[Results of treatment with clozapine in schizophrenic adolescents].

The indications for administering clozapine to 21 adolescent inpatients with schizophrenia were insufficient response to other neuroleptic agents, likelihood of psychotic symptoms becoming chronic and extensive extrapyramidal side-effects from other neuroleptic agents. Marked improvement in or complete disappearance of most of the remaining psychopathological symptoms was seen in 11 of the 21 subjects, with at least some improvement seen in an additional 6. Vegetative side-effects such as daytime weariness, dizziness, orthostatic hypotonia and hypersalivation were usually transient. Deviations from the normal range of values for the hematological and serum enzyme values assessed were of no clinical significance and occurred no more frequently than with other neuroleptic agents, and the values returned to normal when treatment was continued. On the whole, clozapine is a useful addition to the group of drugs available for treating adolescents with schizophrenia.

[Paradoxical reactions and interactions of psychopharmaca in children and adolescents (author's transl)].

After a discussion of the various basic problems concerning the application of psychopharmacological drugs in children and adolescents, paradoxical reactions and interactions of some psychopharmaca are considered. Stimulantia can especially cause disturbances of growth and as a paradoxial reaction the increase of hypermotoric behaviour. Neuroleptics may lead to tardive dyskinesia, to interactions with hypotensors and occasionally to epileptic seizures. As to tricyclic antidepressive it is known that they can cause interactions with hypotensors and thyroid hormones, as well as paradoxical aggressive reactions. In connection with tranquilizers paradoxical reactions as anxiety and agitation as well as the so-called paradoxical rage reaction could be observed. Combinations of antiepileptica and psychopharmaca should only be used if there is no other possibility for adequate treatment. After all, the field of indications of psychopharmacological drugs in children and adolescents seems to be rather narrow.

[Disturbances of mental development in infants and preschool children. Their recognition and treatment (author's transl)].

After a short introduction setting out the importance of development dimensions for the understanding of disturbances of development and some statistics on the frequency of peculiarities of behavior in the preschool age, some disorders of mental development of practical importance in the first years of life are dealt with. Psychomotor disturbances, disorders of speech and speaking and emotional and contact disturbances are emphasized.

[Psychosocial consequences of head injuries in children (author's transl)].

Psychosocial consequences of head injuries in children can be understood in terms of an interaction between pretraumatic conditions, conditions and sequelae of the trauma (injury) and posttraumatic factors. The results of these interactions in the area of psychosocial functioning can be defined as effects on development in general as well as effects on school behaviour, occupational and family situation. Taking into account these factors, and the reaction of the famility, two major reactions can be identified: (1) Insisting on the until now practised attitude to the child, (2) Abrupt changing of the attitude to the child after head injury. Both reactions lead to an inadequate handling of the child and in consequence to overtax or overprotection. Finally, also the reaction of the child has to be taken into account. It should be integrated into the rehabilitation program. There seem to be differences between children with slight and severe head injuries.
