Doppler and real-time ultrasound in renal stone surgery.

In stone surgery the operative approach aims not only at complete removal of stones, but also at minimizing damage to the intrarenal arteries and parenchyma. In complicated staghorn or calyceal stones, our concept is to remove as many stones as possible through an extended pyelocalicotomy and the residual calyceal stones via minimal radial nephrotomies. To accomplish an atraumatic nephrotomy, the shortest transparenchymal route should be chosen, and damage to the intrarenal arteries must be avoided. Doppler sonography for intraoperative localization of the intrarenal arteries and combined B-scan sonography for intraoperative localization of stones allow an avascular, atraumatic nephrotomy, in which clamping of the renal artery and cooling are no longer necessary. In 35 operations with a total of 109 radial nephrotomies, clamping of the renal artery was required in only 2 cases. By comparing the preoperative and 6-week postoperative functional results, split 131I-hippuran clearance demonstrated only 7% loss of function of the operated kidneys.

Posterior urethral valves: theoretical considerations on embryological development.

Among congenital urethral malformations, posterior urethral valves represent one of the main causes of severe infravesical obstruction in children, the etiology and embryological development of which remains unclear. Posterior urethral valves of types I and III are supposed to be related to either the persistence of connective tissue along the pathway of migration of the Wolffian duct (type I) or a residual part of the endodermal urogenital membrane (type III). A possible reason for supramontane urethral valves (type II) is seen in the persistence of crest-shaped tissue along the ureter's pathway in embryological development to its final site, although there is some doubt as to the existence of type II valves and they were not seen in the 65 cases of posterior urethral valves in our own material.

Ferritin - another tumor marker for testicular malignancies.

On the basis of results obtained with other malignancies, the usefulness of serum ferritin as a tumor marker in the diagnosis of testicular tumors was studied. In 68 patients with malignant testicular tumors and 22 patients with benign testicular processes, serial determinations of beta-human chorionic gonadotropin (beta-HCG), alpha-fetoprotein (AFP) and serum ferritin were performed. The serum ferritin values showed no relationship to tumor stage, and no difference in nonseminomatous and seminomatous tumors. In particular, no diagnostic improvement was achieved in those cases in which the conventional markers AFP and beta-HCG were unsuccessful (falsely negative results). Furthermore, serum ferritin values were influenced by hepatic factors and anemia, so that serum ferritin - on the basis of the present material-could not be considered a useful tumor marker in testicular malignancies. Further studies on the determination of carcinofetal isoferritin and other new tumor markers are warranted.

[Diffusion of co-tetroxazin into prostatic tissue (author's transl)].

For 2,4-diamino-5-[3,5-dimethoxy-4-(2-methoxyethoxy)]-benzylipyrimidine (tetroxoprim, TXP) and 4-amino-N-(2-pyrimidinyl)benzenesulfonamide (sulfadiazine, SDZ) tissue levels in prostate adenoma patients were 10.75 +/- 5.83 micrograms/g an 8.52 +/- 3.88 micrograms/g, respectively. In accordance with the determined plasma levels of 1.54 +/- 0.53 micrograms/ml TXP and 17.39 +/- 6.11 micrograms/ml SDZ, the prostate/plasma coefficients were found to be 8.26 (TXP) and 0.48 (SDZ).
