[The need for psychotherapy in school-age children].

The article reports the results of an investigation of N = 1.969 school children (aged 6 to 17) of three rural communities, who had been assessed by a German version of the Child Behavior Checklist (Achenbach u. Edelbrock 1983). It was intended to determine the prevalence of clinical maladjustment and to investigate the effectiveness of selective factors in referring to child psychiatric facilities. The case definition was based on those items (behavior disorders), which were rated as in need for counseling or treatment by experts. After a statistical comparison with a sample of referred children the criterion was set up at two or more of such critical items. 12.7% of the school children fulfilled this condition, but most of them (87.6%) had not used institutional services. The rate of children and adolescents, who need psychotherapy was estimated lower (less than 10%). The readiness to refer clinical maladjusted children to child psychiatric facilities was influenced by age and socio economic status, but not by sex and foreign nationality.

[Significance of family relations for coping with psychological disorders--results of an empirical study of therapy prognosis of children and adolescents with psychological disorders].

The importance of family relations for the therapy of psychiatric disorders in children and adolescents is discussed reporting results from an empirical study carried out in an inpatient sample of a child and adolescent psychiatric unit with regard to the quality of mother-child resp, father-child relation and its influence on coping processes. Whether adolescents suffering from a psychiatric disorder are able to cope with their problems during the course of a therapy, depends on their individual and social resources. Therapy outcome is to a great extent influenced by the emotional quality of their family relations. Hostility and rejection as well as unstructured and disorganized parental behavior contribute to a negative outcome. A differentiated analysis shows further that the relation between the adolescents and their fathers is particularly important for therapy prognosis. Nevertheless, one has to consider the respective relations with both parents for therapy prognosis, as problems with one parent may be partly compensated by a good relationship with the other parent. Finally, the cooperation between parents and clinical staff is discussed. Data and experiences show that interest and readiness of parents (mothers as well as fathers) for a close cooperation are higher than generally expected.

[Developmental trends in child and adolescent psychiatry].

Since 1968 child and adolescent psychiatry has become an independent medical specialty in Germany. As psychotherapy has always been an essential part of the discipline, it has meanwhile also been incorporated into the name of the specialty which now reads: child and adolescent psychiatry and psychotherapy. From a historical point of view, child and adolescent psychiatry in Germany has its main roots in adult psychiatry and neurology as well as in pediatrics. Furthermore, it has been stimulated in many ways by other neighbouring disciplines. During the last decades, three trend-setting developments put forward the improvement of child and adolescent psychiatric care in Germany: the "Psychiatrie-Enquête", the model program for psychiatry of the federal government and the "Psychiatry Personnel Act" (Psychiatrie-Personalverordnung). The improvement in the field of care as well as new scientific findings can open important perspectives for the future, but these must always be seen within the frame of society development.
