False aneurysm of the femoral artery due to an osteochondroma.

Two cases of primary misdiagnosis of a posttraumatic false aneurysm are reported. In the first case a twenty-year-old patient was hospitalized under the diagnosis of a malignant soft tissue tumor five weeks after a kick against the right distal thigh. Diagnostic procedures performed in our clinic, however, led to the expectation of a false aneurysm of the femoral artery, caused by perforation of the wall of the vessel by an osteochondroma. Intraoperative findings confirmed this and led to the supposition of a connection with the mechanical trauma. The postoperative course was uneventful. The second case had a similar history. Prophylactic excision of osteochondromas in areas with increased risk of perforation, as in the region of the femoropopliteal junction, is recommended.

Frequencies of segmental perfusion and ventilation abnormalities in lung scintigraphy.

The segmental ventilation-perfusion mismatch is almost invariably a sign of pulmonary embolism (PE). As ventilation scintigraphy is an expensive and time-consuming procedure, it is rarely performed in patients presenting as emergencies. In such patients PE is diagnosed by the presence of a segmental perfusion defect (SPD) in combination with a normal chest X-ray. However, little is known about SPD frequencies in different groups of patients or its value in predicting a mismatch. To determine this frequency, as well as its predictive value for a mismatch (PVM), we examined 764 patients with suspected Pe (PE? group), 359 patients at low risk for PE (low risk group), and 234 patients without suspected PE but at high risk for PE (high risk group), all by perfusion and ventilation scintigraphy. Frequencies of SPD were determined in each group as a function of age. PVM was calculated for each age subgroup using Bayes' theorem. In the low risk group the frequency of segmental mismatches (SM) was about 4%, while one-third of patients at high risk had a SM. This was not age dependent in contrast to the PE? group, where the frequency decreased with age. In the youngest subgroups the PVM for a SPD was higher than 0.9 as well as in a subgroup of the PE? group with a known thrombo-embolic disease. In these subgroups a ventilation scintigraphy is not required for the scintigraphic diagnosis of PE; in all other patients an additional ventilation study is necessary.(ABSTRACT TRUNCATED AT 250 WORDS)

Regeneration and function of autologous splenic grafts in pigs.

The growth kinetics, perfusion and immune response of autotransplanted splenic tissue were studied in pigs. Splenic fragments were transplanted subfascially or in the greater omentum and regenerated to small splenules with a normal histologic structure 6 months later. The grafts showed a normal function of the white pulp after antigenic stimulation, and of the red pulp as demonstrated scintigraphically. The blood flow was reduced. The growth kinetic was less rapid than in rodents and resulted in a relatively lower mass of splenic tissue, but these results are closer to findings in human splenosis.

Regeneration of heterotopically transplanted autologous splenic tissue.

Normal young pigs were splenectomized and thin slices of autologous splenic tissue grafted in pouches of the large omentum and underneath the fascia of the abdominal muscles. The regeneration of the transplanted splenic fragments was studied histologically. Within the first few days, the grafts underwent almost complete necrosis. After 14 days no signs of regeneration could be seen. All typical structures of splenic tissue could be found after 3 months but the red and white pulp were populated to a lesser extent than normal with lymphoid cells. After 6 months and 12 days after i.v. injection of sheep red blood cells, the number of follicles increased dramatically and all the morphological signs of an immune response could be seen. After heterotopic transplantation in young pigs splenic fragments regenerate to small splenules of normal appearance. The regeneration is less rapid and results in smaller masses of tissue than in rodents.

Selective labelling of lymphoid tissues by extracorporeal perfusion.

A method for selective labelling of lymphoid tissues by means of an extracorporeal perfusion system is described. The applicability of this technique was tested by local labelling of a part of the small intestine with its mesenteric lymph nodes, spleen and bone marrow in normal young pits. Tritiated thymidine was added to the perfusate and the selectivity of the labelling procedure tested by scintillation counting of serum samples, of specimens of lymphoid tissues and by autoradiography. There was only negligible spillover of the isotope and the perfused organs were well preserved.
