[Is endogenous creatinine clearance a sound basis for estimation of elimination half life of gentamicin?].

The elimination half life of gentamicin after intravenous administration of 80 mg of Refobacin was determined experimentally in 24 patients with normal and variably reduced renal function in the agar diffusion test. Experimentally found elimination half lives were compared to values determined from the creatinine clearance. Although there was good correlation between both parameters, satisfactory agreement between experimentally determined and calculated half life was found only in patients with normal or slightly reduced renal function. With decreasing creatinine clearance the calculated value was constantly bigger than the measured value, and the range of both values increased. Gentamicin treatment in patients with grossly decreased renal function should thus be performed under control of serum levels.

[Effects of hexobendine on peripheral circulation (author's transl)].

Hexobendine (Ustimon) given i.v. increases the arterial blood flow in the musculus tibialis anterior ninefold and improves the peripheral circulation for a limited time. Also the better arterial perfusion caused by adenosine and similar substances is intensified by hexobendine given i.v. These results are discussed with the already known effects of other coronary vasodilators.

[Biological availability of spironolactone in two different galenic forms (author's transl)].

The biological availability of spironolactone in two different galenic preparations (Spiro-Tablinen and Aldactone-100 Caps) was investigated in a single-blind, randomised, cross-over study in six healthy probands. The serum concentration of canrenone after oral intake of spironolactone (100 mg) was investigated in the steady state as well as the renal excretion of canrenone. There were no statistically significant differences between the two preparations either in the course of the serum concentration of canrenone or in the cumulative renal excretion.

[Digitalis therapy in practice: correlation between clinical evaluation and plasma digoxin concentration (author's transl)].

In a prospective study 73 patients on maintenance digitalis treatment at the Paracelsus Institute, Bad Hall, were clinically examined and the dosage of the drug was adjusted according to cardiac symptoms. The clinical effects were correlated to digoxin concentrations measured on the day following admission to hospital and on the 21st day of treatment. The following practical conclusions were reached: 1. More than 50% of the patients were underdigitalized. 2. There is often no indication for digitalis therapy in patients with a low daily maintenance digoxin dosage and normal renal funciton. 3. The usual recommended maintenance dosage of digoxin provides serum digoxin levels in the lower region of the therapeutic range. 4. Patients with symptoms of decompensation taking an average dosage of digoxin need more digitalis. There is generally no danger of toxicity when the dosage is increased. 5. The serum digoxin concentration in patients with slightly reduced renal function lies in the upper region of the therapeutic range with usual doses of digoxin.

Hemodynamic changes in hypertensive patients at rest and during physical exercise before and after acute i.v. administration of bufuralol-HCl or propranolol.

The hemodynamic effects of 20 mg Bufuralol-HCl and of 15 mg Propranolol given to hypertensives i.v. at rest and under physical exercise conditions were examined. It could be shown that Bufuralol-HCl lowered the diastolic BP and PR at rest already in the acute experiment, contrary to Propranolol. Under physical exercise conditions the diastolic BP is lowered, the PR remains unchanged in spite of reduced CO. After exclusion of other possible explanations, Bufuralol-HCl may lower the diastolic BP acutely at least partly by inhibition of cerebral beta-receptors. A faster and better liquor diffusion could be the reason for these results. It can be assumed that the acute BP lowering effect is mediated by the same mechanism as the chronic effect of the other beta-receptor blocking drugs.

[Intravenous leucine load in patients with cirrhosis of the liver (author's transl)].

In 6 patients with cirrhosis of the liver and in 6 healthy controls the elimination half life of the serum leucine level (t 1/2 Leu) was determined after intravenous administration of 50 mg leucine per kg bodyweight. Examinations were repeated with simultaneous administration of 0,33 g glucose per kg bodyweight and during a continuous infusion of 500 microgram somatostatin over 60 minutes. The following results were obtained: 1. In cirrhotics and in healthy controls the t 1/2 Leu was shortened during the additional glucose administration compared with the t 1/2 Leu after leucine administration alone and was prolonged by inhibiting insulin secretion by somatostatin. 2. In spite of significantly higher serum insulin levels the t 1/2 Leu in patients with cirrhosis of the liver was significantly prolonged compared with healthy controls after leucine administration as well after leucine and glucose administration. Therefore we conclude, that in patients with cirrhosis of the liver in spite of elevated serum insulin levels leucine as well as glucose are metabolized slower. These results are indicating peripheral insulin resistance regarding the assimilation of glucose and leucine (at least in patients with portocaval shunts).

"VA", a new type of erythrocyte polyagglutination characterized by depressed H receptors and associated with hemolytic anemia. I. Serological and hematological observations.

This report describes a case of persistent polyagglutinability restriced to the red blood cells, and associated with hemolytic anemia in a 20-year-old male. AIME WITH NORMAL INTERVALS. His red blood cells were weakly agglutinated by almost all adult sera. No autoagglutination was noted and the direct antihuman globulin test was negative. Polyagglutinability of his red blood cells was clearly distinguishable from T, Tn and Tk. No reaction was noted with Dolichos biflorus and peanut extracts. The cells differed from normal erythrocytes in their positive reactions with various snail agglutinins and their weak reaction with a range of anti-H reagents. There was normal aggregation by Polybrene. MN determinants were normally expressed. The symbol "VA" is roposed for this type of polyagglutination.

[Acetylcholinesterase activity in spiral ganglion cells in rats].

Acetylcholinesterase has been demonstrated in the spiral ganglion cells by the method of Karnovsky and Roots. After incubation for 24 h the content of AchE was measured with the help of a three-stage scale. 22% of the cells showed strong, 50% medium and 28% weak AchE activity. Within the spiral ganglion perikarya, the enzyme was equally distributed.
