[Immunohistologic findings in the liver, spleen, brain and skeletal muscles in fatal cholestatic viral hepatitis (following double infection with hepatitis B and non-A/non-B?)].

We report immunohistological findings in liver, spleen, brain, and skeletal muscle of a 23-year-old woman with hepatitis nonA/nonB caused by contaminated anti-D-globulin. She died in a liver coma. At autopsy, a chronic liver dystrophy with cirrhosis was diagnosed. The necrotic areas of the liver showed a collapse of the reticulin framework, newly formed collagen fibres, and diffuse inflammation with immunohistological evidence of IgG, CIq, C9, and fibrinogen. C4 and C9 could be localised in bile thrombi and in the cytoplasm of pseudotubular transformed hepatocytes. In addition, C9 was found in blood vessel walls. A local distribution of HBsAg was found in the cytoplasm and/or the periphery of liver cells. HBcAg could not be detected in any of 5 different regions of the liver. A serum with antibodies to acute phase antigen of nonA/nonB hepatitis stained the cytoplasm and nuclei of (mostly intact) liver cells focally and their cell membranes diffusely. Patchy deposits of IgA and IgM were demonstrated in liver, brain und spleen. Circulating antibodies to cell nuclei and smooth muscle reacted with the patient's own liver and brain but not with spleen and skeletal muscle.--It is suggested that the manifold immunohistological findings in this patient are an expression of the vain attempt of the organism to clear away antigenic material, probably induced by different hepatitis viruses.

[Guillain-Barré polyradiculitis in acute viral hepatitis type B--case report].

A polyneuritis of the Guillain-Barré type developed in a 29 year-old female patient during the icteric stage of viral hepatitis B. The main symptoms were diffuse paresthesias and increasing motor weakness of the limbs, ending up in paralysis of the legs. The analysis of the CSF (albumin raised, cell count normal) and electroneurographic findings were diagnostically reaffirming. The patient was given prednisolone. All symptoms disappeared within two months.

Non A, non B hepatitis: ultrastructural findings in human liver biopsies.

Dense intranuclear particles 30 to 40 nm diameter, could be identified in liver specimens from female patients, who immediately after delivery had been inoculated with sera contaminated with non A, non B hepatitis. The same particles were described previously by KENDREY et al. (1975) in a case of acute hepatitis without clinical or serological evidence for hepatitis A and B. The joint occurrence of dense intranuclear particles with typical core particles in cases of chronic B hepatitis show that they are not specific for non A, non B hepatitis. The observed structures are likely to be nonspecific responses of hepatocytic nuclei to different hepatitis viruses. The nature of relationship to the infectious agent remains to be elucidated. Tubular structures in the endoplasmic reticulum and other cytoplasmic inclusions which had been demonstrated in liver cells from chimpanzees inoculated with non A, non B hepatitis, did not occur in our material.

[Epidemiology, clinical aspects, immunology and morphology of non-A, non-B hepatitis].

Examinations were carried out in a population group of 106 potentially contaminated females and in five donors, affected one year earlier within the frames of a restricted epidemic, caused by NANB-hepatitis virus. It was possible to differentiate a specific antibody-antigen system, identical to that of sirahi et al. The antigen was confirmed in 20 per cent of the females, 21 per cent of the contaminated did not get ill. The high SGPT level came in the foreground in the rest; 38 per cent had the signs of cholestasis. After one year, high transaminases were found in 43 per cent of the diseased, and biopsically--a chronic persistent hepatitis was found or a hepatitis with low activity. The chronification tendency correlates with the favourable clinical findings during the initial acute phase. The patients with prodromal phenomena, high transaminases and cholestasis showed a tendency to noncomplicated course and completely healing. The extraparenteral infection was confirmed, being conditioned by the close contact between mother and child or during breast feeding on one hand, on the base of the clinical manifestations, and biopsically--on the other.

[Demonstration of hepatitis A virus in the feces].

On the basis of own examinations methodology and results of the demonstration of the virus A in the stools and of the proof of antibodies in the serum are described. The parameters of the virus-A-hepatitis which are important for practice are further the basis for diagnosis, therapy and surrounding prophylaxis in routine work. They were compiled in a table.

The internistic accessory documentation as a partial region of the information processing referred to the patient in a hospital fulfils the demands of diagnostic and therapeutic kind from the point of view of the hospital physician to rationalise and optimize the demands of administrative work, such as identification and duration of stay, and demands of the physician performing the follow-up treatment to get a quick and sufficient information with the threefold aim of treatment (including prevention, diagnostics and after-treatment), documentation (including statistics, archivation, information) and research (any connection of data for cross-section and longitudinal section examinations you like).
