Codon 17 polymorphism of the cytotoxic T lymphocyte antigen 4 gene in Hashimoto's thyroiditis and Addison's disease.

Endocrine autoimmune disorders share susceptibility and resistance factors of the human leukocyte antigen system on the short arm of chromosome 6, but other gene loci also contribute to predisposition and protection. Because the cytotoxic T lymphocyte antigen 4 (CTLA4) alanine-17 encoded by the CTLA4 gene on chromosome 2q33 confers susceptibility to Graves' disease, as well as to type 1 (insulin-dependent) diabetes mellitus, we investigated this dimorphism in the other endocrine autoimmune disorders: Hashimoto's thyroiditis and Addison's disease. We analyzed the CTLA4 exon 1 polymorphism (49 A/G) in 73 patients with Hashimoto's thyroiditis, 76 with Addison's disease, and 466 healthy controls. This dimorphism corresponds to an aminoacid exchange (Thr/Ala) in the leader peptide of the expressed protein. CTLA4 alleles were defined by PCR, single-strand conformational polymorphism analysis, and restriction fragment length polymorphism analysis using BbvI. Patients with Hashimoto's thyroiditis had significantly more Ala alleles than controls, both as homozygotes (22% vs. 15%) and heterozygotes (53% vs. 46%), and less Thr than controls as homozygotes (25% vs. 39%), P < 0.04. The phenotypic frequency for Ala was significantly higher in patients (75%), compared with controls (61%), P < 0.03. Patients with Addison's disease did not differ significantly from controls, but those carrying the suceptibility marker, human leukocyte antigen DQA1*0501, were significantly more CTLA4 Ala17 positive than controls with the same DQA1 allele (P < 0.05). In conclusion, an alanine at codon 17 of CTLA4 confers genetic susceptibility to Hashimoto's thyroiditis, whereas this applies only to the subgroup of DQA1*0501+ patients with Addison's disease.

[Prognostic factors in gallbladder carcinoma as a decision aid for reoperation].

In a retrospective study of patients with gallbladder carcinoma, predictive factors for survival were evaluated. TNM stage, according to the UICC and complete tumor resection, turned out to be the strongest values in multivariate analysis. So it is important to make an early correct diagnosis and to reach complete tumor resection during surgical procedure. In case of an incidental tumor after simple cholecystectomy, hepatic wedge resection and lymph node dissection should be performed in T2 stage and above.

Waveform reliability of the phase related external suction baroreceptor manipulation method.

The phase related external suction (PRES) method of controlled carotid baroreceptor manipulation (a neck cuff with cardiac cycle timed pressure variations) has been used to investigate not only the human baroreflex, but extrahomeostatic effects of baroreceptor activity (such as the modulation of cortical activity, peripheral reflexes, and pain) as well. The present study provides quantitative within subject test-retest reliability estimates of PRES effects on 6-s trial-length heart rate waveforms. Mean within subject test-retest reliability for each averaged heart rate response waveform was 0.77 for the baroreceptor stimulation (systolic negative/diastolic positive cuff pressure) condition, 0.56 for the control condition (diastolic negative/systolic positive cuff pressure), and 0.69 for the difference between conditions. The importance of analyzing time-intensity dynamics, rather than merely the mean or maximum effect of any treatment variable is emphasized, and recommended for investigations of a wide variety of short duration, induced physiological time series processes.

PRES- and orthostatic-induced heart-rate changes as markers of labile hypertension: magnitude and reliability measures.

Split-half and test-retest reliabilities of heart-rate responses to a baroreceptor manipulation and an orthostatic maneuver were compared between subjects with either normal or elevated blood-pressure. Ten subjects showing elevated resting blood-pressure and II normotensive subjects participated in two experimental sessions, each including heart-rate recordings during baroreceptor manipulation and orthostatic challenge. Carotid baroreceptors were manipulated by applying the baroreceptor-specific phase-related external suction (PRES) technique. The orthostatic stimulation procedure (OSP) was a change of body position from lying to standing. Heart rate responses evoked by OSP failed to discriminate significantly between the groups either in the magnitude or the (test, retest) reliability measure. The PRES procedure also failed to discriminate with the conventional magnitude measure, but the reliability measures showed significant differences. Paradoxically, the high-blood-pressure group manifested the higher baroreceptor reliability. The present findings are consistent with the view that operant conditioning produces phasic blood-pressure increases. In this view, blood-pressure increases activate the arterial baroreceptors which, in turn, dampen pain and/or stress sensitivity. Individuals showing high consistency (reliability) in their cardiovascular responses are more likely to learn this form of conditioning, and hence to eventually increase their tonic blood-pressure. High reliability of cardiovascular responses may therefore constitute a risk for hypertension. Aside from such theoretical considerations, the findings indicate that less conventional dependent variables like reliability may be worth exploring in the search for the etiology of essential hypertension, and that, in this search, specificity (relative to baroreceptor function) is more important than the magnitude of the heart-rate changes that are produced.

Gas-potentiometric method with solid electrolyte oxygen sensors for the investigation of combustion.

Gas-potentiometric analysis using oxide-ion-conducting solid electrolytes as stabilized zirconia is a worthwhile method for the investigation of combustion processes. In the case of gas and oil flames specific parameters like the flame contour, the degree of burn-out and mixing can be determined and information about flame turbulence and reaction density can be gained from the temporal resolution of the sensor signal. Measurements carried out with solid electrolyte oxygen sensors in a fluidized bed show that combustion processes of solid fuels are also analyzable. This analysis results in fuel specific burn-out curves finally leading to burn-out times and to parameters of a macrokinetics of the combustion process as well as to ideas about the burn-out mechanism. From the resulting constants of the effective reaction rate a reactivity relative to bituminous coal coke can be given for any solid fuel.

Endogenous retroviral long terminal repeats of the HLA-DQ region are associated with susceptibility to insulin-dependent diabetes mellitus.

HLA-DQ genes are the main inherited factors predisposing to IDDM. This gene region harbors long terminal repeat (DQ LTR) elements of the human endogenous retrovirus HER V-K, which we analyzed for a possible association with disease. We first investigated whether LTR segregate with DQ alleles in families. Members (n = 110) of 29 families with at least one diabetic child, unrelated patients with IDDM (n = 159), and healthy controls (n = 173) were analyzed. Genomic DNA was amplified for DQ LTR3 by a nested primer approach as well as for DQA1 and DQB1 second exons, to assign DQA1 and DQB1 alleles. DQ LTR segregated in 24 families along with DQ alleles. Of the 29 families, 20 index patients were positive for DQ LTR. The DQ LTR was in all patients on the haplotype carrying the DQA1 *0301 and DQB1 *0302 alleles. A majority of patients had DQ LTR (62%) compared with controls (38%) (p < 1.3 x 10(-5)), even after matching for the high-risk alleles DQA1 *0501, DQB1 *0201-DQA1 *0301, and DQB1 *0302 (79% of patients and 48% of controls; p < 0.02). Subtyping for DRB1 *04 alleles in all DQB1 *0302+ individuals showed 56% DRB1 *0401, DQB1 *0302 [LTR' patients vs. 29% controls with the same haplotype (p < 0.002)]. In conclusion, these data demonstrate the segregation of DQ LTR with DQA1, DQB1 alleles on HLA haplotypes. Furthermore their presence on DRB1 *0401-, DQA1 *0301-, and DQB1 *0302-positive haplotypes suggest that they contribute to DQ-related susceptibility for IDDM.

Genetic markers in diagnosis and prediction of relapse in Graves' disease.

Hyperthyroidism of Graves' disease takes an unpredictable clinical course in the long-term follow-up. Whereas roughly 30-60% of patients relapse after their first antithyroid drug treatment, the likelihood of remission in the remaining group can not be foreseen. We have analysed-retrospectively-patients with Graves' disease that had been on antithyroid drug treatment for one year and were followed up thereafter. Patients were investigated for a variety of clinical parameters like ophthalmopathy status and relapse or remission as well as gene polymorphisms of the HLA and other regions. Of the 259 patients analysed so far, patients with ophthalmopathy did not differ from those without for HLA DQA1 and CTLA4 alleles tested. Also, the subgroup of patients with relapses after antithyroid drug treatment showed no different distribution of those alleles from the group with long-term remission. This study also confirms that the allele HLA DQA1* 0501 confers susceptibility to Graves' disease, furthermore, that the CTLA4-alanine 17 allele is an additional predisposing factor.

Lipopolysaccharide of Rhodospirillum salinarum 40: structural studies on the core and lipid A region.

The structural elucidation of lipid A of the cell wall lipopolysaccharide (LPS) of Rhodospirillum salinarum 40 by chemical methods and laser desorption mass spectrometry revealed the presence of a mixed lipid A composed of three different 1,4'bisphosphorylated beta (1 --> 6)-linked backbone hexosaminyl-hexosamine disaccharides, i.e. those composed of GlcN --> GlcN, 2,3-diamino-2,3-dideoxy-D-Glc-(DAG --> DAG, and DAG --> GlcN. Lipid A of R. salinarum contained preferentially 3-OH-18:0 and 3-OH-14:0 as amide-linked and cis delta 11-18:1 and c19:0 as ester-linked fatty acids. The mass spectra of the liberated acyl-oxyacyl residues proved the concomitant presence of 3-O-(cis delta 11-18:1)-18:0 and 3-O-(c19:0)-14:0 as the predominating diesters in this mixed lipid A. The glycosidically linked and the ester-linked phosphate groups of the backbone disaccharide were neither substituted by ethanolamine, phosphorylethanolamine, nor by 4-amino-4-deoxy-L-arabinose, in contrast to most of the enterobacterial lipid As. In the core oligosaccharide fraction, a HexA (1 --> 4)HexA(1 --> 5)Kdo-trisaccharide was identified by methylation analysis. The terminal HexA (hexuronic acid) is possibly 4-OMe-GalA, a component described here as an LPS constituent for the first time. LPS of R. salinarum showed a lethality in C57BL/10 ScSN (LPS-responder)-mice) of an order of 10(-1)-10(-2) of that reported for Salmonella abortus equi LPS, and it was also capable of inducing TNF alpha and IL6 in macrophages of C57BL/10ScSN mice.

Baroreceptor cortical effects, emotions and pain.

The specificity of baroreceptor-dependent inhibition of pain reactions to electrical stimuli was investigated during induction of different emotional states in 27 subjects. Baroreceptors were stimulated through the PRES (Phase Related External Suction) technique, while emotions were induced by means of pleasant, neutral and unpleasant slides. The dependent variables were pain ratings, somatic evoked potentials (N150 and P260) recorded from Fz, Cz and Pz, and skin conductance response (SCR), while heart rate was recorded as a PRES requirement. Valence and arousal ratings were obtained in front of each slide. During suction (external baroreceptor activation) reduced pain ratings, cortical disfacilitation (from Pz, as revealed by N150) and lower SCR were found as compared to pressure (baroreceptor deactivation). Moreover, brain evoked potentials (N150 and P260) reflecting cortical inhibition were found under condition of baroreceptor stimulation during unpleasant slides, but not during pleasant or neutral ones: this result was found in the high blood pressure subjects only. Data showed also a valence effect on pain ratings: pain was evaluated to be higher during unpleasant slides, than neutral and pleasant ones. Results are discussed in the light of "baroreceptor reward" hypothesis, which proposes a learning mechanism for the development of essential hypertension.

The relationship between habitual anger coping style and serum lipid and lipoprotein concentrations.

The relationship between habitual anger coping styles, especially anger expression in a socially assertive manner and serum lipid concentrations was examined in 86 healthy subjects. Habitual anger expression was measured by the Müller Anger Coping Questionnaire (MAQ) on four scales: Aggression, Controlled Affect, Guilt, and Social Inhibition. A positive correlation between serum cholesterol and Aggression was found, as was a negative correlation between LDL/HDL ratio and Controlled Affect. These correlations remained significant after adjustments for age, Body Mass Index (BMI), athletic activity, alcohol, nicotine and coffee consumption. After these adjustments, significant negative partial correlations between Controlled Affect and both total cholesterol and LDL were also found. We found no significant gender differences on any lipid measure. This study demonstrated that unfavourable lipid profiles are linked to a predominantly aggressive anger coping style, whereas favourable lipid profiles are associated with more socially assertive anger coping (Controlled Affect). Thus the expression of anger in a socially assertive manner can be seen as health promoting. The MAQ scales were better predictors of total serum cholesterol concentrations than the health related behaviors and characteristics. Results are discussed in terms of a psychophysiological risk model of inadequate anger coping.

[The percutaneous treatment of surgical and catheter-angiographic vascular complications].

PURPOSE: Technical innovations of angioplasty have introduced new therapeutic options in the treatment of symptomatic arteriosclerotic disease. Atherectomy devices, aspiration catheters and vascular endoprostheses can be used not only for angioplasty purpose but also for correction of iatrogenic dissections and thromboembolic complications. MATERIAL AND METHODS: 54 patients with vascular complications after surgical (n = 7) and percutaneous (n = 47) interventions were observed in a 5 years period. The complications included acute dissections in 34 cases, acute thrombosis in 4 cases, distal embolism after angioplasty in 14 cases, and two cases with arterial rupture after balloon dilatation. RESULTS: 34 patients with dissections and 18 patients with acute thrombosis were treated. Stent implantation for repair of vessel dissection was successful in 33 of 34 cases. Percutaneous catheter aspiration in combination with local thrombolysis was successful in 4 cases with local thrombosis. Distal emboli after angioplasty were successfully removed using aspiration catheters in 13 of 14 patients. Vessel rupture after angioplasty was closed with prolonged balloon dilatation in two patients avoiding emergency surgery. CONCLUSION: Using newly developed intravascular technologies such as atherectomy, stents, and catheter aspiration, many iatrogenic complications after vascular surgery or vascular interventions can be treated. These technologies should be available in every radiology department performing angioplasty.

PVU II polymorphism of LST-1 (leucocyte specific transcript-1) in type I diabetes mellitus, Graves' disease and healthy controls.

The polymorphism of the LST-1 gene (the human homologue of the mouse B144 gene) can be identified by Pvu II restriction enzyme digestion. We investigated the contribution of this RFLP to disease susceptibility in 117 patients with type I diabetes mellitus (IDDM), 110 with Graves' disease (GD) and 93 healthy controls. The distribution of the different LST-1 alleles (LST-1*1:1323 bp, LST-1*2:610 bp/713) was similar among IDDM and GD patients as well as in controls. The combination of DQA1*0501, DQB1*0201 and DQB1*0301, all predisposing to endocrine autoimmune disease, with LST-1*1 or LST-1*2 was not increased in patients. Analysis of two informative families with IDDM demonstrated cosegregation of DQA1 and DQB1 alleles with LST-1 alleles. No association of LST-1 polymorphisms with IDDM nor GD could be demonstrated.

Susceptibility and resistance alleles of human leukocyte antigen (HLA) DQA1 and HLA DQB1 are shared in endocrine autoimmune disease.

Because particular human leukocyte antigen (HLA) DQ alleles are the major predisposing factors for type 1 diabetes mellitus (IDDM), we investigated whether they are shared by other endocrine autoimmune diseases. We, therefore, analyzed the HLA DQ genotypes of 171 patients with IDDM, 271 with Graves' disease (GD), 65 with Hashimoto's thyroiditis, 51 with postpartum thyroiditis, 53 with Addison's disease (AD), and 271 healthy controls. HLA DQA1 and DQB1 alleles were defined by polymerase chain reaction and sequence-specific oligonucleotide hybridization as well as by single strand conformational polymorphism analysis. HLA DQA1*0501 was significantly more frequent in IDDM (60%), GD (65%), and AD (70%) than in controls (43%); DQA1*0301 was significantly more frequent only in IDDM (67% vs. 30% controls). The heterozygous state DQA1*0301/*0501 was found in 9% of controls and 35% of IDDM (relative risk, 5.6). An arginine at position 52 on either DQA1 allele was significantly more frequent in patients with IDDM (94%), GD (80%), and AD (89%) compared with controls (66%). HLA DQB1*0201 and DQB1*0302 were more frequent in IDDM patients (*0201, 62% vs. 36% in controls, *0302, 59% vs. 19% controls), whereas DQB1*0602 was less frequent in IDDM (4%) and GD (18% vs. 31% of controls). In conclusion, endocrine autoimmunity has a common immunogenetic background; susceptibility is conferred by DQA1*0501 as well as an arginine at position 52 of DQA1 alleles, and protection against IDDM and GD is conferred by DQB1*0602.

[Janeway laparoscopic gastrostomy].

The percutaneous endoscopic gastrostomy is the procedure of first choice for the creation of a gastrostomy. If that is not possible a laparoscopic access should be preferred to a laparotomy. In tumor patients there seem to be some advantages of the laparoscopic Janeway gastrostomy compared to Kader-Stamm fistulas. We describe the technique and discuss the results of 5 patients without complications referring to current publications. In conclusion the Janeway gastrostomy is a good palliation because it is an effective, safe and simple method with easy handling, and little requirement for nursing care.

Comparison of two mechanical carotid baroreceptor stimulation techniques.

We compared two mechanical carotid baroreceptor stimulation techniques, the phase related external suction (PRES) method and the conventional neck suction techniques concerning their effects on blood pressure and heart rate responses in a group of 10 normotensive men. The cuff pressure using the PRES method was phase-locked in time to the R-wave of the ECG. During the conventional neck suction technique the cuff pressure changes were not related to the cardiac cycle, it was either negative or positive. Blood pressure was measured in four of the patients both invasively and noninvasively to compare the two baroreceptor stimulating methods. The results have indicated that (1) both mechanical carotid baroreceptor stimulation technique showed a significant heart rate deceleration to baroreceptor stimulation. (2) The heart rate changes were more pronounced during the continuous neck cuff technique, and the heart rate recovered sooner to the baseline. The variation of baroreceptor activity as induced by the PRES method seems to prevent habituation much more than the continuous neck suction method. (3) The systolic blood pressure decrease was significant both during PRES and continuous neck suction stimulation. A higher decrease in systolic blood pressure was shown during continuous neck suction stimulation compare to the PRES stimulation. (4) The diastolic blood pressure changes showed the same alteration for baroreceptor stimulation as compared to the control condition but there was no difference between the two stimulation methods. (5) The noninvasive Finapres blood pressure device measures blood pressure reliably.
