[Atlas fractures with paralysis of the caudal cranial nerves. A case report].

The fracture of the Atlas is a serious trauma, associated with important diagnostic traps. Whether operative or noninvasive treatment should be used depends on the type of fracture. Paralysis of brain nerves are exceptionally rare. We present a case in which fracture of the atlas was associated with paralysis of the 9th through 12th brain nerves, to which differential diagnostic considerations are discussed.

[Long-term results in complex knee injuries using sutures and reconstruction of the posterior cruciate ligament].

There is still a great discussion what's the best treatment for disruptions of the posterior cruciate ligament. The results of the operations are often bad. In Rheinfelden from 1979 to 1990 34 Patients underwent an operation of the posterior cruciate ligament. In 1991 we made a follow-up evaluation with 30 of these patients. The results according the OAK-Score were 8 x excellent, 11 x good, 3 x fair and 5 x poor, 3 patients were reoperated elsewhere. The average Lysholm score was 79 points. The results after treatment of the freshly ruptured ligaments were much better than the results of the treatment of chronic instabilities. A significant acute posterior instability should be operated.

[Pain as the main symptom in knee joint disorders].

Knee pain as the leading symptom can show the way to the diagnosis. Transmission and character of pain indicates the different diagnosis. The theories of pain formation are shown and explain. The information given by the patient should not be the only reason for an active therapy. Also there should be some objective clinical statements in order to permit active therapy. The article shows the combination between pain and objective statements.
